Helicobacter pylori eradication to prevent gastric cancer: underlying molecular and cellular mechanisms

Numerous cellular and molecular events have been described in development of gastric cancer. In this artide, we overviewed roles of Helicobacter pylori （H pylori） infection on some of the important events in gastric carcinogenesis and discussed whether these cellular and molecular events are reversible after cure of the infection. There are several bacterial components affecting gastric epithelial kinetics and promotion of gastric carcinogenesis. The bacterium also increases risks of genetic instability and mutations due to NO and other reactive oxygen species. Epigenetic silencing of tumor suppressor genes such as RUNX3 may alter the frequency of phenotype change of gastric glands to those with intestinal metaplasia. Host factors such as increased expression of growth factors, cytokines and COX-2 have been also reported in non-cancerous tissue in Hpylori-positive subjects. It is noteworthy that most of the above phenomena are reversed after the cure of the infection. However, some of them including overexpression of COX-2 continue to exist and may increase risks for carcinogenesis in metaplastic or dysplastic mucosa even after successful H pylori eradication. Thus, H pylori eradication may not completely abolish the risk for gastric carcinogenesis. Efficiency of the cure of the infection in suppressing gastric cancer depends on the timing and the target population, and warrant further investigation.

Esophageal early basaloid squamous carcinoma with unusual narrowband imaging magnified endoscopy findings

Basaloid squamous carcinoma (BSC) is a rare variant of esophageal cancer. There are very few reports of “early” BSC. Here we report a case of early BSC with unusual findings by narrowband imaging magnified endoscopy (NBI-ME). A 70-year-old man with a middle thoracic esophageal tumor was referred to our hospital. White-light endoscopy revealed a reddish depressed lesion 5 mm in diameter having a subepithelial tumor-like prominence with a gentle rising slope. NBI-ME revealed irregular loop-shaped microvessels coexistent with thick irregularly branched non-looped vessels. Iodine staining revealed a pale brown lesion. We performed endoscopic submucosal dissection for diagnostic treatment. Histologic examination showed the proliferation of basal cell-like hyperchromatic tumor cells in the lamina propria and with slight invasion into the submucosa at a depth of 320 μm. The tumor cells formed solid nests and microcystic structures, containing an Alcian blue-positive mucoid matrix. The surface was covered with squamous epithelium without cellular atypia. Thin vessels were observed in the intra-epithelial papilla and thick vessels were observed around the solid nests beneath the epithelium. Based on these findings together, we diagnosed the lesion as BSC. In this case, the NBI-ME findings differed from those of typical squamous cell carcinoma in that both non-invasive cancer-like irregular loop-shaped microvessels coexisted with massively invasive cancer-like thick non-looped vessels. We speculate that the looped and non-looped vessels observed by NBI-ME histologically corresponded to thin vessels in the intra-epithelial papilla and thick vessels around the tumor nests, respectively. These NBI-ME findings might be a feature of early esophageal BSC.

Clinical problems with antithrombotic therapy for endoscopic submucosal dissection for gastric neoplasms

Linking of Chemical Reactions and Silica Nanoparticle Contact Using Synthetic Helical Molecules

Biological cells exhibit diverse phenomena induced through linking of chemical reactions of molecules and solid surface *** is then a significant topic in the field of chemistry to study phenomena induced through this linking using synthetic systems,which can promote our understanding of biological phenomena and can be applied to the development of novel *** nanoparticles(sNPs),which are synthetic inorganic materials,are attractive for such purposes,because of their following characteristics:they can adsorb large amounts of molecules on their surfaces,they can aggregate through contact between SNPs as well as contact between molecules and SNPs,and the molecules can be easily removed from solutions by *** contact of sNP surfaces with molecules then affects chemical reactions of molecules and also behaviors of *** article describes systems derived from synthetic helical molecules and sNPs,which exhibit notable phenomena including selective adsorption and molecular recognition,equilibrium shift,step kinetics with induction period,precipitation with flow and sweeping,and disaggregation and desorption by sonication,in which the high affinity of helical molecules with sNP surfaces plays important *** models that explain the phenomena are *** applications are also discussed,including the separation of molecules,capture of intermediates,the storage and release of molecules,equilibrium shift,clocking,and the translation of mechanical stimulations into chemical reactions.

