Role of serial multiparametric magnetic resonance imaging in prostate cancer active surveillance

AIM:To examine whether addition of 3T multiparametric magnetic resonance imaging(mp MRI)to an active surveillance protocol could detect aggressive or progressive prostate ***:Twenty-three patients with low risk disease were enrolled on this active surveillance study,all of which had Gleason score 6 or less *** patients had clinical assessments,including digital rectal examination and prostate specific antigen(PSA)testing,every 6 mo with annual 3T mp MRI scans with gadolinium contrast and minimum sextant prostate *** MRI images were anonymized of patient identifiers and clinical information and each scan underwentradiological review without the other results *** statistics for demographics and follow-up as well as the sensitivity and specificity of mp MRI to identify prostate cancer and progressive disease were ***:During follow-up(median 24.8 mo)11 of 23 patients with low-risk prostate cancer had disease progression and were taken off study to receive definitive *** progression was identified through upstaging of Gleason score on subsequent biopsies for all 11 patients with only 2 patients also having a PSA doubling time of less than 2 *** 23 patients had biopsy confirmed prostate cancer but only 10 had a positive index of suspicion on mp MRI scans at baseline(43.5% sensitivity).Aggressive disease prediction from baseline mpM RI scans had satisfactory specificity(81.8%)but low sensitivity(58.3%).Twentytwo patients had serial mp MRI scans and evidence of disease progression was seen for 3 patients all of whom had upstaging of Gleason score on biopsy(30% specificity and 100% sensitivity).CONCLUSION:Addition of mp MRI imaging in active surveillance decision making may help in identifying aggressive disease amongst men with indolent prostate cancer earlier than traditional methods.

An accurate theoretical description of the dynamic properties of correlated quantum many-body systems,such as the dynamic structure factor S(q,ω),is important in many ***,highly accurate quantum Monte Carlo methods are usually restricted to the imaginary time domain,and the analytic continuation of the imaginary-time density–density correlation function F(q,τ)to real frequencies is a notoriously hard ***,it is argued that often no such analytic continuation is required because by definition,F(q,τ)contains the same physical information as does S(q,ω),only represented ***,it is shown how one can directly extract key information such as the temperature or quasi-particle excitation energies from theτdomain,which is highly relevant for equation-of-state measurements of matter under extreme conditions[*** et al.,***.13,7911(2022)].As a practical example,ab initio path-integral Monte Carlo results for the uniform electron gas(UEG)are considered,and it is shown that even nontrivial processes such as the roton feature of the UEG at low density[*** et al.,***.5,304(2022)]are manifested straightforwardly in F(q,τ).A comprehensive overview is given of various useful properties of F(q,τ)and how it relates to the usual dynamic structure *** fact,working directly in theτdomain is advantageous for many reasons and opens up multiple avenues for future applications.

Soil properties and habitats determine the response of bacterial communities to agricultural wastewater irrigation

Increasing temperatures and variability of precipitation events due to climate change will lead in the future to higher irrigation demands in ***,the use of secondary treated wasterwater(TWW)could have consequences for the receiving soil environment and its resident microbial *** objective of this study was to characterize the importance of soil properties and habitats to the response of soil bacteria and archaea to irrigation with *** agricultural soils with contrasting textures(loamy sand or silt loam)and,for each,three variants differing in soil organic carbon and nitrogen,as generated by long-term fertilization,were *** each of these six soils,prokaryotic communities from two habitats,i.e.,root-free bulk soil and the rhizosphere of developing cucumber plants in the greenhouse,were *** were analyzed by the quantity and diversity of their polymerase chain reaction(PCR)-amplified 16S rRNA *** account for TWW-associated nutrient effects,potable water(PW)served as a *** sequence analysis showed that prokaryotic communities mainly consisted of bacteria(99.8%).Upon irrigation,regardless of the water quality,prokaryotic diversity declined,p H increased,and no bacterial growth was detected in bulk *** contrast,the growth of cucumbers was stimulated by TWW,indicating that plants were the main ***,strong responses were seen in the rhizosphere,suggesting an indirect effect of TWW by altered *** main bacterial responders to TWW were Proteobacteria,bacteroidetes,Actinobacteria,and *** in bacterial communities due to TWW were less pronounced in all variants of the silt loam,indicating the importance of clay and soil organic carbon for buffering effects of TWW on soil bacterial ***,soil organic carbon and soil texture are important parameters that need to be considered when applying TWW in agriculture.

Geoscience and high-level nuclear waste disposal： the Nordic scene

In Norden, two countries, Sweden and Finland, are actively engaged in site investigations for the location of deep repositories for spent nuclear fuel from power-producing nuclear reactors. These investigations are being carried out in crystalline rocks of the Fennoscandian Shield. In Sweden, a long history of site selection has led to the identification and investigation of two sites, Forsmark and Laxemar/Simpevarp, based on a strategy of combining favourable bedrock with consent by the local population. Surface-based geoscientific investigations of the two candidates, with extensive deep drilling, are now drawing to a close. A proposal as to which of the sites would be most suitable for the development of a deep repository will be submitted to the governmental regulatory authorities in 2009. In Finland, the site selection process was shorter and less politically controversial, and led to a "Decision in Principle" by the Finnish parliament, in May 2001, to develop a deep repository at the Olkiluoto site. The access tunnel to an underground rock characterisation facility at 400-500 m depth is at present under construction, accompanied by extensive geoscientific investigations in the subsurface. An application for a construction licence for a deep repository will be submitted in 2012. Although all sites are located in Precambrian crystalline rocks, the Swedish sites both lie in relatively homogeneous granitic rocks, whilst the Finnish site is located in an heterogeneous migmatite complex. The Nordic approach to high-level nuclear waste disposal in crystalline rock will be the theme of a Topical Symposium at the 33rd International Geological Congress at Oslo, in August 2008, and the three sites mentioned above will be the focus of Congress Excursion no. 14.

Linking monitoring and modelling for river basin management:Danish experience with combating nutrient loadings to the aquatic environment from point and non-point sources

Nationwide monitoring of the aquatic environment was initiated in 1988 in Denmark as a means to follow the outcome of the Action Plans for nutrient pollution of the aquatic environment. Five Action Plans have been adopted by the Danish parliament since 1985 and the nationwide monitoring programme can be used to quantify the outcome as shown by reductions in nutrient discharges from both point and non-point sources. Moreover, the empirical experience gathered from nearly 20 years of monitoring is assisting the development and calibration of models for simulation of nitrogen leaching, nitrogen removal in groundwater and surface waters and the establishment of a P-index all covering the entire land area of Denmark.

2016年9月18日俄罗斯国家杜马选举结果显示,统一俄罗斯党获得了宪法多数,与其同时进入国家杜马的还有俄罗斯共产党、俄罗斯自由民主党和公正俄罗斯党。从选举结果来看,影响各党在国家杜马选举结果的主要原因在于,现政府总是在选举前制造一种对己有利的形势,即在其监督下的大众媒体故意宣传在社会中充斥着一种反西方、反自由的情绪。这次选举,一方面,国家杜马完全被俄罗斯总统办公厅所控制;另一方面,明显削弱了议会同俄罗斯社会的逆向关系。对选举结果的统计分析表明,在俄罗斯许多地区,选举结果不但没有引起人们对当选的联邦权力机关的充分信任,反而降低了其合法性。
