Outcomes of patients older than 75 years with non-metastatic prostate cancer

Objective:Prostate cancer in elderly patients was formerly treated with androgen deprivation therapy.Since the latter of the 1990s new technologies were introduced into treatments,then strategies have varied.We aimed to observe the outcomes of elderly patients treated during transition period and compare each stage with others.Methods:During 2008 and 2010,255 patients with prostate cancer older than 75 years were sequentially treated.With exception of patients with bone and/or visceral metastasis,outcomes of 199 patients with localized and locally advanced stages were examined.Complete records were obtained by the end of 2015.Results:In total,122(61%),28(14%),37(19%)and 12(6%)of patients were in stages T1c-T2a,T2b-c,T3 and T4,respectively.Patients generally presented with abnormal screening or lower urinary tract symptom.Seventy-one percent of patients received androgen deprivation therapy as monotherapy and 22% of the radiation-treated patients added androgen deprivation therapy.Patients in stage T1c-T2a and T2b-c showed a favorable prognosis.Some cancer death appeared in patients with T3 and T4 during observation periods.Twenty-seven percent of patients died from prostate cancer-independent complications:pneumonia,heart disease,and brain vascular disease.Tendency is similar to that of Japanese elderly male population.No remarkable side effects from androgen deprivation therapy were noticed.

Artificial intelligence and machine learning have been increasingly applied for prediction in agricultural science.However,many models are typically black boxes,meaning we cannot explain what the models learned from the data and the reasons behind predictions.To address this issue,I introduce an emerging subdomain of artificial intelligence,explainable artificial intelligence(XAI),and associated toolkits,interpretable machine learning.This study demonstrates the usefulness of several methods by applying them to an openly available dataset.The dataset includes the no-tillage effect on crop yield relative to conventional tillage and soil,climate,and management variables.Data analysis discovered that no-tillage management can increase maize crop yield where yield in conventional tillage is<5000 kg/ha and the maximum temperature is higher than 32°.These methods are useful to answer(i)which variables are important for prediction in regression/classification,(ii)which variable interactions are important for prediction,(iii)how important variables and their interactions are associated with the response variable,(iv)what are the reasons underlying a predicted value for a certain instance,and(v)whether different machine learning algorithms offer the same answer to these questions.I argue that the goodness of model fit is overly evaluated with model performance measures in the current practice,while these questions are unanswered.XAI and interpretable machine learning can enhance trust and explainability in AI.

The proposed heavy ion inertial fusion(HIF)scenarios require ampere class low charge state ion beams of heavy species.The laser ion source(LIS)is recognized as one of the promising candidates of ion beam providers,since it can deliver high brightness heavy ion beams to accelerators.The design of LIS for the HIF depends on the accelerator structure and accelerator complex following the source.In this article,we discuss the specifications and design of an appropriate LIS assuming two major types of the accelerators:radio frequency(RF)high quality factor cavity type and non-resonant induction core type.We believe that a properly designed LIS would satisfy the requirements of both types,while some issues need to be verified experimentally.

The conversion of methane and carbon dioxide into syngas(dry reforming of methane;DRM)has attracted attention owing to the potential to reuse greenhouse gases.Titanium dioxide(TiO_(2))-based photocatalysts,which have been widely commercialized owing to their high efficiency,non-toxicity,and low cost,are strongly desired in DRM.Here,we report a monoclinic-phase TiO_(2)-B nanobelts-supported rhodium(Rh/TiO_(2)-B nanobelts)catalyst that efficiently promotes DRM under ultraviolet light irradiation at low temperatures.Photogenerated holes in the TiO_(2)-B nanobelts were used to oxidize methane,while the electrons were trapped in rhodium to reduce carbon dioxide.Rh/TiO_(2)-B nanobelts exhibited considerably higher durability and activity than Rh-loaded conventional TiO_(2)(anatase and rutile),owing to the lattice and/or surface oxygen reactivity in TiO_(2)-B nanobelts,which was suggested by X-ray photoelectron spectroscopy measurements and photocatalytic performance tests under an atmosphere of methane alone.This study paves the path for the effective utilization of methane by constructing active TiO_(2)-based nanometal photocatalysts.

Squamous papilloma is a benign mass lesion of the oral mucosa. For papillomas of the tongue, surgery is recommended owing to their malignant potential;however, certain complications may be associated with surgery. A traditional Japanese(Kampo) herbal medicine, Keishibukuryogan-ka-yokuinin(KBGY),has been used to treat viral warts and various skin diseases in Japan. Therefore, the effect of KBGY on papillomas is promising. A 49-year-old Japanese man presented with a wart on his tongue that was about3 months old. He smoked 5 cigarettes per day. He did not drink alcohol. He had no history of malignant illnesses. He was taking alprazolam for panic disorder. The patient was diagnosed with a suspected papilloma of the tongue at the Department of Otolaryngology and was advised to undergo an excision biopsy to exclude malignancy. However, he refused owing to the fear of an invasive procedure. After informed consent was obtained from the patient, KBGY was prescribed. Three months later, the wart on his tongue spontaneously prolapsed. The histopathological diagnosis was squamous papilloma. There was no indication of malignancy, and the patient discontinued Kampo treatment. He has had no recurrence in the past 3 years. KBGY is a combination of Keishibukuryogan and yokuinin(adlay seeds).Keishibukuryogan may be beneficial for skin or oral mucosal remodeling, and yokuinin may have antiviral properties. The present case report suggests the use of KBGY as an appropriate complementary therapy for squamous papilloma.

It is well known that the Cauchy problem for Laplace equations is an ill-posed problem in Hadamard’s sense.Small deviations in Cauchy data may lead to large errors in the solutions.It is observed that if a bound is imposed on the solution,there exists a conditional stability estimate.This gives a reasonable way to construct stable algorithms.However,it is impossible to have good results at all points in the domain.Although numerical methods for Cauchy problems for Laplace equations have been widely studied for quite a long time,there are still some unclear points,for example,how to evaluate the numerical solutions,which means whether they can approximate the Cauchy data well and keep the bound of the solution,and at which points the numerical results are reliable?In this paper,the authors will prove the conditional stability estimate which is quantitatively related to harmonic measures.The harmonic measure can be used as an indicate function to pointwisely evaluate the numerical result,which further enables us to find a reliable subdomain where the local convergence rate is higher than a certain order.

Induced pluripotent stem cells(iPSCs)are considered to be ideal and promising cell sources for various applications such as regenerative medicine and drug screening.However,effective mass production systems for the stable supply of desired numbers of iPSCs are yet to be developed.This review introduces the various approaches that are currently available for stable iPSC production.We start by discussing the limiting factors to be controlled during iPSC culture,such as nutrient supply,waste removal,and oxygen availability.We then introduce recent investigations on iPSC culture systems based on adhesion,suspension,and scaffolds.We also discuss the downstream processes that follow the culture process,such as filling and freezing processes,which limit the production scale due to decreased cell viability during suspension in cryopreservation medium.Finally,we summarize the possibility of the stable mass production of iPSCs and highlight the limitations that remain to be overcome.We suggest that multidisciplinary investigations are essential to understand the different factors that influence cell growth and quality in order to obtain an optimal and stable iPSC mass production system.
