A Review on D 614G Mutation with Bangladesh Scenario

With the COVID-19 pandemic, disparities between the infection rate and death rate in different countries become a major concern. In some countries, lower mortality rate compared to others can be explained by better testing capacity and intensive care facilities. Complete SARS-CoV-2 genome sequences from different countries of the world are continually submitted to Global Initiative for Sharing All Influenza Data using Next Generation Sequencing method. A SARS-CoV-2 variant with a D 614G Mutation in the spike (S) protein has become the most dominant form in the global pandemic. There are a number of ongoing studies trying to relate this mutation with the infectivity, mortality, transmissibility of the virus and its impact on vaccine development. This review aims to accumulate the major findings from some of these studies and focus its future implication. Some studies suggested D 614G strain has increased binding capacity, it affects more cells at a faster rate, so has a high transmissibility. Patients infected with this strain were found with high viral load. But still now there is no such evidence that this strain produces more severe disease as well as increased mortality. The structural change of spike protein produced by D 614G mutation was minor and did not hamper the vaccine efficacy. Some studies showed antibodies produced against D614 strain can neutralize G614 strain and vice versa. Whenever a mutation occurs in spike protein there are always chances of affecting the infectivity, transmissibility, vaccine efficacy. Therefore, more studies are required to find out the overall effect of D 614G mutation.

Background: Acute Kidney Injury (AKI) stands as a prominent postoperative complication in on-pump cardiac surgery, with repercussions on morbidity, mortality, and hospitalization duration. Current diagnostic criteria relying on serum creatinine levels exhibit a delayed identification of AKI, prompting an exploration of alternative biomarkers. Aims and Objectives: This study is designed to overcome diagnostic constraints and explore the viability of serum Cystatin C as an early predictor of Acute Kidney Injury (AKI) in individuals undergoing on-pump cardiac surgery. The investigation aims to establish the relationship between serum Cystatin C levels and the onset of AKI in patients subjected to on-pump cardiac surgery. Primary objectives involve the assessment of the diagnostic effectiveness of serum Cystatin C, its comparison with serum creatinine, and the exploration of its potential for the early identification and treatment of AKI. Methodology: Conducted as a single-center study at the cardiac surgery department of BSMMU in Bangladesh from September 2020 to August 2022, a comparative cross-sectional analysis involved 31 participants categorized into No AKI and AKI groups based on Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Data collection encompassed preoperative, post-CBP (cardiopulmonary bypass) conclusion at 2 hours, postoperative day 1, and postoperative day 2 intervals. Statistical analyses included Chi-squared tests, independent Student’s t-tests, and one-sample t-tests. Significance was set at P Results: The study revealed no significant differences in baseline characteristics between the No AKI and AKI groups, except for CPB time and cross-clamp time. Serum Cystatin C levels in the AKI group exhibited statistical significance at various time points, highlighting its potential as an early detector. Development and Characterization of Calcium Based Biocomposites Using Waste Material (Calcite Stones) for Biomedical Applications

Calcium-based biocomposite materials have a pivotal role in the biomedical field with their diverse properties and applications in combating challenging medical problems. The study states the development and characterization of Calcium-based biocomposites: Hydroxyapatite (HAP), and PVA-Gelatin-HAP films. For the preparation of Calcium-based biocomposites, an unconventional source, the waste material calcite stone, was used as calcium raw material, and by the process of calcination, calcium oxide was synthesized. From calcium oxide, HAP was prepared by chemical precipitation method, which was later added in different proportions to PVA-Gelatin solution and finally dried to form biocomposite films. Then the different properties of PVA/Gelatin/HAP composite, for instance, chemical, mechanical, thermal, and swelling properties due to the incorporation of various proportions of HAP in PVA-Gelatin solution, were investigated. The characterization of the HAP was conducted by X-ray Diffraction Analysis, and the characterization of HAP-PVA-Gelatin composites was done by Fourier Transform Infrared Spectroscopy, Thermomechanical Analysis, Tensile test, Thermogravimetric Differential Thermal Analysis, and Swelling Test. The produced biocomposite films might have applications in orthopedic implants, drug delivery, bone tissue engineering, and wound healing.

