Outcomes of nonresected main-duct intraductal papillary mucinous neoplasms of the pancreas

AIM:To compare characteristics and outcomes of resected and nonresected main-duct and mixed intraductal papillary mucinous neoplasms of the pancreas(IPMN).METHODS:Over a 14-year period,50 patients who did not undergo surgery for resectable main-duct or mixed IPMN,for reasons of precluding comorbidities,age and/or refusal,were compared with 74 patients who underwent resection to assess differences in rates of survival,recurrence/occurrence of malignancy,and prognostic factors.All study participants had dilatation of the main pancreatic duct by ≥ 5 mm,with or without dilatation of the branch ducts.Some of the nonsurgical patients showed evidence of mucus upon perendoscopic retrograde cholangiopancreatography or endoscopic ultrasound and/or after fine needle aspiration.For the surgical patients,pathologic analysis of resected specimens confirmed a diagnosis of IPMN with involvement of the main pancreatic duct or of both branch ducts as well as the main pancreatic duct.Clinical and biologic follow-ups were conducted for all patients at least annually,through hospitalization or consultation every six months during the first year of follow-up,together with abdominal imaging analysis(magnetic resonance cholangiopancreatography or computed tomography) and,if necessary,endoscopic ultrasound with or without fine needle aspiration.RESULTS:The overall five-year survival rate of patients who underwent resection was significantly greater than that for the nonsurgical patients(74% vs 58%; P =0.019).The parameters of age( 10 mm,branch ductdiameter > 30 mm,or presence of extra pancreatic cancers did not significantly influence the prognosis.In the nonsurgical patients,pancreatic malignancy occurred in 36% of cases within a mean time of 33 mo(median:29 mo; range:8-141 mo).Comparison of the nonsurgical patients who experienced disease progression with those who did not progress showed no signi

Deciphering the controlling factors for phase transitions in zeolitic imidazolate frameworks

Zeolitic imidazolate frameworks（ZIFs） feature complex phase transitions, including polymorphism,melting, vitrification, and polyamorphism. Experimentally probing their structural evolution during transitions involving amorphous phases is a significant challenge, especially at the medium-range length scale. To overcome this challenge, here we first train a deep learning-based force field to identify the structural characteristics of both crystalline and non-crystalline ZIF phases. This allows us to reproduce the structural evolution trend during the melting of crystals and formation of ZIF glasses at various length scales with an accuracy comparable to that of ab initio molecular dynamics, yet at a much lower computational cost. Based on this approach, we propose a new structural descriptor, namely, the ring orientation index, to capture the propensity for crystallization of ZIF-4(Zn（Im）2, Im = C3H3N2-) glasses, as well as for the formation of ZIF-zni(Zn（Im）2) out of the high-density amorphous phase. This crystal formation process is a result of the reorientation of imidazole rings by sacrificing the order of the structure around the zinc-centered tetrahedra. The outcomes of this work are useful for studying phase transitions in other metal-organic frameworks（MOFs） and may thus guide the development of MOF glasses.

Protocol for lower back pain management: Insights from the French healthcare system

In this editorial we comment on the article published in a recent issue of the World Journal of Clinical Cases.This article described a novel ultrasound-guided lateral recess block approach in treating a patient with lateral recess stenosis.The impact of spinal pain-related disability extends significantly,causing substantial human suffering and medical costs.Each county has its preferred treatment strategies for spinal pain.Here,we explore the lower back pain(LBP)treatment algorithm recommended in France.The treatment algorithm for LBP recommended by the French National Authority for Health emphasizes early patient activity and minimal medication use.It encourages the continuation of daily activities,limits excessive medication and spinal injections,and incorporates psychological assessments and non-pharmacological therapies for chronic cases.However,the algorithm may not aggressively address acute pain in the early stages,potentially delaying relief and increasing the risk of chronicity.Additionally,the recommended infiltrations primarily involve caudal epidural steroid injections,with limited consideration for other injection procedures,such as transforaminal or interlaminar epidural steroid injections.The fixed follow-up timeline may not accommodate patients who do not respond to initial treatment or experience intense pain,potentially delaying the exploration of alternative therapies.Despite these limitations,understanding the strengths and weaknesses of the French approach could inform adaptations in LBP treatment strategies globally,potentially enhancing patient outcomes and satisfaction across diverse healthcare systems.

