Comparison of sedative effectiveness of thiopental versus midazolam in reduction of shoulder dislocation

BACKGROUND: Various sedative drugs have been proposed to control anxiety and agitation in shoulder dislocation, but none of them has been diagnosed as the best sedative and relaxant agent. The study aimed to compare the sedative effectiveness of thiopental versus midazolam in reduction of shoulder ***: A randomized double-blind controlled trail was performed in 80 patients with shoulder dislocation recruited from the emergency department. Ten patients were excluded and 70 patients were enrolled in the study. Case group received intravenous thiopental 2 mg/kg+2 μg/kg fentanyl while control group received intravenous midazolam 0.1 mg/kg+2 μg/kg fentanyl. Number of times, patients and physician's satisfaction, difficulty of procedure, degree of muscle relaxation, time of sedation and complete recovery, number of patients with apnea episode, O2 saturation, patient's pain score and adverse events were all ***: Muscular tone had significant difference between the two groups(P-value=0.014) and thiopental was more muscle relaxant than midazolam. Replacement of shoulder dislocation in thiopental group was easier than midazolam group(P-value=0.043). There was no need to use multiple methods of reduction in either group. Before drug infusion the mean±SD VAS scores were 8.37±2.21 in the midazolam group(A) and 8.94±1.78 in the thiopental group(B); mean difference 0.57, 95% CI= –0.38 to 1.52. After completion of the procedure, the mean±SD VAS scores in group(A) and(B) were 3.20±1.30 vs. 3.65±1.30; mean difference –0.45, 95% CI= –1.07 to ***: Thiopental might be more effective and relaxant than midazolam for reduction of shoulder dislocation.

Evaluation of factors affecting psychological morbidity in emergency medicine practitioners

BACKGROUND: Assessing and evaluating mental health status can provide educational planners valuable information to predict the quality of physicians' performance at work. These data can help physicians to practice in the most desired way. The study aimed to evaluate factors affecting psychological morbidity in Iranian emergency medicine practitioners at educational hospitals of ***: In this cross sectional study 204 participants(emergency medicine residents and specialists) from educational hospitals of Tehran were recruited and their psychological morbidity was assessed by using a 28-question Goldberg General Health Questionnaire(GHQ-28). Somatization, anxiety and sleep disorders, social dysfunction and depression were evaluated among practitioners and compared to demographic and job related ***: Two hundreds and four participants consisting of 146(71.6%) males and 58(28.4%) females were evaluated. Of all participants, 55(27%) were single and 149(73%) were married. Most of our participants(40.2%) were between 30–35 years old. By using GHQ-28, 129(63.2%) were recognized as normal and 75(36.8%) suffered some mental health disorders. There was a signifi cant gender difference between normal practitioners and practitioners with disorder(P=0.02) while marital status had no significant difference(P=0.2). Only 19(9.3%) declared having some major mental health issue in the previous ***: Females encountered more mental health disorders than male(P=0.02) and the most common disorder observed was somatization(P=0.006).

BACKGROUND:Numerous drugs have been proposed to alleviate pain in patients with biliary colic,especially opioids,but still there is a tendency to use less narcotics because of their side effects and the unwillingness of some *** present study aimed to compare the analgesic effect of paracetamol combined with low-dose morphine versus morphine alone in patients with biliary ***:A randomized double-blind controlled trial was performed in 98 patients with biliary colic,recruited from two emergency departments from August 2012 to August *** patients were excluded and the remaining were randomized into two groups:group A received 0.05mg/kg morphine+1 000 mg paracetamol in 100 m L normal saline and group B received 0.1 mg/kg morphine+normal saline(100 m L)as *** scores were recorded using visual analogue scale(VAS)at baseline and 15 and 30 minutes after drug *** effects and the need for rescue medication(0.75 g/kg intravenous fentanyl)were also reported within 60 minutes of drug ***:Before the infusion,the mean±SD VAS scores were 8.73±1.57 in group A and8.53±1.99 in group *** 15 minutes after drug administration,the mean±SD VAS scores were2.16±1.90 in group A vs.2.51±1.86 in group B;mean difference was–0.35,and 95%CI–1.15 to 0.45(P=0.38).At 30 minutes the mean±SD VAS scores were 1.66±1.59 in group A vs.2.14±1.79 in group B;mean difference was–0.48,and 95%CI–1.20 to 0.24(P=0.19).The mean pain scores in the two groups at 15 and 30 minutes demonstrated no significant ***:Paracetamol combined with low-dose morphine may be effective for pain management in patients with biliary colic.

