There is a clear clinical need for efficient physiotherapy and rehabilitation programs during and after bone lengthening and reconstruction for gaining the optimal effect and also prevention or treatment of lengthening side effects.Pin tract infection is the most prevalent side effect during lengthening which could be prevented and treated initially via proper wound care.Muscle contractures are typically a consequence of the generated tension on the distracted muscle.It can be managed by physiotherapy initially and surgically in later severe stages.Furthermore,it is essential to avoid muscle contracture development,which is the demonstration of the imbalanced muscle appeals on the joint to inhibit the following subluxation.The knee is the furthermost affected joint by the aforementioned problem due to the inherent lack of ligamentous and bony stability.Joint stiffness is the other possible unfavorable effect of lengthening.It happens because of extensive muscle contractures or may possibly be attributed to rigidity of the joint following the amplified pressure on the joint surface during the process of lengthening.Physiotherapy and occupational therapy including endurance and strength exercise as well as stretching play an important role during the rehabilitation periods for the prevention and also the treatment of muscle contracture and the following deformity and also joint stiffness.Likewise,the effect of mental and physical rehabilitation programs should not be overlooked.

The varieties and capabilities of artificial intelligence and machine learning in orthopedic surgery are extensively expanding.One promising method is neural networks,emphasizing big data and computer-based learning systems to develop a statistical fracture-detecting model.It derives patterns and rules from outstanding amounts of data to analyze the probabilities of different outcomes using new sets of similar data.The sensitivity and specificity of machine learning in detecting fractures vary from previous studies.AI may be most promising in the diagnosis of less-obvious fractures that are more commonly missed.Future studies are necessary to develop more accurate and effective detection models that can be used clinically.

BACKGROUND The management of caustic esophageal burns in the pediatric population has changed over the years,while the most optimal management with regards to effectiveness,availability,and cost-beneficent stays controvertible.AIM To describe how to utilize a chest tube for esophageal stenting in pediatrics.METHODS Data regarding the etiology,treatment,and complications of caustic injury in pediatrics over 10 years was collected retrospectively.Furthermore,data regarding the patient's follow-up who underwent esophageal chest tube(ECT)were collected.The ECT was prepared by carving a narrowed section in the chest tube while maintaining the radiopaque section.The ECT will then be positioned from the cricopharyngeal and exited through the nostril and fixed on the patient's cheek.RESULTS During the period of our study,data from 57 patients with an average age of 2.5 years(range 1-12;SD=1.7)were obtained.The results showed that 89%of esophageal injury was due to alkaline and 9.4%were caused by acidic agents.The treatment methods showed that 29 patients(50.8%)recovered with dilatation alone.In 16 patients(28.06%),the esophageal repair was performed by using the colon,and in 5 patients(8.7%),other surgical methods were used and in 7 patients(12.2%),the ECT stents were used.ECT was inserted in 7 cases with a mean age of 2(range:1.5-3)years who were classified as grade IIB or III.Grading was performed by endoscopy assessment on the first day.Antibiotics and corticosteroids were administrated as initial medical management for all patients.ECT implantation was done during the first 8 d for 5 out of 7 cases(mean:3.8 d).For the 2 patients,ECT was used after 27(patient 6)d and 83(patient 7)d.The reason for late stenting in these patients was a postponed referral to our center,in which patient 7 even received 4 dilation episodes before visiting our center.ECT was removed after an average of 44 d in the first 5 patients,while in the other 2 patients(6 and 7)was 2 and 1 wk,respectively.There was no complication related to,or failu

The incidence of human immunodeficiency virus(HIV)-infected cases that need total joint replacement(TJR)is generally rising.On the other hand,modern management of HIV-infected cases has enabled them to achieve longevity while increasing the need for arthroplasty procedures due to the augmented degenerative joint disease and fragility fractures,and the risk of osteonecrosis.Although initial investigations on joint replacement in HIV-infected cases showed a high risk of complications,the recent ones reported acceptable outcomes.It is a matter of debate whether HIV-infected cases are at advanced risk for adverse TJR consequences;however,the weak immune profile has been associated with an increased probability of complications.Likewise,surgeons and physicians should be aware of the complication rate after TJR in HIV-infected cases and include an honest discussion of the probable unwelcoming complication with their patients contemplating TJR.Therefore,a fundamental review and understanding of the interaction of HIV and arthroplasty are critical.

BACKGROUND It has been said that the number of orthopaedic claims has increased in the last few years. Investigation through the most prevalent cause would help to prevent further cases.AIM To review the cases of medical complaints in orthopedic patients who had been involved in a traumatic accident.METHODS A retrospective multi-center review of trauma orthopaedic-related malpractice lawsuits from 2010 to 2021 was conducted utilizing the regional medicolegal database. Defendant and plaintiff characteristics along with fracture location, allegations, and litigation outcomes were investigated.RESULTS A total of 228 claims referred to trauma-related conditions with a mean age of 31.29 ± 12.56 were enrolled. The most common injuries were at hand, thigh, elbow, and forearm, respectively.Likewise, the most common alleged complication was related to malunion or nonunion. In 47% of the cases, the main problem that led to the complaint was the inappropriate or insufficient explanation to the patient, and in 53%, there was a problem in the surgery. Eventually, 76% of the complaints resulted in a defense verdict, and 24% resulted in a plaintiff verdict.CONCLUSION Surgical treatment of hand injuries and surgery in non-educational hospitals received the most complaints. The majority of litigation outcomes were caused by a physician’s failure to fully explain and educate the traumatic orthopedic patients and technological errors.

