Achalasia: A review of Western and Iranian experiences

Achalasia is a primary motor disorder of the eso-phagus, in which esophageal emptying is impaired. Diagnosis of achalasia is based on clinical fi ndings. The diagnosis is confirmed by radiographic, endoscopic, and manometric evaluations. Several treatments for achalasia have been introduced. We searched the PubMed Database for original articles and meta-analyses about achalasia to summarize the current knowledge regarding this disease, with particular focus on different procedures that are used for treatment of achalasia. We also report the Iranian experience of treatment of this disease, since it could be considered as a model for medium-resource countries. Myotomy, particularly laparoscopic myotomy with fundoplication, is the most effective treatment for achalasia. Compared to other treatments, however, the initial cost of myotomy is usually higher and the recovery period is longer. When performing myotomy is not indicated or not possible, graded pneumatic dilation with slow rate of balloon inflation seems to be an effective and safe initial alternative. Injection of botulinum toxin into the lower esophageal sphincter before pneumatic dilation may increase remission rates. However, this needs to be conf irmed in further studies. Due to lack of adequate information regarding the role of expandable stents in the treatment of achalasia, insertion of stents does not currently seem to be a recommended treatment. In summary, laparoscopic myotomy can be considered as the procedure of choice for treatment of achalasia. Graded pneumatic dilation is an effective alternative when the performance of myotomy is not possible for any reason.

The interaction of bovine serum albumin (BSA) with a new palladium(II) complex [Pd(bpy)(Oct-Gly)]NO 3 (bpy, 2,2′-bipyridine; Oct-Gly, octyl-glycine) was studied by isothermal titration UV-visible spectrophotometry and microcalorimetry in 30 mmol/L Tris buffer, pH 7.0. There is a set of 18 binding sites for this complex on BSA at 300 and 310 K with positive cooperativity in the binding process. The Hill coefficients at 300 and 310 K are 2.2 and 2.4, respectively. The binding of this palladium complex on BSA is endothermic with mean association binding constant of 21.0 and 16.4 (mmol/L) -1 at 300 and 310 K, respectively. The complex can denature the protein as surfactants. The stability of BSA in the interaction study with the complex is 84 and 58 kJ/mol at 300 and 310 K, respectively. Also, the enthalpy of BSA denaturation due to the interaction with the complex is 842 kJ/mol.

The presence of groundwater is strongly related to its geological and geohydrological *** is,however,important to study the groundwater potential in an area before it is utilized to provide clean ***-Schlumberger’s method was used to analyze the groundwater potential while hydraulic properties such as soil porosity and hydraulic conductivity were used to determine the quality and ability of the soil to allow water’s movement in the *** results show that the aquifer in the Sekara and Kemuning Muda is at a depth of more than 6 meters below the ground level with moderate groundwater *** is also found that the aquifer at depths of over 60 m have high groundwater ***,soil porosity in Kemuning is found to be average while the ability to conduct water was *** makes it possible for some surface water to seep into the soil while the remaining flows to the rivers and ditches.

Internal development is applying of all the power and existing potential and actual capacities in the city to achieve sustainable and urban participation, and improves all social, physical, economic, and political structures to achieve a sustainable quality and quantity equilibrium. The policy of urban development in the world is to improve the productivity and optimization of land use and the possibility of infill development and improving access of city distressed fabric habitants to urban and social services. In this regard, the present study of Analysis of vitalization strategies of historical fabric with the approach of endogenous development Case study (1, 2 & 3 regional of Zahedan city). The research method is analytic-descriptive and based on library, documentary and field studies and VIKOR model has been also used for data analysis. Based on the results of VIKOR model, the 3 regional of Zahedan city with score 3 and the region 1 with a score of 0.436 were located on the highest and lowest rank. At the end, we suggested strategies according of the research result.
