Hepatic osteodystrophy and liver cirrhosis

AIM: To investigate the correlation between hepatic osteodystrophy and osteoporosis in patients with liver cirrhosis. METHODS: Bone mineral density of the patients (n = 55) and that of the control group (n = 30) were measured by dual-energy X-ray absorptiometry. All the women in the study were premenopausal. Deoxypyridinoline, pyridinoline and urinary Ca 2+ were measured as bone destruction markers, while alkaline phosphatase (ALP), osteocalcin and insulin-like growth factor-1 (IGF-1) were measured as bone formation markers. Furthermore, interleukin-1 (IL-1), IL-6, tumor necrosis factor α (TNF-α), vitamin D3, direct bilirubin, albumin, cortisol and parathyroid hormone (PTH) levels were measured. The independent Student t test and χ 2 test were employed in comparing both groups, and the Pearson correlation test was used to determine associations. RESULTS: Comparing cirrhosis and control groups, lumbar total T-score (-1.6 ± 1.2 g/cm 2 vs -0.25 ± 1.3 g/cm 2 , P < 0.001), lumbar total Z-score (-1.2 ± 1.23 g/cm 2 vs -0.6 ± 1.3 g/cm 2 , P < 0.001), total femur T-score (-0.05 ± 1 g/cm 2 vs -0.6 ± 0.9 g/cm 2 , P = 0.003) and total femur Z-score (-0.08 ± 1.5 g/cm 2 vs 0.7 ± 0.9 g/cm 2 , P =0.003) showed significantly lower values in the cirrhosis group. Blood ALP level (109.2 ± 57 U/Lvs 62.6 ± 32.5 U/L, P < 0.001), IL-6 level (27.9 ± 51.6 pg/mL vs 3.3 ± 3.1 pg/mL, P = 0.01), TNF-α level (42.6 ± 33.2 pg/mL vs 25.3 ± 12.3 pg/mL, P = 0.007) and direct bilirubin level (0.9 ± 0.7 mg/dL vs 0.3 ± 0.2 mg/dL, P < 0.001) were significantly higher in the cirrhosis group. IGF-1 level (47.7 ± 26.2 ng/mL vs 143.4 ± 53.2 ng/mL, P < 0.001), osteocalcin level (1.05 ± 2.5 ng/mL vs 7.0 ± 13 ng/mL, P = 0.002) and 24 h urinary Ca 2+ (169.6 ± 227.2 mg/dL vs 287 ± 168.6 mg/dL, P = 0.003) were significantly lower in the cirrhosis group. Hydrogeochemistry and Water Quality Evaluation along the Flow Path in the Unconfined Aquifer of the Düzce Plain, North-western Turkey

The Diizce Plain has a multi-aquifer system, which consists of a near surface unconfined aquifer, along with first and second deeper confined aquifers. Hydrochemical evolution and water quality are related to infiltration of the precipitation, recharge from the formations surrounding the plain, flow path of groundwater and the relationship between surface and groundwater. The groundwater in the unconfined aquifer flows towards the Efteni Lake and the Biiyiik Melen River. Surface waters are divided into two main hydrochemical facies in the study area：（a） Ca2＋-HCO3-; and （b） Ca2＋, Mg2＋-HCOc-, SO4^2-. The groundwater has generally three main hydrochemical facies：（a） Ca2＋-HCO3-; （b） Ca2＋, Mg2＋-HCO3-; and （c） Ca2＋, Mg2＋-HCO3-, Cl-. The hydrochemical facies ＂a＂ and ＂b＂ dominate within shallow depths in recharge areas under rapid flow conditions, while hydrochemical facies ＂c＂ characterizes shallow and mixed groundwater, which dominate intermediate or discharge areas （near Efteni Lake and Biiyiik Melen River） during low flow conditions and agricultural contamination. Calcium and bicarbonate ions, total hardness and electrical conductivity of total dissolved solids （EC-TDS） values increase along the groundwater flow path; but these parameters remain within the limits specified by the standards set for industrial and agricultural usages.

It has been already shown that Parkinson’s disease (PD) is characterized by a prominent degeneration in substantia nigra (SN) neurons. Growing evidence suggests that there is a latent period of PD associated with slight non-motor findings such as olfactory dysfunction (Dickson et al., 2018). However, the potential biomarker role of olfactory dysfunction in PD has been a topic of great interest in the last years (Raskin et al., 1990;Dickson et al., 2018). The classical hypothesis of Braak suggests that PD begins as a synucleinopathy in the lower brainstem or in the olfactory bulb (OB) that progresses rostrally to the SN and amygdala to cause parkinsonism at a later stage of the disease (Burke et al., 2008).

