Chronological age when health care transition skills are mastered in adolescents/young adults with inflammatory bowel disease

AIM To describe the longitudinal course of acquisition of healthcare transition skills among adolescents and young adults with inflammatory bowel diseases.METHODS We recruited adolescents and young adults(AYA) with inflammatory bowel diseases(IBD), from the pediatric IBD clinic at the University of North Carolina. Participants completed the TRx ANSITION Scale? at least once during the study period(2006-2015). We used the electronic medical record to extract participants' clinical and demographic data. We used ordinary least square regressions with robust standard error clustered at patient level to explore the variations in the levels and growths of healthcare transition readiness.RESULTS Our sample(n = 144) ranged in age from 14-22 years. Age was significantly and positively associated with both the level and growth of TRx ANSITION Scale? scores(P < 0.01). Many healthcare transition(HCT) skills were acquired between ages 12 and 14 years, but others were not mastered until after age 18, including self-management skills.CONCLUSION This is one of the first studies to describe the longitudinal course of HCT skill acquisition among AYA with IBD, providing benchmarks for evaluating transition interventions.

Acceptability of Caesarean Section among Pregnant Women Seeking Antenatal Care at Women and Newborn Hospital-University Teaching Hospitals, Lusaka Zambia

Caesarean section remains one of the most common obstetric and major operations performed on women during pregnancy and has contributed to improving maternal and foetal health. Consequently, significant mortality and morbidity from pregnancy and labour-related causes can be eliminated by a timely caesarean section. The acceptance of the life-saving procedure remains unsatisfactory in most sub-Saharan nations. The study aimed to assess the acceptability of caesarean section and associated factors among pregnant women seeking antenatal care at Women and Newborn Hospital-University Teaching Hospitals in Lusaka Zambia. The study utilised a quantitative cross-sectional analytical study design involving 420 pregnant women selected using systematic random sampling at Women and Newborn Hospital-University Teaching Hospitals Lusaka Zambia. Data was obtained using an Interview schedule and analysed using R software version 4.3.2. Chi-square, Fisher’s exact test, Wilcoxon rank sum test and binary logistic regression for statistical analysis at a 5% level of significance. The study found that the acceptability of Caesarean section as a mode of delivery was low (29%) among respondents at Women and Newborn Hospital-University Teaching Hospitals. This was influenced by marital status (P = 0.002), socioeconomic status (P = 0.050), parity (P = 0.004), gestation age (P = 0.008), previous history of caesarean section (P = 0.003), knowledge (P < 0.001), attitude (P < 0.001), and sociocultural beliefs (P = 0.045). Low acceptability of Caesarean section delivery is common among women at Women and Newborn Hospital-University Teaching Hospitals, despite its potential benefits for maternal and neonatal health. Socio-demographic and maternal characteristics, knowledge, attitude, perception, and socio-cultural beliefs play a major role in determining acceptability. Acceptability of Kangaroo Mother Care (KMC) by Mothers with Low-Birth-Weight Babies at Arthur Davison Children’s Hospital, in Ndola, Zambia

Background: Kangaroo Mother Care is a simple safe method used to care for low-birth-weight babies. Low-birth-weight is a global public health issue that pose significant challenge to perinatal care systems. Globally, complications due to low-birth-weight are the leading cause of neonatal mortality, resulting in an estimated 1 million deaths annually. Kangaroo Mother Care (KMC) is a low-cost method of care for low-birth-weight infants in areas with inadequate incubators and power outages with positive outcomes. Objectives: To assess factors influencing acceptability of Kangaroo Mother Care (KMC) in NICU at Arthur Davison Children’s Hospital in Ndola, Zambia. Methodology: A cross sectional quantitative analytical study design was used. The study was conducted at Arthur Davison Children’s Hospital (ADCH) in Ndola, Zambia. The purposive sampling method was used to select the study participants and a total of 129 mothers with Low Weight Babies A were selected to participate structured closed ended questionnaire was used to collect data from the participants using an interview. Data was analyzed using a Statistical Package for Social Scientists (SPSS) software version 26. Cross tabulations were done to determine association of variables using a Chi square (x2) test at 95% confidence interval and were assumptions where not met, fishers exact test was used. Multivariate binary logistic regression analysis was used to quantify the relationship between the dependent and independent variables. Results: Most of the respondents had male babies 77%, 69% of the babies weighed 1000 g - 1400 g, majority 79% had normal delivery, 71% of the respondents were multigravida and most of the respondents 79% were married. The study reviewed that 85% of those who attained secondary education accepted KMC, 74% of the respondents without monthly income accepted KMC, 80% of the respondents had positive attitude and accepted KMC. 82% of the respondents who had positive perception accepted KMC. Physiologic surgical options,including vascularized lymph node transplant and lymphovenous bypass are becoming increasingly popular interventions for the treatment of lymphedema of both the upper and lower extremities.Many different lymph node donor sites have been described,including submental,lateral thoracic,superficial groin,supraclavicular,and various intraabdominal sites.This paper describes a step-by-step approach to the harvest of vascularized lymph nodes from the supraclavicular area,which is the preferred donor site for most patients with both upper and lower extremity lymphedema.

