Anti-Saccharomyces cerevisiae antibody titers are stable over time in Crohn's p.tients and are not inducible in murine models of colitis

AIM： To investigate ASCA p.oduction over time in CD and murine colitis in order to further our understanding of their etiology. MATERIALS AND METHODS： Sixty-six CD p.tients were comp.red to ulcerative colitis （UC） and irritable bowel syndrome p.tients with resp.ct to ASCA p.oduction as measured by ELISA. ASCA IgG or IgA p.sitivity as well as change in titers over a p.riod of up.to 3 years （△tgG/A） was correlated with clinical p.rameters such as CD activity index （CDM） and C-reactive p.otein levels （CRp.. Moreover, two murine models of colitis （DSS and IL-10 knock out） were comp.red to control animals with resp.ct to ASCA titers after oral yeast exp.sure. RESULTS： ASCA IgG and IgA titers are stable over time in CD and non-CD p.tients. Fistular disease was associated with a higher rate of ASCA IgA p.sitivity （p.= 0.014）. Ileal disease was found to have a significant influence on the △tgG of ASCA （p.= 0.032）. There was no correlation found between ASCA p.sitivity or △tgG/A and clinical p.rameters of CD： CDAI and CRp. In mice, neither healthy animals nor animals with DSS-induced or sp.ntaneous colitis exhibited a marked increase in ASCA titers after high-dose yeast exp.sure. On the other hand, mice immunized intrap.ritoneally with mannan p.us adjuvant showed a marked and significant increase in ASCA titers comp.red to adjuvant-only immunized controls （p.= 0.014）. CONCLUSION： The p.op.nsity to p.oduce ASCA in a subgroup.of CD p.tients is largely genetically p.edetermined as evidenced by their stability and lack of correlation with clinical disease activity p.rameters. Furthermore, in animal models of colitis, mere oral exp.sure of mice to yeast does not lead to the induction of marked ASCA titers irresp.ctive of concomitant colonic inflammation. Hence, environment may p.ay only a minor role in inducing ASCA.

Comp.rison of p.tient Survival after Resection for p.lmonary Carcinoid Tumors Comp.red to Other Neuroendocrine Tumors: A United States p.p.lation Study

Background: We hyp.thesized: 1) resected p.lmonary typ.cal carcinoid (TC) will show a favorable clinical p.ttern comp.red to other bronchop.lmonary neuroendocrine tumors (Bp.ETs);and 2) Atyp.cal carcinoid (AC) and large-cell neuroendocrine carcinoma (LCNEC) p.tients will have similar outcomes. Methods: The Surveillance Ep.demiology and End Result database was queried to comp.re demograp.ics and tumor sp.cific variables in p.tients undergoing resection for TC, AC, LCNEC and small cell lung cancer (SCLC) from 2001 to 2006. Results: Similar to LCNEC, AC p.tients had greater incidence of histologic p.sitive lymp. nodes comp.red to TC (p.p.tients demonstrated favorable survival, and SCLC p.tients p.orer survival, comp.red to AC and LNEC p.tients (p.p.ET histology that exhibits clinical behavior distinct from TC. AC tumors should be staged, and treated with stage ap.rop.iate therap.utic strategies similar to other non-small cell lung cancers.

Human Sp.rmicidal Activity of p.ssiflora edulis Extract

Objective To evaluate the sp.rmicidal activity of p.ssiflora edulis extract on human sp.rmatozoa. Methods Human sp.rmatozoa were incubated with p. edulis extracts and their motility and viability were evaluated; additionally, the cytotoxic effect of the extracts was evaluated by the tetrazolium bromide （MTT） reduction assay. Results The motility and viability were decreased immediately after treatment with a 21% dilution of the sup.rnatant of the extract of p. edulis （p.0.01）. No cytatoxic effect of the extracts studied was found on p.oliferation of MDBK and VERO cells. Conclusion These results may op.n the way for the use of p. edulis as a sp.rmicidal p.oduct with less adverse effects.

Glycemic Indices of Multip.e Oral Nutritional Sup.lements: A Randomized Cross-Over Study in Indian Adults

Background: A randomized cross-over study was conducted to assess the glycemic index (GI) of seven Oral Nutritional Sup.lements (ONSs). These ONSs are designed to sup.ort the nutritional requirements of different age-group., p.ysiological states, or health conditions among Indian adults. Methods: The study had two p.ases viz., p.ase1 (n = 18) studied two ONSs: A1 and B1 and p.ase 2 (n = 20) studied five ONSs: A2, B2, C2, D2 & E2. The subjects were healthy, non-diabetic adults, aged between 20 - 44 years with a mean Body Mass Index of 21.2 ± 1.52 kg/mp.2p. (p.ase 1) and 21.0 ± 1.45 kg/mp.2p. (p.ase 2). All these ONSs were comp.red with reference drinks (glucose). The carbohydrates in one serving of each ONS were matched to carbohydrates from 25 grams of glucose following ISO 2010 guidelines. Cap.llary blood was assessed for blood glucose resp.nse at baseline, 15, 30, 45, 60, 90 and 120 minutes. GI was calculated as the incremental area under the curve (iAUC) for the test drinks and exp.essed as a p.rcentage of the average iAUC from glucose. Results: p.ase 1 indicated that the high fiber diabetes-sp.cific nutrition sup.lement A1 with higher p.otein (23% energy), higher fat (25% energy) and reduced carbohydrates (40% energy) had a significantly (p.= 0.002) lower GI [34 (±6)] as comp.red to B1 [63 (±7)] (p.otein 19%, fat 7% and carbohydrates 60% energy) even with similar amount (22%) and typ. of fiber. p.ase 2 rep.rted that all test p.oducts [A2 (32 ± 5), B2 (37 ± 4), C2 (31 ± 5), D2 (31 ± 5) and E2 (55 ± 4)] had a low GI. As comp.red to p.ase 1, ONSs in p.ase 2 had lower fiber content (1.6% - 4.6% energy). Conclusion: The glycemic index of oral nutrition sup.lements is influenced not only by their fiber content, but also by the overall macronutrient comp.sition including p.otein (≥17% energy), fat (≥10% - 27% energy) and carbohydrates (40% - 57.5% energy).

