In this article,we discuss evidence supporting the effective implementation of e Health,telehealth,and telemedicine during the coronavirus disease 2019 pandemic,with a view towards its permanent future integration in *** performed a literature search for articles describing the use of telehealth/telemedicine in the pandemic context using five *** articles selected describe the use of telemedicine as its advantages in terms of practicality and *** synthesis of articles is applicable to high-,middle-and lowincome *** of the notable benefits include breaking down geographical and time barriers,reducing waiting lists and crowding in healthcare facilities,and saving on national healthcare ***,there are a number of difficulties with the widespread implementation of telemedicine services that mainly relate to bureaucratic and regulatory ***,it is also important to make healthcare professionals and providers aware of the limits of this tool to avoid potential cases of *** in turn will have to be made aware of and be educated on the use of this new healthcare modality before it is accepted by *** the current socio-economic climate,it is therefore essential to implement a telehealth model aimed at efficiency and continuity of healthcare,as well as leading to an improvement in the quality of life of patients,whilst optimising existing resources and reducing *** that regard,the adoption of e Health,telehealth,and telemedicine services should be considered highly timely,despite current existing limitations.

Endoscopic submucosal dissection was introduced in Japan for the mini-invasive treatment of early gastric cancer,as part of national screening program considering high prevalence of disease in these *** technique allows en-bloc curative oncological excision and to obtain in a single step R0-resection,characterization,histological staging and potential cure of the tumor with a very high cost-benefit *** the years,Western endoscopists have adopted endoscopic submucosal dissection,achieving good rates of efficacy,long-term improved outcomes and safety,with low risk of local recurrence comparable to those obtained in Asian ***,according to some authors,the excellent outcomes from East country could not be representative of the Western *** epidemiological differences of early gastric cancer,scant volume data and limitations in training opportunities between Western and Eastern countries,European Society of Gastrointestinal Endoscopy have adopted Japanese guidelines and developed a European core curriculum for endoscopic submucosal dissection *** should be able to estimate the probability of performing a curative resection by considering the benefit/risk relationship case-by-case in order to implement a correct decision-making process.

Conversion of laparoscopic colorectal resection for cancer: What is the impact on short-term outcomes and survival?

Laparoscopic resection for colon and rectal cancer is associated with quicker return of bowel function, reduced postoperative morbidity rates and shorter length of hospital stay compared to open surgery, with no differences in long-term survival. Conversion to open surgery is reported in up to 30% of patients enrolled in randomized control trials comparing open and laparoscopic colorectal resection for cancer. In this review, reasons for conversion are anatomical-related factors, disease-related-factors and surgeon-related factors. Body mass index, local tumour extension and co-morbidities are independent predictors of conversion. The current evidence has shown that patients with converted resection for colon cancer have similar outcomes compared to patients undergoing a laparoscopic completed or open resection. The few studies that have assessed the outcomes after conversion of laparoscopic rectal resection reported significantly higher rates of complications and longer length of hospital stay in converted patients compared to laparoscopically treated patients. No definitive conclusions can be drawn when converted and open rectal resections are compared. Early and pre-emptive conversion appears to have more favourable outcomes than reactive conversion; however, further large studies are needed to better define the optimal timing of conversion. With regard to long-term oncologic outcome, overall and disease-free survival in the case of conversion in laparoscopic colorectal cancer surgery seems to be worse than those achieved in patients in whom resection was successfully completed by laparoscopy. Although a worse long-term oncologic outcome has been suggested, it remains difficult to draw a proper conclusion due to the heterogeneity of the long-term outcomes as well as the inclusion of both colon and rectal cancer patients in most of the studies. In vitro Ruminal Gas Production Kinetics of Four Fodder Trees Ensiled With or Without Molasses and Urea