Choroidal and cutaneous metastasis from gastric adenocarcinoma

Choroidal or cutaneous metastasis of gastric cancer is rare. Gastrointestinal cancer was found in only 4% in patients with uveal metastasis. Choroidal metastasis from gastric cancer was reported in two cases in earlier literature. The frequency of gastric cancer as a primary lesion was 6% in cutaneous metastasis of men, and cutaneous metastasis occurs in 0.8% of all gastric cancers. We report a patient with gastric adenocarcinoma who presented with visual disorder in his left eye and skin pain on his head as his initial symptoms. These symptoms were diagnosed to be caused by choroidal and cutaneous metastasis of gastric adenocarcinoma. Two cycles of chemotherapy consisted of oral S-1 and intravenous cisplatin (SPIRITS regimen); this was markedly effective to reduce the primary gastric lesion and almost all the metastatic lesions.

Early detection of early gastric cancer(EGC)is important to improve the prognosis of patients with gastric *** advances in endoscopic modalities and treatment devices,such as image-enhanced endoscopy and high-frequency generators,may make endoscopic treatment,such as endoscopic submucosal dissection,a therapeutic option for gastric intraepithelial ***,short-term outcomes of endoscopic resection(ER)for EGC have ***,surveillance with endoscopy after ER for EGC is becoming more important,but how to perform endoscopic surveillance after ER has not been established,even though the follow-up strategy for more advanced gastric cancer has been ***,a surveillance strategy for patients with EGC after ER is needed.

CONTROLLED SYNTHESIS OF AMPHIPHILIC BLOCK POLYMERS HAVING GLUCOSE RESIDUES AND THEIR STRUCTURE FORMATION

Vinyl ether (VE)-based amphiphilic block copolymers with D-glucose residues as hydrophilicpendants were synthesized by CH_3 CH(OiBu)Cl/ZnI_2-initiated sequential living cationic copolymerization of3-O-(vinyloxy)ethyl-1, 2:5, 6-di-O-isopropylidene-D-glucofuranose (IGVE) and isobutyl VE (IBVE ) andsubsequent deprotection. The precursor block copolymers had a narrow molecular weight distribution(M_w/M_n～1.1) and a controlled segmental composition. The solubility characteristics of the amphiphiliccopolymer depended strongly on composition. Their solvent-cast thin films were examined, under atransmission electron microscope, and could be seen to exhibit various microphase-separated surfacemorphologies such as spheres, cylinders, and lamellae, depending on composition. The amphiphiliccopolymers with the appropriate segmental composition were found to form a stable monolayer at the air-water interface, which was successfully transferred onto a substrate by the Langmuir-Blodgett (LB)technique. The layered strucfure of the built-up LB films was controlled by blending the homopolymer.

Basic Research on the Burden of Dropping off and Picking up Children: Differences by Employment Type and Gender in the Tokyo Metropolitan Area

There is a need to reduce the burden of child drop-off and pick-up for child-rearing generations, but most studies on the actual situation in Japan are based on survey results. In this study, we analyzed differences in child drop-off and pick-up by employment type and gender, utilizing the “Metropolitan Area Person Trip Survey,” which is a statistical data set. The study targeted households in which both spouses were between 30 and 49 years old, had children under the age of 6, and included the following three groups. 1) Dual-income Group 1 (both spouses employed/on contract/temporary);2) Dual-income Group 2 (husband employed/on contract/temporary, wife part-time);3) Full-time housewife group (husband employed, wife unemployed). The analysis revealed that a) wives are almost always responsible for dropping off and picking up their children;b) husbands drop off and pick up their children less frequently in dual-income households;and c) households with children raising within 10 to 30 km of Tokyo Station have longer commuting times and need to reduce the burden of dropping off and picking up their children.

Factors Influence Hanoians’ Choice of Non-Motorized Transport Mode to Access Bus Stations

Hanoi’s rapid urbanization has led to a surge in private vehicle ownership, particularly motorcycles, amidst inadequate public transportation infrastructure. Despite government efforts, many still prefer motorized transport, citing mobility and safety concerns, exacerbated by insufficient pedestrian infrastructure. This study examines the motivations behind this reliance on motorized vehicles, particularly motorcycles, in Hanoi. Findings reveal safety and convenience as primary factors driving motorized transport use, especially for accessing bus stations. Economic incentives could promote non-motorized travel and public transport adoption. Policy implications highlight the importance of addressing economic factors and improving access infrastructure to manage motorized vehicle reliance and foster sustainable urban mobility in Hanoi.