Background and purpose Retinal pathologies are an independent risk factor for ischaemic stroke,but research on the predictive value of retinal abnormalities for recurrent vascular events in patients with prior stroke is inconclusive.We investigated the association of retinal pathologies with subsequent vascular events.Methods In a substudy of the Intensified secondary prevention intending a reduction of recurrent events in TIA and minor stroke patients(INSPiRE-TMS)trial,we enrolled patients with recent transient ischaemic attack(TIA)or minor stroke with at least one modifiable risk factor.Primary outcome was the composite of subsequent vascular events.Retinal photographs were taken at baseline and categorised into three different fundus groups by a telemedically linked ophthalmologist.Results 722 patients participated in the current study and 109 major vascular events occurred.After multivariable adjustments,we did not find a significant association between fundus categories and risk for subsequent vascular events(HRs for moderate vascular retinopathy and vascular retinopathy with vessel rarefaction in comparison to no vascular retinopathy 1.03(95%CI 0.64 to 1.67),p=0.905 and 1.17(95%CI 0.62 to 2.20),p=0.626).In a selective post hoc analysis in patients with diabetes mellitus and hypertension,patients with vascular retinopathy with vessel rarefaction had a higher risk for recurrent stroke(HR 24.14(95%CI 2.74 to 212.50),p=0.004).Conclusions Retinal changes did not predict major subsequent vascular events in patients with recent TIA or minor stroke.Further studies are needed to examine the utility of fundus photography in assessing the risk of stroke recurrence in patients with diabetes mellitus and hypertension.

Anomaly Detection in Textured Images with a Convolutional Neural Network for Quality Control of Micrometric Woven Meshes

Industrial woven meshes are composed of metal materials and are often used in construction,industrial and residential activities or applications.The objective of this work is defect detection in industrial fabrics in the quality control stage.In order to overcome the limitations of manual methods,which are often tedious and time-consuming,we propose a strategy that can automatically detect defects in micrometric steel meshes by means of a Convolutional Neural Network.The database used for such a purpose comes from real problem data for anomaly detection in micrometric woven meshes.This detection is performed through supervised classification with a Convolutional Neural Network using a VGG19 architecture.We define a pipeline and a strategy to tackle the related small amount of data.It includes i)augmenting the database with translation,rotation and symmetry,ii)using pre-trained weights and iii)checking the learning curve behaviour through cross-validation.The obtained results show that,despite the small size of our databases,detection accuracy of 96%was reached.

Arterial switch operation(ASO)is a complex neonatal operation in which transfer of the coronary arteries origins is the key to success.Coronary events after a successful ASO are not uncommon.We describe a rare case of a child who underwent an ASO in the neonatal period with one coronary(LAD)described as atretic left in place.At age seven,he developed myocardial ischemia due to retrograde flow with a steal phenomenon from the LAD into the pulmonary artery.The patient underwent a late LAD reimplantation.This case underscores that even very small ostia should be translocated at the time of ASO.

Accuracy of ultrasound to identify chronic liver disease

AIM:To identify and assess studies reporting the diagnostic performance of ultrasound imaging for identifying chronic liver disease(CLD)in a high risk population. METHODS:A search was performed to identify studies investigating the diagnostic accuracy of ultrasound imaging for CLD.Two authors independently used the quality assessment of diagnostic accuracy studies(QUADAS)checklist to assess the methodological quality of the selected studies.Inter-observer reliability of the QUADAS tool was assessed by measuring the degree of agreement(percent agreement,κstatistic)between the reviewers for each assessment prior to a consensusmeeting.The characteristics of each study population, sensitivity and specificity results for the index tests, and results of any testing for observer agreement were extracted from the reports.Receiver Operator Characteristic plots were generated using Microsoft Excel 2003 software and used to graphically display the diagnostic performance data and to explore the relationships between the reported ultrasound techniques and study characteristics,and methodology quality. RESULTS:Twenty-one studies published between 1991 and 2009 were retained for data extraction,analysis and assessment for methodological quality.Assessment of methodology quality was performed on the 21 selected studies by two independent reviewers(RA&KT) using the QUADAS assessment tool.Across all studies the mean number of responses within the QUADAS assessment tool was 10(range 7-13)for"Yes",1(range 0-3)for"No"and 3(range 0-6)for"unclear".Interrater agreement for assessment of methodology quality was significantly greater than chance when assessing for representative spectrum,clear selection criteria, appropriate delay between reference and index tests, adequate descriptions of the index and reference tests, reference and index test blinding,and if relevant clinical information was provided.Seven studies reported moderate to high observer agreement for ultrasound techniques.Studies which clearly reported blinding performed bet
