Patterns of Blood Supply and Venous Drainage in Pediatric Intralobar Pulmonary Sequestration: A Retrospective Analysis of 30 Pediatric Cases from a Single Center

Purposes: Intralobar pulmonary sequestration is a rare congenital anomaly that causes nonspecific respiratory symptoms. This report presented our surgical experience for pediatric cases of intralobar pulmonary sequestration in our institute. Methods: Between 2002 and 2014, 30 pediatric patients with intralobar pulmonary sequestration underwent lobectomy. The clinical characteristics of patients were retrospectively analyzed using medical records and imaging data. Results: The median age at operation was 23 months. The sequestered lung was situated in the right lower lung in 15 cases and in the left lower lung in 15 cases. Eight cases were prenatally diagnosed by fetal ultrasound and magnetic resonance imaging. One aberrant artery was seen in 26 cases and two aberrant arteries in 4 cases. The drainage vein from the sequestered lung was: 1) the pulmonary vein in 27 cases;2) the inferior vena cava in 1 case;and 3) the azygos vein and the pulmonary vein in 2 cases. All patients underwent lower lobectomy of the affected side without postoperative early and long-term complications. Conclusion: Due to the increased rate of prenatal diagnoses, the age at operation has decreased. With respect to safety, it is vital to ascertain the number of aberrant arteries and the venous drainage route before lobectomy.

Drag-reduction behavior of an unusual nonionic surfactant in a circular pipe turbulent flow

The Alkyl Polyglucoside（APG） is a nonionic surfactant with no toxicity and with high biodegradability, its drag-reduction behavior in a circular pipe flow is measured, and the rheological characteristics are investigated with a rheometer with a cone-plate flow cell. From the measured results, the APG is shown to have a high drag-reduction capacity, whose shear viscosity is shear-ratedependent at high concentrations, while its solution at concentrations with drag-reduction effects is non-viscoelastic as verified by zero relaxation time in the relaxation process of the shear stress, which contradicts the general viewpoint that there is a correlation between the viscoselastic characteristics and the turbulent drag reduction for the drag-reduction surfactant. However, the APG solution is birefringent as observed through a birefringent test, which indicates that there are rod-shaped micelles in the solution under the shearing flow. The higher extensional viscosity inferred from the extensional phenomenon observed in the measurements of the shear viscosity could be responsible for the drag reduction property of this nonionic surfactant.

Technical tips of endoscopic ultrasound-guided choledochoduodenostomy

Endoscopic ultrasound(EUS) is clinically useful not only as a diagnostic tool during EUS-guided fine needle aspiration,but also during interventional ***-guided biliary drainage has been developed and performed by experienced ***-guided choledocoduodenostomy(EUS-CDS) is relatively well established as an alternative biliary drainage method for biliary decompression in patients with biliary *** reported technical success rate of EUS-CDS ranges from 50% to 100%,and the clinical success rate ranges from 92% to 100%.Further,the over-all technicalsuccess rate was 93%,and clinical success rate was 98%.Based on the currently available literature,the overall adverse event rate for EUS-CDS is 16%.The data on the cumulative technical and clinical success rate for EUS-CDS is ***,EUS-CDS can still lead to several problems,so techniques or devices that are more feasible and safe need to be ***-CDS has the potential to become a first-line biliary drainage procedure,although standardizing the technique in multicenter clinical trials and comparisons with endoscopic biliary drainage by randomized clinical trials are still needed.

Technical tips for endoscopic ultrasound-guided hepaticogastrostomy

Interventional procedures using endoscopic ultrasound(EUS) have recently been developed. For biliary drainage, EUS-guided trans-luminal drainage has been reported. In this procedure, the transduodenal approach for extrahepatic bile ducts is called EUSguided choledochoduodenostomy, and the transgastric approach for intrahepatic bile ducts is called EUSguided hepaticogastrostomy(EUS-HGS). These procedures have several effects, such as internal drainage and avoiding post-endoscopic retrograde cholangiopancreatography(ERCP) pancreatitis, and they are indicated for an inaccessible ampulla of Vater due to duodenal obstruction or surgical anatomy. EUS-HGS has particularly wide indications and clinical impact as an alternative biliary drainage method. In this procedure, it is necessary to dilate the fistula, and several devices and approaches have been reported. Stent selection is also important. In previous reports, the overall technical success rate was 82%(221/270), the clinical success rate was 97%(218/225), and the overall adverse event rate for EUS-HGS was 23%(62/270). Adverse events of EUS-biliary drainage are still high compared with ERCP or PTCD. EUSHGS should continue to be performed by experienced endoscopists who can use various strategies when adverse events occur.

