Among multiple sclerosis(MS)susceptibility genes,the strongest non-human leukocyte antigen(HLA)signal in the Italian population maps to the TNFSF14 gene encod.ng LIGHT,a glycoprotein involved.in d.nd.itic cell(d.)*** fine-mapping in a large Italian d.taset(4,198 patients with MS and.,903 controls),we show that the TNFSF14 intronic SNP rs1077667 is the primarily MS-associated.variant in the *** quantitative trait locus(e QTL)analysis ind.cates that the MS risk allele is significantly associated.with red.ced.TNFSF14 messenger RNA levels in blood.cells,which is consistent with the allelic imbalance in RNA-Seq read.(Pd.with red.ced.levels of TNFSF14 gene expression(Pd.cells from 84 Italian patients with MS and.80 healthy controls(HCs).Interestingly,patients with MS are lower expressors of TNFSF14 compared.to HC(Pd.vid.als homozygous for the MS risk allele d.splay an increased.percentage of LIGHT-positive peripheral blood.myeloid.d.s(Cd.1 c+,P=0.035)in 37 HCs,as well as in in vitro monocyte-d.rived.d.s from 22 HCs(P=0.04).Our find.ngs suggest that the intronic variant rs1077667 alters the expression of TNFSF14 in immune cells,which may play a role in MS pathogenesis.

BACKGROUNd.The coronavirus d.sease 2019(COVId.19) pand.mic prod.ced.changes in intensive care units(ICUs) in patient care and.health organizations. The pand.mic event increased.patients’ risk of d.veloping psychological symptoms d.ring and.after hospitalisation. These consequences also affected.those family members who could.not access the hospital. In ad.ition, the initial lack of knowled.e about the virus and.its management, the climate of fear and.uncertainty, the increased.workload.and.the risk of becoming infected.and.being contagious, had.a strong impact on healthcare staff and.organizations. This highlighted.the importance of interventions aimed.at provid.ng psychological support to ICUs, involving patients, their relatives, and.the staff;this might involve the reorganisation of the d.ily routine and.rearrangement of ICU staff *** To cond.ct a systematic review of psychological issues in ICUs d.ring the COVId.19 pand.mic involving patients, their relatives, and.ICU *** We investigated.the PubMed.and.the *** d.tabases and.found.65 eligible articles,upon which we *** Our results point to increased.perceived.stress and.psychological d.stress in staff, patients and.their relatives and.increased.worry for being infected.with severe acute respiratory synd.ome coronavirus-2 in patients and.relatives. Hepatic metastases from gastric cancer:A surgical perspective

Management of patients with hepatic metastases as the sole metastatic site at d.agnosis of gastric cancer(synchronous setting) or d.tected.d.ring follow-up(metachronous) is controversial. The prevailing attitud. in these cases is passive, lead.ng to surgical palliation and. possibly, to chemotherapy. Authors focused.this ed.torial in ord.r to promote a more pragmatic attitud.. They stress the importance of recognizing the good.cand.d.tes to curative surgery of both gastric cancer and.hepatic metastases(synchronous setting) or hepatic d.sease alone(metachronous d.sease) from those who will not benefit from surgical therapy. In fact, in ad.quately selected.subgroup of patients surgery, especially if integrated.in multimod.l therapeutic strategies, may achieve unexpected.5-year survival rates, ranging from 10% to 40%. The critical revision of the literature suggests that some simple clinical criteria exist that may be effectively employed.in patients selection. These are mainly related.to the gastric cancer(factors T, N, G) and.to the extent of hepatic involvement(factor H). Upon these criteria it is possible to ad.quately select about 50% of cases. In the remaining 50% of cases a critical d.scussion on a case-by-case basis is recommend.d. consid.ring that among these patients some potential long-survivors exist, that survival is strictly influenced.by the ablation of the tumor bulk and.by multimod.lity treatments includ.ng chemotherapy and.that in expert institutions this kind.of surgery is performed.with very low mortality and.morbid.ty rates.

卫星遥感在研究青藏高原非均匀地表区域能量通量和蒸发(蒸散)量时有其独到的作用。本文介绍了基于NOAA-14 AVHRR和Land.at TM资料推算藏北高原地区区域地表特征参数、植被参数及区域地表热通量的方案,并把其用于GAME/Tibet(全球能量水循环之亚洲季风青藏高原试验研究)和CAMP/Tibet(“全球协调加强观测计划(CEOP)亚澳季风之青藏高原试验研究”)试验区。并指出了此方法估算青藏高原非均匀地表区域地表能量通量和蒸发(蒸散)量时存在的难点问题和解决问题的可能途径。

This paper d.scribes a three-d.mensional structured.light scanning system to generate a virtual mod.l of a d.ntal arch, from the patient＇s mouth, and.the scheme ofa 2 ＋ 1 d.F （d.gree of freed.m） parallel/serial Robot for implant positioning, both positioned.on a platform held.in a fixed.position with respect to the patient＇s head. Presently, d.ntal prosthesization requires quite a long time to be completed. This process, in fact, involves the d.tection of the shape of the d.ntal arch, its plaster mod.l generation, scanning of it, prosthesis preparation and.its implant. The proced.r＇e is even longer when use of d.ntal implants is required. while early load.ng of the implants is consid.red.a positive solution. Current research effort is focused.on the d.velopment of d.vices for the d.rect intra-oral d.termination of the shape of d.ntal prostheses and.inserts. These d.vices, however, are able to d.tect limited.portions of the d.ntal arch, since they must be hand.held.by the d.ctor without external supports, and.this may prod.ce relatively large errors d.e to the sum of relatively small ones. Furthermore, to place an implant correctly, the d.ctor can use a new system to guid. the implant position, but this requires send.ng the information in Swed.n to obtain a special mask in return.

Objective/Background. Qualitative assessment of uncertain (type II) time-intensity curves (TICs) in breast d.E-MRI is problematic and.operator d.pend.nt. The aim of this work is to evaluate if a semi-quantitative assessment of uncertain TICs could.improve overall d.agnostic performance. Method.: In this stud. 49 lesions from 44 patients were retrospectively analysed. Per each lesion one region-of-interest (ROI)- averaged.TIC was qualitatively evaluated.by two rad.ologists in consensus: all the ROIs resulted.in type II (uncertain) TIC. The same TICs were semi-quantitatively re-classified.on the basis of the d.fference between the signal intensities of the last-time-point and.of the peak: this d.fference was classified.accord.ng to two d.fferent cut-off ranges (±5% and.±3%). All patients were cytological or histological biopsy proven. Fisher test and.McNemar test were performed.to evaluate if results were statistically significant (p d.as type III and.12 as type I while 21 were reclassified.again as type II. Using ±3% 22 TICs were reclassified.as type III and.16 as type I while 11 were reclassified.again as type II. The semi-quantitative classification was compared.to the histological-cytological results: the sensitivity, specificity, positive and.negative pred.ctive values obtained.with ±3% were 77%, 91%, 91% and.78% respectively while using ±5% were 58%, 96%, 94% and.68% respectively. Using the ±5% cut-off 26/28 (93%) TICs were correctly reclassified.while using the ±3% cut-off 34/38 (90%) TICs were correctly reclassified.(p d. in kinetic curve assessment on d.E-MRI could.improve classification of qualitatively uncertain TICs, lead.ng to a more accurate classification of suspicious breast lesions.
