Background and Aims:Spontaneous bacterial peritonitis(SBP)is a serious complication of liver cirrhosis and a prognostic model to predict it is *** study was designed to test the ability of different laboratory tests and the new scoring system by Wehmeyer and colleagues(consisting of age,C-reactive protein(CRP)and platelet count)to predict ***:Three-hundred patients admitted to the National Liver Institute,University of Menoufia,Egypt(2015-2016)with liver cirrhosis and ascites were included in our *** was diagnosed if ascetic neutrophil count was≥250/μL with no sign of secondary ***:The patient population had age range of 29-81 years old,was 60%men and showed a majority(91.7%)with primary cause of liver disease being hepatitis *** univariate analysis,associations with age,total bilirubin,aspartate aminotransferase level,creatinine level,international normalized ratio,model for endstage liver disease score,total leucocytic count,platelet count and CRP level were *** multivariate analysis,independent predictors were age,platelet count and CRP level(p=0.004,0.013 and<0.001,respectively).CRP at a cut-off point≥13.5 mg/L could predict SBP(sensitivity of 86.4%and specificity of 66.0%).Wehmeyer's SBP scoring system was predictive(p<0.001);only 4%of patients with 0 score developed SBP(CRP cut-off of 30 mg/L),while 92.8%with score of 3 or 4 developed *** using our modified Wehmeyer score with CRP cut-off value of 13.5 mg/L,no patient with 0 score developed ***:Age,CRP level and platelet count are independent predictors for SBP and a scoring system including them could easily predict the *** diagnosis could be excluded in patients with 0 score,using CRP cut-off value of 13.5 mg/L.

Incidence of Haematoma in Post Bariatric Abdominoplasty Cases

Objective: This study aims at assessing the incidence of haematoma formation in abdominoplasty cases who had previous bariatric surgery and in those who did not have bariatric surgery before. Patients and Methods: This is a 4 years retrospective study involving patients who underwent abdominoplasty surgery between the period of July 2015 and June 2019. The incidence of haematoma formation was assessed in relation to history of previous bariatric surgery in these patients. Results: 164 patients met the inclusion criteria of the study;46 of them were males and 118 were females. The mean age of the patients included in the study was 38.6 (21 - 60 years). The mean BMI was 27 kg/m2 (21.4 - 34.7 Kg/m2). The mean hospital stay was 6.6 days (4 - 19 days). 107 patients had history of previous bariatric surgery performed and 57 had not. 21 patients developed haematoma (12.8%). 16 of the patients who had haematoma had previous bariatric surgery done (15% of post bariatric surgery cases) while 5 patients developed haematoma with no history of previous bariatric surgery (8.8%). Sixteen male patients out of the 46 male patients in the study developed postoperative haematoma (34.8% of the male patients) while only 5 of the 118 female patients developed postoperative haematoma (4.2% of the female patients) which shows a significant difference in haematoma formation when comparing males to females in general (p Conclusion: Our study shows that the incidence of post abdominoplasty haematoma formation is significantly higher in male patients who had bariatric surgery before when compared to those who had not (p < 0.05). The incidence was higher in males when compared to females (p < 0.001) indicating that males with previous history of bariatric surgery have the highest chance of developing haematoma formation. Semen parameters in men with spinal cord injury： changes and aetiology

Aim： To assess the changes in semen parameters in men with spinal cord injury （SCI） and the possible causes of these changes. Methods： The study included 45 subjects with SCI. Semen retrieval was done by masturbation （2）, vigorous prostatic massage （n = 13）, penile vibratory stimulation （n = 13） or electroejaculation （n = 17）. Results： The semen of men with SCI showed normal volume （2.3 ± 1.9 mL） and sperm count （85.0 × 10^6 ± 83.8 × 10^6/mE） with decreased motility （11.6% ± 10.1%）, vitality （18.5 % ± 15.2%） and normal forms （17.5 ± 13.4%）, and pus cells has been increased （6.0 × 10^6 ± 8.2 × 10^6/mL）. Total （13.4 ± 9.9 vs. 7.1 ± 6.8） and progressive （4.4 ± 3.9 vs. 2.2 ± 2.1） motility were significantly higher in subjects with lower scrotal temperatures. There was no statistical significant difference between electroejaculation and penile vibratory stimulation groups as regards any of the semen parameters. Subjects＇ age, infrequent ejaculation, injury duration and hormonal profile showed no significant effect on semen parameters. Conclusion： The defining characteristics of the seminogram in men with SCI are normal volume and count with decreased sperm motility, vitality and normal forms, and the increased number of pus cells. The most acceptable cause of the deterioration of semen is elevated scrotal temperature.

