Anatomic and functional results of idiopathic macular epiretinal membrane surgery

AIM: To assess the impact of macular surgery on the functional and anatomic outcomes in patients with grade 2 epiretinal membrane(ERM), and the effect of internal limiting membrane(ILM) peeling on visual acuity and to analyze the long-term effect of pars plana vitrectomy(PPV) on intraocular pressure(IOP).METHODS: Pseudophakic eyes(62 eyes) diagnosed as idiopathic grade 2 ERM with at least 6 mo postoperative follow-up were included in this retrospective study. The fellow eye was nonvitrectomized. Patients were divided into two groups: group 1(29 eyes) treated with ERM and ILM peeling and group 2(33 eyes) with only ERM peeling. Preoperative and postoperative best corrected visual acuity(BCVA), slit-lamp, and a dilated fundus examination was performed. IOP was measured with Goldman applanation tonometer before, day 1 and first week and each visit after surgery. The incidence of significant IOP elevation was compared between vitrectomized eyes and nonvitrectomized fellow ***: Visual improvement was statistically significant and similar in both groups(P=0.008 in group 1, P=0.002 in group 2, P=0.09 inter-group). The amount of decrease in central macular thickness was statistically significant and similar in both groups(P=0.005 group 1, P=0.008 group 2, P=0.37 intergroup). At the final follow-up(14.1±9.6 mo) the incidence of significant IOP elevation was 4% in vitrectomized eyes(three eyes) and 3%(two eyes) in the nonvitrectomized fellow eyes(P=0.12). Four eyes(12.1%) had recurrent ERM after a mean follow-up of 8.6±1.1 mo in group 2, there was no recurrence in group 1(P=0.01).CONCLUSION: Recurrence of ERM may be decreased by ILM peeling during ERM surgery. However, it seems that ILM peeling do not affect the functional outcome and 23-gauge PPV alone do not have a significant effect on IOP.

Orbital decompression surgery for the treatment of Graves＇ ophthalmopathy：comparison of different techniques and long-term results

AIM： To evaluate the long-term results of different orbital decompression techniques performed in patients with Graves＇ ophthalmopathy（GO）.METHODS： Totally 170 cases with GO underwent orbital decompression between 1994 and 2014. Patients were divided into 4 groups as medial-inferior, mediallateral（balanced）, medial-lateral-inferior, and lateral only according to the applied surgical technique. Surgical indications, regression degrees on Hertel exophthalmometer, new-onset diplopia in the primary gaze and new-onset gaze-evoked diplopia after surgery and visual acuity in cases with dysthyroid optic neuropathy（DON） were compared between different surgical ***： The study included 248 eyes of 149 patients. The mean age for surgery was 42.3±13.2 y. DON was the surgical indication in 36.6% of cases, and three-wall decompression was the most preferred technique in these cases. All types of surgery significantly decrease the Hertel values（P〈0.005）. Balanced medial-lateral, and only lateral wall decompression caused the lowest rate of postoperative new-onset diplopia in primary gaze. The improvement of visual acuity in patients with DON did not significantly differ between the groups（P=0.181）. CONCLUSION： The study show that orbital decompression surgery has safe and effective long term results for functional and cosmetic rehabilitation of GO. It significantly reduces Hertel measurements in disfiguring proptosis and improves visual functions especially in DON cases.

First report of the histopathological effect of electrocautery using on the urethral taste rosea during glans penis injury by incision in rabbits

Comparison of the Retinomax hand-held autorefractor versus table-top autorefractor and retinoscopy

