Both combined hepatocellular-cholangiocarcinoma(cHCCCCA)and cholangiolocarcinoma are rare primary liver ***-CCA is believed to originate from transformed hepatocellular carcinoma or liver stem/progenitor *** is characterized by ductular reaction-like anastomosing cords and glands resembling cholangioles or canals containing hepatocellular carcinoma components and adenocarcinoma *** to the 2019 revision of the World Health Organization criteria,a subtype with stem cell features as a subclassification of cHCC-CCA was abolished for lack of conclusive evidence of the stem cell origin *** led to the classification of cholangiolocarcinoma with hepatocytic differentiation as ***,cholangiolocarcinoma without hepatocytic differentiation is classified as a subtype of small-duct cholangiocarcinoma and is assumed to originate from the bile ***,we report the first case of double primary cHCC-CCA and cholangiolocarcinoma without hepatocytic differentiation in different hepatic segments of a cirrhotic *** believe this case supports the validity of the new World Health Organization criteria because the pathological finding of cHCC-CCA in this case shows the transformation of hepatocellular carcinoma to ***,this case may demonstrate that immature ductular cell stemness and mature hepatocyte cell stemness in hepatocarcinogenesis can coexist in the same *** results provide valuable insights into the mechanisms of growth,differentiation,and regulation of liver cancers.

Comparison of Oxidative Stability of Monogalactosyl Diacylglycerol, Digalactosyl Diacylglycerol, and Triacylglycerol Containing Polyunsaturated Fatty Acids

Oxidative stability of three different lipid classes, namely, monogalactosyl diacylglycerol (MGDG) and digalactosyl diacylglycerol (DGDG) from spinach and edible brown seaweed (Akamoku) and triacylglycerol (TAG) of linseed oil was compared. Analysis of oxygen consumption and polyunsaturated fatty acid (PUFA) composition demonstrated that spinach DGDG had the highest oxidative stability, followed by Akamoku DGDG, Akamoku MGDG, spinach MGDG, and linseed TAG. These results disagree with the order of oxidative stability expected from the average number of bis-allylic positions of each lipid. Additionally, DGDG constituents of both spinach and Akamoku showed higher oxidative stability than their MGDG constituents. The unusual oxidative stability of MGDG and DGDG could be conferred by the protection of bis-allylic positions of the PUFA against oxidative attack by the galactosyl moiety of the GL.

Repeated Exposure to Cruciferous Allyl Nitrile Protects against Chemically Induced Skin Inflammation in the Mouse

Repeated exposure to cruciferous allyl nitrile can induce antioxidant and phase 2 detoxification enzymes in various tissues. In the present study, we examined the effect of five days repeated exposure to allyl nitrile at subtoxic levels (0 - 400 μmol/kg/day) on the mouse ear. There was an increase in catalase activity in the ear at 100 - 400 μmol/kg/day, while elevated quinone reductase activity was observed at 400 μmol/kg/day only. Next, after repeated allyl nitrile exposure (0 - 400 μmol/kg/day), the skin irritant croton oil was applied to the ear to induce skin acute inflammation (oedema). Compared with the 0 μmol/kg/day group, animals in the 100 and 400 μmol/kg/day pre-treatment groups showed reduced oedematous response to croton oil. The reduced oedematous response was inversely associated with enhanced myeloperoxidase activity used as index of the presence of neutrophils. These data suggest that repeated exposure to allyl nitrile at subtoxic levels contributes to protection against croton oil-induced ear dermatitis, potentially through decreasing reactive oxygen species and through infiltration of neutrophils.

AIM To identify certain clinical factors other than the type of gastrectomy which affect the postoperative quality of life(QOL) of patients after *** The postgastrectomy syndrome assessment scale(PGSAS)-45 was designed to assess the severity of symptoms, the living status and the QOL of gastrectomized patients. It consists of 45 items, of which 22 are original items while 23 were retrieved from the SF-8 and Gastrointestinal Symptoms Rating Scale questionnaires with permission. A nationwide surveillance study to validate PGSAS was conducted and 2368 gastric cancer patients who underwent various types of gastrectomy at 52 medical institutions were enrolled. Of these, 1777 patients who underwent total gastrectomy(TG) reconstructed with Roux-Y(n = 393), distal gastrectomy(DG) reconstructed with Billroth-I(n = 909), or DG reconstructed with Roux-Y(n = 475) were evaluated in the current study. The influence of the type of gastrectomy and other clinical factors such as age, sex, duration after surgery, the symptom severity, the degree of weight loss, dietary intake, and the ability for working on the postoperative QOL(i.e., dissatisfaction for daily life subscale, physical component summary and mental component summary of the SF-8) were examined by multiple regression analysis(MRA). In addition, importance of various symptoms such as esophageal reflux, abdominal pain, meal-related distress, indigestion, diarrhea, constipation and dumping on the postoperative living status and QOL were also appraised by *** The postoperative QOL were significantly deteriorated in patients who underwent TG compared to those after DG. However, the extent of gastrectomy was not an influential factor on patients' QOL when adjusted by the MRA. Among various clinical factors, the symptom severity, ability for working, and necessity for additional meals were the most influential factorsto the postoperative QOL. Prognostic Nutritional Index Predicts Life Expectancy of Patients with End-Stage Oral Cancer: A Retrospective Study

Background: Generally, clinicians do not accurately estimate life expec-tancy in terminally ill patients with cancer. Aim: To evaluate the value of the Prognostic Nutritional Index (PNI) for accurately estimating the life expectancy of patients with end-stage oral cancer. Design: A longitudinal section study. Setting/participants: Fifteen patients (12 men;mean age: 71.7 years) who died of oral cancer between 2005 and 2014 (the terminal group) were included. The mean PNI values at the initial visit and at 3, 2, and 1 months before the deaths were comparatively analyzed. Results: The mean follow-up period was 133 days. At the initial examination, the PNI values were 49.1 ± 4.5 (p = 0.6723). The PNI value of the terminal group was 35.6 ± 5.1 at 2 months before death and 28.6 ± 3.0 at 1 month before death. The PNI values at 3, 2, and 1 months before death in the terminal group significantly differed from each other and from that at the initial visit and steadily decreased until death. Conclusions: Our findings suggest the utility of PNI as a prognostic index in patients with end-stage oral cancer patients. Furthermore, the PNI should be routinely considered in the nutritional management of patients with oral cancer nearing death.

