High platelet distribution width can independently predict testicular survival in testicular torsion among patients with steady-state sickle cell anemia

Objective This study aimed to evaluate the predictive value of platelet volume indices(PVI),such as mean platelet volume(MPV),platelet distribution width(PDW),and plateletcrit(PCT),as prognostic indicators of testicular viability in torsion patients with steady-state sickle cell anemia(SCA)who underwent surgical *** Forty-eight patients with SCA with testicular torsion and 46 male control subjects were enrolled in the *** patients underwent scrotal color Doppler ultrasonography before surgery,and PVI(MPV,PDW,and PCT)values were measured in all *** duration and testicular volume were also *** The testicular salvage rate in patients with SCA was 73%after *** showed that MPV,PDW,and PCT values were significantly higher in torsed SCA as compared with controls(p<0.05).Orchiectomy in patients with SCA showed significantly higher MPV,PDW,and PCT values than the orchiopexy group(p<0.05).The MPV values of orchiectomy patients showed a higher significant cut-off of≥11.5 fL,which is higher than in torsed patients without SCA,as an indicator of testis *** also demonstrated a higher significant cut-off of≥12.7 fL for detorsion outcomes in patients with *** duration of less than 7 hours was also significantly correlated with orchiopexy(p≤0.001).Univariate analysis showed that higher MPV,increased PDW,and symptom duration were indicative of the outcome of testicular detorsion in *** analysis showed that increased PDW and symptom duration are prognostic parameters for testicular viability in *** Increased PDW and symptom duration can be used as parameters for predicting testicular detorsion outcomes in patients with steady-state SCA.

Application of microalgal-ZnO-NPs for reusing polyester/cotton blended fabric wastes after modification by cellulases enzymes

Polyester/cotton(PET/C)blended fabric wastes are produced daily in huge amounts,which constitutes an economic loss and an environmental threat if it is not reused *** textile waste recycling technologies put much effort into developing fabric materials with unique properties,such as bioactivity or new optical goods based on modern technologies,especially *** this study,zinc oxide nanoparticles(ZnO-NPs)were biosynthesized using the aqueous extract of Dunaliella *** immobilized on PET/C waste fabrics after enzymatically activated with *** produced Dunaliella-ZnO-NPs(10–20 nm with a spherical shape)were characterized by High-resolution transmission electron microscopy(HRTEM),Fourier-transform infrared spectroscopy(FTIR),X-Ray diffraction analysis(XRD),and Scanning electron microscopy-energy dispersive X-ray analyzer(SEM-EDAX),and some functional groups,such as CH,CO,NH,and CN(due to the presence of carboxyl,proteins and hydroxyl groups),were detected,revealing the biosynthesis of *** analysis showed that the resulting ZnO-NPS had potent antimicrobial effects,Ultraviolet(UV)protection capabilities,and no cytotoxic effects on the normal human fibroblast cell line(BJ1).On the other hand,enzymatic treatments of PET/C fabric waste with cellulases enhanced the immobilization of biosynthetic nanoparticles on their *** PET/C fabrics loaded with Dunaliella-ZnO-NPs showed antibacterial and UV protection capabilities making them an eco-friendly and cost-effective candidate for numerous *** applications can include the manufacture of active packaging devices,wastewater treatment units,and many other environmental applications.

BACKGROUND Helicobacter pylori(***)is a significant human pathogen that is responsible for a variety of illnesses,including mucosa-associated lymphoid tissue lymphoma,gastric cancer,peptic ulcers,and *** To investigate the frequency of *** infection and its resistance patterns among Egyptian patients and to determine the influence of *** virulence genetic determinants on the eradication success of 14-d triple therapy *** *** infections were investigated in 72 patients with gastroduodenal complications suggestive of *** *** cagA and vacA genotypes of cultured strains were studied using polymerase chain *** patients underwent 14 d of triple-therapy *** treatment response was examined using histology and a rapid urease test 6 wk after therapy *** The intention-to-treat eradication rate was 59.2%(95%CI:48.2%-70.3%).Rates of *** resistance to clarithromycin,amoxicillin,and metronidazole were 52.8%,81.9%,and 100%,*** eradication of *** was more significantly associated with vacA s1-positive strains[adjusted odds ratio(aOR)=0.507,95%CI:0.175-0.822].A significant association was found between failed eradication rate and *** strains resistant to clarithromycin(aOR=0.204,95%CI:-0.005 to 0.412)and amoxicillin(aOR=0.223,95%CI:0.026-0.537).CONCLUSION This study’s low *** eradication rate following 14-d triple therapy is concerning and *** pan-resistance to metronidazole followed by the high resistance to ciprofloxacin,amoxicillin,and clarithromycin in this research is challenging and of great concern.

Volumetric Overload Shocks: Why Is Starling’s Law for Capillary Interstitial Fluid Transfer Wrong? The Hydrodynamics of a Porous Orifice Tube as Alternative

Based on clinical and experimental work two new types of volumetric overload shocks are reported: volumetric overload shock type one and type two depending on the type of fluid causing their induction. Volumetric overload shock type one is induced by sodium-free fluids such as glycine, glucose, mannitol and sorbitol and is characterized with acute dilutional hyponatraemia. Volumetric overload shock type 2 is induced with sodium-based fluids normal saline and plasma substitutes used for resuscitation of the critically ill and has no serological marker. It presents with the multiple vital organs dysfunction or failure syndrome or the adult respiratory distress syndrome. Hypertonic sodium is an effective treatment when given early adequately. The underlying pathophysiology is discussed. An alternative to Starling’s law for the capillary interstitial fluid transfer is given. Hydrodynamic of a porous orifice tube akin to capillary with a surrounding Chamber akin to the interstitial fluid space demonstrated a rapid dynamic magnetic field-like fluid circulation between the surrounding chamber and the lumen of the G tube that represent an adequate replacement for Starling’s law.

