Prevalence and risk factors of gastroesophageal reflux disease in Qashqai migrating nomads, southern Iran

AIM: To investigate the prevalence and risk factors of gastroesophageal re? ux disease (GERD) symptoms in Qashqai migrating nomads with a different life style in Fars province, southern Iran. METHODS: In summer 2006, 748 Qashqai migrating nomads aged 25 years or more were enrolled using a multiple-stage stratified cluster random sampling method. A questionnaire consisting of demographic characteristics, lifestyle and GERD symptoms (heartburn, regurgitation, chest pain, dysphagia, hoarseness and cough) as completed for each subject. RESULTS: The questionnaire was completed in 717 subjects. The prevalence rate of GERD, defined as reflux occurring at least one time per week in the preceding year, was 33% (237 subjects). The prevalence was higher in older individuals (36.0% vs 28.9%, P < 0.05) and in those with other gastrointestinal complaints (51.0% vs 27.8%, P < 0.001), but not different in obese and non-obese subjects. It was also higher in those consuming fruitsand vegetables more than once a week (36.2% vs 17.3%, P < 0.001). GERD had a positive correlation with smoking (42.1% vs 27.8%, P < 0.001), but a negative relation with non-alcoholic beverages. The association between GERD and non-steroidal antiinflammatory drugs (NSAIDs) consumption was also signifi cant (40.2% vs 25.4%, P < 0.001). CONCLUSION: The prevalence of GERD (33%) is very high in Qashqai migrating nomads which may be due to a lower socioeconomic and educational level of these people and difference in the life style. Older age, frequent consumption of fruits and vegetables, smoking and NSAIDs are risk factors for GERD in this population.

Halothane hepatitis in Iran:A review of 59 cases

AIM:To study halothane hepatitis (HH) in Iran and its associated risk factors. METHODS: We retrospectively studied files of all cases diagnosed with HH referred to three referral hospitals and four private centers in Iran from April 1994 to September 2006. Information on age at surgery, gender, medications history, obesity, history of previous exposure, previous reaction to halothane, familial history, type of surgery, perioperative hypoxia or sepsis, morbidity and mortality were recorded and analyzed. RESULTS: A total of 59 cases were identifi ed. Forty- eight (81%) were women. The median age at the time of surgery was 44 years (range, 18 to 80 years). Sixty percent of patients were above 40-year-old. Obesity was observed in 22.2%. Previous history of exposures to halothane was noted in 61% of which 50% had history of post-exposure reaction. Coronary artery bypass graft (CABG), cholecystectomy, and cosmetic surgeries (mainly weight reduction) were the most frequent surgeries. The mortality rate was 12.2%. In patients developing encephalopathy, it was as high as 50%. CONCLUSION: HH remains an important cause of morbidity and mortality in centers still using this anesthetic. However, a large percentage of these casescould have been avoided. To lessen occurrence of further cases of HH, the authors suggest that in female patients having a history of surgery (or delivery) with general anesthesia, the use of halothane should be absolutely avoided. Utilization of proper substitutes in adults’ anesthesia is advocated.

AIM: To determine the prevalence and symptoms of gastroesophageal reflux disease (GERD) in a healthy general population in relation to demographic,lifestyle and health-seeking behaviors in Shiraz,southern Iran. METHODS: A total of 1978 subjects aged > 35 years who referred to Gastroenterohepatology Research Center and who completed a questionnaire consisting of 27 questions for GERD in relation to demographic,lifestyle and health-seeking behaviors were included in this study for a period of five months. The validity and reliability of the questionnaire were determined. RESULTS: The prevalence of GERD was 15.4%,which was higher in females (17.3%),in rural areas (19.8%),and in illiterate subjects (21.5%) and those with a mean age of 50.25 years. The prevalence was significantly lower in subjects having fried food (14.8%),and fruit and vegetables (14.6%). More symptoms were noticed in subjects consuming pickles (22.1%),taking aspirin (21%) and in subjects with psychological distresses (27.2%) and headaches (22%). The correlation was statistically significant between GERD and halitosis (18.3%),dyspepsia (30.6%),anxiety (19.5%),nightmares (23.9%) and restlessness (18.5%). Their health seeking behavior showed that there was a significant restriction of diet (20%),consumption of herbal medicine (19%),using over-the-counter drugs (29.9%) and consulting with physicians (24.8%). Presence of GERD symptoms was also significantly related to a previous family history of the disease (22.3%).CONCLUSION: GERD is more common in females,rural and illiterate subjects and correlated with consumption of pickles,occurrence of headache,psychological distress,dyspepsia,halitosis,anxiety,nightmare and restlessness,and a family history of GERD and aspirin intake,but the correlation was negative with consumption of fat and fiber intake.

