Saline intrusion is a hot issue and has always been of concern in the VMD(Vietnamese Mekong Delta),especially in the context of many changes of impact factors such as upstream flows and SLR(Sea Levels Rise).Vulnerability to changes in the upstream flows and SLR ismust-have reasons for updated and interpreted *** information is used for exploiting of soil and water *** 11 model was successfully applied to assess the saline *** study provided the picture of the saline intrusion in the dry season from January to may in the VMD in the existing situation(2015 and 2016)and the future(2030 and 2050)under the impact of flow at Kratie in various frequencies of 18%,50%and 85%based on the time series of 2001-2016,and SLR according to RCP(Representative Concentration Pathway)4.5 scenario of MONRE(Ministry of Natural Resources and Environment)of Vietnam issued in *** results show that in the year 2015 the ASI(Saline Intrusion Area)in the VMD was relatively low due to moderate tidal level and high Kratie discharge(P=18%).The scenario like the situation in 2016 and in the future ASI increased significantly compared to the 2015 baseline scenario which shows that the VMD is very vulnerable to saline *** on multivariate regression analysis,the study also presented the formulas for the relationship between the ASI of 0.25 g/L,2.5 g/L and 4.0 g/L thresholds and the impact factors such as the average discharge at Kratie and the maximum daily tidal level in East Coast during the dry season from January to *** an adjusted R2 at 0.913-0.974,these formulas are believed to be reliable for predicting ASIs based on the Kratie flow and the East Coast tidal level.

Nutrient stimulation of mesenteric blood flow-implications for older critically ill patients

Nutrient ingestion induces a substantial increase in mesenteric blood flow. In older persons(aged ≥ 65 years), particularly those with chronic medical conditions, the cardiovascular compensatory response may be inadequate to maintain systemic blood pressure during mesenteric blood pooling, leading to postprandial hypotension. In older ambulatory persons, postprandial hypotension is an important pathophysiological condition associated with an increased propensity for syncope, falls, coronary vascular events, stroke and death. In older critically ill patients, the administration of enteral nutrition acutely increases mesenteric blood flow, but whether this pathophysiological response is protective, or precipitates mesenteric ischaemia, is unknown. There are an increasing number of older patients surviving admission to intensive care units, who are likely to be at increased risk of postprandial hypotension, both during, and after, their stay in hospital. In this review, we describe the prevalence, impact and mechanisms of postprandial hypotension in older people and provide an overview of the impact of postprandial hypotension on feeding prescriptions in older critically ill patients. Finally, we provide evidence that postprandial hypotension is likely to be an unrecognised problem in older survivors of critical illness and discuss potential options for management.

目的:探讨在急性氧化应激状态下人晶状体上皮细胞株SRA01/04内衰老标记蛋白30(SMP30)、Caspase-3、Ca^2+-ATP表达的变化。方法:通过慢病毒转染建立SMP30过表达(OE)组、过表达空载对照(NC-OE)组、SMP30沉默(KD)组及沉默空载(NC-KD)组SRA01/04细胞,RT-PCR和Western blotting检测SMP30含量鉴定细胞是否转染成功;用300μmol/L H2O2 作用2 h,构建急性氧化应激状态时,PCR检测各组细胞SMP30基因的表达情况,Western blotting检测各组细胞SMP30、Caspase-30和Ca^2+-ATP蛋白的表达量。结果:在正常状态时,OE组SMP30基因及其蛋白表达量高于NC-OE组(P<0.05),KD组的SMP30基因及其蛋白表达量低于NC-KD组(P<0.05)。OE组及KD组细胞SMP30基因表达量均出现增加(P<0.05),与正常状态时相比,急性氧化应激状态时OE组细胞SMP30蛋白表达上调(P<0.05);正常状态时,OE组Caspase-3蛋白表达量低于NC-OE组,KD组Caspase-3蛋白表达量高于NC-KD组(P<0.05)。急性氧化应激状态时,OE组Caspase-3蛋白表达量低于NC-OE组,KD组Caspase-3蛋白表达量低于NC-KD组(P<0.05)。在急性氧化应激状态时OE组Caspase-3蛋白表达量低于正常状态时;KD组的Caspase-3蛋白表达量高于正常状态时KD组(P<0.05)。在正常状态时,OE组Ca^2+-ATP蛋白表达量高于NC-OE组,KD组Ca^2+-ATP蛋白表达量低于NC-KD组(P<0.05)。在急性氧化应激状态时,OE组Ca^2+-ATP蛋白表达量高于NC-OE组,KD组Ca^2+-ATP蛋白表达量低于NC-KD组(P<0.05)。在急性氧化应激状态时OE组的Ca^2+-ATP蛋白表达量高于正常状态时(P<0.05)。结论:在急性氧化应激状态时,SMP30含量出现反应性增加,过表达SMP30可能对人晶状体上皮细胞株SRA01/04具有抗氧化、抗凋亡、增加钙调节活性的作用。