Let C be a semidualizing module for a commutative ring R. It is shown that the ：IC-injective dimension has the ability to detect the regularity of R as well as the Pc-projective dimension. It is proved that if D is dualizing for a Noetherian ring R such that idR（D） = n 〈 ∞, then ：ID-idR（F） ≤ n for every flat R-module F. This extends the result due to Enochs and Jenda. Finally, over a Noetherian ring R, it is shown that if M is a pure submodule of an R-module N, then/TC-idR（M） ≤ IC-idR（N）. This generalizes the result of Enochs and Holm.

The blockchain technology has been parenting Bitcoins and many other cryptocurrencies that have strived to be traded on the global markets.It has been only within the last two decades that eventually the concept of blockchain technology was flourished through evolution of IoT,or Internet of Things,and the first well defined cryptocurrency,namely Bitcoin,as a main biproduct of it was evolved to what we know as blockchain technology.Unlike what most people recognize cryptocurrency,as a main concept of its parenting blockchain technology,the cryptos are not just the main or everything about blockchain.Blockchain technology,is rather much more powerfully designed to interconnect and facilitate trade among many businesses in commerce,including insurance,banking,healthcare industry,etc.,through an amazing cryptocurrency,a non-fiat currency and medium of exchange.Bitcoin’s extension into the 21st century’s monetary system was and will continue to be registered as a historical turning point of the medium of exchange,store of value,and unit of account,among other desirable properties of money.We dare to have concluded that the blockchain technology is another revolution inside the capitalistic ecosystem for enforcing its powerful potential in the conversion of centralized businesses into somewhat more decentralized,where competition and efficiency would be more practically fostered than ever before.There will be more reliable and verifiable information,permanently stored on electronic ledgers,connected to enormous other nodes merged with those already accessible through the blockchain platform,operated by entrepreneurs,such as“Your First Venture Companion,”just as a tentative example.

Propose:In this study,we re-assessed the criteria defined by the radiological society of North America(RSNA)to determine novel radiological findings helping the physicians differentiating COVID-19 from pulmonary contusion.Methods:All trauma patients with blunt chest wall trauma and subsequent pulmonary contusion,COVID-19-related signs and symptoms before the trauma were enrolled in this retrospective study from February to May 2020.Included patients(Group P)were then classified into two groups based on polymerase chain reaction tests(Group Pa for positive patients and Pb for negative ones).Moreover,44 patients from the prepandemic period(Group PP)were enrolled.They were matched to Group P regarding age,sex,and trauma-related scores.Two radiologists blindly reviewed the CT images of all enrolled patients according to criteria defined by the RSNA criteria.The radiological findings were compared between Group P and Group PP;statistically significant ones were re-evaluated between Group Pa and Group Pb thereafter.Finally,the sensitivity and specificity of each significant findings were calculated.The Chi-square test was used to compare the radiologica丨findings between Group P and Group PP.Results:In the Group PP,73.7%of all ground-glass opacities(GGOs)and 80%of all multiple bilateral GGOs were detected(p<0.001 and p=0.25,respectively).Single bilateral GGOs were only seen among the Group PP.The Chi-square tests showed that the prevalence of diffused GGOs,multiple unilateral GGOs,multiple consolidations,and multiple bilateral consolidations were significantly higher in the Group P(p=0.001,0.01,0.003,and 0.003,respectively).However,GGOs with irregular borders and single consolidations were more significant among the Group PP(p=0.01 and 0.003,respectively).Of note,reticular distortions and subpleural spares were exclusively detected in the Group PP.Conclusion:We concluded that the criteria set by RSNA for the diagnosis of COVID-19 are not appropriate in trauma patients.The clinical signs and symptoms are not always usefu

Objective: The extremity injury pattern after a major earthquake is not well understood because data on this type of injury and management are limited. The aim of this study was to analyze the data of trauma patients with ex-tremity injury in the earthquake of Barn Iran, registering 6.6 on the Richter scale.Methods: We reviewed medical records of 486 patients admitted to hospitals of Tehran University of Medical Sciences. Among them, 274 patients sustained extremity injuries. This group was composed of 138 females (50.4%) and 136 males (49.6%) and 213 cases (77.7%) were under 40 years of age.Results: Fracture was the major type of injury (58.4% of extremity injuries). The most common sites of injury were lower extremities (185 patients, 67.5% of all victims). Pelvic & rib fractures and abdominal injuries were the most fre-quently associated injuries. The mean ISS was 6.2±4.0, and 61% of the patients had ISS≤7. Amputation and death occurred in 2.9% and 2.5% of cases, respectively.Conclusions: Special attention should be given to deal-ing with orthopedic injuries in similar disasters in the future.
