Hyperbaric-oxygen treatment: An adjunctive therapy in acute renal failure due to crush injury

Objective: Major seismic events leave their survivors trapped under the rubble leading to extensive muscle damage and its devastating sequale of hemodynamic and metabolic disturbances and acute renal failure. Hypoxemia aggravated during each hemodialysis (HD) session is important in acute renal failure patients with massive tissue injury. We retrospectively analyzed the adjunctive role of hyperbaric-oxygen (HBO) therapy in patients treated with HD for acute renal failure due to crush injury in terms of dialysis duration. Patients and methods: 16 patients admitted after the Marmara earthquake to Istanbul Faculty of Medicine were treated with HBO. Only 8 (2M, 6F, mean age: 24.8 + 7.3 years) of them had required HD treatment. 29 (16M, 13F, mean age: 34.6 + 12.9 years) crush syndrome patients treated with HD but not with HBO were taken as controls and the clinical and laboratory data of the two groups were compared. Results: The mean duration time under the rubble for the HBO group was 9.4 + 3.2 hours. Mainly lower extremity fasciotomies were performed at 15.6 + 14.8 hours after extrication. There were no amputations in this group. There were three amputations in the control group. The mean number of HD sessions was 9.2 + 6.7/patient for 10.9+9.6 days until renal functions recovered and the patients had 27.4 + 15.6 HBO sessions until the recovery of their lesions. There were no correlations between the number of HBO sessions and any laboratory parameter nor the number of HD sessions. There was no statistical difference in the need for HD between the HBO and control group. Conclusions: Our results could not demonstrate any beneficial effect of HBO treatment in terms of HD duration;however, the valuable contribution of the HBO treatment was to increase the salvage of crushed limbs.

What If I Told You Camouflage is a Myth? Animal Coloration is Mainly A-biotic and not Biotic (Camouflage)

In the present article,the author posits that the perception that animals apparently display a strategy of avoiding detection by means of camouflage—i.e.,by disguising themselves in the natural colours of their environment—is not the actual case in nature but,rather,merely *** coloration is mainly a-biotic(eco-physiological)and not biotic(camouflage).The contention regarding the absence of the phenomenon of camouflage among animals as a common evolutionary response is based on three arguments:(1)that reflecting the natural colours of the environment is linked to ecophysiology;(2)that predator and prey constitute“an evolutionary pair”and,accordingly,they know how to identify one another(in order to survive they employ different strategies,of which camouflage is not one of them);and (3)that the approach of relating animal camouflage to reflecting the colours of the environment is an anthropocentric *** than the accepted biotic-ethological approach(colour camouflage),the present article suggests the recognition of a-biotic and eco-physiological conditions as a distinct research field,whose title“Reflection of environmental colours by animals”,along with this article,calls for eco-physiologists to demonstrate that this approach indeed offers a special contribution to the understanding of colouration in animals.

The objective was to examine the effects of optimal leaf nitrogen levels>2.0%and suboptimal levelssynthetic rate,stem diameter increment,height growth increment and acorn mass of pedunculate oak during 2010 in the absence of drought stress and during 2011 under the impact of moderate drought *** to the results,moderate drought stress significantly reduced net photo synthetic rate,stem diameter increment and height growth increment,while acorn mass was not *** nitrogen nutrition significantly reduced the net photo synthetic rate and stem diameter increment only in the wet year,acorn mass in both wet and dry years,while height growth increment was not significantly reduced by suboptimal nitrogen nutrition in either *** results indicate that optimal nitrogen levels can stimulate photo synthetic rate and stem diameter increment of pedunculate oak only in the absence of moderate drought ***,the results show that moderate drought stress is a more dominant stressor for photosynthesis and growth of pedunculate oak than suboptimal nitrogen nutrition,while for acorn development,it is the more dominant stressor.

Purpose:Digitoxin is a cardiac glycoside used in the treatment of heart *** by its known antiinflammatory effect,this study aims to investigate the effect of digoxin in a sepsis model and to bring to light its effect and underlying mechanism in acute lung injury(ALI).Method:28 wistar albino rats were divided into 4 *** model is performed by the feces intraperitoneal-injection procedure(FIP).Results:TNF-a,CRP,IL-6,IL 1-Beta,lactic acid,and MDA values were significantly decreased in the FIP+digitoxin group compared to the FIP+saline *** the same groups were examined,histological improvements such as decrease in alveolar inflammation and decrease in septal thickening in the digitoxin group and thorax CT were found to be significantly higher in the Hounsfield unit digitoxin group compared to the saline ***:Digitoxin has shown biochemical improvement in sepsis with all known mechanisms of action,and healing effects in both computerized tomography and histology in the lung.

Energy states of the Hulthen plus Coulomb-like potential with position-dependent mass function in external magnetic fields

We need to solve a suitable exponential form of the position-dependent mass （PDM） schr6dinger equation with a charged particle placed in the Hulthen plus Coulomb-like potential field and under the actions of the external magnetic and Aharonov-Bohm （AB） flux fields. The bound state energies and their corresponding wave functions are calculated for the spatially-dependent mass distribution function of interest in physics. A few plots of some numerical results with respect to the energy are shown.

Exact solutions of Effective Mass Dirac Equation with Non-PT-symmetric and Non-Hermitian Exponential-type Potentials

By using a two-component approach to the one-dimensional effective mass Dirac equation, bound states are investigated under the effect of two new non-PT-symmetric and non-Hermitian exponential type potentials. It is observed that the Dirac equation can be mapped into a schrodinger-like equation by rescaling one of the two Dirac wave functions in the case of the position-dependent mass. The energy levels and the corresponding Dirae eigenfunctions are found analytically.

Effect of BMI on outcomes of surgical treatment for tibial plateau fractures： A comparative retrospective case series study

OBJECTIVE:Tibia plateau fracture (TPF) treatment aims at achieving a stable, aligned, mobile, painless knee and preventing post-traumatic osteoarthritis. To achieve this goal, surgeons consider criteria such as patients' characteristics, severity, risk of complications, fracture displacement/depression, degree of soft tissue injury. However, body mass index (BMI) is not considered as a risk factor in literature. Our study was conducted to find out any possible correlation between BMI and functional scores or radiological score separately.;METHODs:Retrospective analysis of case series between 2011 and 2014 was done on the database of a tertiary hospital in Istanbul. There were 67 TPF patients (54 males, 13 females) in the study. Relationship between BMI and functional knee scores or radiological score was compared statistically. Closed fractures with both high-energy and low-energy injury were included in the study. Patients with open fracture, multi-trauma presence, meniscus and/or ligamentous injury, increased co-morbidity, inadequate records (25 cases in all) were excluded. surgery type, schatzker classification, injury side, trauma energy, and gender were considered as possible risk factors. Binary regression analysis was done for possible factors affecting functional knee scores and radiologic score.;REsULTs:Model summary calculations were done as Nagelkerke R test for Knee society score, Lysholm knee score, and Ahlback and Rydberg radiologic scores, which were 0.648, 0.831, and 0.327 respectively. Homer-Lemeshow test values were 0.976, 0.998, and 0.362, respectively. There is negative correlation between BMI and both knee function scores. There is no correlation between BMI and radiologic score.;CONCLUsIONs:An increase in BMI has a negative effect on functional knee scores after surgical treatment of TPFs. Therefore, BMI should be considered as a risk factor for surgical treatment of TPFs.
