Method to measure tree-ring width,density,elemental composition,and stable carbon and oxygen isotopes using one sample

Tree-ring width(RW),density,elemental com-position,and stable carbon and oxygen isotope(δ^(13)C,δ^(18)O)are widely used as proxies to assess climate change,ecology,and environmental pollution;however,a specific pretreat-ment has been needed for each proxy.Here,we developed a method by which each proxy can be measured in the same sample.First,the sample is polished for ring width meas-urement.After obtaining the ring width data,the sample is cut to form a 1-mm-thick wood plate.The sample is then mounted in a vertical sample holder,and gradually scanned by an X-ray beam.Simultaneously,the count rates of the fluorescent photons of elements(for chemical characteriza-tion)and a radiographic grayscale image(for wood density)are obtained,i.***.the density and the element content are obtained.Then,cellulose is isolated from the 1-mm wood plate by removal of lignin,and hemicellulose.After producing this cellulose plate,cellulose subsamples are separated by knife under the microscope for inter-annual and intra-annual stable carbon and oxygen isotope(δ^(13)C,δ^(18)O)analysis.Based on this method,RW,density,elemental composition,δ^(13)C,and δ^(18)O can be measured from the same sample,which reduces sample amount and treatment time,and is helpful for multi-proxy comparison and combination research.

Evaluation of bacterial contamination and medium-term oncological outcomes of intracorporeal anastomosis for colon cancer:A propensity score matching analysis

BACKGROUND Although intracorporeal anastomosis(IA)for colon cancer requires longer operative time than extracorporeal anastomosis(EA),its short-term postoperative results,such as early recovery of bowel movement,have been reported to be equal or better.As IA requires opening the intestinal tract in the abdominal cavity under pneumoperitoneum,there are concerns about intraperitoneal bacterial infection and recurrence of peritoneal dissemination due to the spread of bacteria and tumor cells.However,intraperitoneal bacterial contamination and medium-term oncological outcomes have not been clarified.abdominal cavity in IA.METHODS Of 127 patients who underwent laparoscopic colon resection for colon cancer from April 2015 to December 2020,75 underwent EA(EA group),and 52 underwent IA(IA group).After propensity score matching,the primary endpoint was 3-year disease-free survival rates,and secondary endpoints were 3-year overall survival rates,type of recurrence,surgical site infection(SSI)incidence,number of days on antibiotics,and postoperative biological responses.RESULTS Three-year disease-free survival rates did not significantly differ between the IA and EA groups(87.2%and 82.7%,respectively,P=0.4473).The 3-year overall survival rates also did not significantly differ between the IA and EA groups(94.7%and 94.7%,respectively;P=0.9891).There was no difference in the type of recurrence between the two groups.In addition,there were no significant differences in SSI incidence or the number of days on antibiotics;however,postoperative biological responses,such as the white blood cell count(10200 vs 8650/mm^(3),P=0.0068),C-reactive protein(6.8 vs 4.5 mg/dL,P=0.0011),and body temperature(37.7 vs 37.5℃,P=0.0079),were significantly higher in the IA group.CONCLUSION IA is an anastomotic technique that should be widely performed because its risk of intraperitoneal bacterial contamination and medium-term oncological outcomes are comparable to those of EA.

Combined laparoscopic and thoracoscopic repair of adult right-sided Bochdalek hernia with massive liver prolapse: A case report

BACKGROUND A Bochdalek hernia(BH)is a congenital diaphragmatic hernia that often develops in the neonatal period.BH typically occurs on the left side of the diaphragm.A right-sided BH in an adult is rare.CASE SUMMARY A 45-year-old man was referred to our hospital because of an abnormal shadow seen on chest radiography during a medical check-up.A chest radiograph showed elevation of the right hemidiaphragm.Computed tomography showed prolapse of multiple intraabdominal organs into the right thoracic cavity,corresponding to a right-sided BH.The herniated contents included the stomach,transverse colon,and left lobe of the liver.The left lobe of the liver was enlarged,particularly the medial segment.Laparoscopic surgery was performed.However,the left lobe of the liver was completely trapped in the thoracic cavity.Therefore,thoracoscopic manipulation had to be performed to return the liver to the abdominal cavity.The hernia was repaired with interrupted nonabsorbable sutures and reinforced with mesh.CONCLUSION Combined laparoscopic and thoracoscopic surgery was successfully performed for right-sided BH with massive liver prolapse and abnormal liver morphology.

Immunotherapy for esophageal cancer:Where are we now and where can we go

Immune checkpoint inhibitor therapy has dramatically improved patient prognosis,and thereby transformed the treatment in various cancer types including esophageal squamous cell carcinoma(ESCC)in the past decade.Monoclonal antibodies that selectively inhibit programmed cell death-1(PD-1)activity has now become standard of care in the treatment of ESCC in metastatic settings,and has a high expectation to provide clinical benefit during perioperative period.Further,anti-cytotoxic T-lymphocyte–associated protein 4(CTLA-4)monoclonal antibody has also been approved in the treatment of recurrent/metastatic ESCC in combination with anti-PD-1 antibody.Well understanding of the existing evidence of immune-based treatments for ESCC,as well as recent clinical trials on various combinations with chemotherapy for different clinical settings including neoadjuvant,adjuvant,and metastatic diseases,may provide future prospects of ESCC treatment for better patient outcomes.

