Efficacy of combination therapy with natriuretic and aquaretic drugs in cirrhotic ascites patients: A randomized study

AIM To assess the effects of a combination therapy with natriuretic and aquaretic drugs in cirrhotic ascites *** A two-center,randomized,open-label,prospective study was conducted. Japanese patients who met the criteria were randomized to trial group and the combination diuretic group(received 7.5 mg of tolvaptan) or the conventional diuretic group(received 40 mg of furosemide) for 7 d in addition to the natriuretic drug which was used prior to enrolment in this study. The primary endpoint was the change in body weight from the baseline. Vital signs,fluid intake,and laboratory and urinary data were assessed to determine the pharmacological effects after administration of aquaretic and natriuretic *** A total of 56 patients were randomized to receive either tolvaptan(n = 28) or furosemide(n = 28). In the combination and conventional diuretic groups,the average decrease in body weight from the baseline was 3.21 ± 3.17 kg(P < 0.0001) and 1.75 ± 2.36 kg(P = 0.0006),respectively,when measured on the final dosing day. Following 1 wk of treatment,a significantly greater reduction in body weight was observed in the combination diuretic group compared to that in the conventional diuretic group(P = 0.0412).CONCLUSION Compared to a conventional diuretic therapy with only a natriuretic drug,a combination diuretic therapy with natriuretic and aquaretic drugs is more effective for patients with cirrhotic ascites.

Isolation and Expression Profiling of a CONSTANS-Like Gene and Two FLOWERING LOCUS T-Like Genes from Spinacia oleracea L.

Spinach (Spinacia oleracea L.) develops leaf rosettes under short-day conditions, and starts reproductive growth including bolting and flowering under long-day conditions. Japanese people prefer Oriental spinach that bolts easily with a shorter photoperiod than European spinach. This is one of the main reasons that Oriental spinach is difficult to grow year-round. In order to understand spinach flowering mechanisms and obtain knowledge for spinach breeding, we isolated one CONSTANS-like (COL) and two FLOWERING LOCUS T (FT) homologs, which are key components of photoperiodic regulation of flowering time, from a Japanese cultivar. The expression of SoCOL1 showed diurnal rhythm with the highest expression at the end of the dark cycle. This diurnal rhythm is similar to the expression of BvCOL1 from sugar beet (Beta vulgaris), whose flower-promoting effect was observed when overexpressed in Arabidopsis. Phylogenetic analysis showed that SoCOL1 is the closest homolog of BvCOL1, suggesting that SoCOL1 is an ortholog of BvCOL1. SoFT1 and SoFT2 are closely related to BvFT1 and BvFT2, respectively. The expression of SoFT1 and SoFT2 were induced in advance of flower bud formation after changing the photoperiod, but the expression level of SoFT1 was much lower than SoFT2. Currently, we are speculating that SoFT2 is a flower-promoting factor of spinach, and that SoFT1 has a role in light signaling because the expression of SoFT1 showed a diurnal rhythm.

Measurement of Tryptophan Metabolites in Healthy Old Men and Patients of Type 2 Diabetes Mellitus (T2DM)

Background: Plasma levels of tryptophan (TRP) metabolites have not been measured extensively in patients of type 2 diabetes mellitus (T2DM). Methods: Metabolites analysis was performed by a liquid chromatograph tandem mass spectrometer, the LCMS-8060 quadrupole mass spectrometer combined with Nexera X2 liquid chromatograph system (Shimadzu Corporation, Kyoto, Japan). Body mass index (BMI) and TRP metabolites have been measured in healthy old men (n = 20) and patients of T2DM (n = 20). TRP metabolites were measured by using the ultrahigh speed liquid chromatography-mass spectros-copy (Shimadzu Corporation). Results: The plasma levels of 5-hydroxytryptophan (5-HTRP), 5-hydroxyindoleacetic acid (5-HIAA), kynurenic acid (KNA), 3-hydroxykynurenine (3-HKN), and 3-hydroxyanthranilic acid (3-HAA) were higher in patients of T2DM than healthy old men. Since 5HTRP and 5-HIAA belong to the serotonin pathway, and KNA, 3-HKN, and 3-HAA belong to the KN pathway of TRP metabolism, these pathways were activated more in the patients of T2DM. Since plasma levels of Indole-3-acetic acid were not elevated in T2DM, that pathway was not activated more in T2DM. Serotonin levels were not increased but 5-HIAA levels were increased in the plasma of T2DM patients, which may mean that serotonin was quickly metabolized to 5-HIAA in the patients of T2DM. Conclusion: Plasma levels of tryptophan metabolites in serotonin and kynurenine pathways increased in T2DM patients. Obesity expressed by BMI may not influence tryptophan metabolism in healthy old men and T2DM patients. These results indicate that our new method of the simultaneous measurements of all the tryptophan metabolites is the powerful measure to identify factor related to endogenous stresses seen in DM.