This study investigated if the addition of urea （U）, molasses （M） or their 1：1 （v/v） mixture during ensiling increases the nutritional value of forage from four fodder trees （Prunus persica, Leucaena esculenta, Acacia farnesiana, and Prunus domestica）. Forage samples of fodder trees were collected in triplicate （three individual samples of each species） and subjected to an in vitro gas production （GP） procedure. Fermentation at 24 h （GP 24）, short-chain volatile fatty acids （SCFA）, and microbial crude protein production （MCP）, in vitro organic matter digestibility （OMD）, metabolizable energy （ME） and dry matter degradability （DMD） were estimated. Forage samples were incubated for 72 h in an incubator at 39oC and the volume of GP was recorded at 2, 4, 6, 8, 10, 12, 24, 48, and 72 h of incubation using the reading pressure technique. The rumen fermentation profiles were highest for P. persica, which showed the highest （P〈0.0001） DMD, ME, OMD, SCFA, GP 24 and MCP. On the other hand L. esculenta had the lowest （P〈0.0001） DMD, SCFA, MCP; P. domestica had the lowest （P〈0.0001） OMD. The addition of M to silage increased （P〈0.0001） ME and OMD, as well as GP. However, the addition of U and the mixture of U and M reduced （P〈0.0001） DMD, ME, OMD, SCFA, GY 24 and MCP. These results show that P. persica has the highest nutritive value and L. esculenta the lowest for ruminants. Additionally, the addition of M to forage from fodder trees increases rumen GP and fermentation, which may improve nutrient utilization in ruminants.

BACKGROUND： A large amount of endotoxin can be detected in the peripheral venous blood of patients with liver cirrhosis, contributing to the pathogenesis of hepatotoxicity because of its role in oxidative stress. The present study aimed to test the effect of the supplementation with red palm oil（RPO）, which is a natural oil obtained from oil palm fruit（Elaeis guineensis） rich in natural fat-soluble tocopherols, tocotrienols and carotenoids, on lipid peroxidation and endotoxemia with plasma endotoxin-inactivating capacity, proinflammatory cytokines profile, and monocyte tissue factor in patients with chronic liver disease. METHODS： The study group consisted of sixty patients（34 males and 26 females; mean age 62 years, range 54-75） with Child A/B, genotype 1 HCV-related cirrhosis without a history of ethanol consumption, randomly enrolled into an 8-week oral daily treatment with either vitamin E or RPO. All patients had undergone an upper gastrointestinal endoscopy 8 months before, and 13 out of them showed esophageal ***： Both treatments significantly decreased erythrocyte malondialdehyde and urinary isoprostane output, only RPO significantly affected macrophage-colony stimulating factor and monocyte tissue factor. Liver ultrasound imaging did not show any change. CONCLUSIONS： RPO beneficially modulates oxidative stress and, not least, downregulates macrophage/monocyte inflammatory parameters. RPO can be safely advised as a valuable nutritional implementation tool in the management of chronic liver diseases.

The development of efficient water oxidation catalysts(WOCs)is of key importance in order to drive sustainable reductive processes aimed at producing renewable ***,two novel dinuclear complexes,[(Cp^*Ir)2(μ-κ^3-O,N,O-H4-EDTMP)](Ir-H4-EDTMP,H4-EDTMP^4-=ethylenediamine tetra(methylene phosphonate))and[(Cp^*Ir)2(μ-κ^3-O,N,O-EDTA)](Ir-EDTA,EDTA^4-=ethylenediaminetetraacetate),were synthesized and completely characterized in solution,by multinuclear and multidimensional NMR spectroscopy,and in the solid state,by single crystal X-Ray *** were supported onto rutile TiO2 nanocrystals obtaining Ir-H4-EDTMP@TiO2 and Ir-EDTA@TiO2 hybrid *** molecular complexes and hybrid materials were found to be efficient catalysts for WO driven by NaIO4,providing almost quantitative yields,and TON values only limited by the amount of NaIO4 *** for the molecular catalysts,Ir-H4-EDTMP(TOF up to 184 min^-1)exhibited much higher activity than Ir-EDTA(TOF up to 19 min^-1),likely owing to the higher propensity of the former to generate a coordination vacancy through the dissociation of a Ir–OP bond(2.123A,significantly longer than Ir–OC,2.0913A),which is a necessary step to activate these saturated ***-H4-EDTMP@TiO2(up to 33 min^-1)and IrEDTA@TiO2(up to 41 min^-1)hybrid materials showed similar activity that was only marginally reduced in the second and third catalytic runs carried out after having separated the supernatant,which did not show any sign of activity,*** observed TOF values for hybrid materials are higher than those reported for analogous systems deriving from heterogenized mononuclear *** suggests that supporting dinuclear molecular precursors could be a successful strategy to obtain efficient heterogenized water oxidation catalysts.
