Gastrointestinal symptoms in a Japanese population:A health diary study

AIM： To investigate the incidence of gastrointestinal symptoms and the nature of consequent utilization of health care services in a Japanese population. METHODS： Using self-report, we conducted a prospective cohort study of a nationally representative sample of the Japanese population over a one-month period to determine the incidence of gastrointestinal symptoms of all kinds and resultant health care utilization. Both information on visits to physicians and use of complementary and alternative medicine therapies were collected. RESULTS： From a total of 3568 in the recruitment sample, 3477 participants completed a health diary （response rate 97%）. The data of 112 participants with baseline active gastrointestinal diseases were excluded from the analysis, leaving 3365 participants in the study. The incidence of gastrointestinal symptoms was 250 and the mean number of symptomatic episodes was 0.66 in a month. Abdominal pain, diarrhea, nausea, constipation and dyspepsia were the most frequent symptoms. Female gender, younger age, and low baseline quality of life were risk factors for developing these symptoms. The participants were more likely to treat themselves, using dietary, complementary or alternative medicines, than to visit physicians, except in the case of vomiting.CONCLUSION： Gastrointestinal symptoms are common in the Japanese population, with an inddence of 25%. Abdominal pain, diarrhea, nausea, constipation and dyspepsia are the most frequent symptoms. Risk factors for developing these symptoms include female gender, younger age, and low baseline quality of life.

Liver cirrhosis in a child associated with Castleman's disease: A case report

BACKGROUND Castleman's disease(CD)is a lymphoproliferative *** syndrome is classified as a variant of CD based on its key clinical manifestations of thrombocytopenia,anasarca(generalized edema and pleural effusion),fever(pyrexia),reticulin fibrosis in the bone marrow and the proliferation of megakaryocytes,and organomegaly(such as hepatosplenomegaly and multiple lymphadenopathies);TAFRO syndrome is mainly reported in Japanese *** our knowledge,this is the first pediatric case report detailing a CD-associated disorder progressing to *** SUMMARY A 10-year old male patient presented with fever and *** months before hospitalization,he had remarkable abdominal ***,he visited a clinic for a fever that lasted 5 *** physical findings were marked hepatosplenomegaly and cervical lymphadenopathy.A blood test revealed leukocytosis,microcytic anemia,aspartate aminotransferase-dominant transaminase elevation,high levels of C-reactive protein,polyclonal hypergammaglobulinemia,and high levels of interleukin-6 and vascular endothelial growth *** contrast computed tomography and magnetic resonance imaging suggested cirrhosis,which was confirmed by liver *** findings in the enlarged hepatic lymph nodes revealed both hyperplasia and atrophy of lymphoid follicles with some vascular hyperplasia and moderate plasmacytosis between the lymphoid follicles,which is compatible with lymph node histology in TAFRO *** was not effective in reducing the patient’s symptoms;therefore,the patient was prescribed *** date,the patient remains free of fever and continues to receive *** We described the clinicopathological features of TAFRO syndrome to highlight the clinical presentation of this rare disease in a pediatric case.

Spontaneous regression of hepatocellular carcinoma: A mini-review

Spontaneous tumor regression is an extremely rare phenomenon in the oncology field. However, there are several case reports resulted in the regression of hepatocellular carcinoma(HCC) and the accumulation of clinical information and analyses of the mechanism can contribute to the development of a novel therapy. For this purpose, we have carefully reviewed 23 cases of spontaneously regressed HCC published in recent 5 years and our case. The information regarding the tumor size, tumor marker, treatments, etc., have been summarized. The mechanism of spontaneous regression has been discussed to date and presumed to be due to many factors, including hypoxia and immunological reactions. In this careful review of the 24 cases based on the clinical information, hypoxia, systemic inflammation, and both upon spontaneous regression were seen in 3, 8, and 4 cases, respectively amo ng t he 15 c as e s f or w hic h t he inf o r mat io n regarding the proposed mechanisms are available. Recent development of immunotherapeutic approaches in oncology shows promising results, therefore, accumulation of additional cases and analysis of mechanisms underlying the spontaneous regression of HCC are essential and could lead to the development of a new generation of immunotherapies including antibodies directed against immune reactions.

Environmental agricultural policy in Japan in harmony with WTO agricultural agreements:current situation and its development (I)

Introduction'Agriculture' could be defined as the human activity of obtaining a larger volume of food and raw materials from a limited land area in order to sustain a growing population. To accomplish this, humans must transform an ecological system of nature into a partly man-made ecological system of arable land. In this ecological system of arable land, crops that require human assistance are predominant, and any other wild species are generally considered undesirable, and so are removed. There are significantly fewer species in an ecological system of arable land than in an ecological system of nature, and the interrelationship among species is weaker. Given these facts,agriculture is in effect accompanied by the destruction of nature, and cannot be said to contribute to the protection of the environment.