AIM:To evaluate joint effects of Methylentetra-hydrofolate reductase(MTHFR) C677T genotypes,and serum folate/vitamin B12 concentrations on promoter methylation of tumor-associated genes among Iranian colorectal cancer patients. METHODS:We examined the associations between MTHFR C677T genotype,and promoter methylation of P16,hMLH1,and hMSH2 tumor-related genes among151 sporadic colorectal cancer patients. The promoter methylation of tumor-related genes was determined by methylation-specific PCR. Eighty six patients from whom fresh tumor samples were obtained and 81 controls were also examined for serum folate and vitamin B12 concentrations by a commercial radioimmunoassay kit. RESULTS:We found 29.1% of cases had tumors with at least one methylated gene promoter. In case-case comparison,we did not find a significant association between methylation in tumors and any single genotype. However,in comparison to controls with the CC genotype,an increased risk of tumor methylation was associated with the CT genotype(OR = 2.5;95% CI,1.1-5.6) . In case-case comparisons,folate/vitamin B12 levels were positively associated with tumor methylation. Adjusted odds ratios for tumor methylation in cases with high(above median) versus low(below median) serum folate/vitamin B12 levels were 4.9(95% CI,1.4-17.7) ,and 3.9(95% CI,1.1-13.9) ,respectively. The frequency of methylated tumors was significantly higher in high methyl donor than low methyl donor group,especially in those with MTHFR CT(P = 0.01) ,and CT/TT(P = 0.002) genotypes,but not in those with the CC genotype(P = 1.0) . CONCLUSION:We conclude that high concentrations of serum folate/vitamin B12 levels are associated with the risk of promoter methylation in tumor-specific genes,and this relationship is modified by MTHFR C677T genotypes.

Sustainable agriculture plays a crucial role in meeting the growing global demand for food while minimizing adverse environmental impacts from the overuse of synthetic pesticides and conventional fertilizers.In this context,renewable biopolymers being more sustainable offer a viable solution to improve agricultural sustainability and production.Nano/micro-structural supramolecular biopolymers are among these innovative biopolymers that are much sought after for their unique features.These biomaterials have complex hierarchical structures,great stability,adjustable mechanical strength,stimuli-responsiveness,and self-healing attributes.Functional molecules may be added to their flexible structure,for enabling novel agricultural uses.This overview scrutinizes how nano/micro-structural supramolecular biopolymers may radically alter farming practices and solve lingering problems in agricultural sector namely improve agricultural production,soil health,and resource efficiency.Controlled bioactive ingredient released from biopolymers allows the tailored administration of agrochemicals,bioactive agents,and biostimulators as they enhance nutrient absorption,moisture retention,and root growth.Nano/micro-structural supramolecular biopolymers may protect crops by appending antimicrobials and biosensing entities while their eco-friendliness supports sustainable agriculture.Despite their potential,further studies are warranted to understand and optimize their usage in agricultural domain.This effort seeks to bridge the knowledge gap by investigating their applications,challenges,and future prospects in the agricultural sector.Through experimental investigations and theoretical modeling,this overview aims to provide valuable insights into the practical implementation and optimization of supramolecular biopolymers in sustainable agriculture,ultimately contributing to the development of innovative and eco-friendly solutions to enhance agricultural productivity while minimizing environmental impact.