Effect of Exchange-Correlation Functional on the Structural, Mechanical, and Optoelectronic Properties of Orthorhombic RbSrBr3 Perovskite

In the present study, the effect of the exchange-correlation functional on the structural, mechanical, and optoelectronic properties of orthorhombic RbSrBr3 perovskite has been investigated using various functionals in Density Functional Theory (DFT) with the CASTEP code. The optimized lattice parameters are quite similar for all the functionals. The electronic properties have shown that RbSrBr3 perovskite is a wide direct band gap compound with a band gap energy ranging from 4.296 eV to 4.494 eV for all the functionals. The mechanical parameters like elastic constants, Young’s modulus, Shear modulus, Poisson’s ratio, Pugh’s ratio, and an anisotropic factor reveal that the RbSrBr3 perovskite has ductile behavior and an anisotropic nature which signifies the mechanical stability of the compound. The Debye temperature might withstand lattice vibration heat. High absorption coefficient (>104 cm−1), high optical conductivity, and very low reflectivity have been found in the RbSrBr3 perovskite for all functions. The computed findings on the RbSrBr3 perovskite suggested that the presented studied material is potentially applicable for photodetector and optoelectronic devices.

Impact of Action Observation Therapy along with Usual Physiotherapy Intervention of Individual with Alzheimer’s Disease

Alzheimer’s disease (AD) is a neurodegenerative disorder characterized by cognitive impairments in the initial stage, which lead to severe cognitive dysfunction in the later stage. Action observation therapy (AOT) is a multisensory cognitive rehabilitation technique where the patient initially observes the actions and then tries to perform. The study aimed to examine the impact of AOT along with usual physiotherapy interventions to reduce depression, improve cognition and balance of a patient with AD. A 67 years old patient with AD was selected for this study because the patient has been suffering from depression, dementia, and physical dysfunction along with some other health conditions like diabetes and hypertension. Before starting intervention, a baseline assessment was done through the Beck Depression Inventory (BDI) tool, the Mini-Cog Scale, and the Berg Balance Scale (BBS). The patient received 12 sessions of AOT along with usual physiotherapy interventions thrice a week for four weeks, which included 45 minutes of each session. After four weeks of intervention, the patient demonstrated significant improvement in depression, cognition, and balance, whereas the BDI score declined from moderate 21/63 to mild 15/63 level of depression. The Mini-Cog score improved from 2/5 to 4/5, and the BBS score increased from 18/56 to 37/56. It is concluded that AOT along with usual physiotherapy intervention helps to reduce depression, improve cognition and balance of people with AD.

Background: Following coronary artery bypass grafting (CABG), delirium emerges as a prevalent complication. This study aimed to assess the correlation between elevated serum cortisol levels and the occurrence of postoperative delirium subsequent to off-pump CABG. Methods: Conducted in the Department of Cardiac Surgery at BSMMU from October 2020 to September 2022, this comparative cross-sectional study included a total of 44 participants. Subjects, meeting specific criteria, were purposefully assigned to two groups based on off-pump CABG. Group A (n = 22) consisted of patients with normal serum cortisol levels, while Group B (n = 22) comprised individuals with high serum cortisol levels on the first postoperative day. Delirium onset was assessed at the bedside in the ICU on the 1st, 2nd, 3rd, 4th, and 5th postoperative days using standard tools, namely the Richmond Agitation Sedation score (RASS) and The Confusion Assessment Method (CAM-ICU). Data were collected based on the presence or absence of delirium. Statistical analysis utilized SPSS version 26.0, employing an independent Student’s t-test for continuous data and chi-square and Fischer’s exact test for categorical data. A p-value ≤ 0.05 was considered statistically significant. Results: Group-A had a mean age of 54.50 ± 17.97, and Group-B had a mean age of 55.22 ± 15.45, both with a male predominance (81.81% and 86.36% respectively). The mean serum cortisol level was significantly higher in Group B (829.71 ± vs. 389.98 ± 68.77). Postoperative delirium occurred in 27.3% of Group B patients, statistically significant compared to the 4.5% in Group A. However, patients in Group B who developed delirium experienced significantly longer postoperative ICU and hospital stays (79.29 ± 12.27 vs. 11.44 ± 2.85, p ≤ 0.05). In this paper,we present a comprehensive system model for Industrial Internet of Things(IIoT)networks empowered by Non-Orthogonal Multiple Access(NOMA)and Mobile Edge Computing(MEC)*** network comprises essential components such as base stations,edge servers,and numerous IIoT devices characterized by limited energy and computing *** central challenge addressed is the optimization of resource allocation and task distribution while adhering to stringent queueing delay constraints and minimizing overall energy *** system operates in discrete time slots and employs a quasi-static approach,with a specific focus on the complexities of task partitioning and the management of constrained resources within the IIoT *** study makes valuable contributions to the field by enhancing the understanding of resourceefficient management and task allocation,particularly relevant in real-time industrial *** results indicate that our proposed algorithmsignificantly outperforms existing approaches,reducing queue backlog by 45.32% and 17.25% compared to SMRA and ACRA while achieving a 27.31% and 74.12% improvement in Qn ***,the algorithmeffectively balances complexity and network performance,as demonstratedwhen reducing the number of devices in each group(Ng)from 200 to 50,resulting in a 97.21% reduction in complexity with only a 7.35% increase in energy *** research offers a practical solution for optimizing IIoT networks in real-time industrial settings.

