Short-and long-term outcomes of endoscopically treated superficial non-ampullary duodenal epithelial tumors

BACKGROUND It is widely recognized that endoscopic resection(ER) of superficial nonampullary duodenal epithelial tumors(SNADETs) is technically challenging and may carry high risks of intraoperative and delayed bleeding and *** adverse events could be more critical than those occurring in other levels of the gastrointestinal tract. Because of the low prevalence of the disease and the high risks of severe adverse events, the curability including short-and long-term outcomes have not been standardized *** To investigate the curability including short-and long-term outcomes of ER for SNADETs in a large case *** This retrospective study included cases that underwent ER for SNADETs at our university hospital between March 2004 and July 2017. Short-term outcomes of ER were measured based on en bloc and R0 resection rates as well as adverse events. Long-term outcomes included local recurrence detected on endoscopic surveillance and disease-specific mortality in patients followed up for ≥ 12 mo after *** In the study, 131 patients with 147 SNADETs were analyzed. The 147 ERs consisted of 136 endoscopic mucosal resections(EMRs)(93%) and 11 endoscopic submucosal dissections(ESDs)(7%). The median tumor diameter was 10 *** pathology diagnosis was adenocarcinoma(56/147, 38%), high-grade intraepithelial neoplasia(44/147, 30%), or low-grade intraepithelial neoplasia(47/147, 32%). The R0 resection rate was 68%(93/136) in the EMR group and73%(8/11) in the ESD group, respectively. Cap-assisted EMR(known as EMR-C)showed a higher rate of R0 resection compared to the conventional method of EMR using a snare(78% vs 62%, P = 0.06). No adverse event was observed in the EMR group, whereas delayed bleeding, intraoperative perforation, and delayed perforation in 3, 3, and 5 patients occurred in the ESD group, respectively. An extremely rare case of pancreatic metastasis of esophageal squamous cell carcinoma

The imbalance between β-amyloid （Aβ） generation and clearance plays a fundamental role in the pathogenesis of Alzheimer＇s disease （AD）. The sporadic form of AD is characterized by an overall impairment in Aβ clearance. Immunotherapy targeting Aβ clearance is believed to be a promising approach and is under active clinical investigation. Autophagy is a conserved pathway for degrading abnormal protein aggregates and is crucial for Aβ clearance. We previously reported that oral vaccination with a recombinant AAV/Aβ vaccine increased the clearance of Aβ from the brain and improved cognitive ability in AD animal models, while the underlying mechanisms were not well understood. In this study, we first demonstrated that oral vaccination with rAAV/Aβ decreased the p62 level and up-regulated the LC3B- II/LC3B-I ratio in APP/PS1 mouse brain, suggesting enhanced autophagy. Further, inhibition of the Akt/mTOR pathway may account for autophagy enhancement. We also found increased anti-Aβ antibodies in the sera of APP/PS1 mice with oral vaccination, accompanied by elevation of complement factors C1q and C3 levels in the brain. Our results indicate that autophagy is closely involved in oral vaccination-induced Aβ clearance, and modulating the autophagy pathway may be an important strategy for AD prevention and intervention.

Surgical resection of a solitary para-aortic lymph node metastasis from hepatocellular carcinoma

Lymph node (LN) metastases from hepatocellular carcinoma (HCC) are considered uncommon. We describe the surgical resection of a solitary para-aortic LN metastasis from HCC. A 65-year-old Japanese man with B-type liver cirrhosis was admitted for the evaluation of a liver tumor. He had already undergone radiofrequency ablation, transcatheter arterial chemoembolization, and percutaneous ethanol injection therapy for HCC. Despite treatment, viable regions remained in segments 4 and 8. We performed a right paramedian sectionectomy with partial resection of the left paramedian section of the liver. Six months later, serum concentrations of alpha-fetoprotein (189 ng/mL) and PIVKA-2 (507 mAU/mL) increased. Enhanced com- puted tomography of the abdomen revealed a tumor (20 mm in diameter) on the right side of the abdominal aorta. Fluorine-18 fluorodeoxyglucose positron emission tomography revealed an increased standard uptake value. There was no evidence of recurrence in other regions. Esophagogastroduodenoscopy and colonoscopy revealed no malignant tumor in the gastrointestinal tract. Para-aortic LN metastasis from HCC was thus diagnosed. We performed lymphadenectomy. Histopathological examination revealed that the tumor was largely necrotic, with poorly differentiated HCC on its surface, which confirmed the suspected diagnosis. After 6 mo tumor marker levels were normal, with no evidence of recurrence. Our experience suggests that a solitary para-aortic LN metastasis from HCC can be treated surgically.

A Study on Relationship among Blade Camber Direction and Pitch Angle and Performance of a Small Straight-Blade Darrieus Wind Turbine by Using Scale Test Model and Gurney Flap

Straight-blade Darrieus vertical axis wind turbines are used as medium and small size wind turbine because of higher power output in vertical axis wind turbine (VAWT). In our previous study, the relationship between the performance and Reynolds number based on airfoil chord length had been investigated by using small-scale test models of lift-type VAWT, and the results showed that the performance of tested wind turbine models with small diameter was clearly lower than that of the large-scale field test machine, and its performance also varies significantly with the blade pitch angle. In this study, we focused on the performance of a small-scale straight-blade Darrieus VAWT, the relationship among the blade airfoil camber direction and the pitch angle, and the performance of the small-scale VAWT was examined experimentally by using a small-scale VAWT test model with Gurney flap which was a small flat plate. Gurney flaps with its height h, as a ratio to the blade chord length c, h/c = 0.036 to 0.055, were attached to the blades of the VAWT test model, in addition, the attaching direction of the Gurney flap on the blade was examined for both inward and outward of the rotor, and the pitch angle was also examined for a range of −5 to 10 degrees. These results are discussed comparing with the result of the VAWT without Gurney flap and considering the numerical results for the single blade with/without the Gurney flap. The results showed that the performance of the tested VAWT was reversed between the inward and outward Gurney flaps around a pitch angle of 10 degrees. That is, the inward Gurney flap was superior at a pitch angle of less than 10 degrees, while the outward Gurney flap was effective at a pitch angle of more than 10 degrees. Purification and characterization of lizard fish pepsinogens and pepsins

Five pepsinogens were identified in lizard fish (Saurida wanieso) stomach and two of them (PG-Ⅰand PG-Ⅱ) were purified to homogeneity by column chromatographies on DEAE-Sephacel, Ultrogel AcA54, DEAE-Sepharose and Poly-L-Lysine Agarose. The molecular weights of the two purified PGs were about 41 kDa. Both pepsinogens were converted into pepsins within a few minutes under pH 2. 5. Corresponding pepsins were also purified by a chromatography on S-Sepharose and their molecular weights were 36 kDa as determined on SDS-PAGE. Optimum pHs of the pepsins were around 3. 5 and optimum temperatures were 45℃using hemoglobin as substrate. Both enzymes showed high sensitivity toward pepstatin A, a specific inhibitor of aspartic proteinases. The N-terminal amino acid sequences of PG-Ⅰand PG-Ⅱas determined to the 16th residue were exactly the same and showed high identity to pepsinogens of other animals.
