Non-alcoholic fatty liver disease and metabolic syndrome in obese children

I read with great interest the article of Fu et al who investigated whether non-alcoholic fatty liver disease(NAFLD)is an early mediator for prediction of metabolic syndrome,and whether liver B-ultrasound could be used for its diagnosis,in a study involving 861 obese children(6-16 years old).In this study,it was reported that NAFLD is not only a liver disease,but also an early mediator that reflects metabolic disorder,and that liver B-ultrasound can be a useful tool for metabolic syndrome(MS)*** authors reported that NAFLD and MS were present in 68.18%and 25.67%of obese children,***,they observed that the prevalence of MS in NAFLD children was 37.64%,which was much higher than that in the non-NAFLD *** analogous to those of the Adult Treatment PanelⅢdefinition for MS were used for children in this *** reported prevalence data on MS in the young has varied markedly,in large part because of disagreement among the variously proposed definitions of ***,in my opinion,a study aiming to assess the association between MS components and NAFLD in obese children has to take into account a simple,easy-to-apply clinical definition proposed by the international diabetes federation for *** of the results of the Fu et al study are limited byanother major caveat:that the diagnosis or exclusion of NAFLD was based on liver enzymes and ultrasound imaging,but was not confirmed by liver ***,it is known that liver enzymes may be within the reference interval in up to 70%of patients with diagnosed NAFLD and that the full histopathological spectrum of NAFLD may be present in patients with normal liver enzymes,which therefore cannot be reliably used to exclude the presence of NAFLD.

Robust Backstepping Control of a Quadrotor Unmanned Aerial Vehicle under Colored Noises

Advances in software and hardware technologies have facilitated the production of quadrotor unmanned aerial vehicles(UAVs).Nowadays,people actively use quadrotor UAVs in essential missions such as search and rescue,counter-terrorism,firefighting,surveillance,and cargo *** performing these tasks,quadrotors must operate in noisy ***,a robust controller design that can control the altitude and attitude of the quadrotor in noisy environments is of great *** researchers have focused only on white Gaussian noise in their studies,whereas researchers need to consider the effects of all colored noises during the operation of the *** study aims to design a robust controller that is resistant to all colored ***,a nonlinear quadrotormodel was created with ***,a backstepping controller resistant to colored noises was *** backstepping controller was tested under Gaussian white,pink,brown,blue,and purple *** and Lyapunov-based controller designswere also carried out,and their time responses(rise time,overshoot,settling time)were compared with those of the backstepping *** the simulations,time was in seconds,altitude was in meters,and roll,pitch,and yaw references were in *** and settling time values were in seconds,and overshoot value was in *** the obtained values are examined,simulations prove that the proposed backstepping controller has the least overshoot and the shortest settling time under all noise types.

Non-Thermal Atmospheric Plasma:Can it Be Taken as a Common Solution for the Surface Treatment of Dental Materials?

This study aimed to evaluate the surface roughness and wetting properties of various dental prosthetic materials after different durations of non-thermal atmospheric plasma（NTAP）*** hundred and sixty discs of titanium（Ti）（n：40）,cobalt chromium（Co-Cr）（n：40）,yttrium stabilized tetragonal zirconia polycrystals（Y-TZP）（n：40）and polymethylmethacrylate（PMMA）（n：40）materials were machined and smoothed with silicon carbide *** surface roughness was evaluated in a control group and in groups with different plasma exposure times [1-3-5 s].The average surface roughness（Ra）and contact angle（CA）measurements were recorded via an atomic force microscope（AFM）and tensiometer,*** changes were examined with a scanning electron microscope（SEM）.Data were analyzed with two-way analysis of variance（ANOVA）and the Tukey HSD test α=0.05）.According to the results,the NTAP surface treatment significantly affected the roughness and wettability properties（P 〈 0.05）.SEM images reveal that more grooves were present in the NTAP *** an increase in the NTAP application time,an apparent increment was observed for Ra,except in the PMMA group,and a remarkable reduction in CA was observed in all *** is concluded that the NTAP technology could enhance the roughening and wetting performance of various dental materials.

Petrogenesis of the late Cretaceous Turnagol intrusion in the eastern Pontides:Implications for magma genesis in the arc setting

A series of Cretaceous plutons is present in the eastern Pontides of northeastern Turkey. The Turnag61 intrusion is the least studied and, thus, the least understood plutons in the orogen. This intrusion consists of hornblende-biotite granodiorites emplaced at 78 Ma based on LA-ICP-MS U-Pb zircon dating. It is of sub- alkaline affinity, belongs to the medium- to high-K calc-alkaline series, and displays features typical of I- type granites. The rocks of the intrusion are enriched in large-ion lithophile elements and light rare earth- elements with negative Eu anomalies （EuJEu＊ = 0.69-0.82）, but are deficient in high-field-strength elements. They have a small range of （87Sr/S6Sr）i （0.7060 0.7063）, ENd （-2.6 to 3.1）, and 6180 （＋8.1 to ＋9.1） values. Their Pb isotopic ratios are 206pb/204pb = 18.63-18.65, ＇207Tpb/204pb = 15.62-15.63, and 208pb/204pb 38.53-38.55. The fractionation of plagioclase, hornblende, and Fe-Ti oxides had key functions in the evolution of the Turnag61 intrusion. The crystallization temperatures of the melts ranged from 758 to 885 ℃ as determined by zircon and apatite saturation thermometry. All these characteristics, combined with the low values of K20/Na20 and （Na20 ＋ K20）/（FeOt ＋ MgO ＋ TiO2）, as well as the high values of （CaO ＋ FeOt ＋ MgO ＋ TiO2）, suggest an origin by dehydration melting from a metabasaltic lower crustal source.