In eukaryotes, gene expression is achieved by four steps: transcription, processing, nuclear export, and translation. Each step requires multiple factors, and frequently two or more pathways are used by a single gene, enabling strictly regulated gene expression. Importantly, eukaryotes, taking advantage of the separated structures of the nucleus and the cytoplasm, have evolved complex and organized mRNA processing mechanisms that permit sophisticated biological activity. The processes are much more complicated than those found in prokaryotes, in which transcription and translation occur linearly in time and place. Here, we review gene expression, focusing on mRNA processing in the nucleus and the gene regulatory systems found at each step. Combination of gene regulation shows the typical phenotype in each cell. Further understanding of the uncertain mechanisms will uncover the gene regulation through mRNA expression.

In this study, fatigue tests under different R ratios were conducted on the AZ61 Mg alloy to investigate its fatigue lifetimes and fatigue crack growth (FCG) behavior. The fracture surface of the failed specimens was investigated using a scanning electron microscope to study the size of the intermetallic compounds from which the pioneer fatigue crack initiated and led to the final failure of the specimen. To determine the maximum size of the intermetallic compounds existing within the cross section of the specimen at higher risk, Gumbel’s extreme-value statistics were utilized. In the present study, the intermetallic compounds contained within the specimen were assumed to be the initial cracks existing in the material before the fatigue tests. A modified linear elastic fracture-mechanics parameter, M, proposed by McEvily et al., was used to analyze the short FCG behavior under different stress ratios, R. The relation between the rate of FCG and M parameter was found to be useful and appropriate for predicting the fatigue lifetimes under different R ratios. Moreover, the probabilistic stress-fatigue life (P-S-N) curve of the material under different R ratios could be predicted with this method, which utilizes both the FCG law and a statistical distribution of sizes of the most dangerous intermetallic compounds. The evaluated results were in good agreement with the experimental ones. This correspondence indicates that the estimation method proposed in the present study is effective for evaluation of the probabilistic stress-fatigue life (P-S-N) curve of the material under different R ratios.

A measure called the Uncertainty in Unruptured Intracranial Aneurysm Patients Undergoing Endovascular Coiling Scale (UUIACS) was developed and its validity and reliability were examined. The 49 questions that comprised the original draft of the UUIACS were created based on interview data. Based on data from 172 participants, exploratory and confirmatory factor analyses were conducted. As a result of exploratory factor analysis, the UUIACS e retained 17 items and extracted four factors (“Lack of decision-making cues”, “Lack of information and complexity of information interpretation”, “The ambiguous nature of the disease”, and “The unpredictable living with UIA”). All of the UUIACS items showed adequate internal consistency. Between the UUIAC scale and the Universal Uncertainty in Illness Scale (UUIS), the Health Locus of Control (HLC) scale, and the SF-36v2® (Japanese version), positive correlations were found between the UUIACS and UUIS, and the HLC scale at a 1% significance level indicating concurrent validity. According to confirmatory factor analysis, the UUIACS had an acceptable goodness of fit. Given these findings, the UUIACS was judged to have satisfied the criteria for use in a clinical setting, although further investigation was required.

Thermochemical Heat Storage Performance in the Gas/Liquid-Solid Reactions of SrCl₂ with NH₃

Thermochemical heat storage is a promising technology for improving energy efficiency through the utilization of low-grade waste heat. The formation of a SrCl2 ammine complex was selected as the reaction system for the purpose of this study. Discharge characteristics were evaluated in a packed bed reactor for both the gas-solid reaction and the liquid-solid reaction. The average power of the gas-solid reaction was influenced by the pressure of the supplied ammonia gas, with greater powers being recorded at higher ammonia pressure. For the liquid-solid reaction, the obtained average power was comparable to that obtained for the gas-solid reaction at 0.2 MPa. Moreover, the lower heat transfer resistance in the reactor was observed, which was likely caused by the presence of liquid ammonia in the system. Finally, the short-term durability of the liquid-solid reaction system was demonstrated over 10 stable charge/discharge cycles.