Plasmonic photodetectors have received increasing attention because their detection properties can be designed by tailoring their metal structures on surfaces without using any additional *** of the plasmonic resonant state in a photodetector is relevant for various applications,including investigating in situ adaptive detection property changes,depending on the situation,and performing single-pixel spectroscopy in geometrically limited ***,the spectral responsivity change with conventional reconfiguration methods is relatively ***,we propose a plasmonic photodetector that reconfigures its spectral responsivity with electromechanical deformation instead of bias *** photodetector consists of a gold plasmonic grating formed on an n-type silicon cantilever,and the spectral responsivity is reconfigured by electromechanically scanning at an incident angle to the grating on the *** photodetector exhibits peak shifts in spectral responsivity in a wavelength range from 1250 to 1310 nm after electromechanical ***,for potential future applications,we demonstrate near-infrared spectroscopy using the *** photodetector has the potential to be adopted as a near-infrared spectrometer in industrial silicon imaging systems because its structure enables subbandgap photodetection on silicon by a Schottky junction.

作为一个全球气候变率的重要模态,北极涛动或北半球环状模态(AO/NAM)一般由北半球海平面气压变率的EOF1来表示。但是通常认为EOF1仅有统计学意义而并不能够表明它是否是一种实际的物理模态。另一方面,现有的波-流相互作用理论也仅能给出纬向风的纬向平均状况,而不能够保证沿着某一纬圈的变化,所以它并不意味着一个具有半球尺度的纬向对称的相干结构能够组织起来。因此AO/NAM的形成机制仍然是一个具有争议的问题。文章提出纬向对称的或环状的正规模态与基本气候态附近的线性(进而非线性)动力学之间有着最直接的联系,因而可能在AO/NAM动力学的理解中扮演重要角色。为了深入探讨AO/NAM的动力学本质,使用p-坐标球面原始方程计算了冬季北半球及全球气候基本态附近的线性动力系统纬向对称的正规模态,发现在半球尺度上的纬向对称正规模态具有AO/NAM的经向偶极子及垂直方向近似正压的空间结构特征。而这些纬向对称正规模的时间变化尺度取决于与其他非纬向对称正规模间的相互作用。从而说明了AO/NAM可能是半球尺度上纬向对称正规模态的动力学行为的反映。

Efficacy of ripasudil in reducing intraocular pressure and medication score for ocular hypertension with inflammation and corticosteroid

·AIM:To investigate the efficacy of ripasudil,a Rho kinase inhibitor,in reducing intraocular pressure(IOP)and medication scores of anti-glaucoma drugs in patients with ocular hypertension with inflammation and corticosteroid.·METHODS:The study included 11 patients diagnosed with ocular hypertension with inflammation and corticosteroid,all of whom were prescribed ripasudil eye drops and followed up for at least 2y after the initiation of *** was measured using a non-contact tonometer before enrollment and at each follow-up *** medication score of glaucoma eye drops was calculated for each patient.·RESULTS:The mean IOP(26.4±2.9 mm Hg before treatment)significantly decreased after ripasudil therapy(13.7±3.3 mm Hg at 3mo)and remained stable in the low-teens during the 2-year follow-up period(P<0.0001).A significant decrease in the medication score was observed at 12mo or later after the initiation of ripasudil therapy(P<0.05).Both baseline medication scores and glaucomatous optic disc change rates were significantly higher in the five eyes that required glaucoma surgery during the 2-year observation period than the 10 eyes that did not require surgery.·CONCLUSION:Our results demonstrate the efficacy of ripasudil,in reducing IOP and the medication score over a 2-year treatment period in patients with ocular hypertension with inflammation and *** findings also suggest that ripasudil could reduce the IOP in uveitic glaucoma patients with both lower baseline medication score and lower glaucomatous optic disc change rate.

Pyrene Nanocrystals Crystallized by a Continuous Flow Microreactor

Pyrene solution and distilled water were flowed through a microreactor at a predetermined flow rate to generate pyrene crystals. Pyrene nanocrystals were crystallized by a Continuous Flow Microreactor. The particle size and luminescence properties of pyrene nanocrystals produced were evaluated. The crystal mean size between 60 nm and 400 nm could be controlled by the operating conditions. The crystal mean size decreased with increasing flow rate and solution concentration. In addition, the crystal morphology also changed. In the case of slow flow conditions, a needle-like crystal morphology was obtained. The crystal morphology became spherical on increasing the flow rate. Pyrene crystals with size about 400 nm exhibited luminescence at about 470 nm. As the crystal size decreased, the intensity of the luminescence also declined. The luminescence wavelength was in the range of about 370 to 400 nm.