Treatment 2.0 Pilot in Vietnam—Early Progress and Challenges

Announced in 2010 at the International AIDS Conference in Vienna and pioneered by the Joint United Nations Programme on HIV/AIDS (UNAIDS) and the World Health Organization's (WHO) leadership at the global level, Treatment 2.0 is a new approach to the HIV response that encourages innovation, efficiency, and sustainability. Building upon WHO and UNAIDS' '3 by 5' Initiative, Treatment 2.0 focuses upon scale-up and universal access of life-saving ART treatment through strategic investments and innovations in five priority pillars that include: 1) Optimize drug regimens, 2) Provide point-of-care (POC) and other simplified diagnostic and monitoring tools, 3) Reduce costs, 4) Adapt service delivery, and 5) Mobilize communities [1-3]. The Treatment 2.0 approach is in line with UNAIDS' 2011-2015 Strategy: Getting to Zero with the vision of, 'Zero New infections;Zero discrimination;and Zero AIDS-related deaths', and as well as the four strategic directions for the health sector response outlined under WHO's Global Health Sector Strategy on HIV/AIDS 2011-2015 [4,5]. At the Sixty-fourth World Health Assembly (WHA) in may 2011, it was formally announced that Vietnam has taken the leadership to pilot Treatment 2.0 in two of its provinces, with support from both the WHO and UNAIDS country offices [6,7]. Given that Vietnam is one of the few countries with a concentrated epidemic to pilot Treatment 2.0, the outcomes and experiences of this initiative can provide valuable insight to other countries who may consider implementation. The objectives of this article are therefore to: 1) Describe the early process for translating Treatment 2.0 concept in Vietnam's context;and 2) Highlight early progress and challenges.

Background In Viet Nam,tuberculosis(TB)represents a devastating life-event with an exorbitant price tag,partly due to lost income from daily directly observed therapy in public sector ***,persons with TB may seek care in the private sector for its flexibility,convenience,and *** study aimed to measure income changes,costs and catastrophic cost incurrence among TB-affected households in the public and private *** Between October 2020 and March 2022,we conducted 110 longitudinal patient cost interviews,among 50 patients privately treated for TB and 60 TB patients treated by the National TB Program(NTP)in Ha Noi,Hai Phong and Ho Chi Minh City,Viet *** a local adaptation of the WHO TB patient cost survey tool,participants were interviewed during the intensive phase,continuation phase and *** compared income levels,direct and indirect treatment costs,catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate *** The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort(USD 868 vs USD 578;P=0.010).However,private sector treatment was also significantly costlier(USD 2075 vs USD 1313;P=0.005),driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants(USD 754 vs USD 164;P<0.001).This resulted in no significant difference in catastrophic costs between the two cohorts(Private:55%vs NTP:52%;P=0.675).Factors associated with catastrophic cost included being a single-person household[adjusted odds ratio[(a OR=13.71;95%confidence interval(CI):1.36-138.14;P=0.026)],unemployment during treatment(a OR=10.86;95%CI:2.64-44.60;P<0.001)and experiencing TB-related stigma(a OR=37.90;95%CI:1.72-831.73;P=0.021)].Conclusions Persons with TB in Viet Nam face similarly high risk of catastrophic costs whether treated in the public or private *** costs coul

Some clinical and epidemiological studies have suggested that Alzheimer’s Disease (AD) and cataract, may share common pathogenetic mechanisms, subsequently a positive association between the prevalence of AD and cataract, although other studies found no significant relationship between dementia and visual impairment including cataract in the elderly. Little is known about the association between Activity of Daily Living (ADL) and the combination of AD and cataract. To examine the association between ADL and the combination of AD and cataract, we performed a national survey in nursing care institutions in Japan, examining the decreased ADL in elderly with and without AD and cataract for 453 elderly aged 85.0 ± 8.13 years. The proportion, 43.5% of AD in subjects without cataract was significantly higher than that, 23.5% with cataract. Almost all ADL in AD with cataract was significantly lower than that without cataract, although all ADL in cataract patients receiving surgery in AD was significantly (all p < 0.002) higher than that in cataract patients without surgery in AD, as was confirmed by the multiple regression analysis incorporating into the relevant factors as independent variables. These results showed that ADL scores including cognitive functions decreased by cataract were increased by the surgery in the patients with AD. A significantly negative association between AD and cataract seen in our data, which was inconsistent with the previous result, might lead to the necessities of the clinical diagnosis for slight severity of cataract for patients with AD.

甘蔗工艺成熟期转化酶及其抑制子与蔗糖积累的相关性研究

引言 Wingard观察烟环斑病毒在烟的接种叶上产生环斑之后,系统感染的叶上也出现环斑,但此后新生叶上的环斑逐渐趋向集中在叶尖部分,终于在再次新生的叶上没有病状了。他在这些已无病状的新生叶上再接种病毒时却不能再出现病状,但是用这些已无病状的叶汁接种健康植株的叶上却产生典型的环斑,这结果说明虽然已无病状但仍有病毒。他认为这是一种免疫现象。Price重复证明了上述的观察,并且注意到这种复原现象的环境条件相当广;用复原植株所作扦插的植株上再接种病毒也不再出现病状,但扦插
