Cholecystoenteric fistula with and without gallstone ileus: A case series

Background: A cholecystoenteric stula (CEF) is an uncommon complication of gallstone disease. The aim of this study was to present our experience of a series of patients with CEF, presenting with or without gallstone ileus, along with their surgical outcomes. Methods: From 2015 to 2018, 3245 consecutive patients unde.went cholecystectomy for gallbladder disease at our institution, of which 15 were diagnosed with a CEF. All electronic medical records were retrospectively reviewed. Results: Fifteen patients presented with CEF. Ten patients presented cholecystoduodenal stula, four pa- tients cholecystocolonic, and one patient cholecystogastric counterparts. Twelve patients were female. The median patient age was 61 years (range 33 86 years). Five patients presented with gallstone ileus treated by laparotomy and enterolithotomy. In ten patients, a laparoscopic approach was attempted, but conversion to open surgery was necessary for eight of them. The median operative time was 140 min (range 60 240 min), and the median operative blood loss was 50 mL (range 10600mL). The procedure-related morbidity and mortality rates were 13.3% and 6.7%, respectively. Conclusions: There is no consensus on the best treatment modality for a CEF, as the treatment outcome is mostly depende.t on the surgeon’s expertise and the patient’s condition. not all CEFs are accompanied by gallstone ileus. For such case, the main purpose is to resolve the intestinal obstruction and, unless necessary, avoidance of the gallbladder area.

As China became the greatest outbound tourism market worldwide,many countries are designing strategies in order to attract Chinese tourists to their ***,Ecuador receives a small market share of 0,02%,which shows that has not been responsive and has not yet developed a tailor-made strategy for this new market;on the contrary is still invested in pursuing traditional markets as the main source of tourism *** this sense,it is imperative for Ecuador to seize the opportunity,and thus to develop a promotion strategy targeted to the Chinese market in order to increase the.number of arrivals,taking into consideration the case of Australia′s *** thesis does a comprehensive analysis of Australia′s Tourism promotion in the Chinese market,the implemented strategies of the past years and the result on arrival′s growth to this ***,it studies Australia through Butler′s Stage Model′s approach,and describes Australia′s marketing mix in order to give the reader a better unde.standing on the country′s tourism promotion ***,it gives insight into Ecuador′s tourism promotion strategies through the SWOT method and concludes by suggesting marketing strategies learned from the Australian experience in order to increase the share for the Chinese market.

Evaluation of Early Action Mechanisms in Peru Regarding Preparedness for El nino

In this article,we provide an impact evaluation of an intervention in Peru regarding preparedness for El nino impacts in Picsi District of Chiclayo Province in Peru’s northwestern coastal Lambayeque *** effort involved the provision of special kits that reduce the potential damage to homes as a consequence of rainfall and floods associated with an El nino-Southe.n Oscillation *** was collected in 2016 when this Forecast-based Financing early action was activated by an El nino forecast,and after a coastal El nino actually struck in *** dual database permits us to estimate the impact of the intervention on the damage level of homes by comparing those homes supported by the program with those homes not receiving pilot-program *** comparison is achieved by using propensity score matching techniques,which identify the most comparable homes to the ones that were supported by the *** main findings of the study suggest a positive impact of the program in terms of its effectiveness in mitigating the damage caused by the 2017 El ni?*** results suggest a drop in the scale of house damage(less damage)by around 63%for a home that received the modular kit *** considering other specifications of the model,the decrease in the scale of house damage improves up to approximately 66%.

Glioblastoma(GBM) is one of the deadliest tumors and has a median survival of 3 months if left untreated. Despite advances in rationally targeted pharmacological approaches, the clinical care of GBM remains palliative in intent. Since the majority of altered signaling cascades involved in cancer establishment and progression eventually affect cell cycle progression, an alternative approach for cancer therapy is to develop innovative compounds that block the activity of crucial molecules needed by tumor cells to complete cell division. In this context, we review promising ongoing and future strategies for GBM therapeutics aimed towards G2/M inhibition such as anti-microtubule agents and targeted therapy against G2/M regulators like cyclin-depende.t kinases,Aurora inhibitors, PLK1, BUB, 1, and BUBR1, and survivin. Moreover, we also include investigational agents in the preclinical and early clinical settings. Although several drugs were shown to be gliotoxic, most of them have not yet entered therapeutic trials. The use of either single exposure or a combination with novel compounds may lead to treatment alternatives for GBM patients in the.near future.

