Use of shear wave elastography for the diagnosis and follow-up of biliary atresia: A meta-analysis

BACKGROUND Timely differentiation of biliary atresia(BA)from other infantile cholestatic diseases can impact patient outcomes.Additionally,non-invasive staging of fibrosis after Kasai hepatoportoenterostomy has not been widely standardized.Shear wave elastography is an ultrasound modality that detects changes in tissue stiffness.The authors propose that the utility of elastography in BA can be elucidated through meta-analysis of existing studies.AIM To assess the utility of elastography in:(1)BA diagnosis,and(2)post-Kasai fibrosis surveillance.METHODS A literature search identified articles that evaluated elastography for BA diagnosis and for post-Kasai follow-up.Twenty studies met criteria for meta-analysis:Eleven for diagnosis and nine for follow-up post-Kasai.Estimated diagnostic odds ratio(DOR),sensitivity,and specificity of elastography were calculated through a random-effects model using Meta-DiSc software.RESULTS Mean liver stiffness in BA infants at diagnosis was significantly higher than in non-BA,with overall DOR 24.61,sensitivity 83%,and specificity 79%.Post-Kasai,mean liver stiffness was significantly higher in BA patients with varices than in patients without,with DOR 16.36,sensitivity 85%,and specificity 76%.Elastography differentiated stage F4 fibrosis from F0-F3 with DOR of 70.03,sensitivity 96%,and specificity 89%.Elastography also differentiated F3-F4 fibrosis from F0-F2 with DOR of 24.68,sensitivity 85%,and specificity 81%.CONCLUSION Elastography has potential as a non-invasive modality for BA diagnosis and surveillance post-Kasai.This paper’s limitations include inter-study method heterogeneity and small sample sizes.Future,standardized,multi-center studies are recommended.

Insight into the Origin of Iron Ore Based on Elemental Contents of Magnetite and Whole-Rock Geochemistry:A Case of the Bipindi Banded Iron Formations,Nyong Complex,SW Cameroon

The Bipindi iron ore district is located in the central section of the Nyong Complex at the northwestern margin of the Congo Craton in Southwest Cameroon.This iron district contains numerous iron mineralization hosted by the Mewongo,Bibole,Kouambo,and Zambi banded iron formations(BIFs).These BIFs contain magnetite as the main iron ore mineral associated with pyrite,and gangue minerals are quartz with minor chlorite and amphibole.The origin of iron ore from these BIFs was investigated using a combination of in-situ magnetite and whole-rock chemistry.The studied BIF ore samples have a narrow range of TFe between 30.90 wt.%and 43.20 wt.%,indicating a low-grade ore.The geochemical signatures of magnetite such as low contents of base metals(e.g.,Cu,Co,V,and Zn)and low Co/Zn ratios600 and>75,respectively),we suggest that magnetite was derived from a mixture of seawater and~0.1%low-temperature hydrothermal fluids in an oxidizing environment.Collectively,low-temperature hydrothermal and later metamorphic fluids were necessary for the transformation of the protolith Nyong Complex BIFs to iron ore.

Results of an Eight Year Long Practice of Saline Bipolar Transurethral Resection of Prostate

