Efficacy of penile low-intensity shockwave treatment for erectile dysfunction:correlation with the severity of cavernous artery disease

We analyzed the efficacy of penile low-intensity extracorporeal shockwave treatment for erectile dysfunction(ED)combined with cavernous artery disease(CAD).ED was evaluated by the International Index of Erectile Function,subdividing patients into mild and moderate/severe *** was assessed using penile color Doppler ***(n=111)with a positive outcome after treatment,based upon the minimal clinically important difference of the International Index of ED,were followed up for 3 months and 6 *** found a significant mean increase in the index of erectile function,with an overall improvement in hemodynamic parameters of the cavernous *** particular,93.9%of the patients with mild ED without CAD responded to treatment and 72.7%resumed normal erectile *** 31.2%of the patients with moderate/severe ED and CAD responded to treatment,and none resumed normal erectile *** patients with mild ED and no CAD maintained the effects of therapy after 3 months,while no patients with moderate/severe ED and CAD maintained the benefits of treatment after 3 ***,patients with mild ED and no CAD have better and longer lasting responses to such treatment,with a higher probability of resuming normal erectile function than patients with moderate/severe ED and CAD.

Hepatocellular carcinoma is one of the most frequent malignant tumors worldwide:Portal vein tumor thrombosis(PVTT)occurs in about 35%-50%of patients and represents a strong negative prognostic factor,due to the increased risk of tumor spread into the bloodstream,leading to a high recurrence *** this reason,it is a contraindication to liver transplantation and in several prognostic scores sorafenib represents its standard of care,due to its antiangiogenetic action,although it can grant only a poor prolongation of life *** scientific evidences lead to consider PVTT as a complex anatomical and clinical condition,including a wide range of patients with different prognosis and new treatment possibilities according to the degree of portal system involvement,tumor biological aggressiveness,complications caused by portal hypertension,patient’s clinical features and tolerance to antineoplastic *** median survival has been reported to range between 2.7 and 4 mo in absence of therapy,but it can vary from 5 mo to 5 years,thus depicting an extremely variable *** this reason,it is extremely important to focus on the most adequate strategy to be applied to each group of PVTT patients.

To the Editor:Chylothorax is a rare pleural effusion with a milky appearance due to triglycerides and chylomicrons[1];About 1%of cases are associated with liver cirrhosis[2].Incidentally,it has been hypothesized that chylothorax originates from the passage of chylous ascites into pleural cavity owing to diaphragmatic defects and negative thoracic pressure[3].Chylous ascites represents the 1%of ascites in liver cirrhosis(1:20,000 hospitalizations)[4].Pathophysiological mechanism is mediated by increased hepatic and gastrointestinal lymphatic flow due to portal hypertension[5]。

Background: Several international studies, confirmed in Italy too, show a hard presence of socio-relational problems inside the celiac population. Methods: Qualitative study involving persons with celiac disease and their families. 25 individual semi-structured interviews were conducted in three Italian regions. Results: Problems of management of social life for celiac persons are experienced, specially in the fields of: school, work, travels and life outside the home. Conclusion: Chronic illness is a “biographical disruption” [1] and the whole society has to be invested to fight the burden of celiac persons in their possibility of access to public life.

Cancer after transplantation is the third cause of death and one of the more relevant comorbidities. Aim of this review is to verify the role of different pathogenetic mechanisms in cancer development in transplant patients and in general population as well. In particular has been outlined the different role exerted by two different families of drug as calcineurin inhibitor and mammalian target of rapamycin(m TOR) inhibitor. The role of m TOR pathways in cell homeostasis is complex but enough clear. As a consequence the m TOR pathway deregulation is involved in the genesis of several cancers. Hence the relevant role of m TOR inhibitors. The authors review the complex mechanism of action of m TOR inhibitors, not only for what concerns the immune system but also other cells as endothelial, smooth muscle and epithelial cells. The mechanism of action is still now not completely defined and understood. It implies the inhibition of m TOR pathway at different levels, but mainly at level of the phosphorylation of several intracellular kinases that contribute to activate m TOR complex. Many prospective and retrospective studies in transplant patients document the antineoplastic role of m TOR inhibition. More recently m TOR inhibitors proven to be effective in the treatment of some cancers also in general population. Kidney cancers, neuroendocrine tumors and liver cancers seem to be the most sensitive to these drugs. Best results are obtained with a combination treatment, targeting the m TOR pathway at different levels.
