Renal Cell Carcinoma Associated with Xp11.2 Translocation/TFE3 Gene Fusion: A Case Report with Immunohistochemical and Cytological Features

Gene fusions involving two of the MiT subfamily factors, such as TFE3, TFEB, TFC and MiTF, have been identified in renal cell carcinoma (RCC). Xp11.2 translocation RCC is a rare pediatric neoplasm that harbors gene fusions involving TFE3, which plays an important role in cell proliferation and survival. We herein present a case of RCC associated with Xp11.2 translocation/TFE3 gene fusion in a 14-year-old Japanese boy presenting gross hematuria and body weight loss. The tumor was characterized by histopathology, cytology and TFE3-immunohistochemistry/immunocytochemistry. Knowledge of distinctive morphological and immunostaining features of this tumor can help to accurately diagnose this rare subset of translocation associated RCC in routine pathological diagnostic procedures.

Plasmablastic lymphoma(PBL) is a rare form of nonHodgkin's lymphoma that is associated with human immunodeficiency virus(HIV) infection. Although PBL is most commonly observed in the oral cavity of HIV-positive patients, it can also be observed at extra-oral sites in HIV-negative patients. This report represents an unusual case of HIV-negative PBL that occurred in the sigmoid colon. This patient had a history of systemic lupus erythematosus and an underlying immunosuppressive state from long term steroid therapy. The lymphoma cells were positive for CD138, kappa light chain restriction and Epstein-Barr virus and negative for CD20/L26, CD3, CD79 a, UCHL1(CD45RO) and cytokeratin(AE1/AE3). The patient died approximately 2 mo after the operation. In the present paper, we review cases of PBL of the colon in HIVnegative patients.

Retained foreign objects in the abdomen and pelvis are serious clinical problems yet the imaging required can present difficulties. Prolonged retention of lipiodized oil used for hysterosalpingography over years is very rare. However, lipiodized oil had previously been misdiagnosed as residual metallic material. We are reporting a case in which the latest computed tomography (CT) equipment seemed inadequate for obtaining a clear pre-operative diagnosis. Here, we describe the case of a 33-year-old Japanese female whose pelvis had contained retained lipiodized oil that had been suspected as residual metallic material. The preoperative diagnosis was very difficult and included three-dimensional computed tomography (3D-CT) of unclear results despite expectations of resolution. By laparoscopic surgery, we removed a cyst of approximately 2 cm containing a yellowish oily fluid. Postoperatively, we demonstrated that the fluid was lipiodized oil. A postoperative experiment to attempt distinguishing lipiodized oil from metal through gemstone spectral CT imaging did not offer clarity either. Distinguishing between retained lipiodized oil and metallic material in the abdominal cavity may still present unexpected difficulties even with the latest medical equipments.

＜正＞We show that femtosecond （fs） laser enables us to produce true three-dimensional （3D） microstructures inside a photosensitive glass （Foturan）, which has superior properties on transparency, hardness, chemical and thermal resistances, and biological compatibility. Exposure to the focused fs laser beam writes latent images inside the glass. Modified regions are developed by a post baking process and then preferentially etched away in dilute hydrofluoric （HF） acid. In general, an embedded microchannel fabricated by scanning focal beam perpendicularly to the direction of laser propagation becomes a cross section of an elliptical shape with a large aspect ratio according to the longitudinal beam profile. We demonstrate that the aspect ratio of channel can be improved merely by placing a slit before the focusing system.

A DC (data center) demands air-conditioning power as large as the 1/3-1/2 of total electricity consumption. Thus, energy saving of cooling power of DC yields considerable effect on both economic and environmental views. PV (Photovoltaic) and absorption refrigerator with CGS (cogeneration systems) or gas boiler are possible power saving options. The waste warm air from DC would be utilized for greenhouse heating when DC and greenhouse locate near in the suburbs. In this study, the authors develop an energy network model to assess the potential contribution of DC as a major electric power and chilled air consumer as well as the warm air supplier in a district to the energy efficiency improvement. The evaporation heat of LNG (liquefied natural gas) utilization is also considered as well as PV, CGS. This model is applied to the cases of the urban area in Tokyo which involves athletic center, shops and hospital and the suburbs including greenhouse and then compared.