Stump appendicitis is an acute inflammation of the residual appendix and one of the rare complications after appendectomy. Paying attention to the possibility of stump appendicitis in patients with right lower abdominal pain after appendectomy can prevent the delay of diagnosis and treatment. In patients with stump appendicitis, CT scan not only assists in making an accurate preoperative diagnosis but also excludes other etiologies. We report a 47-year old man with preoperatively diagnosed stump appendicitis by CT, who underwent an open appendectomy 20 years ago.

Comparison of brimonidine-timolol and dorzolamidetimolol in the management of intraocular pressure increase after phacoemulsification

AIM: To compare the effectiveness of brimonidine/timolol fixed combination(BTFC) and dorzolamide/timolol fixed combination(DTFC) in the management of short-term intraocular pressure(IOP) increase after phacoemulsification surgery.·METHODS: Eighty eyes of 80 patients undergoing phacoemulsification and intraocular lens(IOL)implantation were randomly assigned into three *** 1 consisted of 28 eyes and represented the control group. Group 2 consisted of 25 eyes undergoing phacoemulsification surgery and BTFC was instilled at the end of surgery. Group 3 consisted of 27 eyes undergoing phacoemulsification surgery and DTFC was instilled at the end of surgery. IOP was measured preoperatively and 6, 24 h and 1wk postoperatively.·RESULTS: There was no statistically significant difference in preoperative baseline IOP among the three groups(P =0.84). However, IOP was significantly lower in groups 2 and 3 compared to the control group(P 25 mm Hg at 6h after surgery(P =0.008). However, IOP decreased and was >25 mm Hg in only one eye in each group at24 h after surgery.·CONCLUSION: BTFC and DTFC have similar effects in reducing increases in IOP after phacoemulsificationsurgery and can both be recommended for preventing IOP spikes after such surgery.

Effects of coal damage on permeability and gas drainage performance

Coal permeability is a measure of the ability for fluids to flow through coal structures. It is one of the most important parameters affecting the gas drainage performance in underground coal mines. Despite the extensive research conducted on coal permeability, few studies have considered the effect of coal damage on permeability. This has resulted in unreliable permeability evaluation and prediction. The aim of this study is to investigate the effect of coal damage on permeability and gas drainage performance. The Cui-Bustin permeability model was improved by taking into account the impact of coal damage on permeability. The key damage coefficient of the improved permeability model is determined based on the published permeability data. A finite-element numerical simulation was then developed based on the improved permeability model to investigate the damage areas and the permeability distribution around roadway. Results showed that the tensile failure occurs mainly on the upper and lower sides of the roadway while the shear failure symmetrically occurs on the left and right sides. With the increase in the friction angle value, the damage area becomes small. A good agreement was obtained between the results of the improved permeability model(c = 3) and the published permeability data. This indicated a more accurate permeability prediction by the improved permeability model. It is expected that the findings of this study could provide guidance for in-seam gas drainage borehole design and sealing, in order to enhance the gas drainage performance and reduce gas emissions into underground roadways.

Optimized Numerical Model Based Assessment of Wave Power Potential of Marmara Sea

Marmara Sea, located between Black Sea and Aegean Sea, is an important sea for ocean engineering activities. In this study, wave power potential of Marmara Sea was investigated using the third generation spectral wind-wave model MIKE 21 SW with unstructured mesh. Wind data was obtained from ECMWF ERA-Interim re-analyses wind dataset at 10 m with a spatial resolution of 0.1? for the period of 1994 to 2014. The numerical model was calibrated with measured wave data from a buoy station located in Marmara Sea. Mesh optimization was also performed to obtain the most suitable mesh structure for the study area. This study is the first that dealt with the determination of wave energy potential of Marmara Sea. The numerical model results are presented in terms of monthly, seasonal and annual average of wave power flux(k W m^(-1)). The maximum wave power flux is 1.13 kW m^(-1) and occurs in November. The overall annual mean wave power flux during 1994–2014 is found to be 0.27 kW m^(-1) in the offshore regions.

Epididymal changes associated with cryptorchidism in rats

Aim: To investigate the effect of a new mechanically induced cryptorchidism model on rat epididymal ***: Eighteen 21-day-old Sprague Dawley rats were randomly divided into 3 groups of 6 animals each. Ani-mals of Groups 1 and 2 were made unilaterally cryptorchid by closing the left inguinal canal, while Group 3 (controls),sham operated. At Day 60 (Group 1) and Day 90 (Group 2 and 3), the rats were sacrificed. The epididymides of bothsides were weighed, and the diameter of the epididymal tubules, the height of the tubular epithelium and the presenceof fibrosis of the caput epididymides were examined under a light-microscope. Results: The cryptorchid epididymishad a significantly lower weight increase than the contralateral ones and the controls, but the epithelial height and diam-eter of caput epididymal tubules were not significantly different. Fibrosis was not observed. Conclusion; The au-thors proposed that this new model is a convenient method to evaluate the epididymal changes associated with experi-mental cryptorchidism.
