Strategy for reliable identification of ischaemic stroke, thrombolytics and thrombectomy in large administrative databases

Background Administrative data are frequently used in stroke *** accurate identification of patients who had an ischaemic stroke,and those receiving thrombolysis and endovascular thrombectomy(EVT)is critical to ensure representativeness and *** examined differences in patient samples based on mode of identification,and propose a strategy for future patient and procedure identification in large administrative *** We used non-public administrative data from the state of California to identify all patients who had an ischaemic stroke discharged from an emergency department(ED)or inpatient hospitalisation from 2010 to 2017 based on International Classification of Disease(ICD-9)(2010-2015),ICD-10(2015-2017)and Medicare Severity-Diagnosis-related Group(MS-DRG)discharge *** identified patients with interhospital transfers,patients receiving thrombolytics and patients treated with EVT based on ICD,Current Procedural Terminology(CPT)and MS-DRG *** determined what proportion of these transfers and procedures would have been identified with ICD versus MS-DRG discharge *** Of 365099 ischaemic stroke encounters,most(87.70%)had both a stroke-related ICD-9 or ICD-10 code and stroke-related MS-DRG code;12.28% had only an ICD-9 or ICD-10 code and 0.02% had only an MS-DRG *** all transfers(99.99%)were identified using ICD *** identified 32433 thrombolytic-treated patients(8.9% of total)using ICD,CPT and MS-DRG codes;the combination of ICD and CPT codes identified nearly all(98%).We identified 7691 patients treated with EVT(2.1%of total)using ICD and MS-DRG codes;both MS-DRG and ICD-9/ICD-10 codes were necessary because ICD codes alone missed 13.2%of *** codes only pertain to outpatient/ED patients and are not useful for EVT *** ICD-9/ICD-10 diagnosis codes capture nearly all ischaemic stroke encounters and transfers,while the combination of ICD-9/ICD-10 and CPT codes are adequate

In vitro and in vivo cytochrome P450 3A enzyme inhibition by Aframomum melengueta and Dennettia tripetala extracts

Obj.ctive: To evaluate the in vitro and in vivo inhibitory effects of two commonly used herbs, Aframomum melengueta(A. melengueta) and Dennettia tripetala(D. tripetala) on CYP 3A enzymes. Methods: In vitro inhibition of the enzymes were assessed with microsomes extracted from female albino rats using erythromycin-N-demethylation assay(EMND) method while their in vivo effects were measured by estimating simvastatin plasma concentrations in rats. Pharmacokinetic parameters were determined using non-compartmental anaysis as implemented in Win Nonlin pharmacokinetic program. Results: EMND assay with intestinal microsomes indicated that aqueous extracts of D. tripetala and A. melengueta significantly(P < 0.05) inhibited intestinal CYP 3A activity at both 50 μg and 100 μg concentrations. Petroleum ether extract of D. tripetala and ethanol extracts of A. melengueta inhibited intestinal CYP3 A activity at 100 μg but not at 50 μg concentrations. All the extracts showed an in vitrodose dependent CYP 3A inhibition with liver microsomes. In vivo analysis showed that pretreatment with the extracts enhanced systemic absorption of simvastatin with reductions in metabolizing enzymes activity as indicated in significant increases in maximal concentration, area under curve, area under moment curve and mean resident time of simvastatin(P < 0.05). Conclusions: Herbal preparations containing these plants' extracts should be used with caution especially in patients on CYP450 3A substrate medications.

Comparative study of a vitrinite-rich and an inertinite-rich Witbank coal (South Africa) using pyrolysis-gas chromatography

This study aims to compare iso-rank vitrinite-rich and inertinite-rich coal samples to understand the impact of coal-forming processes on pyrolysis chemistry.A medium rank C bituminous coal was density-fractionated to create a vitrinite-rich and an inertinite-rich *** vitrinite-rich sample has 83 vol% total vitrinite (mineral-matter-free basis),whereas the inertinite-rich counterpart has 66 vol% total *** vitrinite-rich sample is dominated by collotelinite and ***,semifusinite,and inertodetrinite are the main macerals of the inertinite-rich *** chemistry was assessed using a pyrolysis gas chromatograph (py-GC) equipped with a thermal desorption unit coupled to a time of flight mass spectrometer (MS) (py-GC/MS) and solid-state nuclear magnetic resonance (13C CP-MAS SS NMR).The pyrolysis products of the coal samples are generally similar,comprised of low and high molecular weight alkanes,alkylbenzenes,alkylphenols,and alkyl-subtituted polycyclic aromatic hydrocarbons,although the vitrinite-rich sample is chemically more *** lack of diversity exhibited by the inertinite-rich sample upon pyrolysis may be interpreted to suggest that maj.r components were heated in their geologic *** on the 13C CP-MAS SS NMR analysis,the inertinite-rich sample has a greater fraction of phenolics,reflected in the py-GC/MS results as substituted and unsubstituted *** greater abundance of phenolics for the inertinite-rich sample may suggest a fire-related origin for the dominant macerals of this *** C2-alkylbenzene isomers (p-xylene and o-xylene) were detected in the pyrolysis products for the vitrinite-rich and inertinite-rich samples,though more abundant in the *** presence of these in both samples likely reflects common source vegetation for the dominant vitrinite and inertinite rnacerals.