Generating one-,two-,three-and four-scroll attractors from a novel four-dimensional smooth autonomous chaotic system

A new four-dimensional quadratic smooth autonomous chaotic system is presented in this paper, which can exhibit periodic orbit and chaos under the conditions on the system parameters. Importantly, the system can generate one-, two-, three- and four-scroll chaotic attractors with appropriate choices of parameters. Interestingly, all the attractors are generated only by changing a single parameter. The dynamic analysis approach in the paper involves time series, phase portraits, Poincare maps, a bifurcation diagram, and Lyapunov exponents, to investigate some basic dynamical behaviours of the proposed four-dimensional system.

Dye-sensitized solar cells based on Cr-doped TiO2 nanotube photoanodes

The effect of chromium doping on the photo- voltaic efficiency of dye-sensitized solar cells （DSSCs） with anodized TiO2 nanotubes followed by an annealing process was investigated. Cr-doped TiO2 nanotubes （CrTNs） with different amounts of chromium were obtained by anodizing of titanium foils in a single-step process using potassium chro- mate as the chromium source. Film features were investigated by scanning electron microscopy （SEM）, X-ray diffraction （XRD）, energy-dispersive X-ray spectroscopy （EDX）, and ultraviolet-visible （UV-Vis） spectroscopy. It is clearly seen that highly ordered TiO2 nanotubes are formed in an anodizing solution free of potassium chromate, and with a gradual increase in the potassium chromate concentration, these nanotube structures change to nanoporous and compact films without porosity. The photovoltaic efficiencies of fabricated DSSCs were characterized by a solar cell measurement sys- tem via the photocurrent-voltage （l-V） curves. It is found that the photovoltaic efficiency of DSSCs with CrTNsl sample is improved by more than three times compared to that of DSSCs with undoped TNs. The energy conversion efficiency increases from 1.05 % to 3.89 % by doping of chromium.

Effect of Hot Working on Secondary Phase Formation in 2205 Duplex Stainless Steel

Repetitive Thermomechanical Processing towards Ultra Fine Grain Structure in 301,304 and 304L Stainless Steels

Thermomechanical processing as a combination of cold rolling and annealing was performed on austenitic stainless steels 301,304 and 304L. Two cold rolling steps each one up to a reduction of 75% were combined with an intermediate annealing at 800℃ for 20 min. The final annealing was performed *** same temperature and time. Cold rolling contributed to martensite formation at the expense of metastable austenite in the studied materials. Austenite in 301 was found to be less stable than that in 304 and 304L. Hence, higher strength characteristics in the as-quenched 301 stainless steels were attributed to the higher volume fraction of martensite. Both α′-martensite and ε-martensite were found to form as induced by deformation. However, the intensity of ε-martensite increased as the stability of austenite decreased. Annealing after cold rolling led to the reversion of austenite with an ultra fine grained structure in the order of 0.5-1 μm from the strain induced martensite. The final grain size was found to be an inverse function of the amount of strain induced martensite. The thermomechanical processing considerably improved the strength characteristics while the simultaneous decrease of elongation was rather low.

Universality of a Critical Magnetic Field in a Holographic Superconductor

We study aspects of holographic superconductors analytically in the presence of a constant external magnetic field. It is shown that the critical temperature and the critical magnetic field can be calculated at nonzero temperature. We detect the Meissner effect in such superconductors. A universal relation between black hole mass and critical magnetic field Hc is proposed to be Hc/M2/3≤0.687365. We also discuss some aspects of phase transition in terms of black hole entropy and Bekenstein＇s entropy to the energy upper bound.

Hot deformation behavior of microstructural constituents in a duplex stainless steel during high-temperature straining

The hot deformation characteristics of 1.4462 duplex stainless steel （DSS） were analyzed by considering strain partitioning between austenite and ferrite constituents. The individual behavior of ferrite and austenite in microstructure was studied in an iso-stress condition. Hot compression tests were performed at temperatures of 800-1100~C and strain rates of 0.001-1 s-1. The flow stress was modeled by a hyperbolic sine constitutive equation, the corresponding constants and apparent activation energies were determined for the studied alloys. The constitutive equation and law of mixture were used to measure the contribution factor of each phase at any given strain. It is found that the contribution factor of ferrite exponentially declines as the Zener-HoUomon parameter （Z） increases. On the contrary, the austenite contribution polynomially increases with the increase of Z. At low Z values below 2.6. x 1015 （lnZ---35.5）, a negative contribution factor is determined for austenite that is attributed to dynamic recrystallization. At high Z values, the contribution factor of austenite is about two orders of magnitude greater than that of ferrite, and therefore, austenite can accommodate more strain. Microstructural characterization via electron back-scattered diffraction （EBSD） confirms the mechanical results and shows that austenite recrystallization is possible only at high temperature and low strain rate.