Differences between the healthcare systems of Quebec and France for the treatment of pain due to spinal disorders

In Quebec,Canada,the public healthcare system offers free medical services.However,patients with spinal pain often encounter long waiting times for specialist appointments and limited physiotherapy coverage.In contrast,private clinics provide expedited care but are relatively scarce and entail out-of-pocket expenses.Once a patient with pain caused by a spinal disorder meets a pain medicine specialist,spinal intervention is quickly performed when indicated,and patients are provided lifestyle advice.Transforaminal epidural steroid injections are frequently administered to patients with radicular pain,and steroid injections are administered on a facet joint to control low back or neck pain.Additionally,medial branch blocks are performed prior to thermocoagulation.France’s universal healthcare system ensures accessibility at controlled costs.It emphasizes physical activity and provides free physical therapy services.However,certain interventions,such as transforaminal and interlaminar epidural injections,are not routinely used in France owing to limited therapeutic efficacy and safety concerns.This underutilization may be a potential cause of chronic pain for many patients.By examining the differences,strengths,and weaknesses of these two systems,valuable insights can be gained for the enhancement of global spinal pain management strategies,ultimately leading to improved patient outcomes and satisfaction.

Two rectangular-shaped lakes,Lake Hulun and Lake Buir,located at the boundary between China and Mongolia,only c.75 km apart and therefore experiencing similar wind fields,have been studied based on satellite images and field surveys in order to compare their geomorphological and sedimentological characteristics.The wind-driven hydrodynamics,which have a significant effect on the development of littoral landforms and on sediment distribution,have been discussed for the two similar lakes that experienced a prevailing wind perpendicular to their long axis.A conceptual model related to wind-driven water bodies and sediment distribution is proposed.Wave-influenced to wave-dominated deltas,beaches,spits,and eolian dune deposits develop around these two lakes,with a strikingly similar distribution pattern.These features locally inform the longshore drift and help reconstruct the water circulation induced by wind forcing.Under the NW prevailing wind regime,the spits developed on the SW coast with a NW—SE extension,which was influenced by the NW—SE longshore currents.The same influence was observed in the delta extension in the NE area.The differences lie in the presence of fan deltas in the NW region of Lake Hulun,but not in Lake Buir.Additionally,the width of the beach and eolian deposits on the downwind coast of Lake Hulun is three times greater than that of Lake Buir which were caused by the differences in sediment supply and wind fetch between the two lakes.Lake Hulun and Lake Buir provide two reliable examples to understand the relationship among the wind field,provenance,hydrodynamics,landforms,and asymmetrical distribution of clastics in elongated lakes.They also represent relevant modern analogs,which may also be of guiding significance to wind-driven sand body prediction in lacustrine basins.

Gold Mineralisation in the Intiédougou Prospect (Diébougou District) in Southwest Burkina Faso, West African Craton

The Intiédougou located in the Houndé Birimian greenstone belt has been the subject of several mining and geoscience studies that have led to the discovery of mineralized gold targets. One of these mineralized targets has prompted work that raises the issue of control factors for the gold mineralization of the prospect. The methodology used in this study combines a study of core drill hole data located in the area and laboratory studies. The Intiédougou sector is based on andesito-basaltic, andesitic interstratified volcanoclastite rocks and Tarkwaïen type detrital sedimentary rocks caught in a vice in the volcano-sedimentary unit. Lithostructural analysis of the sector shows that the subvolcanic rocks bearing gold mineralization are subjected to heterogeneous ductile to brittle deformations and affected by hydrothermalism evolving at stages marked by large fissure fillings. These hydrothermal phases evolve in the zones of expansion created by the brittle deformations that have contributed to the deposits of different types of gold-enriched sulphides. These different phases of hydrothermal destabilization generally of low degree accompany the tardi to post-eburnean brittle tectonics. This deformation system is favorable to the establishment of gold mineralization in the form of vein bodies. The overimposition of deformed and altered areas suggests a genetic relationship between deformation and hydrothermal activity. In conclusion, the mineralization of Intiédougou in vein styles, set up in a volcanic arc environment with a paragenesis of gold-pyrite deposit ± chalcopyrite would be controlled by the structural aspect and accompanied by hydrothermal alteration.

