Recurrent Sigmoid Volvulus after Sigmoidectomy: 3 Clinical Cases

Introduction: Recurrent sigmoid volvulus is a medical and surgical emergency. It is the recurrence of a twisting of the sigmoid loop around its mesenteric axis initiating an occlusion by strangulation. The risk factors of colonic volvulus are: a dolichosigmoid, a chronic constipation, a tumor, …In Western countries, recurrences of sigmoid volvulus after surgery are rare and due to a non resection of a dolicho-megacolon. Most African studies do not report any recurrence of sigmoid volvulus after sigmoidectomy. This work aims to describe three original cases of recurrence of sigmoid volvulus after sigmoidectomy recorded at the University hospital of Brazzaville and to analyze their causes and surgical management. Clinical Observation: We studied the cases of three male patients, whose average age was 73 years, each of them having a history of chronic constipation and sigmoidectomy for sigmoid volvulus with an average delay of recurrence of 5.3 years. They all underwent emergency laparotomy for acute intestinal obstruction on scarred abdomen. The surgery permitted to unfold a recurrent volvulus of the sigmoid associated with circumferential scar fibrosis on the volvulated sigmoid loop. A second colectomy was performed. The histological results of the surgical specimens were normal for two patients and revealed signs of tissue necrosis for the third patient. The postoperative outcome was without incident for two patients. The third patient died after developing a septic shock. Conclusion: Recurrent sigmoid volvulus after sigmoidectomy is rare. The etiological factors for our three patients were incomplete sigmoidectomy during the first operation and chronic constipation.

Contribution of Hydrogeochemical and Isotopic Tools to the Management of Upper and Middle Cheliff Aquifers

In the alluvial aquifers of Upper and Middle Cheliff(North-West Algeria), the groundwater quality is deteriorating. The objective of this study was to characterize the physical and chemical properties of these aquifers;and to evaluate the groundwater quality and its appropriateness for drinking and agricultural use. An investigation was carried out by estimating of the physiochemical parameters(Ca^2+, Mg^2+, Na^+, K^+, Cl^-, SO4^2-, HCO3^-, NO3^-, Br^- and TDS) to identify the chemical characteristics of groundwater. Morever, the isotopic composition was examined to identify the sources of recharge of these aquifers. The groundwater geochemistry for the high water level(May, 2012 and June, 2017) and low water level(November, 2012 and October, 2017) was studied. Accordingly, water samples from 39 water sampling points were collected(October, 2017 and June, 2018), for the purpose of analyzing stable isotopes(18O, 2H). The results show that the groundwater is mainly characterized by Ca-Cl and Na-Cl type. The chemical quality of the water is from fair to poor with the presence of nitrates used in agricultural and urban discharge. Also, the Br/Cl ratio gives indications on the origin of the salinity. This salinity is due to the leaching of chlorinated fertilizers, the dissolution of evaporite deposits and the rise of deep salty water by the fault of Chellif. While, the diagram of δ2H=f(δ18O) indicates that the origin of the recharge of these aquifers is the Atlantic and Mediterranean oceanic meteoric rainwater.

Spatial Variability and Cartography of Maximum Annual Daily Rainfall under Different Return Periods in Northern Algeria

The estimation of precipitation quantiles has always been an area of great importance to meteorologists, hydrologists, planners and managers of hydrotechnical infrastructures. In many cases, it is necessary to estimate the values relating to extreme events for the sites where there is little or no measurement, as well as their return periods. A statistical approach is the most used in such cases. It aims to find the probability distribution that best fits the maximum daily rainfall values. In our study, 231 rainfall stations were used to regionalize and find the best distribution for modeling the maximum daily rainfall in Northern Algeria. The L-moments method was used to perform a regionalization based on discordance criteria and homogeneity test. It gave rise to twelve homogeneous regions in terms of LCoefficient of variation(L-CV), L-Skewness(L-CS) and L-Kurtosis(L-CK). This same technique allowed us to select the regional probability distribution for each group using the Z statistic. The generalized extreme values distribution(GEV) was selected to model the maximum daily rainfall of 10 groups located in the north of the steppe region and the generalized logistic distribution(GLO) for groups representing the steppes of Central and Western Algeria. The study of uncertainty by the bias and RMSE showed that the regional approach is acceptable. We have also developed maximum daily rainfall maps for 2, 5, 10, 20, 50 and 100 years return periods. We relied on a network of 255 rainfall stations. The spatial variability of quantiles was evaluated by semi-variograms. All rainfall frequency models have a spatial dependence with an exponential model adjusted to the experimental semi-variograms. The parameters of the fitted semi-variogram for different return periods are similar, throughout, while the nugget is more important for high return periods. Estimation and inter-comparison of infiltration models in the agricultural area of the Mitidja Plain, Algeria

