We aimed to clarify the effects of troglitazone to prevent the development of fatty liver in Otsuka Long-Evans tokushima fatty (OLEtF) rats, an obese type2 diabetes model. treatment of 0.2% troglitazone for 16 weeks significantly decreased blood glucose, serum insulin and free fatty acid (FFA) concentration (P＜0.05 for each) in the OLEtF rats leaving their food intake, body weight and general fat pad weight unaffected compared with those of their lean littermates, Long-Evans tokushima Otsuka (LEtO) rats. troglitazone restored increase in liver weight and hepatic triglyceride(tG) content of the OLEtF rats (P＜0.05 for each). these findings were accertained by histological examination which showed that fat deposition, necrosis and vacuolization of hepatocyte, markers for fatty liver, were diminished by troglitazone treatment. Messenger RNA expression of microsomal triglyceride transfer protein in the liver, a marker for hepatic tG output, was not significantly different between OLEtF and LEtO rats with or without troglitazone treatment. Fatty acid composition such as C18∶1(oleic acid) showed no remarkable change after troglitazone treatment in OLEtF rats, indicating no dietary influence to hepatic lipogenesis. these indicate that reduction in circulating FFA level may be one of the main mechanisms for troglitazone to prevent the development of insulin resistance and fatty liver.

AIM: to elucidate the relationship between the microvessel count (MVC) by CD34 analyzed by immunohistochemical method and prognosis in hepatocellular carcinoma (HCC) patients who underwent hepatectomy based on our preliminary study. MEtHODs: We examined relationships between MVC and clinicopathological factors in 128 HCC patients. the modifi ed Japan Integrated staging score (mJIs) was applied to examine subsets of HCC patients. REsULts: Median MVC was 178/mm^2, which was used as a cut-off value. MVC was not signif icantly associated with any clinicopathologic factors or postoperative recurrent rate. Lower MVC was associated with poor disease-free and overall survivals by univariate analysis (P = 0.039 and P = 0.087, respectively) and lower MVC represented an independent poor prognostic factor in disease-free survival by Cox’s multivariateanalysis (risk ratio, 1.64; P = 0.024), in addition to tumor size, vascular invasion, macroscopic fi nding and hepatic dysfunction. signifi cant differences in disease-free and overall survivals by MVC were observed in HCC patients with mJIs 2 (P = 0.046 and P = 0.0014, respectively), but not in those with other scores. CONCLUsION: tumor MVC appears to offer a useful prognostic marker of HCC patient survival, particularly in HCC patients with mJIs 2.

Background:the end-stage liver disease causes a metabolic dysfunction whose most prominent clinical feature is the loss of skeletal muscle mass(sMM).In living-donor liver transplantation(LDLt),liver graft regeneration(GR)represents a crucial process to normalize the portal hypertension and to meet the metabolic demand of the *** data are available on the correlation between pre-LDLt low sMM and ***:Retrospective study on a cohort of 106 LDLt patients receiving an extended left liver lobe *** skeletal muscle index(sMI)at L3 level was used for muscle mass measurement,and the recommended cut-off values of the Japanese society of Hepatology guidelines were used as criteria for defining low *** was evaluated as rate of volume increase at 1 month post-Lt[graft regeneration rate(GRR)].Results:the median GRR at 1 month post-Lt was 91%(IQR,65-128%)and a significant correlation with graft volume-to-recipient standard liver volume ratio(GV/sLV)(rho-0.467,Pt-to-recipient weight ratio(GRWR)(rho-0.414,Pth post-Lt cholinesterase serum levels(rho 0.397,P=0.002)and pre-Lt low muscularity[absent *** GRR 97.5%(73.1-130%)vs.83.5%(45.2-110.9%),P=0.041]was *** in male recipients,but not in women,it was shown a direct correlation with pre-Lt sMI(rho 0.352,P=0.020)and inverse correlation with 1 month post-Lt sMI variation(rho-0.301,P=0.049).A low GRR was identified as an independent prognostic factor for recipient overall survival(HR 6.045,Psions:Additionally to the hemodynamic factors of portal circulation and the quality of the graft,the metabolic status of the recipients has a significant role in the GR process.A pre-Lt low sMM is associated with impaired GRR and this negative impact is more evident in male recipients.

硝酸甘油诱导的血管舒张作为血流介导的血管舒张功能（flow-mediated vasodilation,FMD）的对照试验,用来区别内皮依赖性与非依赖性的血管舒张功能。近来,硝酸甘油诱导的血管舒张已被报道是动脉粥样硬化分级的有用标志物。该研究旨在评估联合FMD与硝酸甘油诱导的血管舒张对未来心血管事件的预测价值。方法与结果：纳入研究对象402例,其中包括心血管病患者,测定FMD与硝酸甘油诱导的血管舒张。

Endoscopic submucosal dissection (EsD) is now the most common endoscopic treatment in Japan for intramucosal gastrointestinal neoplasms (non-metastatic). EsD is an invasive endoscopic surgical procedure, requiring extensive knowledge, skill, and specialized equipment. EsD starts with evaluation of the lesion, as accurate assessment of the depth and margin of the lesion is essential. the devices and strategies used in EsD vary, depending on the nature of the lesion. Prior to the procedure, the operator must be knowledgeable about the treatment strategy(ies), the device(s) to use, the electrocautery machine settings, the substances to inject, and other aspects. In addition, the operator must be able to manage complications, should they arise, including immediate recognition of the complication(s) and its treatment. Finally, in case the EsD treatment is not successful, the operator should be prepared to apply alternative treatments. thus, adequate knowledge and training are essential to successfully perform EsD.
