Endoscopic submucosal dissection in early gastric cancer in elderly patients and comorbid conditions

The prognosis of early gastric cancer(EGC) is good if there is no concomitant lymph node metastasis. Therefore, the early detection of EGC is important to improve the prognosis of patients with gastric cancer. In Japan, 40% to 50% of all gastric cancers are EGC, and endoscopic submucosal dissection(ESD) is widely accepted as a local treatment for these lesions, particularly for large lesions that at one time were an indication for gastrectomy because of the difficulty of en-bloc resection. Consequently, this procedure can preserve the entire stomach and the patient's postoperative quality of life. ESD has become a general technique with improved procedures and devices, and has become the preferred treatment for EGC rather than gastrectomy. Therefore, ESD may demonstrate many advantages in patients who have several comorbidities, particularly elderly population, patients taking antithrombotic agents, or patients with chronic kidney disease, or liver cirrhosis. However, it is not yet clear whether patients with both EGC and comorbidities are feasible candidates for ESD and whether they would consequently be able to achieve a survival benefit after ESD. In this review, we discuss the clinical problems of ESD in patients with EGC and those comorbid conditions.

研究了柿树品种“禅寺丸”(2 n=6x=90 )的减数分裂和小孢子形成过程。同源染色体配对和分离基本正常 ,中期构型为 45 ,但出现拟多价体 (次级配对 )。观察发现了不减数分裂现象。不减数发生在第 1次分裂 ,第 2次分裂时可见 2个 n=90的子核。未减数子核在四分体期形成二分体 ,小孢子期形成大型花粉粒。观察结果提示了柿树多倍体由未减数配子受精产生的可能性。

＜正＞ Objective: Diabetic neuropathy is associated with development of intestinal motilitydysfunction and autonomic neuropathy. We studied the effects of cyclohexenoniclong - chain fatty alcohol （FA） on isolated- ileum in the diabetes induced *** and Results: The rats divided into 5 groups. One is the non - diabetic group,others induced diabetes. 4 weeks after induction of diabetes, one group killed immediately, while other 3 groups were administered FA （0,2 or 8 mg/kg） for more 4 weeks. The serum glucose and serum insulin levels were unchanged by FA. The contractile responses to carbachol and KCl were argumentedin the isolated - diabeticileum. Real - time PCR and histological study showed changes of muscarinic M2 and M3 receptors of the diabetic ileum. Treatment with FA improved the thickness of intestine wall and diabetic - induced hyperreactivityof the rat ileum. Furthermore. FA reversed the diabetes - induced upregulation of muscarinic mRNAs in the diabetic rat ileum. Conclusion: These results indicate that FA has therapeutic effects on hyperreactivity in the diabetic ileum by ameliorating over expression of muscarinic M2 and M3 receptors mRNAs.

Gene fusions involving two of the MiT subfamily factors, such as TFE3, TFEB, TFC and MiTF, have been identified in renal cell carcinoma (RCC). Xp11.2 translocation RCC is a rare pediatric neoplasm that harbors gene fusions involving TFE3, which plays an important role in cell proliferation and survival. We herein present a case of RCC associated with Xp11.2 translocation/TFE3 gene fusion in a 14-year-old Japanese boy presenting gross hematuria and body weight loss. The tumor was characterized by histopathology, cytology and TFE3-immunohistochemistry/immunocytochemistry. Knowledge of distinctive morphological and immunostaining features of this tumor can help to accurately diagnose this rare subset of translocation associated RCC in routine pathological diagnostic procedures.

An experience of hyperbaric oxygen and transurethral coagulation combination therapy for abnormal vessels and intractable macrohematuria due to pelvic arteriovenous malformations

A 78-year-old man was admitted to our hospital with a chief complaint of gross hematuria. The patient was diagnosed with pelvic arteriovenous malformations and repeated arterial embolization. A cystoscopy demonstrated that bladder trigone was covered by many clots. At the same time, bleeding mucosa at the right side of the bladder dome was confirmed. We administered hyperbaric oxygen therapy 7 days after admission. Macrohematuria recovered gradually and we then performed transurethral coagulation at the dome with failed mucosa 14 days after admission. Macrohematuria recovered completely and there was no recurrence during hospitalization. Thirty days after admission, we performed cystoscopy and found clear mucosa in the trigone. The patient was discharged 32 days after admission.

The Ability of Edible Mushrooms to Act as Biocatalysts: Preparation of Chiral Alcohols Using Basidiomycete Strains

To examine the potential ability of edible mushrooms to act as biocatalysts, 19 basidiomycete strains were screened. Modified media (PG, O, and PGO medium) for liquid cultivation of these basidiomycete strains were designed and tested. Wet cells (>10 g) of 4 basidiomycete strains (Pleurotus salmoneostramineus H7, P. salmoneostramineus H13, Ganoderma lucidum NBRC31863, Flammulina velutipes NBRC31862) were harvested from PGO medium for 7 days. The stereoselective reduction of α-keto esters using the 4 strains was tested. It was found that each of these strains had a reducing activity toward 6 aliphatic α-keto esters. In the presence of L-alanine as an additive, the reduction of ethyl 2-oxobutanoate and ethyl 2-oxopentanoete by P. salmoneostramineus H7 produced the corresponding alcohol with a high conversion ratio and with excellent enantiomeric excess (>99% e.e. (R)). Furthermore, ethyl pyruvate, ethyl 2-oxobutanoate, and ethyl 2-oxopentanoate were predominantly reduced to the corresponding (R)-hydroxy ester (>99% e.e.) by G. lucidum. Thus, we found that these edible mushrooms have great potential to be used as biocatalysts for the stereoselective reduction of carbonyl compounds.

Recovery of serum testosterone following neoadjuvant and adjuvant androgen deprivation therapy in men treated with prostate brachytherapy

AIM: To investigate the time course of testosterone(T) recovery after cessation of androgen deprivation therapy(ADT) in patients treated with brachytherapy. METHODS: One-hundred and seventy-four patients treated between June 1999 and February 2009 were studied. Patients were divided into a short-term usage group(≤ 12 mo, n = 91) and a long-term usage group(≥ 36 mo, n = 83) according to the duration of gonadotropin-releasing hormone agonist therapy. Median follow-up was 29 mo in the short-term group and was 60 mo in the long-term ***: Cumulative incidence rates of T recovery to normal and supracastrate levels at 24 mo after cessationwere 28.8% and 74.6%, respectively, in the long-term usage group, whereas these values were 96.4% and 98.8% in the short-term usage group. T recovery to normal and supracastrate levels occurred significantly more rapidly in the short-term than in the long-term usage group(P < 0.001 and P < 0.001, respectively). Five years after cessation, 22.6% of patients maintained a castrate T level in the long-term usage group. On multivariate analysis, lower T levels(< 10 ng/d L) at cessation of ADT was significantly associated with prolonged T recovery to supracastrate levels in the longterm usage group(P = 0.002). CONCLUSION: Lower T levels at cessation of ADT were associated with prolonged T recovery in the longterm usage group. Five years after cessation of longterm ADT, approximately one-fifth of patients still had castrate T levels. When determining the therapeutic effect, especially biochemical control, we should consider this delay in T recovery.
