Predictive factors for body weight loss and its impact on quality of life following gastrectomy

AIM To determine the predictive factors and impact of body weight loss on postgastrectomy quality of life(QOL). METHODS We applied the newly developed integrated questionnaire postgastrectomy syndrome assessment scale-45, which consists of 45 items including those from the Short Form-8 and Gastrointestinal Symptom Rating Scale instruments, in addition to 22 newly selected items. Between July 2009 and December 2010, completed questionnaires were received from 2520 patients with curative resection at 1 year or more after having undergone one of six types of gastrectomy for Stage Ⅰ gastric cancer at one of 52 participating institutions. Of those, we analyzed 1777 eligible questionnaires from patients who underwent total gastrectomy with Roux-en-Y procedure(TGRY) or distal gastrectomy with Billroth-I(DGBI) or Roux-en-Y(DGRY) procedures. RESULTS A total of 393, 475 and 909 patients underwent TGRY, DGRY, and DGBI, respectively. The mean age of patients was 62.1 ± 9.2 years. The mean time interval between surgery and retrieval of the questionnaires was 37.0 ± 26.8 mo. On multiple regression analysis, higher preoperative body mass index, total gastrectomy, and female sex, in that order, were independent predictors of greater body weight loss after gastrectomy. There was a significant difference in the degree of weight loss(P 25 kg/m2). Multiple linear regression analysis identified lower postoperative body mass index, rather than greater body weight loss postoperatively, as a certain factor for worse QOL(P < 0.0001) after gastrectomy, but the influence of both such factors on QOL was relatively small(R2, 0.028-0.080).CONCLUSION While it is certainly important to maintain adequate body weight after gastrectomy, the impact of body weight loss on QOL is unexpectedly small.

生态群落自下而上和自上而下的调控被广泛的用于描述物种相互作用的机制。传统方法往往使用时间序列间的相关系数来辨别这两种调控机制。这里,我们展示不宜使用传统方法的三类情形,它们是不稳定性,时间序列的错置和复杂群落结构。在这些情况下使用传统方法将会得到错误的结论。在本文中,当只有高营养级种群影响低营养级种群的动态时,我们称其为自上而下的调控,反之则为自下而上的调控。我们新引进了一个统计学方法,叫做协整检验,来检测这两种调控机制。这个方法可以有效的处理上面提及的三类不适合传统方法的情形。最后,我们使用墨西哥湾的渔业调查数据来演示这一新方法。结果显示,墨西哥湾褐虾种群和三种底栖鱼类存在自下而上的调控,而且墨西哥湾不同区域也存在着群落结构的差异。

Background factors influencing postgastrectomy syndromes after various types of gastrectomy

BACKGROUND Postgastrectomy syndromes(PGS) after curative gastrectomy for gastric cancer are influenced by not only gastrectomy type but also by background factors. Recently, a nationwide PGS study was performed using the Postgastrectomy Syndrome Assessment Scale-45(PGSAS-45) *** To determine the influence of each background factor on PGS for each gastrectomy type using PGS assessment study(PGSAS) data as an additional analysis. METHODS The data of 2368 patients were obtained from the PGSAS. This included patients undergoing distal gastrectomy(DG) with Billroth I reconstruction, DG with Roux-en-Y reconstruction, total gastrectomy with Roux-en-Y, proximal gastrectomy, pylorus-preserving gastrectomy(PPG), and local resection. Multiple regression analysis was performed to explore the independent effects of each background factor on the main outcome measures(MOMs) of PGSAS-45 for each gastrectomy type. The background factors included postoperative period, age, sex, surgical approach(laparoscopic or open), and the status of the celiac branch of the vagal *** The MOMs of DG and PPG were highly affected by background factors, whereas those of total gastrectomy with Roux-en-Y, proximal gastrectomy, and local resection were not. Worse PGS were found in females, whereas a longer postoperative period alleviated some of the MOMs. For DG and PPG, a laparoscopic approach and preservation of the celiac branch improved several *** Various background factors affected PGS, and their influence varied with the type of gastrectomy performed. Laparoscopic surgery and celiac branch preservation can improve PGS in patients undergoing DG and PPG.

Electro-Anatomical Approach to Membranous Septal Aneurysm: Potential as a Focus of Critical Ventricular Arrhythmias

Membranous septal aneurysm (MSA) is a rare anomaly and known to cause ventricular tachycardia and atrioventricular block. However, underlying mechanisms have not been addressed in its long history. We report first 3-D electro-anatomical mapping of MSA during and three years following the surgery. An elderly patient underwent a surgery for MSA. In the mapping, we located the His bundle near MSA and observed delayed potentials around MSA. Our report showed that electrophysiological character of myocardium was changed around MSA and this change might be a reason for ventricular tachycardia and atrioventricular block. An ordinary surgery for MSA might not resolve this problem since we still observed delayed potentials three years after the surgery.

