Acute lymphoblastic leukemia (ALL) is the most common malignancy in children, with the majority of cases being of precursor B-cell phenoltype. Conventional cytogenetic analysis plays an important role in the diagnosis of B-cell ALL, identifying characteristic chromosomal abnormalities associated with a given prognosis therein facilitating optimized treatment. The more recent introduction of microarray technology to the analysis of B-cell ALL has afforded both higher resolution for the detection of known abnormalities and an ability to identify novel copy number abnormalities (CNAs) with potential clinical relevance. In the current study, microarray analysis was performed on 20 cytogenetically abnormal B-cell ALL cases (10 pediatric and 10 adult), while a novel microarray-based balanced-translocation detection methodology (translocation CGH or tCGH) was applied to that subset of cases with a known or suspected recurrent balanced translocation. Standard microarray analysis identified that CNAs was not detected by previous conventional cytogenetics in 75% (15/20) cases. tCGH identified 9/9 (100%) balanced translocations defining BCR/ABL1 (x4), ETV6/RUNX1 (x3), and MLL/AFF1 (x2) breakpoints with high resolution. The results illustrate the improved molecular detail afforded by these technologies and a comparison of translocation breakpoints, CNAs and patient age offers new insights into tumor biology with potential prognostic significance.

Insights into myelodysplastic syndromes from current preclinical models

In recent years, there has been significant progress made in our understanding of the molecular genetics of myelodysplastic syndromes(MDS). Using massively parallel sequencing techniques, recurring mutations are identified in up to 80% of MDS cases, including many with a normal karyotype. The differential role of some of these mutations in the initiation and progression of MDS is starting to be elucidated. Engineering candidate genes in mice to model MDS has contributed to recent insights into this complex disease. In this review, we examine currently available mouse models, with detailed discussion of selected models. Finally, we highlight some advances made in our understanding of MDS biology, and conclude with discussions of questions that remain unanswered.

Muscle energy technique for chronic obstructive pulmonary disease:A feasibility study

Objective:This study aimed to investigate the feasibility of implementing a manual therapy technique(muscle energy technique,MET)protocol in a hospital pulmonary rehabilitation(PR)program for patients with moderate to severe chronic obstructive pulmonary disease(COPD).Methods:Participants aged 40 years and over,with moderate to severe COPD,were recruited into this 12-week *** primary outcome measures were feasibility(acceptability of the intervention and attendance/adherence to the trial)and safety(adverse events,AEs).All participants received the MET and PR *** and assessors were ***-standardized MET was delivered on 6 occasions(a maximum of once per week)at the hospital directly before a PR *** undertook PR sessions as per the hospital program at a frequency of two days per week for 8 *** were contacted 4 weeks after their final MET treatment via a telephone call to assess acceptability of the ***:Thirty-three participants were enrolled,with a median age of 74 years(range 45-89 years).The median number of MET sessions that participants attended was 5(range O-6)out of a possible 6 sessions(83%attendance).At follow-up,participants overwhelmingly enjoyed the MET treatment with some subjectively reporting improved *** were no major AEs related to the intervention,with the majority of AEs classified as expected events related to COPD ***:It is feasible to implement a manual therapy protocol using MET as an adjunct to PR in a hospital *** rates were satisfactory and there were no AEs related to the MET component of the intervention.

Economic Assessment and Review of Waterless Fracturing Technologies in Shale Resource Development: A Case Study

Our database tracking of USA water usage per well indicates that traditionally shale operators have been using, on average 3 to 6 million gallons of water; even up to 8 million for the en- tire life cycle of the well based on its suitability for re-fracturing to stimulate their long and lateral ho- rizontal welis. According to our data, sourcing, storage, transportation, treatment, and disposal of this large volume of water could account for up to 10% of overall drilling and completion costs. With in- creasingly stringent regulations governing the use of fresh water and growing challenges associated with storage and use of produced and flowback water in hydraulic fracturing, finding alternative sources of fracturing fluid is already a hot debate among both the scientific community and industry experts. On the other hand, waterless fracturing technology providers claim their technology can solve the concerns of water availability for shale development. This study reviews high-level technical issues and opportunities in this challenging and growing market and evaluates key economic drivers behind water management practices such as waterless fracturing technologies, based on a given shale gas play in the United States and experience gained in Canada. Water costs are analyzed under a variety of scenarios with and without the use of （fresh） water. The results are complemented by surveys from several oil and gas operators. Our economic analysis shows that fresh water usage offers the greatest economic return. In regions where water sourcing is a challenge, however, the short-term economic advantage of using non-fresh water-based fracturing outweighs the capital costs required by waterless fracturing methods. Until waterless methods are cost competitive, recycled water usage with low treatment offers a similar net present value （NPV） to that of sourcing freshwater via truck, for in- stance.

Prolonged Disease Stabilization and Tolerability in a Nuclear Protein in Testis Midline Carcinoma Patient Treated with Dual Histone Deacetylase and Phosphoinositide 3-Kinase Inhibitor CUDC-907

Introduction: Nuclear protein in testis midline carcinoma (NMC) is a rare and extremely aggressive carcinoma (median survival Case Report: A 61-year old female NMC patient enrolled on study CUDC-907-102 (NCT02307240) after rapidly progressing through two prior treatments. The patient’s assessable sites of disease consisted of right pleural effusion, right hilar soft tissue, and segment IV liver. Treatment was well tolerated with toxicities primarily consisting of manageable diarrhea and thrombocytopenia. The patient remains on active treatment after more than 32 months of stable disease. Discussion: Dysregulation of MYC in NMC is believed to play a central role in pathogenesis. CUDC-907 has demonstrated potent suppression of MYC expression and anti-tumor activity in preclinical NMC models, providing a mechanistic rationale for the prolonged disease stabilization observed here. The treatment of additional NMC patients with CUDC-907 is needed to further evaluate this promising report. Conclusion: This case demonstrates a rare success in the treatment of a devastating disease using only a novel small molecule, warranting further investigation of CUDC-907 in NMC.
