Place of sodium-glucose co-transporter type 2 inhibitors for treatment of type 2 diabetes

Inhibitors of sodium-glucose co-transporter type 2(SGLT2), such as canagliflozin and dapagliflozin, are recently approved for treatment of type 2 diabetes. These agents lower blood glucose mainly by increasing urinary glucose excretion. Compared with placebo, SGLT2 inhibitors reduce hemoglobin A1c(Hb A1c) levels by an average of 0.5%-0.8% when used as monotherapy or add-on therapy. Advantages of this drug class include modest weight loss of approximately 2 kg, low risk of hypoglycemia, and decrease blood pressure of approximately 4 mm Hg systolic and 2 mm Hg diastolic. These characteristics make these agents potential add-on therapy in patients with Hb A1 c levels close to 7%-8.0%, particularly if these patients are obese, hypertensive, and/or prone for hypoglycemia. Meanwhile, these drugs are limited by high frequency of genital mycotic infections. Less common adverse effects include urinary tract infections, hypotension, dizziness, and worsening renal function. SGLT2 inhibitors should be used with caution in the elderly because of increased adverse effects, and should not be used in chronic kidney disease due to decreased or lack of efficacy and nephrotoxicity. Overall, SGLT2 inhibitors are useful addition for treatment of select groups of patients with type 2 diabetes,but their efficacy and safety need to be established in long-term clinical trials.

Background Bowel obstruction is a common surgical emergency in ***-fourth of neonates with obstruction suffer postoperative complications,with high mortality,especially in low-income *** attributed to mortality include prematurity,late presentation and associated multiple congenital *** prevalence and the predictors of mortality in our setting have not been well *** aimed to describe the early postoperative outcomes and to determine the predictors of mortality among neonates with bowel *** We conducted a prospective cohort study during 4 months,when we recruited postoperative neonates from the pediatric surgery unit of Mulago *** collected data on demographics,clinical presentations,maternal characteristics,the surgical procedure performed,postoperative outcomes,*** follow-up for 2 weeks,we analyzed the data using Cox proportional hazards regression models of predictors of *** A total of 76 neonates were recruited,with a male-to-female ratio of 2.2:*** age ranged from 1 to 26 days,a median of 3[interquartile range(IQR):2,7].About 67.1%had birth weights ranging between 2.5 kg and 3.5 kg,a mean of 2.8[standard deviation(SD)=0.64];76.3%were termed;with anorectal malformation(ARM),31.6%as the leading cause,followed by jejunoileal atresia(JIA),25%.Nearly 55.3%of neonates developed complications;53.9%with post-operative fever,15.8%had wound *** mortality rate was 44.7%(34/76)and was highest among cases of JIA,41.2%.The predictors of mortality included prematurity,fever at admission,breastfeeding status,and mother’s parity as well as the cadre of healthcare providers(p<0.005).Conclusions ARM is the predominant cause of bowel obstruction among neonates,followed by *** morbidity and mortality due to bowel obstruction among neonates are unacceptably *** major predictors of mortality were prematurity,fever at admission,and the cadre of the healthcare providers.

Textual data streams have been extensively used in practical applications where consumers of online products have expressed their views regarding online *** to changes in data distribution,commonly referred to as concept drift,mining this data stream is a challenging problem for *** majority of the existing drift detection techniques are based on classification errors,which have higher probabilities of false-positive or missed *** improve classification accuracy,there is a need to develop more intuitive detection techniques that can identify a great number of drifts in the data *** paper presents an adaptive unsupervised learning technique,an ensemble classifier based on drift detection for opinion mining and sentiment *** improve classification performance,this approach uses four different dissimilarity measures to determine the degree of concept drifts in the data *** a drift is detected,the proposed method builds and adds a new classifier to the *** add a new classifier,the total number of classifiers in the ensemble is first checked if the limit is exceeded before the classifier with the least weight is removed from the *** this end,a weighting mechanism is used to calculate the weight of each classifier,which decides the contribution of each classifier in the final classification *** experiments were conducted on real-world datasets and the resultswere evaluated on the false positive rate,miss detection rate,and accuracy *** proposed method is also compared with the state-of-the-art methods,which include DDM,EDDM,and PageHinkley with support vector machine(SVM)and Naive Bayes classifiers that are frequently used in concept drift detection *** all cases,the results show the efficiency of our proposed method.

AIM: To assess the inter-observer agreement and reliability as well as intra-observer repeatability for lacrimal scintigraphy(LS) reports with and without considering the irrigation test ***: A prospective, observational, cross sectional study. Two masked clinicians(lacrimal surgeon and nuclear medicine specialist) independently reported 100 LS images(50 patients of >6 years of age with unilateral anophthalmic socket) in a university hospital. The lacrimal surgeon performed a diagnostic irrigation test and repeated the report of the same LS images 2 y after the first report(intra-observer agreement). A weighted Kappa analysis was performed to determine inter-observer agreement and reliability as well as intra-observer repeatability for the type(normal, partial and complete obstruction) and location(presac, preduct, and intraduct) of the obstruction. Subgroup analysis was also performed with consideration of irrigation test results. RESULTS: A significantly moderate agreement was found between lacrimal surgeon and nuclear medicine specialist for both the type(Kappa=0.55) and location(Kappa=0.48) of obstruction. Agreement values were higher for the type(Kappa=0.61 vs 0.41) and location(Kappa=0.56 vs 0.31) of obstruction in cases with normal than abnormal irrigation test. Strong and significant intraobserver(lacrimal surgeon) repeatability was found for both the type(Kappa=0.66) and location(Kappa=0.69) of obstruction. LS showed no to slight reliability based on irrigation ***: A moderate agreement is found between lacrimal surgeon and nuclear medicine specialist regarding the interpretation of LS suggesting the importance ofconsensus groups among nuclear medicine specialists and lacrimal surgeons to create a common language for interpretation of LS. Intra-observer repeatability is strong for the lacrimal surgeon.

Ocular manifestations of children with atopic dermatitis in Saudi Arabia

AIM:To examine the incidence of ocular abnormalities in children with atopic dermatitis(AD)in Saudi Arabia and its association with the severity of ***:This is a cross-sectional study on 50 children with AD who were between 5 and 16 years of *** severity of AD was evaluated using the SCORing Atopic Dermatitis(SCORAD)*** the children underwent slit lamp exams,visual acuity assessment,intraocular pressure measurement,and corneal *** children were considered to have an ophthalmic abnormality if one or more of the following signs were present:glaucoma,keratoconus suspicion,in addition to lid,conjunctival,corneal,lenticular,or retinal ***:Based on the SCORAD severity index,14%of children had mild AD(7/50),38%had moderate AD(19/50),and nearly half had severe *** than half the children exhibited facial involvement,and half had peri-orbital *** mean SCORAD index was *** mean age was 10.48±3.6y,and the cohort showed a slight male predominance(54%males).Both eyes of the 50 children in the cohort were *** on the ocular examinations,92%of the patients showed ocular abnormalities:lid abnormalities(27/50)followed by keratitis(22/50).Four patients had moderate risk for keratoconus in one eye and eight patients were suspected to have ***,SCORAD severity index was not associated with age,sex,or the number or presence of ophthalmic ***:This is the first study in Saudi Arabia to evaluate the prevalence of ocular manifestations in children with *** results indicate that the majority of children with AD have ocular abnormalities that mainly include lid *** on these findings,larger scale studies are needed to affirm whether regular screening for ophthalmic abnormalities would be beneficial for children with AD in terms of early intervention and prevention of sight-threatening complications.