Applications of shear thickening fluids: a review

Shear thickening fluids (STFs) are a type of non-Newtonian fluids. In the equilibrium state, the STFs are in liquid-state. However, they become solid-state under a sudden strike or impact. And when the strike or impact is removed, the STFs can turn back to their equilibrium properties. This paper provides a detailed review on relevant literature related to the applications with shear thickening fluids. It introduced the factors affecting shear-thickening behaviour including volume fraction, particle size, particle shape, carrier fluid viscosity, temperature and some other factors.

In the context of global climate change it is of practical importance to study changes in the thermal state of upper *** permafrost temperatures are significantly influenced by active-layer freezeback whose duration varies from year to *** later is complete freezing of the active layer,the shorter is the period of winter heat losses from the ground and the higher is its mean annual temperature,and vice *** have been conducted since 1996 in the watershed between the Lena and Kenkeme Rivers,30 km north-west of *** study area is situated within the zone of continuous permafrost with temperatures of-2.5 to-3.5℃.In Central Yakutia,duration of active-layer freezing primarily depends on two environmental factors:pre-winter soil moisture content and early-winter snow *** with low pre-winter soil moisture contents and thin snow covers have higher freezing rates and shorter duration of the freezeback *** such winters during the observation period,the rates of freezing and the lengths of the freezeback period were,respectively,1.0-3.8 cm/day and 60-110 days for the forest landscapes on sandy soils,1.3-3.5cm/day and 40-75 days for the forest landscapes on silty soils,and 0.8-3.7 cm/day and 70-110 days for the meadows on silty *** with high pre-winter soil moisture contents and thick snow covers are characterized by lower freezing rates and by longer duration of the freezeback *** the observation period,the rates of freezing and the lengths of the freezeback period in these winters were,respectively,0.4-2.5 cm/day and 83-145 days for the forest landscapes on sandy soils,0.5-2.1 cm/day and 80-130 days for the forest landscapes on silty soils,and 0.6-2.6cm/day and 95-155 days for the meadows on silty *** on the results of investigations at the experimental sites,empirical relationships between mean annual permafrost temperature and duration of active-layer freezing have been ***

In this paper, a data-driven design method of a feedback gain for loop shaping is proposed for a single input plant. This method can be applied to the tuning of a static feedback gain and also that of the output matrix of a dynamical controller. Constraints on the feedback gain are represented by linear matrix inequalities based on the unfalsified concept using many responses obtained by filtering a plant *** feedback gain is a solution of a linear matrix inequality problem, where the integral gain of the loop transfer function is maximized subject to the constraints. Numerical examples demonstrate the usefulness.

AIM To identify certain clinical factors other than the type of gastrectomy which affect the postoperative quality of life(QOL) of patients after *** The postgastrectomy syndrome assessment scale(PGSAS)-45 was designed to assess the severity of symptoms, the living status and the QOL of gastrectomized patients. It consists of 45 items, of which 22 are original items while 23 were retrieved from the SF-8 and Gastrointestinal Symptoms Rating Scale questionnaires with permission. A nationwide surveillance study to validate PGSAS was conducted and 2368 gastric cancer patients who underwent various types of gastrectomy at 52 medical institutions were enrolled. Of these, 1777 patients who underwent total gastrectomy(TG) reconstructed with Roux-Y(n = 393), distal gastrectomy(DG) reconstructed with Billroth-I(n = 909), or DG reconstructed with Roux-Y(n = 475) were evaluated in the current study. The influence of the type of gastrectomy and other clinical factors such as age, sex, duration after surgery, the symptom severity, the degree of weight loss, dietary intake, and the ability for working on the postoperative QOL(i.e., dissatisfaction for daily life subscale, physical component summary and mental component summary of the SF-8) were examined by multiple regression analysis(MRA). In addition, importance of various symptoms such as esophageal reflux, abdominal pain, meal-related distress, indigestion, diarrhea, constipation and dumping on the postoperative living status and QOL were also appraised by *** The postoperative QOL were significantly deteriorated in patients who underwent TG compared to those after DG. However, the extent of gastrectomy was not an influential factor on patients' QOL when adjusted by the MRA. Among various clinical factors, the symptom severity, ability for working, and necessity for additional meals were the most influential factorsto the postoperative QOL.
