Gastric emptying evaluation by ultrasound prior colonoscopy:An easy tool following bowel preparation

AIM: To investigate the gastric emptying after bowel preparation to allow general anaesthesia. METHODS: A prospective, non-comparative, and nonrandomized trial was performed and registered on Eudra CT database(2011-002953-80) and on ***(NCT01398098). All patients had a validated indication for colonoscopy and a preparation using sodium phosphate(NaP) tablets. The day of the procedure, patients took 4 tablets with 250 mL of water every 15 min, three times. The gastric volume wasestimated every 15 min from computed antral surfaces and weight according to the formula of Perlas et al(Anesthesiology, 2009). Colonoscopy was performed within the 6 h following the last ***: Thirty patients were prospectively included in the study from November 2011 to May 2012. The maximum volume of the antrum was 212 mL, achieved 15 min after the last intake. 24%, 67% and 92% of subjects had an antral volume below 20 mL at 60, 120 and 150 min, respectively. 81% of patients had a Boston score equal to 2 or 3 in each colonic segment. No adverse events leading to treatment discontinuation were ***: Gastric volume evaluation appeared to be a simple and reliable method for the assessment of gastric emptying. Data allow considering the NaP tablets bowel preparation in the morning of the procedure and confirming that gastric emptying is achieved after two hours, allowing general anaesthesia.

Toward an easier indigocarmine chromoendoscopy

Indigocarmine chromoendoscopy has been proven to improve the detection of colonic lesions during screening colonoscopy, and is associated with increased adenoma detection rates. Furthermore, it is commonly used to help in the delineation and characterization of colorectal neoplasms. However, it usually requires the use of a spraying catheter that decreases the suction capacity of the endoscope, and is time- consuming. Herein, we report on the feasibility of indigo carmine chromoendoscopy during colonoscopy without using a spraying catheter, with the dye being administered through the air/water channel of the endoscope. Since the suction channel remains free, the air can be exsufflated and the staining then applies uniformly onto the colonic walls with the excess indigocarmine dye being immediately eliminated. In our experience with various types of colonoscopes and cap-assisted colonoscopy, this procedure makes indigocarmine chromoendoscopy much easier and quicker to perform, and might save the use of a spray catheter.

Optimization of the generator settings for endobiliary radiofrequency ablation

AIM:To determine the optimal generator settings for endobiliary radiofrequency ablation. METHODS:Endobiliary radiofrequency ablation was performed in live swine on the ampulla of Vater,the common bile duct and in the hepatic parenchyma. Radiofrequency ablation time,"effect",and power were allowed to vary. The animals were sacrificed two hours after the procedure. Histopathological assessment of the depth of the thermal lesions was performed. RESULTS:Twenty-five radiofrequency bursts were applied in three swine. In the ampulla of Vater(n = 3),necrosis of the duodenal wall was observed starting with an effect set at 8,power output set at 10 W,and a 30 s shot duration,whereas superficial mucosal damage of up to 350 μm in depth was recorded for an effect set at 8,power output set at 6 W and a 30 s shot duration. In the common bile duct(n = 4),a 1070 μm,safe and efficient ablation was obtained for an effect set at 8,a power output of 8 W,and an ablation time of 30 s. Within the hepatic parenchyma(n = 18),the depth of tissue damage varied from 1620 μm(effect = 8,power = 10 W,ablation time = 15 s) to 4480 μm(effect = 8,power = 8 W,ablation time = 90 s). CONCLUSION:The duration of the catheter application appeared to be the most important parameter influencing the depth of the thermal injury during endobiliary radiofrequency ablation. In healthy swine,the currently recommended settings of the generator may induce severe,supratherapeutic tissue damage in the biliary tree,especially in the high-risk area of the ampulla of Vater.

We demonstrate a Ⅲ-Ⅴ-on-silicon-nitride mode-locked laser through the heterogeneous integration of a semiconductor optical amplifier on a passive silicon-nitride cavity using the technique of micro-transfer printing. In the initial phase of our study, we focus on optimizing the lasing wavelength to be centered at 1550 nm. This optimization is achieved by conducting experiments with 27 mode-locked lasers, each incorporating optical amplifiers featuring distinct multiple-quantum-well photoluminescence values. Subsequently we present a comprehensive study investigating the behavior of the mode-locking regime when the electrical driving parameters are varied. Specifically, we explore the impact of the gain voltage and saturable absorber current on the locking stability of a tunable mode-locked laser. By manipulating these parameters, we demonstrate the precise control of the optical spectrum across a wide range of wavelengths spanning from 1530 to 1580 nm. Furthermore, we implement an optimization approach based on a Monte Carlo analysis aimed at enhancing the mode overlap within the gain region. This adjustment enables the achievement of a laser emitting a 23-nm-wide spectrum while maintaining a defined 10 dB bandwidth for a pulse repetition rate of 3 GHz.

Production of Wood Fibers from Thermally Treated Wood

The reduction of the hygroscopicity of wood fibers was investigated through a Thermal Treatment(TT)on wood chips performed before the defibering *** TT and defibering tests were both carried out on a continuous pilot at semi-industrial *** thermal treatment study of wood chips,equivalent to a low temperature pyrolysis,was achieved for four conditions(280°C–320°C)for a duration of 10 *** quantification of solids,condensables and gases(FTIR)at the outcome of the thermal treatment allowed to achieve the mass balances for each *** increase of the reactor temperature from 280°C to 320°C leads to a lower solid yield(94%–82%)while gaseous(1%–3.8%)and condensable(3%–11%)products increase *** treated wood samples were afterwards successfully defibered in different conditions to produce suitable fibers for insulation panel *** aim of the study is also to evaluate the effects of the TT on the lowering of energy consumption necessary for the defibering process while producing good quality *** consumption during defibering process shows a significant decrease with the increase the TT *** morphology is affected by TT and the morphological quality of the fibers decreases as TT severity *** mass percentage of dust was also quantified as a quality marker of produced *** of equilibrium moisture(at 20°C and 65%RH)of the different materials(wood chips before and after TT,produced fibers)show a significant effect of the TT on wood chips hygroscopicity(8.2%for untreated wood to 4.1%for TT at 320°C).However,the effect of the TT on the hygroscopicity reduction of thermally treated wood fibers is drastically less significant due to breaking of the wood structure during defibering process.