Data-Based Filters for Non-Gaussian Dynamic Systems With Unknown Output Noise Covariance

This paper proposes linear and nonlinear filters for a non-Gaussian dynamic system with an unknown nominal covariance of the output noise.The challenge of designing a suitable filter in the presence of an unknown covariance matrix is addressed by focusing on the output data set of the system.Considering that data generated from a Gaussian distribution exhibit ellipsoidal scattering,we first propose the weighted sum of norms(SON)clustering method that prioritizes nearby points,reduces distant point influence,and lowers computational cost.Then,by introducing the weighted maximum likelihood,we propose a semi-definite program(SDP)to detect outliers and reduce their impacts on each cluster.Detecting these weights paves the way to obtain an appropriate covariance of the output noise.Next,two filtering approaches are presented:a cluster-based robust linear filter using the maximum a posterior(MAP)estimation and a clusterbased robust nonlinear filter assuming that output noise distribution stems from some Gaussian noise resources according to the ellipsoidal clusters.At last,simulation results demonstrate the effectiveness of our proposed filtering approaches.

Reinforcement Learning-Empowered Graph Convolutional Network Framework for Data Integrity Attack Detection in Cyber-Physical Systems

The massive integration of communication and information technology with the large-scale power grid has enhanced the efficiency, safety, and economical operation of cyber-physical systems. However, the open and diversified communication environment of the smart grid is exposed to cyber-attacks. Data integrity attacks that can bypass conventional security techniques have been considered critical threats to the operation of the grid. Current detection techniques cannot learn the dynamic and heterogeneous characteristics of the smart grid and are unable to deal with non-euclidean data types. To address the issue, we propose a novel Deep-Q-Network scheme empowered with a graph convolutional network (GCN) framework to detect data integrity attacks in cyber-physical systems. The simulation results show that the proposed framework is scalable and achieves higher detection accuracy, unlike other benchmark techniques.

A novel mitigation measure for normal fault-induced deformations on pile-raft systems

Evidence from recent earthquakes has shown destructive consequences of fault-induced permanent ground movement on structures.Such observations have increased the demand for improvements in the design of structures that are dramatically vulnerable to surface fault ruptures.In this study a novel connection between the raft and the piles is proposed to mitigate the hazards associated with a normal fault on pile-raft systems by means of 3D finite element(FE)modeling.Before embarking on the parametric study,the strain-softening constitutive law used for numerical modeling of the sand has been validated against centrifuge test results.The exact location of the fix-head and unconnected pile-raft systems relative to the outcropping fault rupture in the free-field is parametrically investigated,revealing different failure mechanisms.The performance of the proposed connection for protecting the pile-raft system against normal fault-induced deformations is assessed by comparing the geotechnical and structural responses of both types of foundation.The results indicate that the pocket connection can relatively reduce the cap rotation and horizontal and vertical displacements of the raft in most scenarios.The proposed connection decreases the bending moment response of the piles to their bending moment capacity,verging on a fault offset of 0.6 m at bedrock.

On the occurrence of Neoplanorbulinella Matsumaru, 1976(Foraminifera) from the late Oligocene of Central Iran(Qom Formation):Palaeobiogeographic implications

The shallow marine carbonates of the upper Oligocene Qom Formation yielded several occurrences of the foraminiferan genus Neoplanorbulinella Matsumaru. Neoplanorbulinella saipanensis Matsumaru has so far been recorded from the late Eocene-early Miocene of Japan and late Oligocene of NE Italy whereas N. malatyaensis Gedik is only reported from the Oligocene from its type locality in the Malatya Basin, eastern Turkey. The new records reported here from the Qom Formation indicate that both species occur in the Dobaradar section, ca. 10 km south of the city of Qom in North-Central Iran. The palaeogeographic distribution of these two species therefore has to be extended as far as the palaeolongitude of current Central Iran.The accompanying larger benthic foraminifers, including Miogypsinoides complanatus(Schlumberger),M. formosensis Yabe and Hanzawa, Spiroclypeus margaritatus(Schlumberger), Operculina complanata(Defrance), and Risananeiza pustulosa Boukhary et al., indicate the upper Chattian SBZ 23 Zone. The coexistence of the N. saipanensis and N. malatyaensis points to suitable palaeobiogeographic conditions of Central Iran to host Western and Eastern Tethyan taxa.
