Background:The epidemiological data on post-burn growth,body composition and motor devel-opment is ambiguous and *** aim of this systematic review was therefore to summarize the current body of evidence on post-burn growth,body composition and motor development in ***:A literature search was conducted in PubMed,EMBASE and Web of Science up to March *** considered observational studies that reported(1)metrics on weight,height,body composition,bone mineral content,bone mineral density or motor development,in(2)paediatric burn patients and(3)published in a peer-reviewed ***:A total of 16 studies were *** of the included studies used quantitative methods,but with differing methodology:prospective cohort studies(n=8),retrospective chart reviews(n=3),case-control studies(n=2),cross sectional studies(n=2)and a retrospective cohort study(n=1).When combined,the included studies represented 2022 paediatric burn patients,with a mean age of 7.7(±3.2)*** average burn sizewas 52.8%(±12.7)of the total body surface *** outcome measures included weight(n=12),height(n=7),muscular strength(n=4),bone mineral content(n=5),bone mineral density(n=5),body mass index(n=3),fat mass(n=5),lean body mass(n=7)and fine and gross motor development(n=1).Conclusions:Following an initial decline,patients’growth and motor development started to recover during the first or second year ***,burns may have a profound and prolonged effect on the paediatric burn patients’muscular strength,bone mineral content and lean body *** should be noted that the vast majority of studies included only patients with burns covering≥30%total body surface *** evidence presented in this review may thus not be representative of the whole paediatric burn population.

Variceal bleeding is one of the major causes of death in cirrhotic *** management during the acute phase and the secondary prophylaxis is well *** recommendations(2015 Baveno VI expert consensus)are available and should be followed for an optimal management,which must be performed as an emergency in a liver or general intensive-care *** is based on the early administration of a vasoactive drug(before endoscopy),an antibiotic prophylaxis and a restrictive transfusion strategy(hemoglobin target of 7 g/dL).The endoscopic treatment is based on band *** should be *** the most severe patients(Child Pugh C or B with active bleeding during initial endoscopy),transjugular intrahepatic portosystemic shunt(TIPS)should be performed within 72 hours after admission to minimize the risk of *** prophylaxis is based on the association of non-selective beta-blockers(NSBBs)and repeated band *** should be considered when bleeding reoccurs in spite of a well-conducted secondary prophylaxis or when NSBBs are poorly *** should also be considered when bleeding is *** transplantation should be discussed when bleeding is not controlled after TIPS insertion and in all cases when liver function is deteriorated.

To drain or not to drain after distal pancreatectomy: reflexions on the PANdORINA trial

We read with great interest the PANdORINA study(1).This international multicenter(mostly from the Netherlands)randomized controlled trial aimed at determining whether omitting drainage after distal pancreatectomy(dP)was equivalent in terms of complications to systematic drainage *** primary outcome was the occurrence of severe complications at 90 days defined as dindo-Clavien grade≥*** main secondary outcome was the occurrence of grade B–C postoperative pancreatic fistula(POPF)at 90 *** both outcomes,the non-inferiority threshold was set at 8%.Randomization was performed intraoperatively,stratified by annual center volume(high volume defined as dP≥40/year)and the risk of POPF according to the distal fistula risk score(d-FRS)combining pancreas thickness(high risk,>19 mm)and main pancreatic duct diameter(high risk,>3 mm)measured both at the isthmus(2).

Prognosis of operated hepatocellular carcinoma developed on non-cirrhotic liver

The incidence of primary liver cancers,and particularly hepatocellular carcinoma(HCC),is increasing throughout the *** vast majority of HCCs develop on cirrhotic livers(CLs)(CL HCC)or livers chronically infected by hepatotropic viruses,mainly hepatitis B virus(HBV)and hepatitis C virus(HCV)(1).As HCC occurring on non-HBV non-HCV non-CL(NCL)HCC occurs much less frequently,its management is less established than for CL ***,many clinicians might confuse the prognosis and management of NCL HCC with the one of CL *** is however demonstrated that NCL HCCs behave differently than CL *** prognostic factors and sensibility to systemic therapy are different,added to the fact that NCLs can undergo major resections allowing curative surgical management or even systemic therapies that are not even possible in CL HCCs due to the failing function of the *** established criteria for liver transplantation of CL HCCs,including the famous Milan criteria,have no role in NCL HCC management(2-4).The Barcelona Clinic Liver Cancer(BCLC)strategy for prognosis prediction and treatment recommendation also concerns only CL HCCs;and should not be used for NCL HCCs(5).