AIM:To compare noncycloplegic and cycloplegic results of Retinomax measurements with findings achieved after cycloplegia using table-top autorefractor and ***:The study included 127 patients(mean age96.7mo,range 21 to 221).Retinomax(Rmax)(Nikon Inc.,Japan)was used to obtain noncycloplegic *** cycloplegia,refraction was measured with Rmax,table-top autorefractor(TTR)(Nikon NRK 8000,Inc.,Japan)and *** values of sphere,spherical equivalent,cylinder and axis of cylinder were recorded for Rmax,TTR and retinoscopy in each *** results were analyzed ***:The mean spheric values(SV),spherical equivalent values(SEV)and cylindrical values(CV)of the noncycloplegic Rmax(SV:0.64 D,SEV:0.65 D and CV:0.03 D,respectively)were found to be significantly lower than cycloplegic TTR(1.43 D,1.38 D and 0.3 D;P=0.012,P=0.011 and P=0.04,respectively)and retinoscopy(1.34 D,1.45 D and 0.23 D;P=0.04,P=0.002 and P=0.045,respectively).Mean cycloplegic SV,SEV,CV were not significantly different between Rmax and TTR,Rmax and retinoscopy,TTR and *** or noncycloplegic axis values were not different between any ***:Rmax may be used successfully as a screening tool but may not be accurate enough for actual spectacle *** Rmax measurements may be able to identify refractive error in children because of approximate results to retinoscopy.

Retinal nerve fiber layer thickness changes in obstructive sleep apnea syndrome: one year follow-up results

·AIM: To investigate the retinal nerve fiber layer(RNFL)thickness changes in patients with obstructive sleep apnea syndrome(OSAS) for one year follow-up. To discuss the possibility of detecting tendency of glaucoma in this population by using spectral domain optical coherence tomography(3D-OCT-2000 Spectral domain).·METHODS: After polysomnographic study, all subjects(64 OSAS patients and 40 controls) underwent detailed ophthalmological examination. After these examinations,patients with glaucoma and patients who had ophthalmological and/or systemic disease were excluded from the ***, 20 patients in OSAS group and five patients in controls were excluded from the study in the first examination and follow-up period. The RNFL thickness was assessed with OCT. Forty-four OSAS patients and 35 control subjects were followed up for12 mo. RNFL thickness change and OSAS patients were evaluated for severity of disease by Apnea-Hypopnea Index(AHI).·RESULTS: Forty-four OSAS patients and 35 controls were enrolled in the study. Statistically significance was found between OSAS patients and controls at the 12 *** RNFL thickness was found to be significantly lower in last measurements in OSAS patients when compared with first measurements and control subjects(P <0.001, 0.002, respectively). There was a statistically significant correlation among AHI, and RNFL thickness(P <0.05).·CONCLUSION: The results suggest that the patients with OSAS were related with a proportional decrease in the RNFL thickness. These patients should be followed up regularly for glaucomatous changes. Detecting more RNFL thinning in severe OSAS was important.

Effects of storage media,supplements and cryopreservation methods on quality of stem cells

Despite a vast amount of different methods, protocols and cryoprotective agents(CPA), stem cells are often frozen using standard protocols that have beenoptimized for use with cell lines, rather than with stem cells. Relatively fewcomparative studies have been performed to assess the effects of cryopreservationmethods on these stem cells. Dimethyl sulfoxide (DMSO) has been a key agent forthe development of cryobiology and has been used universally for ***, the use of DMSO has been associated with in vitro and in vivotoxicity and has been shown to affect many cellular processes due to changes inDNA methylation and dysregulation of gene expression. Despite studies showingthat DMSO may affect cell characteristics, DMSO remains the CPA of choice, bothin a research setting and in the clinics. However, numerous alternatives to DMSOhave been shown to hold promise for use as a CPA and include albumin,trehalose, sucrose, ethylene glycol, polyethylene glycol and many more. Here, wewill discuss the use, advantages and disadvantages of these CPAs for cryopreservationof different types of stem cells, including hematopoietic stem cells,mesenchymal stromal/stem cells and induced pluripotent stem cells.