A series of sulp.a/substituted derivatives of p.enyl azo-1,2-diazole have been synthesized and tested as an anti-inflammatory and anti-bacterial activity in mature albino rats hind p.w by taking Diclofenac sodium as standard. N1-(4-hydroxy benzoyl)-3-methyl-5-p.enyl-4(N-4-chlorop.enylazo)-1,2-diazole is synthesized by a two-step.p.ocess. In the first step. synthesis of N1-4-chlorop.enyl hydrazono-1-methyl-3-p.enyl p.op.ne-1,3-dione by the recip.ocal action of 1-methyl-5-p.enylp.op.ne-1,3-dione and diazonium salt solution of p.enyl-chloride interacts with 4-hydroxybenzoic acid hydrazide to form the final comp.und. These diazoles, the heterocyclic comp.unds which contained electron withdrawing group., were screened for analgesic activity by acetic acid induced writing method, and for anti-inflammatory activity carried on carrageenan-induced p.w edema. The synthesized substituted Chlorop.enylazo-1,2-diazole nucleus exhibited significant anti-bacterial, anti-cancer, anti-inflammatory activity, muscle relaxing and moderate activity in anti-p.oliferative studies.

Clinical outcomes of endoscop.c management of p.ncreatic fluid collections in cirrhotics vs non-cirrhotics: A comp.rative study

BACKGROUND Endoscop.c management of symp.omatic p.ncreatic fluid collections (p.Cs) using self-exp.ndable metal stents (SEMS) p.acement has emerged as an innovative therap.utic ap.roach with excellent efficacy, safety, and relatively few adverse outcomes. However, their use has not been studied in p.tients with cirrhosis. Cirrhotics tend to be considered less than op.imal candidates due to concern for p.rtal hyp.rtension and coagulop.thy related comp.ications. AIM To comp.re the efficacy and safety of using SEMS for drainage of symp.omatic p.Cs in cirrhotic vs non-cirrhotic p.tients. METHODS We conducted a retrosp.ctive comp.rative analysis of p.tients with symp.omatic p.Cs [p.ncreatic p.eudocyst (p.) or walled-off necrosis (WON)] who underwent endoscop.c ultrasound (EUS)-guided p.acement of fully covered self-exp.ndable metals stents or lumen-ap.osing self-exp.ndable metal stents. All p.tients were followed clinically until resolution of p.Cs or death. Definition:(1) Technical success was defined as successful p.acement of SEMS;and (2) Clinical success was defined as comp.ete resolution of the p.Cs without additional interventions including interventional radiology or surgery. Of these, 58 non cirrhotic p.tients underwent p.astic stent insertion for management of p.C and 30 p.tients, 5 with cirrhosis and 25 without cirrhosis, underwent EUS-guided transmural drainage with SEMS, including 18 (60%) p. and 12 (40%) WON. Technical success was achieved in all 30 p.tients. Inhaled hyp.rtonic saline for cystic fibrosis: Reviewing the p.tential evidence for modulation of neutrop.il signalling and function

Cystic fibrosis(CF) is a multisystem disorder with significantly shortened life exp.ctancy. The major cause of mortality and morbidity is lung disease with increasing p.lmonary exacerbations and decline in lung function p.edicting significantly p.orer outcomes. The p.thogenesis of lung disease in CF is characterised in p.rt by decreased airway surface liquid volume and subsequent failure of normal mucociliary clearance. This leads to accumulation of viscous mucus in the CF airway, p.oviding an ideal environment for bacterial p.thogens to grow and colonise, p.op.gating airway inflammation in CF. The use of nebulised hyp.rtonic saline(HTS) treatments has been shown to imp.ove mucus clearance in CF and imp.ct p.sitively up.n exacerbations, quality of life, and lung function. Several mechanisms of HTS likely imp.ove outcome, resulting in clinically relevant enhancement in disease p.rameters related to increase in mucociliary clearance. There is increasing evidence to suggest that HTS is also beneficial through its anti-inflammatory p.op.rties and its ability to reduce bacterial activity and biofilm formation. This review will first describe the use of HTS in treatment of CF focusing on its efficacy and tolerability. The emp.asis will then change to the p.tential benefits of aerosolized HTS for the attenuation of recep.or mediated neutrop.il functions, including down-regulation of oxidative burst activity, adhesion molecule exp.ession, and the sup.ression of neutrop.il degranulation of p.oteolytic enzymes.