Background: Open prostatectomy has long been the only prostatic bladder outlet obstruction’s surgery available in Benin. It is prone to postoperative bleeding and infections and is not suitable for prostate cancer-induced bladder outlet obstruction. Objective: To evaluate the first results of endoscopic surgery of prostatic bladder outlet obstruction in our environment. Patients and Method: We conducted an observational study of saline bipolar transurethral resection of prostate (B-TURP) at the former Military Teaching Hospital of Cotonou. We retrospectively collected the data from patients’ medical records from November 17, 2014, to September 7, 2022. We used Excel 2019 to make a descriptive analysis of the data. Results: Saline B-TURP was performed in 60 consecutive patients. Their mean age was 69.1 years (range: 48-85). The procedure was indicated in 22 (36.7%), 32 (53.3%), 3 (5%), and 3 (5%) patients, respectively for acute urinary retention, chronic urinary retention, obstructive renal failure, and unresponsiveness to alpha-blockers. 35 patients (58.3%) had prostate cancer (PCa). 25 patients (41.7) had benign prostate hyperplasia (BPH). No patient got a blood transfusion. The duration of postoperative hospitalization was 3 days (88.3%) to 5 days (11.7%). 5 patients (8.3%), i.e., 3 BPH and 2 prostate cancer patients got preoperative urinary tract infection. The causative bacteria were Escherichia coli in 3 patients (60%), Klebsiella pneumoniae in 2 patients (40%), and multi-resistant Acinetobacter as a metachronous infection to Escherichia coli in 1 patient (20%). All the infections were associated with indwelling Foley catheter. 7 patients (11.7%), i.e., 3 BPH and 4 prostate cancer patients, got a second resection to recover full spontaneous micturition. Prostate size was available in the medical record of 21 BPH patients and 15 prostate cancer patients. Gastroenteritis constitutes a group of diarrheal diseases of infectious origin, responsible for absenteeism from work, morbidity and mortality, especially among aged people. This study aimed to evaluate the antibacterial activity of Mitragyna inermis extracts on the in vitro growth of Staphylococcus aureus, Escherichia coli and Pseudomonas aeruginosa involved in gastroenteritis. Phytochemical screening was carried out using two distinct methods. The detection of phytochemical compounds by tube coloring and chromatography on a thin layer of silica gel. The sensitivity of organisms was evaluated by the agar well method. The dilution method in liquid medium coupled with spreading on Mueller Hinton agar helped determine the CMB/MIC activity ratios. The investigations show that the extract has the best extraction yield (75.86% ± 0.20%) compared to the aqueous macerated (61.8% ± 0.08%) and decocted (66.6% ± 0.12%). These extracts contain several phytochemical compounds such as flavonoids, polyphenols, tannins, alkaloids, saponosides, coumarins and sterols and terpenes. These substances are endowed with biological activities and could be at the origin of antibacterial activity observed with M. inermis extracts. The analysis of antibacterial activity showed that the germs are sensitive to the extracts with inhibition diameters ranging from 8.30 ± 0.53 to 17.87 ± 0.58 mm. The ethanolic extract was the most active with diameters varying from 15.07 ± 0.62 to 17.87 ± 0.58 mm on all the germs tested. E. coli and S. aureus were the most sensitive germs to the extracts. P. aeruginosa was the least sensitive germ. Activity reports indicate that the extracts exert bactericidal activity on E. coli and S. aureus but bacteriostatic activity on P. aeruginosa. These results justify the use of M. inermis leaves in a traditional environment to treat gastroenteritis.

The West African Monsoon (WAM) is characterized by strong decadal and multi-decadal variability and the impacts can be catastrophic for the local populations. One of the factors put forward to explain this variability involves the role of atmospheric dynamics, linked in particular to the Saharan Heat Low (SHL). This article addresses this question by comparing the sets of preindustrial control and historical simulation data from climate models carried out in the framework of the CMIP5 project and observations data over the 20th century. Through multivariate statistical analyses, it was established that decadal modes of ocean variability and decadal variability of Saharan atmospheric dynamics significantly influence decadal variability of monsoon precipitation. These results also suggest the existence of external anthropogenic forcing, which is superimposed on the decadal natural variability inducing an intensification of the signal in the historical simulations compared to preindustrial control simulations. We have also shown that decadal rainfall variability in the Sahel, once the influence of oceanic modes has been eliminated, appears to be driven mainly by the activity of the Arabian Heat Low (AHL) in the central Sahel, and by the structure of the meridional temperature gradient over the inter-tropical Atlantic in the western Sahel.