Pharmaceutical co-crystals have been explored by many researchers as a strategy to optimize physicochemical properties of solid-state *** improvements of solubility,bioavailability,and the reduced tendency for phase transformation occurrence,are factors that highlight benefits of pharmaceutical co-crystals among other solid *** to the Biopharmaceutical Classification System(BCS),non-steroidal anti-inflammatory drugs(NSAIds)are class Ⅱ drugs,which have low aqueous solubility and therefore co-crystallization has the potential to optimize NSAId product *** this review,we highlight the recent progress made on NSAIds co-crystals,their co-formers,synthesis,methods and use,while we underline some promising results on in vitro and in vivo co-crystal properties.A celecoxib-tramadol co-crystal reaches phase Ⅲ clinical trials,showing greater analgesic activity than both individual *** aqueous solubility of the co-crystal formed between l-proline and diclofenac is very high in comparison with the pure *** co-crystals with urea and thiourea have an increase of drug release of almost 60%.Co-crystal design brings a new perspective in drug development since the co-former used can also be a biologically active component allowing to combine different anti-inflammatory drugs,which have an incredible spectrum of application due to the analgesic,anti-pyretic and anti-inflammatory properties.

dispersal and its evolution play a key role for population persistence in fragmented landscapes where habitat loss and fragmentation increase the cost of between-habitat *** such contexts,it is important to know how variation in dispersal and other traits is structured,and whether responses to landscape fragmentation are aligned with underlying dispersal-trait correlations,or dispersal ***,therefore,studied trait variation in Erigone longipalpis,a European spider species specialist of(often patchy)salt *** collected spiders in two salt-marsh landscapes differing in habitat *** then reared lab-born spiders for two generations in controlled conditions,and measured dispersal and its association with various key *** longipalpis population densities were lower in the more fragmented *** this,we found no evidence of differences in dispersal,or any other trait we studied,between the two *** a dispersal syndrome was present at the among-individual level(dispersers were more fecund and faster growing,among others),there was no indication it was genetically driven:among-family differences in dispersal were not correlated with differences in other ***,we showed that the observed phenotypic covariations were mostly due to within-family *** hypothesize that the dispersal syndrome is the result of asymmetric food access among siblings,leading to variation in development rates and carrying over to adult *** results show we need to better understand the sources of dispersal variation and syndromes,especially when dispersal may evolve rapidly in response to environmental change.

Vocal communication is a crucial aspect of animal behavior. The mechanism which most mam- mals use to vocalize relies on three anatomical components. First, air overpressure is generated in- side the lower vocal tract. Second, as the airstream goes through the glottis, sound is produced via vocal fold vibration. Third, this sound is further filtered by the geometry and length of the upper vocal tract. Evidence from mammalian anatomy and bioacoustics suggests that some of these three components may covary with an animal＇s body size. The framework provided by acoustic al- Iometry suggests that, because vocal tract length （VTL） is more strongly constrained by the growth of the body than vocal fold length （VFL）, VTL generates more reliable acoustic cues to an animal＇s size. This hypothesis is often tested acoustically but rarely anatomically, especially in pinnipeds. Here, we test the anatomical bases of the acoustic allometry hypothesis in harbor seal pups Phoca vitulina. We dissected and measured vocal tract, vocal folds, and other anatomical features of 15 harbor seals post-mortem. We found that, while VTL correlates with body size, VFL does not. This suggests that, while body growth puts anatomical constraints on how vocalizations are filtered by harbor seals＇ vocal tract, no such constraints appear to exist on vocal folds, at least during puppy- hood. It is particularly interesting to find anatomical constraints on harbor seals＇ vocal tracts, the same anatomical region partially enabling pups to produce individually distinctive vocalizations.

经过数年对神经电极微尺寸技术的研究,研究人员期望获得对神经组织更小的侵入性伤害,但是现在微尺寸技术对降低植入‘损伤的性能改善已经触及了机械损伤的极限。最近一些新的电极形状设计（正弦型形状）为减小电极植入的侵入性伤害、提高电极记录性能开辟了新的可能性。本文旨在继续探索这种新的电极设计方法,并使其适应最新的神经组织刺激技术:光遗传学技术。本文的目标是找到导光性能良好的柔性形状。首先开展设计可行性研究:形状、材料和制作工艺。在形状和制作工艺层面主要遵循已经发表的有关正弦型电极的结论。这部分还要求必须为光学导向器找到具有良好柔韧性、生物相容性和适宜折射率的芯/包层材料。出于以上考虑,研究中放弃了将Parylene-C/Parylene-N组合作为芯/包层材料的选择。其次,使用两种不同的方法进行光学仿真:光束和光线追踪方法。结果显示了不同的形状参数对光传导的影响。由此,选择了若干组光传导性能较好的形状设计。最后,将在光学仿真软件中找到的各种形状进行力学仿真,得到表征其柔韧性的数据,将其与正弦型电极的侵入性数据进行比较。由于研究中保持了与正弦型电极相同的全局性设计,可以得出新设计电极的侵入性伤害的结论。本文最后得出了一组光学正弦型电极的形状参数、机械和光学特征,理论上可以在改善长期植入电极的植入损伤与记录性能。