Parallel personal measurements of diesel engine exhaust and crystalline silica using dual sampling port

Diesel engine exhaust(DEE) and crystalline silica exposures occur simultaneously in the mining industry,and occupational sampling campaigns can be time-and cost-consuming. The authors evaluated a dualport system for simultaneous sampling of DEE and crystalline silica in laboratory and field *** tests evaluated the operation of pumps during 8 h sampling and the intensity of the flow variation for various filter loading conditions and for different modes of operation. Field validation was performed in an underground mine. Pumps operated in constant flow or constant pressure *** in constant flow mode showed that when the flowrate increased on one side of the system, it decreased on the opposite side according to the loading intensity. Tests in constant pressure mode showed that flowrates systematically decreased when using loaded cassettes. However, the higher the backpressure setting, the lower the flow variation was. Flow variations during field tests were generally within the acceptable ±5% range. However, significant flow variations were identified in higher concentrations. A significant negative correlation was found between flowrate variation and total carbon concentration. While the majority of tests support the use of the dual-port for evaluating concomitant exposures, results highlight the possibility of filter overloading as a cause of flowrate changes.

Exposure to diesel engine exhaust(DE) is a major concern in underground mines. It has been linked to cardiopulmonary diseases and is classified as a human carcinogen. The goal of this study is to assess DE exposures in workers at two underground gold mines, to compare exposure levels within and between the mines, and to compare different methods of measuring DE exposures, namely respirable combustible dust(RCD), elemental carbon(EC) and total carbon(TC). Ambient and personal breathing zone(PBZ) measurements were taken. Side-by-side monitoring of RCD and of the respirable fraction of EC and TC(EC_Rand TC_R) was carried out in the workers' breathing zone during full-shift *** ambient measurements, in addition to EC_R, TC_Rand RCD, a submicron aerosol fraction(less than 1 mm) of EC and TC was also sampled(EC_1and TC_1). Average ambient results of 240 mg/m^3 in RCD, 150 mg/m^3 in EC_Rand 210 mg/m^3 in TC_Rare obtained. Average PBZ results of 190 mg/m^3 in RCD,84 mg/m^3 in EC3Rand 150 mg/min TC_Rare obtained. Very good correlation is found between EC_Rand EC_1 with a Pearson correlation coefficient of 0.99(p < 0.01) calculated between the two logtransformed concentrations. No differences are reported between EC_Rand EC_1, nor between TC_Rand TC_1, since ratios are equal to 1.04, close to 1, in both cases. Highest exposures are reported for loadhaul-dump(LHD) and jumbo drill operators and conventional miners. Significant exposure differences are reported between mines for truck and LHD operators(p < 0.01). The average TC_R/EC_Rratio is 1.6 for PBZ results, and 1.3 for ambient results. The variability observed in the TC_R/EC_Rratio shows that interferences from non-diesel related organic carbon can skew the interpretation of results when relying only on TC data.

Low Cost Filtration of Domestic Wastewater for Irrigation Purpose

Water scarcity in developing countries has forced farmers to use sewage as an alternative source of irrigation water. However, the usage of sewage for vegetable production has been known to cause excessive and often-unbalanced addition of nutrients hence posing a threat to food safety. The objective of this study was to determine the efficacy of slow sand filter and wetland plant in domestic wastewater treatment. To achieve this objective, samples were collected from the domestic wastewater collection pond within Jomo Kenyatta University of Agriculture and Technology (JKUAT). Laboratory tests were conducted on the collected samples and they revealed the presence of BOD, DO, pH, TDS, Sulfates, Chloride, Turbidity, Salinity, Conductivity, Alkalinity?and Coliform;whose values varied when compared with that of the parameters for standard irrigation water. This gave insight to the kind of treatments and filtration medium that were required to transform domestic wastewater into water fit for irrigation. A slow sand filter bed was designed and constructed using precisely six samples materials;sand, sand and wetland plants, gravel, gravel and wetland plants, mixture of gravel and sand, mixture of gravel and sand with wetland plants. These materials were used to identify the chemical and biological changes in domestic wastewater within a seven-day period. The water collected from the slow sand filter was tested, results showed that, of all six samples, slow sand filter using the mixture of gravel, sand with wetland plants had an average percentage efficient of 90% in removing all impurities from domestic wastewater thereby turning it into water suitable for irrigation. It is hoped that this study will provide a safe, easy, eco-friendly and cheap method of wastewater treatment while ensuring the sustainability of wastewater for irrigation and the expansion of green spaces in urban and peri-urban areas.

How botulinum toxin in neurogenic detrusor overactivity can reduce upper urinary tract damage?

Intradetrusor injections of botulinum toxin are the cornerstone of medical treatment of neurogenic detrusor overactivity. The primary aim of this treatment is to ensure a low pressure regimen in the urinary bladder, but the mechanisms leading to long-term protection of the urinary tract remain poorly understood. In this paper, we highlight the potential benefits of intradetrusor injections of botulinum toxin regarding local effects on the bladder structures, urinary tract infections, stone disease, vesico ureteral reflux, hydronephrosis, renal function based on a comprehensive literature review.