Sarcoidosis is a non caseating granulomatous disease of unknown etiology. Many factors may be attributed to the occurrence of that granuloma including the chemical factors. We present this case aiming to highlight Synthol injection which is chemical substance frequently used by athletics to increase their muscle size as it can trigger the occurrence of musculocutanous sarcoidosis. In conclusion Synthol should be added to the chemical list which can induce musculocutanous sarcoidosis.

Experimental Investigation of Intake Diesel Aerosol Fuel Homogeneous Charge Compression Ignition (HCCI) Engine Combustion and Emissions

External mixture formation (PFI) of a diesel fuel aerosol has employed to investigate the diesel HCCI engine combustion and emissions characteristics. The key to the external mixture formation with diesel fuel is the proper fuel/air mixture preparation. A proposed intake diesel fuel aerosol system mainly consists of a small chamber, in which the diesel fuel is fully vaporized by means of fuel cavitation inside the diesel injector nozzle. Nozzle cavitation is mainly affected by the injection pressure and the fuel system temperature. Results obtained reveal that the proposed method determines the possibility of producing a complete homogeneous fuel/air mixture, which can be applied to the diesel HCCI engine. With this method, the combustion and emission behavior were entirely optimized and the engine is capable of running in HCCI combustion mode with nearly ideal mixture preparation. In the present investigation, a methodology for the HCCI combustion mode of the diesel aerosol/air mixtures based on the fuel cavitation inside the injector nozzle parameters (such as the injection pressure and the fuel system temperature where fuel premixed ratio, NOx, CO, CO2, and HC emissions) have analyzed. Based on the engine performance and emissions characteristics the fuel injection pressure and the fuel system temperature have optimized to produce a suitable fuel premixed ratio and the perfect fuel/air mixture homogeneity at different engine operating conditions. The optimal injection pressure ranges between 150 - 200 bars, while the fuel system temperature lies within 175℃ - 200℃. Loops of exhaust gas recirculation (EGR) are used to extend the engine load by controlling the combustion phasing.

Hydrodynamic Characteristics of Three Rows of Vertical Slotted Wall Breakwaters

In this study, we examine the hydrodynamic characteristics of three rows of vertical slotted wall breakwaters in which the front and middle walls are permeable and partially immersed in a water channel of constant depth, whereas the third wall is impermeable. The wave–structure interaction and flow behavior of this type of breakwater arrangement are complicated and must be analyzed before breakwaters can be appropriately designed. To study the hydrodynamic breakwater performance, we developed a mathematical model based on the eigenfunction expansion method and a least squares technique for predicting wave interaction with three rows of vertical slotted wall breakwaters. We theoretically examined the wave transmission, reflection, energy loss, wave runup, and wave force under normal regular waves. Comparisons with experimental measurements show that the mathematical model results adequately reproduce most of the important features. The results of this investigation provide a better understanding of the hydrodynamic performance of triple-row vertical slotted wall breakwaters.

Production of geopolymer composites enhanced by nano‐kaolin material

Nowadays kaolin raw material is usually used to produce nano-kaolin for geopolymer enhancement by using firing *** the present study,kaolin used was taken from the Naqus Formation(Cambro-Ordovician age),west of Gabal El Gunna,Sinai,***-kaolin material is an ultrafine material and was prepared from the taken kaolin by the firing process at 800 for 2℃h with a heating rate of 5/*** mixes were prepared and their laboratory specimens were made℃and cured up to 90 *** cooled slag was used as starting material,and sodium hydroxide and sodium silicate were used in the study as activators for the used *** formed geopolymer mixes with different ratios(1%,1.5%,3%,5%,and 7%)of nano-kaolin as a partial replacement for the raw kaolin were *** Ace super plasticizer was added in the ratio of 4%from the dry weight to ensure good dispersing of the used nano *** showed that increasing the percentage of nano-kaolin up to 3%results in an enhancement in the mechanical properties as compared with the control mix up to 90 days of curing,while higher ratios are not preferable where they lead to agglomeration of the added nano materials and matrix dilution.

采用高速可控闪光摄影技术搭建了喷油器喷孔内流动及近场喷雾可视化试验装置,针对柴油、汽油、乙醇柴油、生物柴油4种燃料,对实际尺寸的透明喷油器喷孔内的流动及近场喷雾进行了对比试验研究。试验研究结果表明:所有试验燃料喷孔内均呈现空穴流动,低运动粘度、高饱和蒸气压的燃料空穴生成时刻更早、空穴强度更强;喷孔空穴强度较大的燃料具有更大的近场喷雾锥角;喷射结束后喷孔内会产生气体倒流现象并形成初始气泡,初始气泡体积随燃料表面张力的减小而增大。同时发现,针阀运动会对压力室和喷孔内的流动产生极大的扰动并影响其流动形态。
