Early colonoscopy and urgent contrast enhanced computed tomography for colonic diverticular bleeding reduces risk of rebleeding

BACKGROUND Colonoscopy within 24 h of hospital admission for colonic diverticular bleeding(CDB)is ***,little is known about rates of rebleeding within 30 *** posited that a group of patients who underwent contrast-enhanced computed tomography(CT)within 4 h of the last hematochezia and colonoscopy within 24 h would experience fewer incidences of *** To evaluate the outcomes of early colonoscopy for CDB among different groups of *** Data from 182 patients with CDB who underwent contrast-enhanced CT and colonoscopy between January 2011 and December 2018 at the study site were retrospectively *** were divided into groups based on the timing of the CT imaging,within or at 4 h were defined as urgent CTs(n=100)and those performed after 4 h were defined as elective CTs(n=82).Main outcomes included rebleeding within 30 d and the identification of stigmata of recent hemorrhage(SRH)(i.e.,active bleeding,non-bleeding visible vessels,or adherent clots).RESULTS In total,182 patients(126 men and 56 women)with median ages of 68.6(range,37-92)and 73.7(range,48-93)years,respectively,underwent CT imaging and colonoscopy within 24 h of the last *** for whom CT was performed within 4 h of the last hematochezia were included in the urgent CT group(n=100)and patients for whom CT was performed after 4 h were included in the elective CT group(n=82).SRH were identified in 35.0%(35/100)of the urgent CT cases and 7.3%(6/82)of the elective CT cases(P<0.01).Among all patients with extravasation-positive images on CT,SRH was identified in 31 out of 47 patients(66.0%)in the urgent CT group and 4 out of 20 patients(20.0%)in the elective CT group(P<0.01).Furthermore,rates of rebleeding within 30 d were significantly improved in the urgent CT and extravasation-positive cases(P<0.05).Results from the evaluation of early colonoscopy did not show a difference in the ability to detect SRH identification or rebleeding *** case

Estimate of Uniaxial Compressive Strength of Hydrothermally Altered Soft Rocks Based on Strength Index Tests

The purpose of this study was to clarify the relationships between results of index tests and uniaxial compressive strength (UCS) in hydrothermally altered soft rocks of the Upper Miocene, which are typical of the soft rock found in northeastern Hokkaido, Japan. Index tests were performed using point load testing machine and needle penetrometer with irregular lump specimens under forced-dry, forced-wet, and natural-moist states. The relationships between irregular lump point load strength (IPLS) index and UCS, and needle penetration (NP) index and UCS were “UCS = approximately 19 IPLS index” and “UCS = 0.848 (NP index)0.619”, respectively, in soft rocks with a UCS below 25 MPa. These relationships could be applied to on-site tests of rocks with natural moisture content. The UCS could be calculated from IPLS and NP tests on soft rocks only when UCS was below 25 MPa, using the equations obtained as a result of this study.

Changes in choroidal thickness in healthy pediatric individuals: a longitudinal study

AIM： To investigate the changes in the choroidal thickness in healthy pediatric children in a longitudinal study, and to determine the ocular and systemic parameters that were significantly correlated with the changes in the choroidal thickness. METHODS： This study included 64 eyes of 34 healthy Japanese children with a mean age（±SD） of 4.4（±0.4）y（range, 3.6-5.8 y） at baseline. Swept-source optical coherence tomography（SS-OCT） was used to record images of the retina and choroid at the baseline and after a mean followup period of about 1.5 y. The 3 D raster scan protocol was used to construct the choroidal thickness map. Mean choroidal thickness was calculated for each of the nine sectors of the Early Treatment Diabetic Retinopathy Study grid. Best-corrected visual acuity, axial length, body height, and weight were also measured. Changes in measurements were defined as the baseline values subtracted from the values at the final visit. A generalized estimating equation was used to eliminate the effect of within-subject intereye correlations. RESULTS： The mean central choroidal thickness was significantly reduced during the follow-up period（baseline, 301.8±8.6 μm; final visit, 286.6±8.0 μm, P〈0.001）. The decrease in the choroidal thickness was greatest in the central sector, followed by the sectors of the inner and outer rings. The inner and outer rings had diameters of 1 to 3 mm and 3 to 6 mm, respectively. The changes in the choroidal thickness in the central, inner ring, and outer ring sectors were significantly and negatively correlated with the age, baseline body height, baseline body weight, and elongation of the axial length. CONCLUSION： These results indicate that the choroidal thickness among preschool-aged Japanese children decreased significantly during the follow-up period. Giant primary angiosarcoma of the small intestine showing severe sepsis

Primary malignant tumors of the small intestine are rare, comprising less than 2% of all gastrointestinal tumors. An 85-year-old woman was admitted with fever of 40 °C and marked abdominal distension. Her medical history was unremarkable, but blood examination showed elevated inflammatory markers. Abdominal computed tomography showed a giant tumor with central necrosis, extending from the epigastrium to the pelvic cavity. Giant gastrointestinal stromal tumor of the small intestine communicating with the gastrointestinal tract or with superimposed infection was suspected. Because no improvement occurred in response to antibiotics, surgery was performed. Laparotomy revealed giant hemorrhagic tumor adherent to the small intestine and occupying the peritoneal cavity. The giant tumor was a solid tumor weighing 3490 g, measuring 24 cm × 17.5 cm × 18 cm and showing marked necrosis. Histologically, the tumor comprised spindle-shaped cells with anaplastic large nuclei. Immunohistochemical studies showed tumor cells positive for vimentin, CD31, and factor VIII-related antigen, but negative for c-kit and CD34. Angiosarcoma was diagnosed. Although no postoperative complications occurred, the patient experienced enlargement of multiple metastatic tumors in the abdominal cavity and died 42 d postoperatively. The prognosis of small intestinal angiosarcoma is very poor, even after volume-reducing palliative surgery.

The molar volumes and bulk moduli of CaSiO perovskite are calculated in the temperature range from 300 to 2800 K and the pressure range from 0 to 143 GPa using molecular dynamics simulations that employ the breathing shell model for oxygen and the quantum correction in addition to the conventional pairwise interatomic potential *** performance of five equations of state,i.e.,the Keane,the general ized-Rydberg,the Holzapfel,the Stacey-Rydberg,and the third-order Birch-Murnaghan equations of state are examined using these *** third-order Birch-Murnaghan equation of state is found to have a clear tendency to overestimate the bulk modulus at very high *** Stacey-Rydberg equation of state degrades slightly at very high pressures along the low-temperature *** comparison,The Keane and the Holzapfel equations of state remain accurate in the whole temperature and pressure range considered in the present study.K’ derived from the Holzapfel equation of state also agrees best with that calculated independently from molecular dynamics *** adiabatic bulk moduli of CaSiO perovskite along lower mantle geotherms are further calculated using the Keane and the Mie-Gr(u|¨)neisen-Debye equations of *** are found to be constantly higher than those of the PREM by～5%,and also very similar to those of the MgSiO *** results support the view that CaSiO perovskite remains invisible in the Earth’s lower mantle.