MIM： To investigate the different impact of genotypes E and C on the development of liver cirrhosis （LC） among different age groups of patients with chronic hepatitis （CH-B）.METHODS： We examined the outcome of 121 patients with CH-B, divided by age and genotype. Univariate analyses were used to compare different groups. The Cox proportional hazard model was employed to evaluate factors affecting the development of LC.RESULTS： In patients 〈 30 years old, there were no significant predictors for development of LC. However, in patients ≥ 30 years old, genotype C was the only significant predictor. In the genotype C group, 8 of 12 patients who progressed to LC were 30-49 years old at initial diagnosis of chronic hepatitis （7 patients were positive for HBeAg）. In the genotype B group, 4 of 8 patients who developed LC were ≥50 years old at initial diagnosis and were HBeAg-negative.CONCLUSION： The rate of development of LC was comparable in patients infected with genotypes B and C when CH-B occurred at 〈 30 years old. However, CH-B patients infected with genotype C showed poor prognosis if they were 30-49 years old and were positive for HBeAg. Age-specific natural course of CH-B should be considered when patients with CH-B are treated with antiviral drugs.

Association of anastomotic leakage with long-term oncologic outcomes of patients with esophagogastric junction cancer

BACKGROUND Despite improvements in surgical procedures and peri-operative patients management,the postoperative complications in esophagogastric junction(EGJ)cancer remain high because of technical *** studies have indicated the negative influence of postoperative infectious complications on long-term survival after gastrointestinal ***,no study has shown the association between postoperative complications and long-term survival of patients with EGJ *** To elucidate influence of postoperative complications on the long-term outcomes of patients with EGJ *** A total of 122 patients who underwent surgery for EGJ cancer at the Keio University were included in this *** examined the association between complications and long-term oncologic *** In all patients,the 3-year overall survival(OS)rate was 71.9%,and the recurrencefree survival(RFS)rate was 67.5%.Compared with patients without anastomotic leakage,those with anastomotic leakage had poor median OS(8 mo vs not reached,P=0.028)and median RFS(5 mo vs not reached,P=0.055).Among patients with cervical anastomosis,there were not significant differences between patients with and without anastomotic ***,among patients who underwent intrathoracic anastomosis,patients with anastomotic leakage had significantly worse OS(P=0.002)and RFS(P=0.005).CONCLUSION Anastomotic leakage was significantly associated with long-term oncologic outcomes of patients with EGJ cancer,especially those who underwent intrathoracic *** anastomosis with subtotal esophagectomy may be an option for the patients who are at high risk for anastomotic leakage.

Mixed ionic electronic conductors(MIECs)have attracted increasing attention as anode materials for solid oxide fuel cells(SOFCs)and they hold great promise for lowering the operation temperature of ***,there has been a lack of understanding of the performance-limiting factors and guidelines for rational design of composite metal-MIEC *** a newly-developed approach based on 3 D-tomography and electrochemical impedance spectroscopy,here for the first time we quantify the contribution of the dual-phase boundary(DPB)relative to the three-phase boundary(TPB)reaction pathway on real MIEC electrodes.A new design strategy is developed for Ni/gadolinium doped ceria(CGO)electrodes(a typical MIEC electrode)based on the quantitative analyses and a novel Ni/CGO fiber-matrix structure is proposed and fabricated by combining electrospinning and tape-casting methods using commercial *** only 11.5 vol%nickel,the designer Ni/CGO fiber-matrix electrode shows 32%and 67%lower polarization resistance than a nano-Ni impregnated CGO scaffold electrode and conventional cermet electrode *** results in this paper demonstrate quantitatively using real electrode structures that enhancing DPB and hydrogen kinetics are more efficient strategies to enhance electrode performance than simply increasing TPB.

Toxicological Evaluation of Dietary Garlic <i>(Allium sativum)</i>Powder in European Sea Bass <i>Dicentrarchus labrax</i>Juveniles

The effects of dietary garlic (Allium sativum) powder as a feed additive on hematological and biochemical health characteristics of European Sea bass Dicentrarchus labrax juveniles were studied. Experimental fish were fed diets supplemented with garlic powder at 0 (control), 2%, 4%, or 6% levels for a period of 60 days. Results showed that the red blood cell count (RBC), hemoglobin (Hb) concentration, hematocrit (%), and mean corpuscular Hb in fish fed garlic powder diets at dietary inclusion levels of 4% and 6%, were significantly lower than the control values. Serum glucose was significantly lower in Sea bass that were fed garlic powder diets (4% and 6%) compared to the control group. Serum triglyceride and globulin levels in fish fed a 4% garlic powder diet were significantly higher than the control values, whereas these two variables in the 2% and 6% garlic treatments were similar to the control values. The cholesterol levels in the 2% and 6% treatment groups were lower than the value recorded for the control group. As a result, it is suggested that garlic powder supplementation in diets for Sea bass juveniles should not exceed 2%. The present study is the first attempt to examine the effects of dietary garlic powder on the hematological and biochemical status in Sea bass juveniles.