Alternatives to norepinephrine in septic shock:Which agents and when?

Vasopressors are the cornerstone of hemodynamic management in patients with septic shock.Norepinephrine is currently recommended as the first-line vasopressor in these patients.In addition to norepinephrine,there are many other potent vasopressors with specific properties and/or advantages that act on vessels through different pathways after activation of specific receptors;these could be of interest in patients with septic shock.Dopamine is no longer recommended in patients with septic shock because its use is associated with a higher rate of cardiac arrhythmias without any benefit in terms of mortality or organ dysfunction.Epinephrine is currently considered as a second-line vasopressor therapy,because of the higher rate of associated metabolic and cardiac adverse effects compared with norepinephrine;however,it may be considered in settings where norepinephrine is un-available or in patients with refractory septic shock and myocardial dysfunction.Owing to its potential effects on mortality and renal function and its norepinephrine-sparing effect,vasopressin is recommended as second-line vasopressor therapy instead of norepinephrine dose escalation in patients with septic shock and persistent arterial hypotension.However,two synthetic analogs of vasopressin,namely,terlipressin and selepressin,have not yet been employed in the management of patients with septic shock,as their use is associated with a higher rate of digital ischemia.Finally,angiotensin II also appears to be a promising vasopressor in patients with septic shock,especially in the most severe cases and/or in patients with acute kidney injury requiring renal replacement therapy.Nevertheless,due to limited evidence and concerns regarding safety(which remains unclear because of potential adverse effects related to its marked vasopressor activity),angiotensin II is currently not recommended in patients with septic shock.Further studies are needed to better define the role of these vasopressors in the management of these patients.

Fusion of spatiotemporal and thematic features of textual data for animal disease surveillance

Several internet-based surveillance systems have been created to monitor the web for animal health surveillance.These systems collect a large amount of news dealing with outbreaks related to animal diseases.Automatically identifying news articles that describe the same outbreak event is a key step to quickly detect relevant epidemiological information while alleviating manual curation of news content.This paper addresses the task of retrieving news articles that are related in epidemiological terms.We tackle this issue using text mining and feature fusion methods.The main objective of this paper is to identify a textual representation in which two articles that share the same epidemiological content are close.We compared two types of representations(i.e.,features)to represent the documents:(i)morphosyntactic features(i.e.,selection and transformation of all terms from the news,based on classical textual processing steps)and(ii)lexicosemantic features(i.e.,selection,transformation and fusion of epidemiological terms including diseases,hosts,locations and dates).We compared two types of term weighing(i.e.,Boolean and TF-IDF)for both representations.To combine and transform lexicosemantic features,we compared two data fusion techniques(i.e.,early fusion and late fusion)and the effect of features generalisation,while evaluating the relative importance of each type of feature.We conducted our analysis using a corpus composed of a subset of news articles in English related to animal disease outbreaks.Our results showed that the combination of relevant lexicosemantic(epidemiological)features using fusion methods improves classical morphosyntactic representation in the context of disease-related news retrieval.The lexicosemantic representation based on TF-IDF and feature generalisation(F-measure=0.92,r-precision=0.58)outperformed the morphosyntactic representation(F-measure=0.89,r-precision=0.45),while reducing the features space.Converting the features into lower granular features(i.e.,generalisation)contributed to impr
