Evaluation of 100 Cases of Mortality after Cardiac Surgery: A Single-Center Experience in Bangladesh

Background: Vigilance ensures safety in cardiac surgery. Performance in cardiac surgery is often measured by short-term mortality. Several risk factors like advanced age, female gender, higher body mass index, decreased left ventricular function, emergent, and redo operations have appeared recurrently as poor prognostic variables. Evaluation of postoperative mortality is crucial to find loopholes to provide proper care and reduce preventable mortality after cardiac surgery in developing countries with limited infrastructures and resources. Methods: This is a retrospective study conducted in the Department of Cardiac Surgery, Bangabandhu Sheikh Mujib Medical University. Perioperative data of 100 cases of mortality after cardiac surgery performed from 1 January 2014 to 30 May 2018 were collected from the university medical record. The data on age, gender, body mass index, preoperative investigations, diagnoses, types of operations, details of cardiopulmonary bypass, and postoperative period of the study populations were evaluated. Results: During the study period, about 1627 cases of cardiac surgery were done with an overall mortality rate of 6.15%. The mean age was 41.05 ± 20.19 (0 - 68) years, and 66% of patients were male. Background: Following coronary artery bypass grafting (CABG), delirium emerges as a prevalent complication. This study aimed to assess the correlation between elevated serum cortisol levels and the occurrence of postoperative delirium subsequent to off-pump CABG. Methods: Conducted in the Department of Cardiac Surgery at BSMMU from October 2020 to September 2022, this comparative cross-sectional study included a total of 44 participants. Subjects, meeting specific criteria, were purposefully assigned to two groups based on off-pump CABG. Group A (n = 22) consisted of patients with normal serum cortisol levels, while Group B (n = 22) comprised individuals with high serum cortisol levels on the first postoperative day. Delirium onset was assessed at the bedside in the ICU on the 1st, 2nd, 3rd, 4th, and 5th postoperative days using standard tools, namely the Richmond Agitation Sedation score (RASS) and The Confusion Assessment Method (CAM-ICU). Data were collected based on the presence or absence of delirium. Statistical analysis utilized SPSS version 26.0, employing an independent Student’s t-test for continuous data and chi-square and Fischer’s exact test for categorical data. A p-value ≤ 0.05 was considered statistically significant. Results: Group-A had a mean age of 54.50 ± 17.97, and Group-B had a mean age of 55.22 ± 15.45, both with a male predominance (81.81% and 86.36% respectively). The mean serum cortisol level was significantly higher in Group B (829.71 ± vs. 389.98 ± 68.77). Postoperative delirium occurred in 27.3% of Group B patients, statistically significant compared to the 4.5% in Group A. However, patients in Group B who developed delirium experienced significantly longer postoperative ICU and hospital stays (79.29 ± 12.27 vs. 11.44 ± 2.85, p ≤ 0.05). Cloud point （CP） determinations of 4-（1,1,3,3-tetramethylbutyl）phenyl-polyethylene glycol （TX-100 （nonionic surfactant）） was carried out in aqueous as well as in the attendance of drug （ceftriaxone sodium trihydrate （CFT））/（CFT ＋ different inorganic salts） and discussed thoroughly. Nonionic surfactants are employed exten- sively in different formulations. In aqueous solution, the values of CP of TX-100 are obtained to increase by means of enhancing of their concentration in the solution. The CP values of TX-100 solutions were found to de- crease in the presence of drug and their values decrease more with rising concentrations of the drug. The values of CP of CFT and TX-100 mixtures were found to further decrease in the attendance of inorganic salts in compar- ison to their absence. The effect of different sodium salts in decreasing CP values of TX-100 was achieved in the following order： NaCO3 〉 Na2SO4 〉 NaCl. However, in the case of potassium and ammonium salts, the decreasing order obtained is K2SO4 〉 KCO3 〉 KCI and （NH4）2SO4 〉 Na2CO3 〉 NH4Cl respectively. Various thermodynamic pa- rameters for example standard free energy （△G c）, standard enthalpy （△H c） as well as standard entropy （△S c） changes of phase separation were also evaluated and discussed in detail on the basis of their behavior.

