Comparison of Public Health Nurse’s Lifestyle Counseling Skills for Clients with Different Readiness of Behavior Changes in Primary Care Settings

Purpose: Public health nurses (PHNs) are required to assess the readiness of the clients and provide lifestyle counseling accordingly. The purpose of this study was to compare the lifestyle counseling provided for clients with different levels of readiness based on self-evaluations and independent assessment. Methods: Participants were PHNs with 10 years’ experience or less. Lifestyle counseling skills were measured through a simulated counseling session in a primary setting lasting up to 30 minutes, followed by a review session, where the participant provided self-evaluations, and the simulated client provided others-evaluations, of the session. The simulated clients played the role of clients at either the pre-contemplation or contemplation stage of preparedness as per the stages of behavior change theory. Results: The self-evaluation results showed that the mean scores for five of the six skill categories, are lower in the precontemplation-stage client cases than in the contemplation-stage client cases, and significantly so for two skill categories. The others-evaluations showed significantly lower mean scores for all skill categories in the precontemplation cases than in the contemplation cases. Conclusion: The PHNs and simulated clients agreed that lifestyle counseling skills were inadequate for the precontemplation cases, as compared with the contemplation cases. The lifestyle counseling skills of PHNs with less experience may not be well-adjusted to the readiness of the client, indicating difficulties in supporting less prepared clients.

Effects of Cultivation Conditions on Chlorophyll Biosynthesis and Carotenoid Accumulation of the Microalga Chlorella Fusca

The purpose of the present study is to optimize the cultivation conditions for the productivities of chlorophyll and carotenoid of the microalga chlorellafusca in a flow bioreactor. The effects of CO2 concentration in aeration and light intensity as the culture condition were firstly examined. After the cultivation conditions as to CO2 concentration and light intensity were determined, the effects of the concentrations of KNO3, CH3COONa, NaC1 and MgSO4 in the culture medium on the chlorophyll biosynthesis and carotenoid accumulation ofC. fusca were examined. KNO3 and CH3COONa were used as the nitrogen and the organic carbon sources, respectively. They are very important nutrient sources to the microalgae. In fact, when the concentration of the nutrient sources was increased, the syntheses of chlorophyll and the carotenoid accumulation of the microalgae were promoted. Cfusca can take an organic carbon as the nutrient sources for its own growth, so the cell is activated compared to the case only in an inorganic nourishment sources. Mg2＋ is known as a raw material of chlorophyll and it can promote carotenoid accumulation. However, when it exists in excessive quantity, the microalgae receive a stress and it was difficult for microalgae to uptake such nutrient sources. In this study, it was found that there existed the optimum amounts of KNO3, CH3COONa, NaCI and MgSO4 which should be added to the culture medium.

Relationship between Reflective Practice Skills and Volume of Writing in a Reflective Journal

Background: According to the diversification of the health needs and the expansion of health disparities, public health nurses need to improve their practical capabilities, starting from basic education in graduate and undergraduate courses. And Reflective Practice with using reflective journal is one way of improving practical capabilities. The purpose of this study was to investigate the relationship between the volume of reflective journal and the quality of progress in the reflective cycle. Methods: The participants in this study were 20 junior students majoring in public health nursing (hereinafter “PHN students”) at a university in the Chugoku area, Japan. We asked the participants to answer the questions on Reflective Practice Skills (RPS) composed of six criteria corresponding to the six questions of Gibbs on the reflective cycle before and after they started writing RJ. The volume of reflective writing was measured by the number of characters written by the PHN students in RJ of the reflective practice for three months. The study plan was approved by the Ethics Committee for Nursing Study, Okayama University. Results: Although the average total RPS score showed a change of about 3 points as a result of the 3-month RJ writing exercise, no correlation was observed between the RPS score and the RJ writing volume (r = 0.175). However, we did observe a moderately positive correlation between the RPS score and the RJ writing volume with regard to Items 5 and 6 (r = 0.475 and r = 0.444, respectively). Conclusion: This study indicated that detailed RJ writing helps to complete the reflective cycle all the way to theorization and action planning, and that the volume of writing may serve as a criterion for qualitative evaluation.

Inference of General Mass Action-Based State Equations for Oscillatory Biochemical Reaction Systems Using <i>k</i>-Step Genetic Programming

Systems biology requires the development of algorithms that use omics data to infer interaction networks among biomolecules working within an organism. One major type of evolutionary algorithm, genetic programming (GP), is useful for its high heuristic ability as a search method for obtaining suitable solutions expressed as tree structures. However, because GP determines the values of parameters such as coefficients by random values, it is difficult to apply in the inference of state equations that describe oscillatory biochemical reaction systems with high nonlinearity. Accordingly, in this study, we propose a new GP procedure called “k-step GP” intended for inferring the state equations of oscillatory biochemical reaction systems. The k-step GP procedure consists of two algorithms: 1) Parameter optimization using the modified Powell method—after genetic operations such as crossover and mutation, the values of parameters such as coefficients are optimized by applying the modified Powell method with secondary convergence. 2) GP using divided learning data—to improve the inference efficiency, imposes perturbations through the addition of learning data at various intervals and adaptations to these changes result in state equations with higher fitness. We are confident that k-step GP is an algorithm that is particularly well suited to inferring state equations for oscillatory biochemical reaction systems and contributes to solving inverse problems in systems biology.

