This study evaluated the capability of hydroxyapatite fiber (HAF) as a carrier and the bone formation by blending simvastatin. The mixture of HAF and simvastatin (0.15, 0.45, 0.75 mg) was placed in 1 ml of tris-buffer and the release of simvastatin from HAF was calculated per 24 hours for 10 days. Bilateral 5 mm-diameter and 3 mm-hight Teflon chambers were fixed on calvaria of adult Japanese white rabbits and filled with 40 mg HAF which containing simvastatin (0, 0.15, 0.45, 0.75 mg). The animals were sacrificed at 4 and 8 weeks and calculated radiologically by Micro-CT. After dyeing by toluidine blue the samples were analyzed histologically. In all of the study groups approximately 25% of simvastatin was released until 10 days. The new bone volume ratio measured by Micro-CT of 4 and 8 weeks group was (22.4%, 21.3%, 41.6%, 26.3%) and (20.2%, 11.7%, 42.1%, 31.2%) in different doses respectively. The 0.45 mg group showed significantly higher new bone volume ratio than 0 mg group and 0.15 mg group. The histological measurement and observations also supported these results. In conclusion, the HAF could be used as a carrier for simvastatin. Combinations of HAF and simvastatin have the potentiality to stimulate new bone formation and approximately 0.45 mg simvastatin in 40 mg HAF is the optimal dose in rabbit chamber model.

In time-of-flight mass spectrometry（TOF MS）, superconducting strip ion detectors（SSIDs） in the parallel configuration are promising for ideal ion detection with a nanosecond-scale time response and a practical large sensitive area. In the parallel configuration, the bias current in one strip is diverted into other parallel strips after each detection event. Under high bias current conditions, the diverted bias current induces cascade switching of all parallel strips. Studies show that cascade switching degrades the ion count rate of SSIDs made from niobium and hence is disliked in TOF MS applications. To suppress the bias current redistribution, we connected resistors in a series with the individual parallel strips using aluminum-bonding wires. Their effect was studied by measuring the pulse height distributions.

Benign esophageal stricture after thermal injury treated with esophagectomy and ileocolon interposition

＜正＞Objectives: We evaluated that feasibility of using Tc-99m tin colloid to identify sentinel nodes （SNs） gastrointestinal malignancies. Methods: The subjects were patients diagnosed with gastrointestinal malignancies considered to be surgically respectable with no clinically obvious nodal metastases. One hundred and sixty-seven patients with gastric cancer, 52 with esophageal cancer, and 61 with colorectal cancer were enrolled in this study. Tc-99m labeled tin colloid （Tc-99m: SnCl2=1:1） was endoscopically injected into the submucosal layer around the tumor. On the following day, patients underwent resection surgery, including resection of regional lymph nodes （LNs）. The radioactivity of resected LNs was measured using a well-type scintillation counter and /or a surgical probe. Any LN with a radioactivity higher than 1/10 of the highest one was defined as a SN. The pathological state of each of the resected LNs was examined using routine H-E staining. Results: The detectability of SNs as well as the sensitivity, false negative （FN） ratio, and diagnostic accuracy of the metastatic status of regional LNs as judged by the pathological results are shown below. Absolute subject numbers （positive cases/total cases examined） are shown in parentheses. Conclusion: Tc-99m tin colloid was able to detect over 90% of the SNs of gastrointestinal malignancies. We suggest that sentinel node concept could be applied to the gastrointestinal tumors as well as breast cancer and malignant melanoma.

Thermal injuries of the esophagus are rare causes of benign esophageal stricture, and all published cases were successfully treated with conservative management. A 28-year-old Japanese man with a thermal esophageal injury caused by drinking a cup of hot coffee six months earlier was referred to our hospital. The hot coffee was consumed in a single gulp at a party. Although the patient had been treated conservatively at another hospital, his symptoms of dysphagia gradually worsened after discharge. An upper gastrointestinal endoscopy and computed tomography revealed a pinhole like area of stricture located 19 cm distally from the incisors to the esophagogastric junction, as well as circumferential stenosis with notable wall thickness at the same site. The patient underwent a thoracoscopic esophageal resection with reconstruction using ileocolon interposition. The pathological findings revealed wall thickening along the entire length of the esophagus, with massive fibrosis extending to the muscularis propria and adventitia at almost all levels. Treatment with balloon dilation for long areas of stricture is generally difficult, and stent placement in patients with benign esophageal stricture, particularly young patients, is not yet widely accepted due to the incidence of late adverse events. Considering the curability and qualityof-life associated with a long expected prognosis, we determined that surgery was the best treatment option for this young patient. In this case, we decided to perform an esophagectomy and reconstruction with ileocolon interposition in order to preserve the reservoir function of the stomach and to avoid any problems related to the reflux of gastric contents. In conclusion, resection of the esophagus is a treatment option in patients with benign esophageal injury, especially in cases involving young patients with refractory esophageal stricture. In addition, ileocolon interposition may help to improve the quality-of-life of patients.

We herein report a case of bronchial bleeding afterradical esophagectomy that was treated with lobectomy.A 65-year-old male who underwent subtotal esophagectomy with three-field lymph node dissection for esophageal carcinoma was referred to our hospital because of sudden *** the esophagectomy,bilateral vocal cord paralysis was observed,and the patient suffered from repeated episodes of aspiration *** revealed hemosputum in the right middle lobe bronchus,and contrast-enhanced computed tomography showed tortuous arteries arising from the right inferior phrenic artery and left subclavian artery toward the right middle lobe *** bronchial arterial embolization was performed twice to control the recurrent hemoptysis,the procedures were *** middle lobectomy was therefore performed via video-assisted thoracic *** bronchial arterys with medial hypertrophy and overgrowth of the small branches were noted near the bronchus in the resected *** patient recovered uneventfully and was discharged on postoperative day 14.

Recent progress in the physics and engineering design study for themodification programme of JT-60 is presented. In order to achieve a steady state high-βplasmaoperation, which is the dominant issue of this programme, physics design for the MHD control and thestability analysis is investigated. Engineering design and the R & D for the superconducting coils,irradiation shield are performed well towards the mission of programme.