Assessment of the Drafting Quality of Request Forms Submitted to the Malaria and Parasitology Units at the Institut Pasteur de Cote d’Ivoire

Biological tests provide information on the medical analysis requested by both the patient and the prescriber. It is a communication link between the prescriber and the laboratory staff. The lack of some information on request forms not only affects the drafting quality of the test and patient care, but could also make thousands of data produced by healthcare centers unusable. The aim of this study was to assess the drafting quality of request forms submitted to the Malaria and Parasitology Units at the Institut Pasteur de Côte d’Ivoire. Methods: It was a descriptive cross-sectional study to assess the drafting quality of request forms of various prescribers received at the Institut Pasteur de Côte d’Ivoire. This study was conducted at the Malaria and Parasitology Units, department of Parasitology and Mycology (Institut Pasteur de Côte d’Ivoire), from 6th December 2020 to 6th December 2021. The information on each request forms was recorded on a data collection form designed for this purpose. Each data collection form corresponds to a request forms and each test to a patient. Results: Out of a total of 1990 request forms received, the patient’s age and sex were missing on 18% and 26.8% of the tests respectively. More than half (51.80%) of request forms did not indicate the patient’s place of residence. Clinical information was not provided on 45.90% of the tests. Prescribers omitting their signatures were 51%, stamps were 50.3% and contacts were 71.2%. Only 5.4% of request forms were of good drafting quality. Providing all the required information on the forms could facilitate the use and analysis of data and samples.

Background: Cancer continues to pose a significant threat to our society, representing one of the most pressing health concerns worldwide. This study aimed to evaluate the chemical composition and the antioxidant activity of aqueous and hydroethanolic extracts from Acacia nilotica (An), Bauhinia reticulate (Br), and Tamarindus indica (Ti) of Fabaceae family, traditionally used in Northern Cameroon for cancer treatment. Methods: The phytochemical screening of the three plants was conducted using conventional colorimetric methods, followed by the measurement of total phenol content, flavonoids, and tannins. The antiradical and antioxidant activities of both plant extracts were assessed through FRAP, ABTS, and DPPH methods. A principal components analysis was employed to correlate the quantities of the evaluated secondary metabolites with the activities. Results: Both types of extracts from the three plants contain alkaloids, saponins, flavonoids, phenolic compounds, tannins, glycosides, terpenoids, coumarins, anthocyanins, and anthraquinones. The aqueous extracts of Br and An are significantly richer (p Conclusion: The three Fabaceae plants from northern Cameroon, prepared in different solvents, can be utilized for their antiradical properties in cancer treatment.

Biological Profile and Cardiovascular Risk in Patients Receiving Neuroleptics at the Psychiatric Department of the University Hospital Center of Brazzaville

Several studies report the problem of cardiovascular tolerance of treatments with neuroleptics, given the important number of morbidities in patients with mental illnesses. This preliminary work aimed to describe the epidemiological and biological profile of patients taking neuroleptics and followed in the psychiatry department of Brazzaville University Hospital, from the angle of cardiovascular risk. Fifty (50) patients (17 men and 33 women), with a mean age of 33.9 10.7 years, were included. Epidemiological data (sex, age, tobacco or alcohol consumption) were collected on pre-established survey forms. Biochemical (total cholesterol, HDL-c, triglycerides and atherogenicity index) and inflammatory parameters (ultra-sensitive CRP, troponin I and NT-ProBNP) were investigated using enzymatic and indirect immunofluorescence technical, respectively. The results obtained showed that 54% of patients were obese, 94% were non-smokers, and 12% had high blood pressure. 10% of patients had high total cholesterol levels and 90% had HDL cholesterol levels below 60 mg/dl. Triglycerides and atherogenicity index were significantly elevated in relation to Body Mass Index (BMI). Ultrasensitive CRP was elevated in 38% of patients. In conclusion, this study revealed an association between lipid parameters (triglycerides and atherogenicity index) in relation to BMI in patients taking neuroleptics followed in the Psychiatry Department of University Hospital Center of Brazzaville.