Geostatistical data plays a vibrant role for surface-based reservoir modeling through outcrop analogues,which is used to understand three-dimensional(3D)variability of petrophysical *** main purpose of this study is to improvise the surface-based 3D geo-modeling to demonstrate petrophysical characteristics and heterogeneities of Sandakan reservoirs,NW *** used point cloud data from Light Detection and Ranging(Li DAR)to build high-resolution virtual outcrop modeling(VOM)onto which we mapped 6 different *** and permeability of core plugs were measured to determine the average variance of petrophysical properties for each *** utilizing the integration of VOMs analogues and petrophysical properties in Petrel^(TM),we demonstrated the distribution and associations of all lithofacies in pseudo wells that have inherent thin beds heterogeneities in 3D geocellular *** results concluded that the heterogeneity of thin beds in lithofacies is dependent on porosity and permeability with input *** to the final model,cross-bedding sandstone(CBS),hummocky cross-stratified sandstone(HCSS)and trough cross-bedding sandstone(TCBS)show good reservoir quality due to high porosity ranging from:25.6%to 20.4%and,19.3%-14.5%,and permeability ranging from:74.03 mD to 66.84 mD and,64.86 mD to 21.01 *** contrast,massive to weak laminae sandstone(MWLS)and bioturbated sandstone(BS)show fair to poor reservoir quality,caused baffling of surrounding mud sediments in the reservoir *** also revealed that Li DAR based VOM with petrophysical properties can significantly reduce the risk and minimize the cost of reservoir modeling in petroleum industry.

High resolution facies architecture and digital outcrop modeling of the Sandakan formation sandstone reservoir, Borneo:Implications for reservoir characterization and flow simulation

Advances in photogrammetry have eased the acquisition of high-resolution digital information from outcrops, enabling faster, non-destructive data capturing and improved reservoir modeling. Geocellular models for flow dynamics with in the virtual outcrop in siliciclastic deposits at different sets of sandstone facies architecture remain, however, a challenge. Digital maps of bedding, lithological contrast, spatial-temporal variations of bedding and permeability characteristics make it more easy to understand flow tortuosity in a particular architecture. An ability to precisely model these properties can improve reservoir characterization and flow modeling at different scales. Here we demonstrate the construction of realistic 2 D sandstone facies based models for a pragmatic simulation of flow dynamics using a combination of digital point clouds dataset acquired from LiDAR and field investigation of the Sandakan Formation, Sabah, ***, we present methods for enhancing the accuracy of outcrop digital datasets for producing high resolution flow simulation. A well-exposed outcrop from the Sandakan Formation, Sabah, northwest Borneo having a lateral extent of 750 m was chosen in order to implement our research approach. Sandstone facies and its connectivity are well constrained by outcrop observations, data from air-permeability measurements, bilinear interpolation of permeability, grid construction and water vector analysis for flow *** proportions were then enumerated in terms of static digital outcrop model(DOM) and facies model based on sandstone facies bedding characteristics. Flow simulation of water vector analysis through each of the four sandstone facies types show persistent spatial correlation of permeability that align with either cross-bedded orientation or straight with more dispersion high quality sandstone(porosity 21.25%-41.2%and permeability 1265.20-5986.25 mD) and moderate quality sandstone(porosity 10.44%-28.75% and permeabili

Spectral Characterization of Laser Induced Plasma from Titanium Dioxide

Optical emission from TiO2 plasma, generated by a nanosecond laser is spectroscopically analysed. The main chemical species are identified and the spatio-temporal distribution of the plasma parameters such as electron temperature and density are characterized based on the study of spectral distribution of the line intensities and their broadening characteristics. The parameters of laser induced plasma vary quickly owing to its expansion at low background pressure and the possible deviations from local thermodynamic equilibrium conditions are tested to show its validity.
