Clinical significance of serum levels of vascular endothelial growth factor and its receptor in biliary disease and carcinoma

AIM: To investigate the clinical significance of serum vascular endothelial growth factor (VEGF) and soluble VEGF receptor-1 (VEGFRl/Flt-1) (sVEGFR1) levels in biliary diseases. METHODS: We analyzed the serum levels of these proteins in patients with acute cholangitis (group 1), biliary malignancies (group 2), and primary biliary cirrhosis or primary sclerosing cholangitis (group 3), and in healthy donors (group 4). The influence of inflammation was also analyzed. Serum VEGF levels were expressed as VEGF per platelet (VEGF/PLT, pg/106) in order to exclude the influence of platelet counts. RESULTS: sVEGFRl levels were significantly higher in groups 1 and 2 than in the control group, but did not correlate with inflammatory markers. VEGF/PLT levels were generally higher in patients with active inflammation than in those with carcinoma. C-reactive protein strongly correlated with the levels of serum VEGF independently of platelet and leukocyte counts, even in cancer patients. In cancer patients, VEGF/PLT and sVEGFRl levels might be indicators for evaluating the effect of medical treatment or the disease progression. CONCLUSION: Serum VEGF and VEGFR1 might be useful markers for gauging the clinical effect of various treatments on patients.

Effects of Fuscoporia obliqua on Postprandial Glucose Excursion and Endothelial Dysfunction in Type 2 Diabetic Patients

＜正＞ There is a great need to develop an appropriate analysis method for the deformation of amorphous materials such as quartz glasses, described here as high viscosity bodies. Since high viscosity bodies satisfy incompressibility during viscous flow, the theory of rigid-plastic flow was considered to be a good potential tool for the prediction of their deformation. In the present article, the rigid-plastic finite element method （RPFEM） is applied to the deformation in incompressible viscous fluids. We lay special emphasis on the formulation of the yield stress for viscous fluids and provide the popular approaches of Lagrange multiplier method and penalty function method to deal with the incompressible condition. Lastly, some ex. aples are provided using quartz glass deformation.

Postprandial hyperglycemia has been reported to elicit endothelial dysfunction and provoke future cardiovascular complications. A reduction of postprandial blood glucose levels by the glucosidase inhibitor Fuscoporia obliqua was associated with a risk reduction of cardiovascular complications, but the effects of Fuscoporia obliqua on endothelial function have never been elucidated. This study is aimed to assess the efficacy of Fuscoporia obliqua on postprandial metabolic parameters and endothelial function in type 2 diabetic patients. Postprandial peak glucose (14.47±1.27 vs. 8.50±0.53 mmol/liter), plasma glucose excursion (PPGE), and change in the area under the curve (AUC) glucose after a single loading of test meal (total 450 kcal; protein 15.3%; fat 32.3%; carbohydrate 51.4%) were significantly higher in the diet-treated type 2 diabetic patients (n=14) than the age-and sex-matched controls (n=12). The peak forearm blood flow response and total reactive hyperemic flow (flow debt repayment) during reactive hyperemia, indices of resistance artery endothelial function on strain-gauge plethysmography, were unchanged before and after meal loading in the controls. But those of the diabetics were significantly decreased 120 and 240 min after the test meal. A prior administration of Fuscoporia obliqua decreased postprandial peak glucose, PPGE, and AUC glucose. The peak forearm blood flow and flow debt repayment were inversely well correlated with peak glucose, PPGE, and AUC glucose, but not with AUC insulin or the other lipid parameters. Even a single loading of the test meal was shown to impair the endothelial function in type 2 diabetic patients, and the postprandial endothelial dysfunction was improved by a prior use of Fuscoporia obliqua. Fuscoporia obliqua might reduce macrovascular complication by avoiding endothelial injury in postprandial hyperglycemic status.

Background: This study aimed to elucidate the experiences of a woman with type 1 diabetes during the course of her pregnancy. Method: This case study involved 1 participant. Data were collected through a semi-structured interview. Results: The data of experiences during pregnancy of Mrs. A comprised 20 codes, 5 categories, and 11 subcategories. The 5 categories were [abnormalities in the delicate body], [difficulty in blood glucose control for safe childbirth], [the burden of visiting both obstetrics] and [internal medicine departments], and [hope for safe childbirth]. Conclusion: The categories extracted in this study included terms that suggested negative experiences, such as “abnormality”, “difficulty”, “inconvenience”, and “burden”. Mrs. A might have used these negative terms due to the difficulties in controlling her blood glucose levels.

