Bridging the mental health treatment gap:effects of a collaborative care intervention(matrix support)in the detection and treatment of mental disorders in a Brazilian city

Objective To analyse temporal trends in diagnosis and treatment of mental disorders in primary care following implementation of a collaborative care intervention(matrix support).Design Dynamic cohort design with retrospective time-series *** secondary data on medical visits to general practitioners of all study clinics were extracted from the municipal electronic records *** changes in the odds of mental disorders diagnoses and antidepressants prescriptions were estimated by multiple logistic regression at visit and patient-year levels with diagnoses or prescriptions as *** changes during two distinct stages of the intervention(stage 1 when it was restricted to mental health(2005-2009),and stage 2 when it was expanded to other areas(2010-2015))were compared by adding year-period interaction terms to each *** 49 primary care clinics in the city of Florianópolis,*** All adults attending primary care clinics of the study setting between 2005 and *** 3131983 visits representing 322100 patients were *** visit level,the odds of mental disorder diagnosis increased by 13%per year during stage 1(OR 1.13,95%CI 1.11 to 1.14,p<0.001)and decreased by 5%thereafter(OR 0.95,95%CI 0.94 to 0.95,p<0.001).The odds of incident mental disorder diagnoses decreased by 1%per year during stage 1(OR 0.99,95%CI 0.98 to 1.00,p=0.012)and decreased by 7%per year during stage 2(OR 0.93,95%CI 0.92 to 0.93,p<0.001).The odds of antidepressant prescriptions in patients with a mental disorder diagnosis increased by 7%per year during stage 1(OR 1.07,95%CI 1.05 to 1.20,p<0.001);this was driven by selective serotonin reuptake inhibitor prescriptions which increased 14%per year during stage 1(OR 1.14,95%CI 1.12 to 1.18,p<0.001)and 9%during stage 2(OR 1.09,95%CI 1.08 to 1.10,p<0.001).The odds of incident antidepressant prescriptions did not increase during stage 1(OR 1.00,95%CI 0.97 to 1.02,p=0.665)and increased by 3%during s

Designing lighting for zero-net energy buildings poses unique and fascinating design *** the challenge of providing lighting loads on the order of.1W/sqft2,these projects require toolsets beyond the typical scope of a light-ing design consultant,including detailed controls design and photometric pre-dictions of available *** new roles are part of a broader trend in lighting design as,driven by new code requirements and a general push towards high-performing buildings,lighting design is becoming a more technical,metrics-driven ***’s a delicate balancing act here,and indeed we have the same responsibility to these projects as any other:lighting that is comfortable and functional for the occupants,that supports the architectural gesture,and that is available within the project *** the task is not simply to design the absolute minimum energy density(although yes,also that),but rather to identify ways in which all of these constraints may be harmonized,and embrace the collaborative and iterative workflow that these projects *** case study is written for architects,engineers,owners and others that want an overview of the lighting design process for ultra-low energy density buildings.I’ll gloss both the absolute basics and the more esoteric technical details in favor of discussing practical techniques that we used successfully to further the project goals,and the trade-offs we confronted.

Reliability-based congestion pricing model under endogenous equilibrated market penetration and compliance rate of ATIS

A reliability-based stochastic system optimum congestion pricing(SSOCP) model with endogenous market penetration and compliance rate in an advanced traveler information systems(ATIS) environment was proposed. All travelers were divided into two classes. The first guided travelers were referred to as the equipped travelers who follow ATIS advice, while the second unguided travelers were referred to as the unequipped travelers and the equipped travelers who do not follow the ATIS advice(also referred to as non-complied travelers). Travelers were assumed to take travel time, congestion pricing, and travel time reliability into account when making travel route choice decisions. In order to arrive at on time, travelers needed to allow for a safety margin to their *** market penetration of ATIS was determined by a continuous increasing function of the information benefit, and the ATIS compliance rate of equipped travelers was given as the probability of the actually experienced travel costs of guided travelers less than or equal to those of unguided travelers. The analysis results could enhance our understanding of the effect of travel demand level and travel time reliability confidence level on the ATIS market penetration and compliance rate; and the effect of travel time perception variation of guided and unguided travelers on the mean travel cost savings(MTCS) of the equipped travelers, the ATIS market penetration, compliance rate, and the total network effective travel time(TNETT).

Colonies of ants allocate exploratory individuals to where they are ecologically needed

Individual differences in behavior have large consequences for the way in which ecology impacts *** differ in how they explore their environment and how exploratory behavior benefits *** group-living animals,behavioral heterogeneity can be beneficial because different individuals perform different *** example,exploratory individuals may discover new food sources and recruit group members to exploit the food,while less exploratory individuals forgo the risks of *** we ask how individual variation in exploratory behavior affects the ability of Argentine ant Linepithema humile colonies to(1)locate novel food sources,(2)exploit known food resources,and(3)respond to disruptions while *** address these questions,we conducted field experiments on *** foraging trails in which we manipulated food availability near and at the foraging trails and disrupted the foraging *** sampled individuals based on their response to the perturbations in the field and tested their exploratory behavior in the *** found that exploratory individuals benefit the colony by locating novel foods and increasing resource exploitation,but they do not play an important role in the recovery of a foraging trail after ***,the benefits of behavioral heterogeneity to the group,specifically in exploratory behavior,differ across ecological contexts.
