Extracorporeal membrane oxygenation(ECMO)has emerged as a vital circulatory life support measure for patients with critical cardiac or pulmonary conditions unresponsive to conventional *** allows blood to be extracted from a patient and introduced to a machine that oxygenates blood and removes carbon *** blood is then reintroduced into the patient’s circulatory *** process makes ECMO essential for treating various medical conditions,both as a standalone therapy and as adjuvant ***-venous(VV)ECMO primarily supports respiratory function and indicates respiratory ***,veno-arterial(VA)ECMO provides hemodynamic and respiratory support and is suitable for cardiac-related *** study reviews recent literature to elucidate the evolving role of ECMO in trauma care,considering its procedural intricacies,indications,contraindications,and associated ***,the use of ECMO in trauma patients,particularly for acute respiratory distress syndrome and cardiogenic shock,has demonstrated promising outcomes despite challenges such as anticoagulation management and complications such as acute kidney injury,bleeding,thrombosis,and *** studies have shown that VV ECMO was associated with significantly higher survival rates than conventional mechanical ventilation,whereas other studies have reported that VA ECMO was associated with lower survival rates than VV *** plays a critical role in managing trauma patients,particularly those with acute respiratory *** research is necessary to explore the full potential of ECMO in trauma *** should have a clear understanding of the indications and contraindications for the use of ECMO to maximize its benefits in treating trauma patients.

Burnishing is a profitable process of surface finishing due to its ability to be automated,which makes burnishing method more desirable than other finishing *** obtain high surface finish,non-stop operation is required for CNC machine and we can attain that by choosing a suitable trajectory of the finishing *** other words,burnishing paths should be multidirectional rather than monotonic,in order to cover uniformly the ***,the burnishing force is also a key parameter of the burnishing process because it determines the degree of plastic deformation,and that makes determining the optimum burnishing force an essential step of the burnishing process a ***,we consider the strategy of ball burnishing path and the burnishing force as variable parameters in this *** this paper,we propose a new strategy of burnishing tool path with trochoid cycles that achieves a multidirectional burnishing of the surface according to various *** into consideration the optimum burnishing force,to improve flat surface finish of AL6061-O samples by reducing the surface roughness parameter(Rz).Experiments carried out on 3-axis milling machine show that the proposed trochoidal path is more effective than the conventional one.

Childhood Papillary Thyroid Carcinoma: A Case Report

Background: Thyroid cancer is a rare disease yet the most common endocrine malignancy in pediatrics. Unlike adult patients, children with thyroid nodules typically don’t complain of pain, soreness, or difficulty swallowing. Additionally, using the recommended therapy for adults to treat paediatrics is not appropriate. There is an unmet need for updated unique guidelines for the management of papillary thyroid carcinoma (PTC) in paediatrics and adolescents. Case Report: A 12-year-old girl had an atypical presentation of metastatic PTC in lymph nodes. She was treated initially with hemi-thyroidectomy, followed by total thyroidectomy. A multidisciplinary team followed her up till successful results were found. Conclusion: Due to the difference in pathophysiology between thyroid tumors in children and adults, a unique approach to PTC management is to be implemented. Further trials are required for a better understanding of risk factors, the likelihood of recurrence, and the long-term side effects of the chosen management plan.

Effects of cropland abandonment and afforestation on soil redistribution in a small Mediterranean mountain catchment

In slopes of Mediterranean mid-mountain areas,land use and land cover changes linked to the aban-donment of cropland activity affect soil quality and degradation and soil redistribution;however,limited attention has been paid to this issue at catchment *** paper evaluates the effects of cropland abandonment and post-land abandonment management(through natural revegetation and afforesta-tion)on soil redistribution rates using fallout ^(137)Cs measurements in the Araguás catchment(0.45 km^(2),Central Spanish Pyrenees).A total of 52 soil core samples,distributed in a regular grid,from the first 30-40 cm and 9 sectioned reference samples were collected across the catchment and soil properties were *** ^(137)Cs was measured in a 5 cm sectioned references samples and in bulk grid samples.^(137)Cs inventories were used to estimate soil erosion and deposition rates across the *** show that the highest erosion rates were recorded under sparsely vegetated sites in the badland area,while the lowest rates were found in the afforested area,but no significant differences were observed between the different uses and covers in soil redistribution rates likely due to a long history of human intervention through cultivation in steep slopes and afforestation ***,the recovery of the soil organic matter in afforested areas suggest that afforestation can reduce soil degradation at long-term *** information gained achieve a better understanding of soil redistribution dynamics and provide knowledge for effective land management after cropland abandonment of agroecosystems in Mediter-ranean mountain areas.

How do Medical Students Learn?

Background & Objectives: In this paper, we try to define learning, to describe how retention of memorized material can be improved and to describe how to help students improve clinical reasoning and problem solving skills. The goal of teaching is to improve learning, but how do we know that students are learning in the proper way? Methods: We depend on our experience in medical and clinical teaching of our undergraduate and postgraduate students during implementation of the curriculum. We interview 1st year (preclinical) and 6th year medical students by certain questionnaires (VARK) in order to know their learning methods and their knowledge about the styles of learning and the techniques of learning. Learning is viewed here as developing a way of thinking and acting that is a characteristic of an expert physician. Such a way of thinking consists of three important elements: We utilize Working Memory in 3 main approaches: First, attention in a way through questions, focus attention and uses of first letter of statement of the word;Second, rule of 7 (Teach < 7 steps);Third, concentration on important relevant materials. We have to keep in our mind that even if we remember something, it doesn't mean we understand it. In addition, the case learning approach & problem solving learning approach are the methods used and we try to be cooperative, active, and experimental and the student must be self directed and the students should be competent in his learning methods, most are confident of their ability. Results: Most of the first year students have poor knowledge in regards to style & learning techniques. For final year students around 30% learn two styles and 20% trimodel, especially more in female students, 10% mention single model in their learning and the remaining learn by Quad modals. In conclusion, learning how to learn must be a standard part of the curriculum in medical school, and student must be aware of that.

