In this investigation,all-optical Toggle flip-flop event-driven memory is explored with data rate of 16 Gbit/*** mode optical fiber model is used as a nonlinear medium to generate the output set and reset pulses of a Toggle flip-flop,and the model is based on the bidirectional optical transmission principle,considering the fundamental effects of cross phase modulation and self-phase modulation with change in polarization *** performance of a flip-flop is evaluated using truth table conditions and performance parameters such as Q factor,which is obtained as 380.92 d b for Q and 272.9 d b for■,and rising and falling times of 7.304 ps and 5.79 ps,respectively are obtained,which makes flip-flop design fast as compared to earlier design techniques.

AIM To assess the real-world effectiveness and cost of simeprevir(SMV), and/or sofosbuvir(SOF)-based therapy for chronic hepatitis C virus(HCV) *** The real-world performance of patients treated with SMV/SOF ± ribavirin(RbV), SOF/RbV, and SOF/RbV with pegylated-interferon(PEG) were analyzed in a consecutive series of 508 patients with chronic HCV infection treated at a single academic medical center. Patients with genotypes 1 through 4 were included. Rates of sustained virological response-the absence of a detectable serum HCV RNA 12 wk after the end of treatment [sustained virological response(SVR) 12]-were calculated on an intention-to-treat basis. Costs were calculated from the payer's perspective using Medicare/Medicaid fees and Redbook Wholesale Acquisition Costs. Patient-related factors associated with SVR12 were identified using multivariable logistic *** SVR 12 rates were as follows: 86%(95%CI: 80%-91%)among 178 patients on SMV/SOF ± RbV; 62%(95%CI: 55%-68%) among 234 patients on SOF/RbV; and 78%(95%CI: 68%-86%) among 96 patients on SOF/PEG/RbV. Mean costs-per-SVR 12 were $174442(standard deviation: ± $18588) for SMV/SOF ± RbV; $223003(± $77946) for SOF/RbV; and $126496(± $31052) for SOF/PEG/RbV. Among patients on SMV/SOF ± RbV, SVR12 was less likely in patients previously treated with a protease inhibitor [odds ratio(OR): 0.20, 95%CI: 0.06-0.56]. Higher bilirubin(OR: 0.47, 95%CI: 0.30-0.69) reduced the likelihood of SVR12 among patients on SOF/RbV, while FIb-4 score ≥ 3.25 reduced the likelihood of SVR 12(OR: 0.18, 95%CI: 0.05-0.59) among those on SOF/PEG/RbV. CONCLUSION SVR 12 rates for SMV and/or SOF-based regimens in a diverse real-world population are comparable to those in clinical trials. Treatment failure accounts for 27% of costs.

blunt traumatic abdominal wall disruptions associated with evisceration (abdominal wall injury grade type Ⅵ) are very rare. We describe a case of large traumatic abdominal wall disruption with bowel evisceration and complete transection of jejunum and sigmoid colon that occurred after a 30-year-old male sustained run over injury to abdomen. Abdominal exploration and primary end to end jejuno-jejunal and colo-colic anastomosis were done. Staged management of giant abdominal wall defect was performed without any plastic reconstruction with good clinical outcome.

Coal is the world＇s most abundant energy source because of its abundance and relatively low cost. Due to the scarcity in the supply of high-grade coal, it is necessary to use low-.grade coal for fulfilling energy demands of modern civilization. However, due to ItS high ash and moisture content, low-grade coal exerts the substantial impact on their consumption like pyrolysis, liquefaction, gasification and combus- tion process. The present research aimed to develop the efficient technique for the production of clean coal by optimizing the operating parameters with the help of response surface methodology. The effect of three independent variables such as hydrofluoric acid （HF） concentration （10-20% by vo!ume ）,. temper- ature （60-100 ~C）, and time （90-180 min）, for ash reduction from the low-grade coal was Investigated.. A quadratic model was proposed to correlate the independent variables for maximum ash reduction at the optimum process condition by using central composite design （CC.D）method. The study reveals that HF concentration was the most effective parameter for ash reduction in comparison with time and temper- ature. It may be due to the higher F-statistics value for HF concentration, which effects to large extent of ash reduction. The characterization of coal was evaluated by Fourier transform infrared spectroscopy （FTIR） analysis and Field-emission scanning electron microscopy with energy-dispersive X-ray （FESEM- EDX） analysis for confirmation of the ash reduction.