We present new lemmas,theorem and corollaries to construct interactions among higher-order rogue waves,n-periodic waves and n-solitons solutions(n→∞)to the(2+1)-dimensional asymmetric Nizhnik-Novikov-Veselov(ANNV)*** examples for theories are given by choosing definite interactions of the wave solutions for the *** particular,we exhibit dynamical interactions between a rogue and a cross bright-dark bell wave,a rogue and a cross-bright bell wave,a rogue and a one-,two-,three-,four-periodic *** addition,we also present multi-types interactions between a rogue and a periodic cross-bright bell wave,a rogue and a periodic cross-bright-bark bell ***,we physically explain such interaction solutions of the model in the 3D and density plots.

A Numerical Study of MHD Laminar Flow in a Rotating Curved Pipe with Circular Cross Section

The incompressible viscous steady flow through a rotating curved pipe of circular cross-section with magnetic field is investigated numerically to examine the combined effects of rotation (Coriolis force), magnetic field and curvature (centrifugal force) on the flow. The curvature of the pipe has been assumed to be small, that is, the radius of the circle in which the central line of the pipe is coiled is large in comparison with the radius of the cross section. Spectral method is applied as a main tool for the numerical technique, where Fourier series, Chebyshev polynomials, Collocation methods, and Iteration method are used as secondary tools. The flow depends on the Taylor number (Tr), Dean Number (Dn), Magnetic Parameter (M) and the dimensionless curvature of the pipe δ. When Tr > 0, the rotation is in the direction so that the Coriolis force enforces the curvature effect. When Tr −1500 ≤ Tr ≤ 1500, Dn ≥ 1000 (large Dean number), M ≥ 0 and δ = 0.01. Due to high magnetic field four-vortex solution is observed in a rotating curved pipe system. Visualization is attained with MAPLE software.

Sustainable Use of Steel Industry Slag (SIS) for Concrete Production: A State Art of Review

This paper presents a review on the sustainable use of steel industry slag (SIS) in the construction industry for sustainable development. This issue also may contribute significantly to attain the sustainable development goal (SDG). If feasible sector is not determined earlier for the use of SIS for the country like Bangladesh then the unproductive uses will be increased and as well without large-scale utilization, direct dumping of SIS will create environmental hazards. On the other hand, the sustainable use of SIS in concrete production can contribute to the economic growth of the country;in addition, it can also contribute to keeping our natural environment safe from hazardous pollution. Another observation of this study is SIS used in cement and concrete production as aggregate or as additional cementing materials has a long-term strength and durability. Besides, replacement of cement with SIS deferred the hydration reaction at early ages, without any significant troubles. Furthermore, it can also be mentioned that high replacement of SIS with cement may raise a problem of volume instability which can be solved by using ground-granulated blast-furnace slag (GGBS) as combined admixture and the slump problem of concrete mixtures can be adjusted with addition of copper slag or class F fly ash. Feasibility analysis of SIS usage and an awareness program for all the stakeholders may be implemented for the sustainable uses of SIS. Both of these will play a vital role to safeguard environment in addition to economic growth of the country.

Dielectric Relaxation and Conductivity of Ba(Mg_(1/3)Ta_(2/3))O_3 and Ba(Zn_(1/3)Ta_(2/3))O_3

