A case of quadruple primary cancer including lip cancer

In recent years, the frequency of occurrence of multiple cancers has tended to increase in association with the progression of diagnostic techniques and the treatment of malignancy or the aging of society。We experienced a case of quadruple cancer (prostate cancer, gastric cancer, larynx cancer, and lip cancer) which developed in a 73-year-old male。Patient: 73-year-old male。First visit: October 4, 2006。Chief complaint: Pain in the lower lip。Past medical history: Radiation therapy for prostate cancer in 1983。A total gastrectomy for gastric cancer in 2004。Laryngomicrosurgery in 2005 and radiation therapy in 2007 for larynx cancer Current medical history: The patient felt pain in his lower lip about 1 month before visiting the hospital。He had been initially treated at a dental clinic in his neighborhood, but the symptoms did not ameliorate, so he visited our department on October 4, 2006, by referral。Present status: Erosion with redness detected in the lower lip。Clinical diagnosis: Leukoplakia of the lower lip。Treatment and progression: On October 5, 2006, the patient had a biopsy taken of his lower lip, and was diagnosed to have differentiated squamous cell cancer based on a histopathological diagnosis。The patient's lower lip was irradiation with 57.5 Gy as part of radiation therapy from December 2006 to January 29, 2007。He is currently being followed up by conservative observations but is making satisfactory progress with no findings that would be indicative of a recurrence。The frequency of occurrence of quadruple cancer is low。Among all cases of malignancy, reported cases that include oral cancer are particularly rare, accounting for only 18 cases in Japan, including the present case。These cases constitutes 12 males and 6 females, with ages ranging from 47 to 86 years old and an average age of 63.7 years old。Among oral cancers, tongue cancer was observed most frequently, accounting for 5 cases, followed by gingival cancer。In other part of cancer, gastric cancer, esop

HAIRCARECUBETM (HCC) is a new additive for hair products that allows the active ingredient to penetrate into the hair. During the course of percutaneous absorption studies, we discovered that lyotropic liquid crystal promotes the skin’s absorptiveness to medicines. We conducted a study to determine whether or not lyotropic liquid crystal could be applied as a penetration-enhancing agent in hair, and as a result we have succeeded in developing HCC. In place of hair dye, we used fluorescein-HCC to evaluate hair permeability, and strong fluorescence was observed as deep as the core of the hair. Moreover, the strength of the fluorescence was dependent on HCC concentration. This result did not change under acidic or basic conditions. In addition, the same trends were observed when using an oxidative hair dye. Furthermore, when HCC was used together with hydrolyzed keratin, which repairs hair damage, a stronger restorative effect was observed. These results confirmed that HCC has the effect of promoting the permeation of pigments and other active ingredients into hair. HCC is expected to be very useful as an additive for developing functional cosmetic hair products.

Limited Effect of Intravenously Administered Indoxyl Sulfate, a Uremic Toxin, on the Hepatic Transport of Pravastatin in Normal Rats

Indoxyl sulfate (IS) is a typical uremic toxin that extensively accumulates in the plasma of patients with seriously impaired renal function. This study seeks to clarify whether IS exerts a potent modulating effect on the hepatic transport of pravastatin, which is a substrate of both organic anion transporting peptides (OATPs) and multidrug resistance-associated protein (Mrp) 2 in rats. When IS is administered intravenously to the normal rats at a dose of 120 μmol/kg;plasma IS levels are approximately 600 μM after 2 min and 100 μM after 120 min. In rats with acute renal failure (ARF) induced by cisplatin, the area under the curve (AUC) was more than 2.5-fold greater compared with that in the normal rats, indicating that IS accumulates in ARF rats. Intravenously administered pravastatin almost disappeared from the plasma by 60 min post-administration and approximately 55% of dose was excreted in the bile within 60 min. This result suggested that pravastatin was efficiently taken up from the sinusoid into hepatocytes via rat OATPs on the sinusoidal membrane and preferentially transported in the bile mediated by Mrp2 on the canalicular membrane. IS administered intravenously at a dose of 120 μmol/kg caused neither an increase in plasma pravastatin levels nor a decrease in its biliary excretion. In conclusion, the present results demonstrate that single intravenous administration of IS does not interfere with the hepatic transport of pravastatin directly in vivo, which is at variance with the results of previous in vitro studies.

A Study on Visual Search during the Trail Making Test: Analysis Using an Eye Tracker

The Trail Making Test A (TMT-A) is a task related to visual search. Therefore, performance in the TMT-A has a high possibility of being influenced by visual information, such as the arrangement of numbers. The purpose of this study was to measure eye movements during the TMT-A using an eye tracker, to analyze differences in enforcement and trace execution, and to examine appropriate inspection drawings as visual search tasks. Study 1 was performed in 14 young healthy subjects and Study 2 was performed in 6 young healthy subjects. The English and Japanese versions of the TMT-A were administered as usual and the subjects performed the traces in a sitting position. The eye movements of the subjects were measured during the task. We analyzed the combined motion angles of both eyes. We compared the average values (30 Hz/s) of movement in the horizontal (X-axis) and vertical (Y-axis) directions during the normal administration of the English and Japanese version of the TMT-A. We also compared the traces performed by the subjects. There were no differences in tracing or enforcement in the English version of the TMT-A. Especially in the vertical visualization operation, it was shown that the normal enforcement was fewer than the tracing, and almost no up-to-down search was performed. In contrast, the subjects performed visual searches in all directions during the Japanese version of the TMT-A.

The purpose of this study was to investigate the effects of a traditional Japanese medicine Goshajinkigan (TJ-107) and Tokishigyakukagoshuyushokyoto (TJ-38) on warm sense threshold, cold sense threshold and the peripheral blood flow. 31 healthy volunteers (control group: 9people, TJ-107 group: 12 people, TJ-38group:10 people) were examined. Drugs administered 2.5 g a dose. Analysis was before and after 1 hour dosage. The warm and cold sense threshold in the thenar of the non-handedness site of these subjects was measured using a thermostimulator (Intercross-200, Intercross Co., Tokyo, Japan). The peripheral blood flow in the finger of the non-handedness site of these subjects was measured using a full-field laser perfusion imager (FLPI, Moor Instruments Ltd., England). Control: The vehicle had no significant effect on the warm sense threshold, cold sense threshold and the peripheral blood flow. TJ-107: The warm sense threshold and cold sense threshold were significantly decreased, and the reaction latency of cold sense was significantly shortening. The peripheral blood flow was significantly increased second and third finger at 115.6%, 119.3%, respectively. TJ-38: The cold sense threshold and the reaction latency of cold sense were significantly increased. The peripheral blood flow was significantly increased second and third finger with 114.3%, 112.8%, respectively. These results suggest that TJ-107 and TJ-38 have effects on the changed warm sense threshold, cold sense threshold and increased peripheral blood flow.

In the present study, the ROCF test was initially conducted involving 30 healthy young individuals, in a quiet environment as Experiment 1 to examine variations in the score among different methods to memorize the figure. In such an environment, no significant differences were observed in the score between the copying and outer speech groups, which suggested the possibility of some of the former groups having used outer speech in a voice too low to be heard or moving their lips without vocalization, achieving the same effect as outer speech, and consequently leading to the absence of differences from the outer speech group. On the other hand, the score markedly varied between the mouthpiece and copying or outer speech groups. As lip movements were suppressed in the former case, the unconscious use of outer speech was also prevented, possibly leading to poor results. Based on these findings, it may be possible to enhance the effects of rehabilitation in a clinical setting by promoting patients’ memorization using outer speech to vocalize the contents of training.
