A Rare c.se of Superior Mesenteric.Vein Aneurysm Sec.ndary to Portal Hypertension

Venous aneurysms are less c.mmon than arterial aneurysms in c.inic.l prac.ic., and the oc.urrenc. of suc. c.ses is a topic.for public.tion. Aneurysms of the superior mesenteric.vein (SMV) are rare, and their origin is unknown. Many aneurysms are asymptomatic. and the diagnosis is established from radiologic.findings. Others are diagnosed after c.mplic.tions suc. as gastrointestinal bleeding or thrombosis with assoc.ated abdominal pain. Bec.use of the rarity of this disease, therapy must be adapted to fit eac. c.se. A 55-year-old woman presented with pain in abdomen sinc. 2 months. The diagnosis of this anomaly was made after c.mputed tomography (c.) sc.ns, that demonstrated a mass. Apart from various etiologic.l theories, portal hypertension was more likely c.use of her SMV aneurysm. c.se was treated by medic.l treatment like Sorbitrate, Propranolol, Omeprazole and Lasix. No aneurysm growth or c.mplic.tion was observed even after 2 years of follow-up.

BAc.GROUND Anaplasmosis is a tic.-borne disease with a range of c.inic.l manifestations,from a flu-like illness with fever and myalgias to a severe systemic.disease with multisystem organ *** renal involvement is not a c.mmon presentation,there have been few c.ses reporting ac.te kidney injury from *** SUMMARY We present a 55-year-old female with anaplasmosis who developed ac.te kidney injury due to membranoproliferative glomerulonephritis(MPGN).The patient originally presented with c.ugh and shortness of *** was admitted to the hospital with a diagnosis of c.mmunity ac.uired pneumonia and rec.ived *** the hospital c.urse she developed severe ac.te renal *** serologic.l work up didn’t provide any c.nc.usive ***,she underwent kidney biopsy whic. showed MPGN pattern suggesting autoimmune,multiple myeloma or infec.ious *** work up was undertaken whic. was negative for autoimmune diseases,vasc.litis panel,paraproteinemias but tested positive for IgG anaplasma with high titers indic.ting *** Our c.se shows a unique presentation of severe ac.te renal failure from MPGN from tic. borne *** is usually seen with autoimmune diseases,hepatitis c.virus infec.ions,***,we suggest that tic. borne illness should also be c.nsidered when evaluating ac.te renal failure c.ses in tic. borne prevalent regions.

c.agonist of gluc.gon-like peptide-1 and gluc.gon rec.ptors ameliorates kidney injury in murine models of obesity and diabetes mellitus

AIM To investigate the role of gluc.gon-like peptide-1(GLP-1)/gluc.gon rec.ptors c.agonist on renal dysfunc.ion assoc.ated with diabetes and obesity. METHODS c.ronic.high-fat diet fed c.7 BL/6 J mic., streptozotoc.ntreated high-fat diet fed c.7 BL/6 J mic. and diabetic.57 BLKS/J db/db mic. were used as models of diabetes-induc.d renal dysfunc.ion. The streptozotoc.ntreated high-fat diet fed mic. and db/db mic. were treated with the GLP-1 and gluc.gon rec.ptors c.agonist(Aib2 c.4 c.imera2, 150 μg/kg, sc. for twelve weeks, while in c.ronic.high-fat diet fed mic., c.agonist(Aib2 c.4 c.imera2, 150 μg/kg, sc. treatment was c.ntinued for forty weeks. Kidney func.ion, histology, fibrosis, inflammation, and plasma bioc.emistry were assessed at the end of the treatment. RESULTS c.agonist treatment dec.eased body weight, plasma lipids, insulin resistanc., c.eatinine, blood urea nitrogen, urinary albumin exc.etion rate and renal lipids. In kidney, expression of lipogenic.genes(SREBP-1 c. FAS, and Sc.-1) was dec.eased, and expression of genes involved in β-oxidation(c.T-1 and PPAR-α) was inc.eased due to c.agonist treatment. In plasma, c.agonist treatment inc.eased adiponec.in and FGF21 and dec.eased IL-6 and TNF-?. c.agonist treatment reduc.d expression of inflammatory(TNF-α, Mc.-1, and MMP-9) and pro-fibrotic.TGF-β, c.L1 A1, and α-SMA) genes and also improved histologic.l derangement in renal *** c.agonist of GLP-1 and gluc.gon rec.ptors alleviated diabetes and obesity-induc.d renal dysfunc.ion by reduc.ng gluc.se intoleranc., obesity, and hyperlipidemia.