An extremely rare case of pancreatic metastasis of esophageal squamous cell carcinoma

We report a rare case of a 68-year-old male with metachronous pancreatic metastasis that was resected2 years after salvage esophagectomy for local recurrence of esophageal squamous cell carcinoma(ESCC).Two years and 8 mo ago,he had undergone definitive chemoradiotherapy for the lower thoracic ESCC and achieved a complete *** used the protocol of the Japan Clinical Oncology Group trial *** 8 mo later,he developed a local recurrence of the ESCC and underwent thoracoscopic salvage esophagectomy followed by reconstruction with a conduit colon graft via a subcutaneous ***,a tumor of the pancreatic body was found on routine follow-up computed tomography(CT).The tumor diameter was 15 mm on CT,and the maximum standardized uptake value of the lesion was 5.49at 18F-2-fluoro-2-deoxy-D-glucose positron-emission tomography,strongly suggesting pancreatic *** addition,all tumor markers were within the reference ***,distal pancreatectomy was performed with the resultant histological diagnosis being confirmed as pancreatic metastasis of the *** was treated with adjuvant chemotherapy,and there has been no evidence of recurrence 9 mo after the *** of pancreatic metastasis offers a good prognosis and should be considered for solitary ESCC metastasis.

自 1976 年以来,日本核废料管理基金会与研究中心(RWMC)一直在进行核废料处置的研究。最近,RWMC非常关注核废料处置库的监测、遥控以及现场勘查技术等重要的社会公众与技术问题。其中,处置库溢出气体是一项重要的安全可靠性评估项目。这项名为“工程屏障的气体渗透试验”(GMT)的研究是在瑞士 Nagra 的 Grimsel现场实验室进行的。GMT 集中研究在粮仓形的处置库中由核废料产生的气体在工程屏障中的渗透问题。在 Grimsel现场,一个按比例缩小的工程屏障库由一个具有透气孔的混凝土仓和砂–膨润土缓冲材料组成,缓冲材料被放置在一个专门建造的洞室中。混凝土仓直径为 2.5 m,高为 2.5 m,放置在直径为 4 m 的从上到下开挖而成的竖井中。在工程屏障饱和及水渗透试验之后,气体分两个阶段被注入地窖中。气体第一次被注入之后,重复进行水渗透试验,以便观察工程屏障库的变化。第二次气体注入的时候,在工程屏障及岩石圈中采用一种可以多点取样的鸡尾酒气体跟踪器。在气体注入之后,小心挖掘出工程屏障库,然后用各种方法来进行鉴定,并采用实验室以及建模程序来进行现场的模拟研究。

AIM： To study a more accurate quantification of hepatic fibrosis which would provide dinically useful information for monitoring the progression of chronic liver disease. METHODS： Using a cDNA microarray containing over 22000 clones, we analyzed the gene-expression profiles of non-cancerous liver in 74 patients who underwent hepatic resection. We calculated the ratio of azanstained： total area, and determined the morphologic fibrosis index （MFI）, as a mean of 9 section-images. We used the MFI as a reference standard to evaluate our method for assessing liver fibrosis. RESULTS： We identified 39 genes that collectively showed a good correlation （r 〉 0.50） between geneexpression and the severity of liver fibrosis. Many of the identified genes were involved in immune responses and cell signaling. To quantify the extent of liver fibrosis, we developed a new genetic fibrosis index （GFI） based on gene-expression profiling of 4 clones using a linear support vector regression analysis. This technique, based on a supervised learning analysis, correctly quantified the various degrees of fibrosis in both 74 training samples （r = 0.76, 2.2% vs 2.8%, P 〈 0.0001） and 12 independent additional test samples （r = 0.75, 9.8% vs 8.6%, P 〈 0.005）. It was far better in assessing liver fibrosis than blood markers such as prothrombin time （r = -0.53）, type IV collagen 7s （r = 0.48）, hyaluronic acid （r = 0.41）, and aspartate aminotransferase to platelets ratio index （APRI）（r = 0.38）. CONCLUSION： Our cDNA microarray-based strategy may help clinicians to precisely and objectively monitor the severity of liver fibrosis.

Sofosbuvir/Ribavirin therapy for patients experiencing failure of ombitasvir/paritaprevir/ritonavir + ribavirin therapy: Two cases report and review of literature

BACKGROUND The effectiveness of sofosbuvir/ribavirin(SOF/RBV) combination therapy,which is one of the 1 st-choice therapeutic options for patients with hepatitis C virus(HCV) genotype 2(HCV-G2) in Japan according to the most recent version of the Japan Society of Hepatology guideline, for patients who experienced failure of the ombitasvir/paritaprevir/ritonavir plus ribavirin(OBV/PTV/r+RBV) combination therapy, which was another option for patients with HCV-G2, is *** SUMMARY We evaluated the effects of SOF/RBV combination therapy in two patients with genotype 2 a who could not achieve a sustained virological response(SVR) by OBV/PTV/r+RBV combination therapy. One patient was complicated with VogtKoyanagi-Harada(VKH) disease. Resistance-associated variations before SOF/RBV combination therapy were not detected in two patients. Both patients had an SVR at 12 wk after the treatment(SVR12). Regarding adverse events(AEs), itching, chill, a dull feeling in the throat and cough as well as increase of alanine transaminase level were shown in one patient, while a headache and deterioration of light aversion probably due to the recurrence of VKH disease were shown in the other patients. In addition, the latter patient developed arthralgia and morning stiffness approximately 7 wk after the therapy and turned out to be diagnosed with rheumatoid *** SOF/RBV therapy might be effective for patients experiencing failure of OBV/PTV/r+RBV therapy, but caution should be taken regarding the AEs.