Feeding behavior and gene expression of appetite-related neuropeptides in mice lacking for neuropeptide Y Y5 receptor subclass

Neuropeptide Y (NPY) is a potent neurotransmitter for feeding. Besides NPY, orexigenic neuropeptides such as agouti-related protein (AgRP), and anorexi- genic neuropeptides such as α-melatonin stimulating hormone (MSH) and cocaine-amphetamine-regulated transcript (CART) are also involved in central feeding regulation. During fasting, NPY and AgRP gene expressions are up-regulated and POMC and CART gene ex- pressions are down-regulated in hypothalamus. Based on the network of peptidergic neurons, the former are involved in positive feeding regulation, and the latter are involved in negative feeding, which exert these feeding-regulated peptides especially in paraventricular nucleus (PVN). To clarify the compensatory mecha- nism of knock-out of NPY system on feeding, change in gene expressions of appetite-related neuropeptides and the feeding behavior was studied in NPY Y5-KO mice. Food intake was increased in Y5-KO mice. Fasting increased the amounts of food and water intake in the KO mice more profoundly. These data indicated the compensatory phenomenon of feeding behavior in Y5-KO mice. RT-PCR and ISH suggested that the compensation of feeding is due to change in gene expressions of AgRP, CART and POMC in hypothalamus. Thus, these fi ndings indicated that the compensatory mechanism involves change in POMC/CART gene expression in arcuate nucleus (ARC). The POMC/CART gene expression is important for central compensatory regulation in feeding behavior.

Does endoscopic ultrasound-guided biliary drainage really have clinical impact?

The well established, gold standard method for treatment of obstructive jaundice involves biliary drainage under endoscopic retrograde cholangiopancreatography(ERCP) performed by pancreatobiliary endoscopists. Recently, interventions using endoscopic ultrasound(EUS) have been developed not only for obtaining cytological and histological diagnosis, but also for biliary drainage as alternative method. EUS-guided biliary drainage(EUSBD) was first reported by Giovannini et al. EUS-BD broadly includes EUS-guided rendezvous technique, EUS-guided choledochoduodenostomy, and EUS-guided hepaticogastrostomy. More recently, EUS-guided antegrade stenting and EUS-guided gallbladder drainage have also been reported. many case reports, series, and retrospective studies on EUS-BD have been reported. However, because prospective studies and comparisons between the different biliary drainage methods have not been reported, the technical success, functional success, adverse events, and stent patency with long-term follow up of EUS-BD are still unclear. Therefore, prospective, randomized controlled studies addressing these issues are needed. Despite this, EUSBD undoubtedly is clinically useful as an alternative biliary drainage method. EUS-BD has the potential to be a first-line biliary drainage method instead of ERCP if results of clinical trials are favorable and the technique is simplified.

Lumen-apposing metal stent deployment for walled-off necrosis using semi-free hand technique from the intestine

Lumen-apposing metal stent (LAMS) deployment under en- doscopic ultrasonography (EUS) guidance has been used for walled-off necrosis (WON) [1] . Recently, LAMS with electrocautery enhanced delivery system has been developed. One of the advan- tages of this stent is that the stent can be delivered without any dilation devices. However, to prevent stent migration into the in- testinal lumen, it is important that stent delivery system should be inserted within the WON. When the diameter of WON is not large enough to insert stent delivery system, contralateral wall of WON may be injured. To prevent this adverse event, guidewire insertion may be useful after puncturing. In addition, according to the re- cent reports [2–4] , EUS-guided LAMS placement for WON drainage has been performed. We herein described a case of LAMS deploy- ment from the second part of the duodenum using semi-free hand technique.

To the Editor：Trauma, inflammation, iatrogenic procedures such as liverbiopsy and percutaneous transhepatic biliary drainage （PTBD）, vascularanomalies, coagulopathy status, and malignancy can causehemobilia [1]. Malignant causes of hemobilia include cholangiocarcinoma,pancreatic cancer, gallbladder tumor, and liver metastasis,but rarely hepatocellular carcinoma （HCC）. Here, we describeachieving endoscopic hemostasis for hemobilia caused by HCC,which was visualized using the single-operator, SPY-DS cholangioscope（Boston Scientific, Tokyo, Japan） under endoscopic retrogradecholangiopancreatography （ERCP） guidance. This scope is a dramaticimprovement over the SpyGlass system in that it is easier to insert into the biliary tract due to a tapered tip, a 120° digitalfield-of-view provides favorable visualization, and new injection and suction functions have been added via a two-port adaptor.

Temporal Structures of the North Atlantic Oscillation and Its Impact on the Regional Climate Variability

In this study, the temporal structure of the variation of North Atlantic Oscillation （NAO） and its impact on regional climate variability are analyzed using various datasets. The results show that blocking formations in the Atlantic region are sensitive to the phase of the NAO. Sixty-seven percent more winter blocking days are observed during the negative phase compared to the positive phase of the NAO. The average length of blocking during the negative phase is about 11 days, which is nearly twice as long as the 6-day length observed during the positive phase of the NAO. The NAO-related differences in blocking frequency and persistence are associated with changes in the distribution of the surface air temperature anomaly, which, to a large extent, is determined by the phase of the NAO. The distribution of regional cloud amount is also sensitive to the phase of the NAO. For the negative phase, the cloud amounts are significant, positive anomalies in the convective zone in the Tropics and much less cloudiness in the mid latitudes. But for the positive phase of the NAO, the cloud amount is much higher in the mid-latitude storm track region. In the whole Atlantic region, the cloud amount shows a decrease with the increase of surface air temperature. These results suggest that there may be a negative feedback between the cloud amount and the surface air *** in the Atlantic region.