Protective effect of low dose of melatonin against cholestatic oxidative stress after common bile duct ligation in rats

AIM: To investigate the role of oxidative injury and the effect of exogenous melatonin administration on liver damage induced by bile duct ligation (BDL), and second, to evaluate the role of nitric oxide (NO), a free oxygen radical, in oxidative injury. METHODS: Thirty-two Sprague-Dawley rats were assigned to four groups: sham operation (SO), BDL, BDL+melatonin, and BDL+vehicle. Cholestasis was achieved by double ligature of the common bile duct. Melatonin was injected intraperitoneally 500 μg/(kg·d) for 8 d. Hepatic oxidative stress markers were evaluated by changes in the amount of lipid peroxides, measured as malondialdehyde (MDA), and reduced GSH. Total nitrite (NOx) concentrations were determined in hepatic homogenates. Histopathological examination was performed using a histological scoring system. RESULTS: The histopathological changes including portal inflammation, necrosis,apoptosis, focal inflammation and fibrosis were severe in the BDL and BDL+vehicle groups. There were numerous large areas of coagulation necrosis. Histological Activity Index scores of these groups were significantly higher than that of the SO group. Treatment with melatonin reduced these alterations significantly. The degree of necro-inflammation and fibrosis showed significant difference between the BDL and BDL+melatonin groups. BDL was accompanied by a significant increase in MDA and NOx, and a significant decrease in GSH levels. Mean±SE values of MDA, GSH and NOx levels of SO group were 147.47±6.69, 0.88±0.33 μmol/g and 180.70±6.58 nm/g, respectively. The values of BDL group were 200.14±21.30, 0.65±0.02 μmol/g, and 400.46±48.89 nm/g, respectively, whereas the values of BDL+melatonin group were 115.93±6.8,0.74±0.02 μmol/g, and 290.38±32.32 nm/g, respectively. Melatonin treatment was associated with a significant recovery of MDA, GSH and NOx levels. Open reduction of a total talar dislocation: A case report and review of the literature

BACKGROUND Total talar dislocation (TTD) is very uncommon for many orthopedic surgeons and emergency/trauma specialists. Scarce cases of TTD have been reported, mainly in the form of open fracture-dislocation injury. CASE SUMMARY We report a very rare injury of closed TTD with a follow-up period of 36 mo. Initial closed reduction was not successful because of a fractured highly unstable medial malleolus displaced into the ankle mortise, blocking the relocation of the talus. The patient was able to walk pain-free after the 3rd month of surgery. At the 36-mo follow-up, there were 10 degrees of flexion loss and 10 degrees of extension loss in the tibiotalar joint. Furthermore, 5 degrees of subtalar joint inversion-eversion loss was present. CONCLUSION Open reduction should be performed for closed TTDs unless closed reduction is successful.

An overlooked effect of systemic anticholinergics:alteration on accommodation amplitude

AIM： To investigate the effect of oral solifenacin succinate, tolterodine-L-tartarate and oxybutinin hydrochloride（HCl） on accommodation amplitude.·METHODS： Female overactive bladder syndrome（OAB）patients who were planned to use oral anticholinergics,patients that uses solifenacin succinate 5 mg（Group I,n =25）, tolterodine-L-tartarate 4 mg（Group II, n =25）,and oxybutinin HCl 5 mg b.i.d.（Group III, n =25） and age matched healthy female subjects（Group IV, n =25） were recruited and complete ophthalmological examination and accommodation amplitude assessment were done at baseline and 4wk after initiation of treatment.·RESULTS： The mean age of 100 consecutive female subjects was 51.6 ±5.7（40-60）y and there were no statistically significant difference with regard to the mean age（P =0.107） and baseline accommodation amplitude（P =0.148） between study groups. All treatment groups showed a significant decrease in accommodation amplitude following a 4-week course of anticholinergic treatment（P =0.008 in Group I, P =0.002 in Group II, P =0.001 in Group III）, but there was no statistically significant difference in Group IV（P =0.065）.·CONCLUSION： A 4-week course of oral anticholinergic treatment have statistically significant effect on accommodation amplitude. Clinicians should avoid both overestimating this result, as this would unnecessarily restrict therapeutic possibilities, and also underestimating it which may lead to drug intolerance.