Autoimmune Diseases and Acquired Von Willebrand Disease in Two Cases of Progeria

Ammar A., a 23-year-old male patient, who lives in Babylon, Haswa District, and his mother describes symptoms of growth retardation, skin changes, hair changes early graying and alopecia. These manifestation started early during his childhood period. There is canseguanity between the patient’s mother & father also one of the patient’s sister has similar illness and one male brother died few months following his birth. We admit the patient to hospital due acute pulmonary infection in Jan 2009, which is controlled after a course of antibiotic and after 5 months he develops generalised mucocuteneous bullous eruption which shows partial response to oral prednisolone 2 mg/Kg. The patient has normal IQ and he is in the secondary school and he has normal blood picture and the only abnormal biochemical abnormalities is mild hyperlipidemia Serum cholestrol of 5.8 mmol/L and Serum Triglyceride of 260 mg/dl. Ammar’s Sister Qawthar A., who has a similar phenotypic manifestations, presented skin vitiligo and hepatosplenomegaly associated with sever anemia and jaundice and her presentation suggestive of autoimmune haemolytic anemia improved following blood transfusion, corticosteroid and azothioprim. In February 2014 Ammer presented with multiple and diffuse cuteneous ecchymymosis with markedly prolonged PTT and slightly proloned bleeding time highly consistent with acquired Von Willebrand’s disease. In conclusion premature aging is a predisposing factor for disturbed immunity and development of autoimmune diseases.

A new three-dimensional elastography using phase based shifted Fourier transform

Elastography is an imaging technique with the ability to determine low quantities of some of the mechanical properties of *** aim of our research is to design a new 3D algorithm using the Shifted Fourier Transform(SFT)to perform a quasi-static *** innovative idea is implementation of a 3D convolution instead of using three 2D *** first,we collected the raw data from Abaqus engineering software in the form of breast tissue with a coefficient of elasticity of healthy tissue and tumor tissue with a coefficient of elasticity of tumor *** primary raw data consists of a number of points with x,y and z specified for tumor and healthy breast *** this step,we simulated the displacements in directions of x,y and z at each point of the prescribed tissues for 15 mm displacement of probe in–Y direction then we collected 1831 points for tumor and 4186 points for breast before and after *** applying a novel reconstruction algorithm,we convolved all images with the 3D Gabor filters to obtain phases,represented displacements of the breast and tumor images for before and after *** reach this goal,we designed a Gabor filter bank based on the dimensions of the input images in different scales,directions,and *** the 3D SFT,we calculated the displacements of the breast and tumor tissues followed by 3D elastogram representation of the ***,we implemented a 2D analysis of SFT in order to investigate validation of the 3D *** 2D algorithm,we used three two-dimensional convulsions in XY,YZ and XZ *** results obtained from the small displacements marked by circles,confirmed the accuracy of the 3D SFT *** areas of interest are the tumor areas in the 2D analysis.

The human spinal column is a dynamic,segmented,bony,and cartilaginous structure that protects the neurologic system and simultaneously provides balance and *** with developmental disorders that affect the patterning or shape of the spine can be at risk of neurologic and other physiologic *** most common developmental disorder of the spine is scoliosis,a lateral deformity in the shape of the spinal *** may be part of the clinical spectrum that is observed in many developmental disorders,but typically presents as an isolated symptom in otherwise healthy adolescent *** idiopathic scoliosis(AIS)has defied understanding in part due to its genetic *** have come from recent genome-wide association studies(GWAS)and next generation sequencing(NGS)of human AIS cohorts,as well as investigations of animal *** studies have identified genetic associations with determinants of cartilage biogenesis and development of the intervertebral disc(IVD).Current evidence suggests that a fraction of AIS cases may arise from variation in factors involved in the structural integrity and homeostasis of the cartilaginous extracellular matrix(ECM).Here,we review the development of the spine and spinal cartilages,the composition of the cartilage ECM,the so-called"matrisome"and its functions,and the players involved in the genetic architecture of *** also propose a molecular model by which the cartilage matrisome of the IVD contributes to AIS susceptibility.

目的设计制备皮肤外用花椒毒素纳米球,探讨花椒毒素纳米球对PUVA疗法光反应毒性的影响,以期优化外用PUVA疗法。方法采用溶剂置换-界面聚合物沉积法制备花椒毒素纳米球,筛选并优化花椒毒素纳米球的处方组成,优化后的纳米球在人工半透膜中考察其体外释放特性,并采用补骨脂素类化合物与DNA的反应模型考察其光反应效率。结果制备并优化后的花椒毒素纳米球其平均粒径为109nm,粒径分布度小于0.1,Zeta电位<-20mV,包封率为65.8%。在体外人工半透膜中的释放特性表明,相比Meladinine溶液,该花椒毒素纳米球具有良好的释放性能。花椒毒素制备成纳米球后,在与胸腺嘧啶的混合反应体系中的光反应速率明显有所降低,能起到控释的效应。结论花椒毒素纳米球有望能降低PUVA疗法的光反应毒性,从而成为改善PUVA疗法的新型制剂。