Adipose-derived mesenchymal stem cells （ADSCs） are a treatment cell source for patients with chronic liver injury. ADSCs are characterized by being harvested from the patient’s own subcutaneous adipose tissue, a high cell yield （ i.e. , reduced immune rejection res-ponse）, accumulation at a disease nidus, suppression of excessive immune response, production of various growth factors and cytokines, angiogenic effects, anti-apoptotic effects, and control of immune cells via cell-cell interaction. We previously showed that conditioned medium of ADSCs promoted hepatocyte proliferation and improved the liver function in a mouse model of acute liver failure. Furthermore, as found by many other groups, the administration of ADSCs improved liver tissue fbrosis in a mouse model of liver cirrhosis. A comprehensive protein expression analysis by liquid chromatography with tandem mass spectrometry show-ed that the various cytokines and chemokines produced by ADSCs promote the healing of liver disease. In this review, we examine the ability of expressed protein com-ponents of ADSCs to promote healing in cell therapy for liver disease. Previous studies demonstrated that ADSCs are a treatment cell source for patients with chronic liver injury. This review describes the various cytokines and chemokines produced by ADSCs that promote the healing of liver disease.

Impedance Spectroscopic Study on Room Temperature Ionic Liquid-Water Mixtures

AC impedance spectroscopy in pure room temperature ionic liquids （RTILs） and RTIL-water mixture was measured at the temperature of range from 30 ℃ down to -30 ℃. The cations of RTILs are N,N-diethyl-N-methyl-N-（2-methoxyethyl） ammonium （[DEME]）, 1-ethyl-3-methylimidazolium （[C2mim]） and l-butyl-3-methylimidazolium （[Camim]）, the anions are tetrafluoroborate （[BF4]） and bis（trifluoromethanesulfonyl）imide （[TFSI]）. In all pure RTILs, there are two kinds of local minima in real part of the AC impedance Zreal. By adding water to [DEME][BF4] （0 mol% 〈 x 〈 94 mol%） at room temperature, the local minimum value at higher frequency decreased remarkably at the fixed frequency with increasing water concentration. Above 94 mol% H20, a quite different profile of the AC impedance spectroscopy was obtained. In addition to Zreal. temperature dependence of an imaginary part of the impedance Zimag had an isosbestic point below 94 mol%. The isosbestic point disappeared above 94 mol%. The isosbestic point in Zing reveals an interaction between [DEME][BFa] and H2O.

To determine the predictive factors and impact of body weight loss on postgastrectomy quality of life (QOL). METHODSWe applied the newly developed integrated questionnaire postgastrectomy syndrome assessment scale-45, which consists of 45 items including those from the Short Form-8 and Gastrointestinal Symptom Rating Scale instruments, in addition to 22 newly selected items. Between July 2009 and December 2010, completed questionnaires were received from 2520 patients with curative resection at 1 year or more after having undergone one of six types of gastrectomy for Stage I gastric cancer at one of 52 participating institutions. Of those, we analyzed 1777 eligible questionnaires from patients who underwent total gastrectomy with Roux-en-Y procedure (TGRY) or distal gastrectomy with Billroth-I (DGBI) or Roux-en-Y (DGRY) procedures. RESULTSA total of 393, 475 and 909 patients underwent TGRY, DGRY, and DGBI, respectively. The mean age of patients was 62.1 ± 9.2 years. The mean time interval between surgery and retrieval of the questionnaires was 37.0 ± 26.8 mo. On multiple regression analysis, higher preoperative body mass index, total gastrectomy, and female sex, in that order, were independent predictors of greater body weight loss after gastrectomy. There was a significant difference in the degree of weight loss (P 25 kg/m2). Multiple linear regression analysis identified lower postoperative body mass index, rather than greater body weight loss postoperatively, as a certain factor for worse QOL (P 2, 0.028-0.080). CONCLUSIONWhile it is certainly important to maintain adequate body weight after gastrectomy, the impact of body weight loss on QOL is unexpectedly small.