The frequency dependent dielectric properties of barium magnesium tantalate （BMT）, Ba（Mgl/3Ta2/3）03 and barium zinc tantalate （BZT）, Ba（Znl/3Ta2/3）03 synthesized by solid state reaction technique have been investigated at various temperatures by impedance spectroscopy. BMT and BZT possess cubic structure with lattice parameter a = 0.708 and 0.451 nm, respectively. The resonance peaks due to dielectric relaxation processes are observed in the loss tangent of these oxides. The relaxation in the samples is polydispersive in nature. The temperature dependence of dc conductivity, the most probable relaxation frequency （corn） obtained from tanδ vs logo） plots and ωm obtained from imaginary parts of the complex electrical modulus vs Iogω plots follow the Arrhenius behavior. According to these Arrhenius plots the activation energies of BMT and BZT are about 0.54 and 0.40 eV, respectively. Thus the results indicate that samples are semiconducting in nature. The frequency-dependent electrical data are analyzed in the framework of conductivity and electric modulus formalisms. Both these formalisms show qualitative similarities in relaxation time. Our study points that for complex perovskite oxides with general formula A（B＇B＇）03, the dielectric properties significantly depend on the atomic radii of both A and B type cations. BMT and BZT exhibit enhancement in dielectric property compared to their niobate counterparts. They may find several technological applications such as in capacitors, resonators and filters owing to their high dielectric constant and low loss tangent.

We investigate the bounded travelling wave solutions of the Biswas-Arshed model(BAM)including the low group velocity dispersion and excluding the self-phase *** integrate the nonlinear structure of the model to obtain bounded optical solitons which pass through the optical fibers in the non-Kerr *** bifurcation technique of the dynamical system is used to achieve the parameter bifurcation sets and split the parameter space into various areaswhich correspond to different phase *** bounded optical solitons and bounded periodic wave solutions are identified and derived conforming to each region of these phase *** also apply the extended sinh-Gordon equation expansion and the generalized Kudryashov integral schemes to obtain additional bounded optical soliton solutions of the BAM *** present more bounded optical shock waves,the bright-dark solitary wave,and optical rogue waves for the structure model via these schemes in different aspects.

Interaction of cetyltrimethylammonium bromide with cefixime trihydrate drug at different temperatures and compositions: Effect of different electrolytes

Herein,conductivity measurements have been carried out to explore the interaction between cetyltrimethylammonium bromide(CTAB,a cationic surfactant) and antibiotic drug(cefixime trihydrate(CMT)) in water and also in occurrence of inorganic salts(NaCl,Na2 SO4 and Na3 PO4) over the temperature range of 303.15-323.15 K with an interval of 5 *** all cases,two critical micelle concentrations(c^*) were achieved for the CMT-surfactant *** of CMT drug to CTAB solution decreases the values of c^* which indicates the interaction between CMT and *** values of c^* for CMT-CTAB mixture in the presence of salts are lower in magnitude compared to the aqueous medium which indicates that micellization of the CMT-CTAB mixed system is favorable in salt *** values of △Gm^0 were obtained to be negative indicating the spontaneity of the micellization process and the extent of spontaneity further increases by means of rising *** obtained outcomes from the ΔHm^0 and ΔSm^0 values disclose that the interactions between CMT and CTAB are mostly electrostatic along with hydrophobic in *** thermodynamic parameters of transfer and enthalpy-entropy compensation phenomenon were also determined and discussed in detail.

Decaying mangrove detritus plays a significant role in nutrient cycling and fueling in both the forests and aquatic habitats where the detritus stems from estuarine and coastal food *** decay rates partly depend on the type of detritus,but most studies have thus far focused on leaf litter decay and decomposition,whereas other detrital sources(except for roots)have largely been *** compare the decay rates of Rhizophora apiculata and Xylocarpus granatum non-leafy detritus(flowers,propagules,stipules,and twigs)with mangrove leaf litter in field studies,using litter bags during the dry and the wet seasons in the tropical mangrove forest of Sibuti,Sarawak,*** observed higher microbial decay rates of stipules,flowers,propagules,and twigs for both the *** and *** the wet *** contrast,leaf decay rates were higher during the dry *** rates depended on the lignin content of the detrital sources of both species,both during dry and wet ***,the half-life(T0.5)and 95%lifespan(T0.95)of non-leaf materials(flowers,propagules,stipules and twigs)for both species were remarkably longer than those of leaf *** decaying non-leafy detritus may play a significant role in nutrient and carbon cycling over longer time,when leaf litter is either being decomposing rapidly and/or being washed away by tidal flush and river ***,non-leafy(flowers,propagules,stipules and twigs)detritus should be taken into account when budgeting organic matter turnover in mangroves.