Immersible ergocycle prescription as a function of relative exercise intensity

Purpose: The purpose of this study was to establish the relationship between various expressions of relative exercise intensity percentage of maximal oxygen uptake(%VO_(2max)), percentage of maximal heart rate(%HR_(max)), %VO_2 reserve(%VO_2R), and %HR reserve(%HRR)) in order to obtain the more appropriate method for exercise intensity prescription when using an immersible ergocycle(IE) and to propose a prediction equation to estimate oxygen consumption(VO_2) based on IE pedaling rate(rpm) for an individualized exercise training ***: Thirty-three healthy participants performed incremental exercise tests on IE and dryland ergocycle(DE) at equal external power output(Pext). Exercise on IE began at 40 rpm and was increased by 10 rpm until exhaustion. Exercise on DE began with an initial load of 25 W and increased by 25 W/min until exhaustion. VO_2 was measured with a portable gas analyzer(COSMED K4b^2) during both incremental tests. On IE and DE, %VO_2R, %HRmax, and %HRR at equal Pext did not differ(p > 0.05).Results: The %HRR vs. %VO_2R regression for both IE and DE did not differ from the identity line %VO_2R IE = 0.99 × HRR IE(%) + 0.01(r^2= 0.91, SEE = 11%); %VO_2R DE = 0.94 × HRR DE(%) + 0.01(r^2= 0.94, SEE = 8%). Similar mean values for %HRmax, %VO_2R, and %HRR at equal Pext were observed on IE and DE. Predicted VO_2 obtained according to rpm on IE is represented by: VO_2(L/min) = 0.000542 × rpm2-0.026 × rpm + 0.739(r = 0.91, SEE = 0.319 L/min).Conclusion: The %HRR–%VO_2R relationship appears to be the most accurate for exercise training prescription on IE. This study offers new tools to better prescribe, control, and individualize exercise intensity on IE.

What is the prognosis of emphysematous pyelonephritis associated with extende.-spectrum beta-lactamases producing microorganisms?

Objective:To describe the microbiological characteristics in emphysematous pyelonephritis(EPn),demonstrate the frequency of extende.-spectrum beta-lactamase(ESBL)microorganisms,and determine if these microorganisms are associated with the prognosis of patients with ***:We conducted a retrospective study in patients with a diagnosis of EPn in a tertiary care hospital of the.northeast region of Mexico during the period from January 2011 to January *** variables were analyzed to determine association with the presence of ESBL-producing *** significance was set with pnts were included;55(87.3%)of them were females,with a median age of 55(interquartile range:45-65)*** management was indicated in 38.1%;42.9%were treated with ureteral stent;12.7%with open or percutaneous drainage;15.8%with early nephrectomy;and 9.5%with delayed *** mortality was 13(20.6%)cases;23(36.5%)cases required admission to the intensive care *** most frequent microorganism isolated was Escherichia coli(n=34,53.9%).ESBL microorganisms were found in 31.7%of the *** significant association of ESBL was found with admission to the intensive care unit,or with increased ***:To our knowledge,this is the first study that evaluates ESBL microorganisms as a prognostic factor in *** factors associated with a poor prognosis in patients with EPn have been *** microbiological factors,specifically ESBL-producing bacteria,do not seem to influence in the prognosis of these patients.

Ethnic minority students are enrolled in formal education everyday around the world. This study explores the influence of the environment upon the participants’native language use. The concept of practice describes the use of the language in this study;it also surveys the role of native languages in connection with the academic performance. In addition, it inquires about the advantanges and the challenges of studying in a second language for Chinese ethnic minority students in northeast normal University. A pilot study was used to test the questions for this study. A bilingual questionnaire in English and Chinese and an interview-focus group were used to collect data;All the information was tape recorded to save the information. The gathering and analysis were guided for three research questions that attempt to show the language used in the university, the role of the.native language in the participants field of study and the experience of use a second language as medium of instruction. The findings show that the.native language plays an important role in the studies of the participants and it is a mark of identity for them. The study wants to contribute to the literature about ethnic minority students in China and other countries.