The Influence of Welding Parameters on Brittle Fracture of Liquefied Natural Gas Storage Tank Welded Joint

Many applications operate at sufficiently low temperature conditions where most structural steels become very brittle and, therefore, unsuitable for use in safety-critical structures. So the materials used in the vessels or storage tanks which keep the natural gas at liquefaction temperatures need to remain ductile and crack resistant with a high level of safety. The material also needs to have high strength in order to reduce the wall thickness of the container and it must permit welding without any risk of brittle fracture. 9% Ni steel plates are one of most common used materials in the LNG storage tank application. However, the welding procedure for 9% Ni steel plates requires high level of skills of welding that is strictly controlled welding parameter for balancing avoidance of cold and hot cracking and maintenance of high strength. Mechanical properties are important characteristics of the weldment that must confirm to the application feasibility as well as functional requirements of the welded joint. The only way to enhanced the mechanical properties of welded joint by controlling the parameters of using welding process. From the main variables of the arc welding process are the heat input and interpass temperature where the two variables control the thermal cycle of welding process. The experiment show that for thin test specimen with thickness ≤ 14 mm, the heat input range from 1.4 to 2 KJ/mm and controlling interpass temperature within 80°C give high tensile strength with improving the toughness properties of welded joint and reduce the probability of brittle fracture happened by increase the ductility and reduce the yield strength and increased the transition temperature.

The effect of treatment package time in head and neck cancer patients treated with adjuvant radiotherapy and concurrent systemic therapy

Objectives:In patients with head and neck carcinoma,“treatment package time”(TPT)was proven to impact outcomes in cases receiving adjuvant radiotherapy *** impact in patients receiving radiotherapy with concurrent systemic therapy has not been studied *** TPT influence on survival endpoints for patients treated with surgery followed by radiation and concurrent systemic therapy was ***:Institutional database to identify head and neck carcinoma cases treated with definitive surgery followed by concomitant chemo(bio)radiotherapy(CRT)was *** was the number of days elapsed between surgery and the last day of radiation.%FINDCUT SAS macro tool was used to search for the cutoff TPT that was associated with significant survival ***-Meier curves,log-rank tests as well as univariate and multivariate analyses were used to assess overall survival(OS)and recurrence free survival(RFS).Results:One hundred and three cases with a median follow up of 37 months were included in the *** tumors were 43%,oral cavity 40%and laryngeal 17%of *** systemic therapy included platinum and cetuximab in 72%and 28%,*** TPT was found to be<100 days with significantly better OS(P=0.002)and RFS(P=0.043)compared to TPT≥100 *** multivariate analysis;TPT<100 days,extracapsular nodal extension,high-risk score,iymphovascular space and perineural invasion were independent predictors for worse OS(P<0.05).T4,extracapsular nodal extension and high-risk score were all significantly detrimental to RFS(P<0.05).Conclusions:Addition of concomitant systemic therapy to adjuvant radiotherapy did not compensate for longer TPT in head and neck squamous cell *** coordinated care must be provided to ensure the early start of CRT with minimal treatment breaks.

Objective:Early treatment of acute ischemia of the heart by performing immediate percutaneous coronary intervention(PCI)to restore blood fl ow in patients with the clinical presentation of an acute coronary syndrome and more specifi cally with ST-elevation myocardial infarction(STEMI)can save *** study aims to identify the mean time(door to balloon time and fi rst contact to balloon time)to primary PCI for STEMI patients and to assess the percentage of primary PCI and its success rate in ***:A registry study of patients presenting to cardiac centers in Egypt was designed,where patients’basic characteristics,the treatment strategy,and the door to balloon time and the fi rst contact to balloon time were ***:One thousand six hundred fi fty STEMI patients with a mean age of 57 years were included in the *** transfer for primary PCI was the most used treatment strategy,representing 74.6%of all treatment strategies *** door to balloon time and the fi rst contact to balloon time were 50 and 60 minutes,respectively,with a primary PCI success rate of 65.1%.Conclusion:The registry study results showed a marked improvement by implementation of the best treatment strategy with respect to the time factor to achieve a better outcome for STEMI patients in Egypt.

A statistically representative questionnaire targeted people using rainwater harvesting(RWH)techniques in rural communities of Sarida catchment,West Bank,Palestine was distributed and *** main objective of this study is to assess the social,economic,and environmental impacts of adopting RWH techniques(***,concrete and clay ponds,Wadi ponds,earth dams,and stone terraces)in different uses to increase water *** results showed a simple sharing of the female component among beneficiaries,while concrete ponds and cisterns were the most used ***,social impacts were noticeable by sharing the same RWH structure and reflected to responsibility skills and role exchange *** the other hand,RWH techniques showed a significant economic impact for end users represented by enhancing domestic,agricultural,and recreational activities leading to good profit *** addition to food security as output,the most important environmental impact was water wasting prevention,which in turn could be linked to sustainable water management and considered as universal challenge for future generations.
