To the Editor The report of the 1992 Atlanta consensus conference provided a framework for standardization of the terminology for description of acute pancreatitis and its complications[1].Terms such as“pancreatic phlegmon”which had historically been subjected to differential interpretation were discontinued and modern terminology was introduced[1].With subsequent clinical usage and with the accrual of new knowledge it became clear that further changes were necessary.

AIM: To measure a broad profile of pro-and anti-inflammatory cytokines in patients with clinically proven chronic pancreatitis (cP) taking either antioxidant therapy or placebo as part of the larger ANTIcIPATE study. METHODS: Patients with chronic pancreatitis were recruited to the ANTIcIPATE study following informed consent and were randomised to intervention with either antox version 1.2-based antioxidant therapy or placebo. After a separate ethics committee amendment a subgroup of 7 patients from either arm of the study were selected for additional analysis of cytokines. cytokines were measured at baseline and after 6 mo of either antox therapy or placebo by biochip array and enzyme-linked immunosorbent assay. RESULTS: Antioxidant therapy and placebo groups were well-matched in terms of age, gender, aetiology of cP, opiate use and disease duration. Baseline antioxidant levels were similar in patients allocated to the antioxidant group as compared to the group al- located to placebo. After 6 mo of antioxidant therapythere was significant elevation in vitamin c levels in the intervention group: 17.6 μg/mL (12.8-29.3 μg/mL) compared to 4.8 μg/mL (1.6-9.1 μg/mL) in placebo (P cI: 9.0-20.2) with similar trends in selenium levels. There was no elevation in a broad array of pro-and anti-inflammatory cytokines in the antioxidant group compared to placebo [interleukin (IL)-1B, IL-4, IL-6, IL-10, tumor necrosis factor-α] either at baseline or after 6 mo of antioxidant therapy. cONcLUSION: cytokine levels were low at baseline and at 6 mo despite a significant elevation in plasma antioxidants. In patients with cP, with opiate-dependent abdominal pain, circulating cytokine levels are low suggesting that pain in this disease is not simply a manifestation of inflammation.

AIM： To validate an optimized ^13c-urea breath test （^13c-UBT） protocol for the diagnosis of H pylori infection that is cost-efficient and maintains excellent diagnostic accuracy. METHODS： 70 healthy volunteers were tested with two simplified ^13c-UBT protocols, with test meal （Protocol 2） and without test meal （Protocol 1）. Breath samples were collected at 10, 20 and 30 rain after ingestion of 50 mg ^13c-urea dissolved in 10 mL of water, taken as a single swallow, followed by 200 mL of water （pH 6.0） and a circular motion around the waistline to homogenize the urea solution. Performance of both protocols was analyzed at various cut-off values. Results were validated against the European protocol. RESULTS： According to the reference protocol, 65.7% individuals were positive for H pylori infection and 34.3% were negative. There were no significant differences in the ability of both protocols to correctly identify positive and negative H pylori individuals. However, only Protocol 1 with no test meal achieved accuracy, sensitivity, specificity, positive and negative predictive values of 100%. The highest values achieved by Protocol 2 were 98.57%, 97.83%, 100%, 100% and 100%, respectively.cONcLUSION： A 10 min, 50 mg ^13c-UBT with no test meal using a cut-off value of 2-2.5 is a highly accurate test for the diagnosis of H pylori infection at a reduced cost.

Having pre-existing comorbidities is described as a risk factor for more severe disease in adult corona virus disease 2019(cOVID-19)and in infections with SARS-coV-1 and MERS-coV[1].In adult SARS-coV-2 infections,patients with pre-existing underlying comorbidities,such as chronic obstructive pulmonary disease,cardiovascular disease,diabetes and obesity,are more likely to have severe disease compared to healthy adults[2].An inconsistency is seen in current fndings on the association with comorbidities and pediatric cOVID-19 *** important limitation in currently available studies is limited data:severe disease is rare in children compared to adults,and most studies describe cOVID-19 severity merely by reporting intensive care unit(IcU)admission or mortality rates instead of detailed data on clinical presentation and outcomes.

Mammalian sperm must undergo a series of biochemical and physiological modifications, collectively called capacitation, in the female reproductive tract prior to the acrosome reaction （AR）. The mechanisms of these modifications are not well characterized though protein kinases were shown to be involved in the regulation of intracellular ca2＋ during both capacitation and the AR. In the present review, we summarize some of the signaling events that are involved in capacitation. During the capacitation process, phosphatidyl-inositol-3-kinase （PI3K） is phosphorylated/activated via a protein kinase A （PKA）-dependent cascade, and downregulated by protein kinase c a （PKca）. PKca is active at the beginning of capacitation, resulting in PI3K inactivation. During capacitation, PKca as well as PP172 is degraded by a PKA-dependent mechanism, allowing the activation of PI3K. The activation of PKA during capacitation depends mainly on cyclic adenosine monophosphate （cAMP） produced by the bicarbonate-dependent soluble adenylyl cyclase. This activation of PKA leads to an increase in actin polymerization, an essential process for the development of hyperactivated motility, which is necessary for successful fertilization. Actin polymerization is mediated by PIP2 in two ways： first, PIP2 acts as a cofactor for phospholipase D （PLD） activation, and second, as a molecule that binds and inhibits actin-severing proteins such as gelsolin. Tyrosine phosphorylation of gelsolin during capacitation by Src family kinase （SFK） is also important for its inactivation. Prior to the AR, gelsolin is released from PIP2 and undergoes dephosphorylation/activation, resulting in fast F-actin depolymerization, leading to the AR.