Novel technique of abdominal wall nerve block for laparoscopic colostomy: Rectus sheath block with transperitoneal approach

A 62-year-old man who had acute rectal obstruction due to a large rectal cancer is presented. He underwent emergency laparoscopic colostomy. We used the laparoscopic puncture needle to inject analgesia with the novel transperitoneal approach. In this procedure, both ultrasound and laparoscopic images assisted with the accurate injection of analgesic to the correct layer. The combination of laparoscopic visualization and ultrasound imaging ensured infiltration of analgesic into the correct layer without causing damage to the bowel. Twentyfour hours postoperatively, the patient's pain intensity as assessed by the numeric rating scale was 0-1 during coughing, and a continuous intravenous analgesic was not needed. Colostomy is often necessary in colon obstruction. Epidural anesthesia for postoperative pain cannot be used in patients with a coagulation disorder. We report the use of a novel laparoscopic rectus sheath block for colostomy. There has been no literature described about the nerve block with transperitoneal approach. The laparoscopic rectus sheath block was performed safely and had enough analgesic efficacy for postoperative pain. This technique could be considered as an optional anesthetic regimen in acute situations.

A Study on Diagnostic Assist Systems of Chronic Obstructive Pulmonary Disease from Medical Images by Deep Learning

In this paper, we propose new diagnostic assist systems of medical images using deep learning algorithms. Specifically, we aim to develop a diagnostic support system for the very early stage of chronic obstructive pulmonary disease (COPD) based on the CT images. It is said that COPD is a disease that develops due to long-term smoking, and it is said that there are a large number of latent onset reserve forces. By discovering this COPD in the very early period 0 and improving the living conditions, subsequent severity can be avoided in many cases, so a system that will help diagnosis by professional radiologists is needed. We show the some experimental results examined by the constructed system.

Assessment of delayed bleeding after endoscopic submucosal dissection of early-stage gastrointestinal tumors in patients receiving direct oral anticoagulants

Delayed bleeding is a major and serious adverse event of endoscopic submucosal dissection(ESD)for early-stage gastrointestinal tumors.The rate of post-ESD bleeding for gastric cancer is higher(around 5%-8%)than that for esophagus,duodenum and colon cancer(around 2%-4%).Although investigations into the risk factors for post-ESD bleeding have identified several procedure-,lesion-,physician-and patient-related factors,use of antithrombotic drugs,especially anticoagulants[direct oral anticoagulants(DOACs)and warfarin],is thought to be the biggest risk factor for post-ESD bleeding.In fact,the post-ESD bleeding rate in patients receiving DOACs is 8.7%-20.8%,which is higher than that in patients not receiving anticoagulants.However,because clinical guidelines for management of ESD in patients receiving DOACs differ among countries,it is necessary for endoscopists to identify ways to prevent post-ESD delayed bleeding in clinical practice.Given that the pharmacokinetics(e.g.,plasma DOAC level at both trough and T_(max))and pharmacodynamics(e.g.,anti-factor Xa activity)of DOACs are related to risk of major bleeding,plasma DOAC level and anti-FXa activity may be useful parameters for monitoring the anti-coagulate effect and identifying DOAC patients at higher risk of post-ESD bleeding.

Coronavirus disease 2019(COVID-19)caused by the novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)has become a major global public health event,resulting in a significant social and economic burden.Although COVID-19 was initially characterized as an upper respiratory and pulmonary infection,recent evidence suggests that it is a complex disease including gastrointestinal symptoms,such as diarrhea,nausea,and vomiting.Moreover,it remains unclear whether the gastrointestinal symptoms are caused by direct infection of the gastrointestinal tract by SARS-CoV-2 or are the result of systemic immune activation and subsequent dysregulation of homeostatic mechanisms.This review provides a brief overview of the mechanisms by which SARS-CoV-2 disrupts the integrity of the gastrointestinal barrier including the mechanical barrier,chemical barrier,microbial barrier,and immune barrier.

Prediction of Photosynthetic Carbon Assimilation Rate of Individual Rice Leaves under Changes in Light Environment Using BLSTM-Augmented LSTM

A model to predict photosynthetic carbon assimilation rate(A)with high accuracy is important for forecasting crop yield and productivity.Long short-term memory(LSTM),a neural network suitable for time-series data,enables prediction with high accuracy but requires mesophyll variables.In addition,for practical use,it is desirable to have a technique that can predict A from easily available information.In this study,we propose a BLSTM augmented LSTM(BALSTM)model,which utilizes bi-directional LSTM(BLSTM)to indirectly reproduce the mesophyll variables required for LSTM.The most significant feature of the proposed model is that its hybrid architecture uses only three relatively easy-to-collect external environmental variables—photosynthetic photon flux density(Q_(in)),ambient CO_(2) concentration(C_(a)),and temperature(T_(air))—to generate mesophyll CO_(2) concentration(C_(i))and stomatal conductance to water vapor(g_(sw))as intermediate outputs.Then,A is predicted by applying the obtained intermediate outputs to the learning model.Accordingly,in this study,1)BALSTM(Q_(in),C_(a),T_(air))had a significantly higher A prediction accuracy than LSTM(Q_(in),C_(a),T_(air))in case of using only Q_(in),C_(a),and T_(air);2)BALSTMC_(i),g_(sw),which had C_(i) and g_(sw) as intermediate products,had the highest A prediction accuracy compared with other candidates;and 3)for samples where LSTM(Q_(in),C_(a),T_(air))had poor prediction accuracy,BALSTMC_(i),g_(sw)(Q_(in),C_(a),T_(air))clearly improved the results.However,it was found that incorrect predictions may be formed when certain factors are not reflected in the data(e.g.,timing,cultivar,and growth stage)or when the training data distribution that accounts for these factors differs from the predicted data distribution.Therefore,a robust model should be constructed in the future to improve the prediction accuracy of A by conducting gasexchange measurements(including a wide range of external environmental values)and by increasing the number of training data samples.