Infiltration is an important part of the hydrological cycle, and it is one of the main abstractions accounted for in the rainfall-runoff modeling. The main purpose of this study is to compare the infiltration models that were used to assess the infiltration rate of the Mitidja Plain in Algeria. Field infiltration tests were conducted at 40 different sites using a double ring infiltrometer. Five statistical comparison criteria including root mean squared error(RMSE), normalized root mean squared error(NRMSE), coefficient of correlation(CC), Nash-Sutcliffe efficiency(NSE), and Kling-Gupta efficiency(KGE) were used to determine the best performing infiltration model and to confirm anomalies between predicted and observed values. Then we evaluated performance of five models(i.e., the Philip model, Kostiakov model, Modified Kostiakov model, Novel model, and horton model) in simulating the infiltration process based on the adjusted performance parameters cited above. Results indicated that the Novel model had the best simulated water infiltration process in the Mitidja Plain in Algeria. however, the Philip model was the weakest to simulate the infiltration process. The conclusion of this study can be useful for estimating infiltration rate at various sites using a Novel model when measured infiltration data are not available and are useful for planning and managing water resources in the study area.

Climatic and anthropogenic impacts on the decrease of sediment discharge to the Mediterranean coast from the largest river of Maghreb

The current study focuses on the issue of the decrease in sediment discharge to the Mediterranean Sea by the largest river in Algeria, the Wadi Cheliff(i.e. Cheliff River). This study clarifies the effect of climatic and anthropogenic factors on the changes in the sedimentary dynamics of the Cheliff River discharging to the sea. The data used(rainfall, water discharge, and sediment discharge) concern the Sidi Bel Attar gaging station on the Cheliff River, only 18 km from discharge to the Mediterranean Sea. A power-type statistical regression model was used to fill the 74.2% gap in Suspended Solids(SS) measurements in the establishment of a SS database for the period 1951 e2012. The study results show that the transport of suspended sediment discharged to the sea is about 487 t/km2/yr. Statistical tests of breaks highlight that rainfall decreased by 26% and that water and sediment input to the sea declined from 74% to 63% for the period of 1981 e2012. The correlation analysis shows a decrease in the contribution of rainfall on sediment and water input at the outlet equal to 77.9% and 77.8%, respectively, during the period of 1980 e2012, compared to the period of 1968 e1980. however, the double mass method reflects the contribution of factors other than rainfall to the decrease of sediment input to the sea. These other factors mainly include large dams, which intercepted about 71% of the total volume of sediment discharged to the sea during the period of 1968 e2010. In addition the contribution of large dams to the reduction of sedimentary input to the sea is more important than that of the decrease in rainfall. The management of large dams also contributes to the increase in the sediment deficit to the sea through the prioritization of interception of sediment at the expense of releases, for socio-economic purposes, 68.4% of the Cheliff River discharge is diverted for human use. The Precipitation Concentration in the Cheliff Watershed, Algeria:A Critical Analysis for Sustainable Water Resource Management

In this study, a statistical analysis of historical and projected values of the annual, seasonal, and supra-seasonal precipitation concentration index(PCI) was conducted for different stations in the Cheliff watershed. Statistical methods such as breakpoint and trend analysis using Pettitt and Mann–Kendall tests were used for the PCI values of annual, monthly, and supra-seasonal precipitation data between 1950 and 2014. The PCI values varied between 14.24% and 24.86% annually, between10.75% and 18.74% seasonally, and between 11.23% and 26.98% supra-seasonally. According to the Pettitt and Mann–Kendall tests, there was an insignificant change in precipitation distribution during the study period. Spatial analysis using the inverse distance weighting(IDW) method confirmed the minor variability in precipitation distribution in the study area. The Pearson correlation coefficient between oscillation indices and PCI values at different scales showed significant values with the Mediterranean Oscillation index(MOi), North Atlantic Oscillation(NAO), and Western Mediterranean Oscillation index(We MOi), highlighting their potential influence on annual PCI values. For the climate projection scenarios, projected PCI values align with historical PCI values. Application of Pettitt and Mann-Kendall tests to the projected PCI series shows that there will not be a significant change in future precipitation distribution.