Polyreactive innate-type B cells account for many B cells expressing self-reactivity in the periphery. Improper regulation of these B cells may be an important factor that underlies autoimmune disease. Here we have explored the influence of self-reactive innate B cells in the development of collagen-induced arthritis （cIA）, a mouse model of rheumatoid arthritis. We show that splenic marginal zone （MZ）, but not B- 1 B cells exhibit spontaneous IgM reactivity to autologous collagen II in na＇=＇ve mice. Upon immunization with heterologous collagen II in complete Freund＇s adjuvant the collagen-reactive MZ B cells expanded rapidly, while the B-1 B cells showed a modest anti-collagen response. The MZ B cells were easily activated by toll-like receptor （TLR） 4 and 9-1igands in vitro, inducing proliferation and cytokine secretion, implying that dual engagement of the B-cell receptor and TLRs may promote the immune response to self-antigen. Furthermore, collagen-primed MZ B cells showed significant antigen-presenting capacity as reflected by cognate T-cell proliferation in vitroand induction of IgG anti-collagen antibodies in vivo. MZ B cells that were deficient in complement receptors I and 2 demonstrated increased proliferation and cytokine production, while Fcy receptor lib deficiency of the cells lead to increased cytokine production and antigen presentation. In conclusion, our data highlight self-reactive MZ B cells as initiators of the autoimmune response in cIA, where complement and Fc receptors are relevant in controlling the self-reactivity in the cells.

Background: Grit is used by birds mainly for grinding hard food items but can also serve a nutritional role as a source of minerals. Ingestion of grit by birds has been documented primarily in species that feed on seeds and *** feeding mainly on nectar and small arthropods, hummingbirds also ingest grit, but why they do so is unclear. We quantified the number of grit particles in the stomachs of six species of hummingbirds during an annual cycle in a seasonal ecosystem of West ***: We compared the number of grit particles in the stomachs of different hummingbird species(Mexican Violetear colibri thalassinus, Amethyst-throated Mountaingem Lampornis amethystinus, White-eared Hummingbird Basilinna leucotis, Rivoli's Hummingbird Eugenes fulgens, Broad-tailed Hummingbird Selasphorus platycercus, and Rufous Hummingbird S. rufus), and between sex and age categories during the different seasons of a year. To deter-mine if grit was used to grind ingested arthropods, we examined the relationships between the number of grit parti-cles, the biomass of arthropods ingested, and their chitin ***: Although species did not differ in the number of grit particles in their stomachs, we found that grit was mostly ingested by female individuals, with only one male of one species(Mexican Violetear) presenting grit in its stomach. We also found that female hummingbirds had grit in their stomachs during the rainy and the cold-dry season(June-February) but not during the warm-dry season(March-May). Our analyses revealed no relationship between the number of grit particles and the amount of ingested arthropods and arthropod chitin content. However,high grit consumption was related to wasp ingestion on Mexican ***: Our results indicate that grit is used mainly by female hummingbirds. This study explored the thermo-chemical properties of industrial hemp hurd with different provenances,maturity stages,and retting *** findings were then compared to hemp hurd used in the fabrication of citric acid-bonded ultra-low-density hemp hurd ***-gas chromatography-mass spectrometry(Py-Gc/MS),Fourier-transform infrared spectroscopy(FTIR),and thermogravimetric analysis(TGA)were employed to document the variability of the hurd and comprehend the potential impact on biobased composite *** choice of cultivar,maturity stage,and processing modality significantly influenced the chemical composition,presence of functional groups,and thermal stability of the ***-Gc/MS revealed substantial variations in the lignin-to-carbohydrate(L/c)ratio,along with the absence of fatty acids in certain *** FTIR signals confirmed consistent functional groups,differences in peak intensities were indicative of carbohydrate variations associated with maturity and retting duration,impacting the availability of hydroxyl groups for,i.e.,interparticle bonding in citric acid-based ***,it was observed that shorter retting durations initially enhanced the thermal resistance,but prolonged retting led to accelerated degradation,significantly reducing the hurd’s residual *** findings indicated notable differences among the samples,emphasizing the importance of investigating variables such as provenance/cultivar,maturity,and processing *** assessment is essential to ensure effective agronomic practices that align the raw material characteristics with the specific requirements of intended applications,such as the fabrication of biobased hemp hurd composites.