Effect of Trimetazidine on Functional Capacity in Patient with Ischaemic Cardiomyopathy (TOFCAPI)

Objective: This study aimed to evaluate the efficacy of trimetazidine on exercise capacity via a six-minute walk test in patients with ischaemic cardiomyopathy and also evaluate the effect of trimetazidine on left ventricular function via echocardiography in the same population. Methods: This prospective observational study, conducted at the National Institute of Cardiovascular Diseases in Dhaka, Bangladesh, enrolled 200 patients with ischaemic cardiomyopathy and a depressed left ventricular ejection fraction (LVEF Results: In this study (n = 200) of ischaemic cardiomyopathy patients, the mean age was 58 years, with 76% of the patients being male. All study subjects received GDMT (Guideline-Directed Medical Therapy) for angina and heart failure. Those who received the modified released form of trimetazidine developed lesions during the 1st and 2nd follow-ups, during which the LVEF, LVIDd, and six-minute walk distance significantly improved (p Conclusion: The findings of the present study demonstrated that the addition of modified-release trimetazidine to GDMT can improve exercise capacity and left ventricular function in patients with ischaemic cardiomyopathy.

In recent years,speculation of an increase in Internet Gaming Disorder(IGD)has surfaced with the growing popularity of internet gaming among Chinese children and *** detrimental impact of IGD on mental health cannot be denied,even though only a small portion of the screen-dependent population exhibits psychopathological and behavioral *** present study aimed to explore a latent proﬁle analysis(LPA)of Internet Gaming Disorder on the mental health of Chinese school *** data were collected from a sample of 1005 Chinese school students(49.8%male;age M=13.32,SD=1.34 years)using a paper-pencil survey through convenience *** explored three latent proﬁles of internet gamers:regular gamers(62.4%),moderate gamers(28.1%),and probable disordered gamers(9.4%).Results showed that the probable disordered gamers had signiﬁcantly higher levels of depression,anxiety,emotional and conduct problems,hyperactivity,and peer problem symptoms as well as lower life satisfaction,and pro-social symptoms compared to regular and moderate gamers(p<0.05).This study would be helpful to mental health professionals in designing interventions for gamers who present IGD *** longitudinal studies should also be undertaken to assess whether mental health worsens for probable disordered gamers.

Is the Sundarbans of Bangladesh in a State of Pollution?

The Sundarbans is the world’s most extensive natural mangrove forest and home to various natural resources. The population in the vicinity has increased, causing more dependency on the resources of the Sundarbans. The increasing industrialization, urbanization, aquaculture, intensive agricultural practices, seaports, tourism facilities, and so on in the peripheral areas of the Sundarbans have made significant changes in the surrounding and upstream land uses of the Sundarbans. This situation may have detrimental influences on the ecosystem components of the Sundarbans. Therefore, it is highly demanded to prepare a piece of baseline information or database of different sources of pollution and their present status in the various components of the Sundarbans. This effort helps to identify issues and concerns, determine the key elements of the ecosystem to monitor the level or overall quality of the Sundarbans ecosystem. The present study systematically collects the potential sources of pollution, types, and current levels in the ecosystem components of the Sundarbans using academic databases, libraries, and online resources. Discharge of industrial waste into water, soil and air, heavy metal pollution, use of agrochemicals, oil (refined and crude) pollution, plastic materials from urban areas, and tourism are the major issues and concerns related to the sustainability of the Sundarbans ecosystem. The air quality of the Sundarbans is in good condition with 0 - 50 AQI of Bangladesh. While BOD, COD, TDS, TSS varied from 2.0 to 3.8 mg/L, 21.6 to 416 mg/L, 146.9 to 24,100 mg/L and 54 to 155 mg/L, respectively. Soil EC, organic carbon, total nitrogen, and total phosphorus ranged from 3.01 - 5.82 mS/cm, 1.41% - 2.69%, 0.51 - 1.05 mg/g, and 0.32 - 0.51 mg/g respectively. The air, water and soil quality parameters varied with the sites and seasons and not much at the state of contamination.