Spontaneous resolution of macular edema after silicone oil removal

AIM: To investigate the macular changes in eyes filled with silicone oil(SO) and course of these changes after SO removal. METHODS: A retrospective optical coherence tomography scan review was conducted for twenty-four patients who underwent uncomplicated pars plana vitrectomy with SO tamponade for complex retinal detachments were detected with optical coherence tomography before, and one week, one month and three months after SO ***: Mean duration of SO tamponade was 3.6±1.0mo(range 3-7mo). Cystoid macular edema(CME) was detected in 3 eyes before SO removal. Submacular fluid was represented in 1 eye before silicone SO *** of CME and submacular fluid was achieved1 mo after SO removal in all eyes. Mean best corrected visual acuity(BCVA) was 1.15 ±0.65(range, hand movement to 0.2) before SO removal in the eyes without macular changes. After SO removal, the mean BCVA values at 1wk and 1 and 3mo, and 0.82 ±0.23, 0.76 ±0.21,and 0.70 ±0.19, all of which were significantly better than baseline(P =0.030, 0.017, 0.006 respectively). In the eyes with macular CME and subretinal fluid the mean BCVA was significantly improved at 3mo after SO removal compared with baseline(P =0.037).CONCLUSION: Decreased visual acuity in eyes filled with SO could be caused by macular complications due to SO. CME and subretinal fluid may resolve without any additional macular surgery after SO removal.

Schiff base ligand （HL） derived from 4-hexylaniline with isatin （1H-indole-2,3-dione） and its complexes with Cu（Ⅱ）, Ni（Ⅱ） were prepared and characterized by analytical, spectroscopic （IR, UV-Vis, Mass） techniques, electrical conductivity, magnetic and thermal measurements. The crystal and molecular structure of [Cu（HL）2Cl2] was determined by a single-crystal X-ray diffraction study. The molecular structure of the title compound has an inversion center on the Cu atom.

Effects of hyperbaric oxygen and Pgg-glucan on ischemic colon anastomosis

AIM： In colorectal surgery, anastomotic failure is still a problem in ischemia. Here, we analyzed the effects of hyperbaric oxygen and beta-glucan on colon anastomoses in ischemic condition. METHODS： Colonic resection and anastomosis in rectosigmoid region were done in forty Wistar-Albino rats of four groups of equal number. Colon mesentery was ligated to induce ischemia. The first group was the control group. The subjects of second group were treated with hyperbaric oxygen; the third group with glucan and the forth group were treated with both. At the forth day, rats were sacrificed, anastomotic segment was resected and burst pressures and hydroxyproline levels of anastomotic line were measured. RESULTS： The burst pressure difference of second and third groups from the control group were meaningful （P〈0.01）; the forth group differed significantly from the control （P〈0.001）. There was no difference between the treated groups on burst pressure level （P〉0.05）. The hydroxyproline levels in all treated groups were different from the control group significantly （P〈0.001）. Hydroxyproline levels in the forth group were higher than those of the second and the third groups （P〈 0.001）. There were no significant differences between the second and the fourth groups in burst pressure and hydroxyproline levels （P〉 0.05）. CONCLUSION： Hyperbaric oxygen and glucan improve healing in ischemic colon anastomoses by anti-microbic,immune stimulating properties and seem to act synergistically when combined together.

Analysis of the Electromagnetic Pollution for a Pilot Region in Turkey

In this paper, electromagnetic (EM) pollution (or radiation) measurements in a transmitter region were performed and statistical analysis of values recorded for the EM sources causing pollution was carried out. The actual measurement values and the estimated values by the analysis model obtained through the statistical analysis were compared. EM radiation levels were measured in the districts of Turkish capital Ankara where cellular base stations and TV/Radio stations are densely populated. EM Radiation (EMR) levels were measured for the GSM900, GSM1800, UHF4, VHF4 and VHF5 stations for certain spectrum ranges under far-field conditions by utilizing isotropic field probe and selective spectrum analyzer. The obtained measurement levels were compared with the limit values given by International Commission for Non-Ionizing Radiation Protection (ICNIRP). The results are discussed, regarding both the obtained values that influence the measurements.
